Mass of multiple myeloma aberrant cells represents at the time of diagnosis a mixture of multiple independent clones that vary in size with time. Population of aberrant cells of minimal residual disease (MRD) represents only a subset of clones from their complex pool, which survived the treatment. The reduction of the number of multiple myeloma clones is associated with decreased genetic variability. Decrease in intratumor heterogeneity after frontline therapy allows implementation of individualized therapy targeting residual clone. Genome wide sequencing combined with gene expression profiling of residual clone will enable to identify key set of mutations and/or affected pathways that can be eliminated by some of the novel targeted therapeutic agents.
Masa aberantních buněk mnohočetného myelomu je v době diagnózy složena ze směsi několika nezávislých klonů s proměnlivou velikostí v čase. Populace aberantních buněk minimální reziduální nemoci (MRN) představuje pouze podmnožinu klonů z původního množství, které přežily léčbu. Zmenšení počtu klonů mnohočetného myelomu je spojeno se snížením celkové genetické variability. Snížení heterogenity uvnitř nádoru po aplikaci primární terapie dovoluje zařazení individualizované léčby zacílené na zbytkový klon. Genomové sekvenování spolu s profilováním genové exprese ve zbytkovém klonu umožní identifikovat sadu klíčových mutací a/nebo poškozených metabolických drah, které mohou být odstraněny některou z nových cílených terapeutických látek.

Inhibitors of antiapoptotic proteins of the BCL2 family can successfully restart the deregulated process of apoptosis in malignant cells. Whereas nonselective agents have been limited by their affinity to different BCL2 members, thus inducing excessive toxicity, the highly selective BCL2 inhibitor venetoclax (ABT-199, Venclexta™) has an acceptable safety profile. To date, it has been approved in monotherapy for the treatment of relapsed or refractory chronic lymphocytic leukemia (CLL) with 17p deletion. Extension of indications can be expected in monotherapy and in combination regimens. Sensitivity to venetoclax is not common in lymphomas, but promising outcomes have been achieved in the mantle cell lymphoma group. Venetoclax is also active in multiple myeloma patients, especially in those with translocation t(11;14), even if high-risk features such as del17p are also present. Surprisingly, positive results are being obtained in elderly acute myeloid leukemia patients, in whom inhibition of BCL2 is able to substantially increase the efficacy of low-dose cytarabine or hypomethylating agents. Here, we provide a summary of available results from clinical trials and describe a specific mechanism of action that stands behind the efficacy of venetoclax in hematological malignancies.

Multiparameter flow cytometry (MFC) has become standard in the management of patients with plasma cell (PC) dyscrasias, and could be considered mandatory in specific areas of routine clinical practice. It plays a significant role during the differential diagnostic work-up because of its fast and conclusive readout of PC clonality, and simultaneously provides prognostic information in most monoclonal gammopathies. Recent advances in the treatment and outcomes of multiple myeloma led to the implementation of new response criteria, including minimal residual disease (MRD) status as one of the most relevant clinical endpoints with the potential to act as surrogate for survival. Recent technical progress led to the development of next-generation flow (NGF) cytometry that represents a validated, highly sensitive, cost-effective and widely available technique for standardized MRD evaluation, which also could be used for the detection of circulating tumor cells. Here we review current applications of MFC and NGF in most PC disorders including the less frequent solitary plasmocytoma, light-chain amyloidosis or Waldenström macroglobulinemia.

Východiska: Díky pokrokům v léčbě mnohočetného myelomu se značně zvýšil počet pacientů dosahujících remise onemocnění, a tedy detekce minimální reziduální choroby (minimal residual disease – MRD) se stala nepostradatelnou ke zhodnocení účinnosti léčby a k posouzení hloubky kompletní léčebné odpovědi. Multiparametrická průtoková cytometrie (multiparametric flow cytometry – MFC) je v současnosti nejpoužívanější metodou pro stanovení a monitorování přítomnosti MRD v kostní dřeni pacientů s mnohočetným myelomem, nicméně mohou být využity také metody na molekulární úrovni. Je zřejmé, že protokol použitý pro stanovení MFC-MRD může významně ovlivnit získané výsledky, přičemž standardizovaný a vysoce senzitivní přístup „next generation flow” je již dostupný. Přínos stanovení MRD pro predikci přežití bez progrese onemocnění a celkového přežití je znám, nicméně nedávné výzkumy ukazují, že MRD negativita dokonce překonává prognostickou hodnotu dosažení kompletní léčebné odpovědi pro přežití bez progrese a celkové přežití. Cíl: Souhrnný článek je zaměřen na využití MFC v analýze MRD u mnohočetného myelomu. Zmíněny jsou také technické aspekty a zhodnocení klinického přínosu tohoto stanovení. Závěr: Informace o hladině MRD stanovené pomocí vysoce senzitivní a reprodukovatelné MFC může být využita jako biomarker k hodnocení účinnosti rozdílných léčebných přístupů, k rozhodování o vhodnosti léčby, ale také může sloužit jako parametr nahrazující celkové přežití u pacientů s mnohočetným myelomem.
Background: Progress in treatment of multiple myeloma extensively increased patient remission rates, so minimal residual disease (MRD) detection becomes essential to assess the effectivity of treatment and depth of complete response. Nowadays, multiparametric flow cytometry (MFC) is the most used method for monitoring of MRD presence in the bone marrow of multiple myeloma patients; however, detection on molecular level can be used as well. It is evident that choice of protocol used for MFC-MRD assessment can significantly affect required results; nevertheless, standardized and highly sensitive approach of “next generation flow” is already available. Although benefit of MRD assessment as an independent predictor of progression-free survival and overall survival is known, very recent research showed that MRD-negative status surpasses the prognostic value of complete response achievement for progression-free survival and overall survival. Aim: This review is focused on use MFC in MRD assessment in multiple myeloma. The technical aspects and clinical benefits of this approach are mentioned as well. Conclusion: The information about MRD level detected by highly sensitive and reproducible MFC can be potentially used as a biomarker to evaluate the efficacy of different treatment strategies, help on treatment decisions and act as a surrogate for overall survival in multiple myeloma patients.

