The aim of this research is to study the effect of body overweight at birth (fetal macrosomia) on the processes of tooth eruption and tooth growth during the first year of life in children in the Kharkiv City (Ukraine) population. One of the research tasks is to examine the features of deciduous teeth eruption in children who were born with macrosomia with different values of the weight-height index at birth. Materials and methods. The medical records of the children born between 1977 and 2013 have been analyzed. The database has been collected in one of the Kharkiv City clinic. The Main Group is comprised of the medical records of the children (separately for boys and girls) born with fetal macrosomia. All the medical records of the Main Group have been divided into subgroups taking into account the gender and the harmonious (well-balanced) development coefficient. The Comparison Group is comprised of the medical records of the children also born within the normal term range, but with weight and height that correspond to the gestation age (fetal normosomia). To determine the average time of the first tooth eruption, as well as the deciduous teeth growth rate for each of the groups under the study, we have used the hypothesis about a linear dependence between the number of erupted teeth and the age of the child. The statistical data processing and verification of the consistency of this hypothesis is performed using the multiple linear regression analysis with the STATISTICA 6.0 software package (Multiple Regression module). The number of delayed eruption and premature eruption cases observed is calculated along with the corresponding confidence intervals for the significance level, p, of less than 0.05, taking into account the binomial distribution of the random variable. The results of the study indicate a slowed growth rate of deciduous teeth in children born with macrosomia, as well as an increased number of cases (by a factor of 2 to 4 times) of deviations in the timing of teeth eruption compared to regional norms. The smallest growth rate of deciduous teeth and the smallest number of teeth at the age of one year are registered in macrosomic boys and macrosomic girls with a long body and a relatively reduced birth weight, as well as in macrosomic girls with intrauterine obesity. The macrosomic girls with intrauterine acceleration with obesity at the background have the largest average tooth growth rate and the largest percentage of premature eruption cases among all subgroups. Conclusions. The somatometric features of fetal macrosomia suggest the influence on the number of teeth that erupt by a certain age. The data on the deviation from the generally accepted terms of teeth eruption in children born with macrosomia, can be the basis for developing new and improving existing prevention programs aimed at preserving dental health.

Gestační diabetes mellitus (GDM) patří mezi nejčastější interní komplikace v těhotenství a podobně jako se v celkové populaci zvyšuje incidence diabetu 2. typu, zvyšuje se i incidence GDM. Příčinou je pravděpodobně životní styl, vyšší incidence nadváhy a obezity a rovněž vyšší věk těhotných. Zatímco starší údaje v české literatuře udávají přítomnost GDM ve 3–4 % všech těhotenství, v současnosti je jeho výskyt při použití stejných diagnostických kritériích více než dvojnásobný. V případě, že ke stávajícím kritériím zahrneme i nepovinné stanovení glykémie v 60. minutě oGTT, záchyt GDM stoupá na více než 20 %. Pokud bychom přijali nová doporučení Mezinárodní asociace pro diabetes v těhotenství IADPSG (International Association of Diabetes and Pregnancy Study Groups), můžeme očekávat záchyt GDM pravděpodobně ještě vyšší. Doporučení IADPSG vzešlo z významné mezinárodní observační studie HAPO (Hyperglycemia and Adverse Pregnancy Outcomes), která na velkém souboru těhotných žen srovnávala vliv mírné hyperglykémie na rozvoj těhotenských a perinatálních komplikací. Přijetí těchto kritérií je předmětem živých diskusí v zahraniční odborné literatuře. V roce 2013 byla IADPSG kritéria akceptována WHO. Výbor České diabetologické společnosti (ČDS ČLS JEP) je přijal v dubnu 2014. Oficiální stanovisko ČGPS ČLS JEP zatím nebylo vydáno. Tento článek analyzuje stávající i nově navrhovaná diagnostická kritéria, shrnuje nejčastěji udávané argumenty pro a proti přijetí IADPSG kritérií a pokusí se otevřít diskusi k tomuto důležitému tématu. Gestational diabetes mellitus (GDM) is the most common internal complication in pregnancy and similarly as the frequency of Type 2 diabetes rises in general population so does the frequency of GDM. The causes include unhealthy life style, growing incidence of overweight and obesity as well as older age of pregnant women. Older data reported the frequency of GDM in 3–4% of all pregnancies in the Czech Republic, whereas nowadays its frequency with using the same diagnostic criteria is more than two times higher. If we include the measurement of glycemia in 1 hour of oGTT, the frequency of GDM reaches more than 20%. In case we accept the new recommendation of the International Association for Diabetes in Pregnancy Study Groups (IADPSG), it is expected that the frequency of GDM will be even higher. IADPSG recommendation resulted from HAPO study (Hyperglycemia and Adverse Pregnancy Outcomes), a large international observational study on the relationship between mild hyperglycemia and pregnancy outcomes. Adoption of these new criteria is intensively discussed among experts all over the world. WHO adopted the IADPSG criteria in 2013. The Czech Diabetes Society committee adopted them in April 2014. Any official statement about the new criteria has not yet been issued by the Czech Gynecology and Obstetrics Society. This article analyzes the old and the new diagnostic criteria, summarizes the most common arguments in favor and against the IADPSG thresholds and tries to open the discussion about this important topic.