Mnohočetný myelom je dyskrazie plazmatických buněk. Je to druhá nejčastější hematologická malignita, která je charakterizována proliferací klonálních plazmatických buněk produkujících škodlivý monoklonální imunoglobulin. Navzdory možnostem léčby, která se v posledním desetiletí velmi vyvinula, se i u pacientů po terapii často nachází malé množství abnormálních reziduálních buněk, které mohou způsobit návrat onemocnění. Charakterizace reziduálních rezistentních klonů může pomoci odhalit důležité terapeutické cíle pro aplikaci účinné personalizované léčby. Pro tento výzkum používáme aberantní plazmatické buňky vysortované podle markerů CD38, CD45, CD56 a CD19, následované sekvenováním nové generace jejich exomu. Ze seznamu 213 genů, ve kterých byla přítomna alespoň jedna varianta, byl přítomen zajímavý gen NRAS, který je jedním z nejčastěji mutovaných genů u mnohočetného myelomu, homology genů z panelu 88 genů dříve použitých pro sekvenování nové generace u mnohočetného myelomu, mezi kterými byl i gen identifikovaný jako významný při rezistenci na bortezomib. Přes to, že jsou výsledky exomového sekvenování prvního pacienta zajímavé, jsou pouze předběžné a při jejich interpretaci je potřeba být obezřetný, neboť závisí na optimalizaci bioinformatické analýzy, která stále probíhá. Získané výsledky je také potřeba validovat Sangerovým sekvenováním. Brzy budou prezentovány definitivní výsledky podpořené větší kohortou pacientů.
Multiple myeloma is a plasma cell dyscrasia. It is the second most common hematological malignancy which is characterized by proliferation of clonal plasma cells producing harmful monoclonal immunoglobulin. Despite treatment modalities greatly evolved during the last decade, small amount of aberrant residual cells reside in patients after therapy and can cause relapse of the disease. Characterization of the residual, resistant clones can help to reveal important therapeutic targets for application of effective and precious treatment. We use CD38, CD45, CD56 and CD19 sorted aberrant plasma cells to perform next generation sequencing of their exome. Among the 213 genes in which at least one variant was present, the most interesting was found gene NRAS, one of the most often mutated gene in multiple myeloma, and homologs of 88 gene panel previously used for multiple myeloma sequencing among which was a gene previously identified as gene meaningful in bortezomib resistance. Nevertheless, the results of next generation exome sequencing need to be interpreted with caution, since they rely on bioinformatical analysis, which is still being optimized. The results of next generation sequencing will also have to be confirmed by Sanger sequencing. Final results supported by larger cohort of patients will be published soon.

The introduction of PD-1/PD-L1 pathway inhibitors is an important landmark in solid oncology with unprecedented practice-changing activity in various types of solid tumours. Among haematological malignancies, PD-1/PD-L1 inhibitors have been successful, so far, only in the treatment of classical Hodgkin lymphoma, which typically exhibits an over-expression of PD-1 ligands (PD-L1, PD-L2) due to alterations in chromosome 9p24.1. Such positive outcomes led to the US Food and Drug Administration approval of nivolumab use in relapsed Hodgkin lymphoma in 2016 as the first haematological indication. Although the results in other lymphoid malignancies have not been so striking, blockade of the PD-1/PD-L1 axis has led to meaningful responses in other lymphoma types such as diffuse large B-cell lymphoma, follicular lymphoma or several T-cell lymphomas. Monotherapy with PD-1/PD-L1 inhibitors in chronic lymphocytic leukaemia and multiple myeloma has been unsatisfactory, suggesting that a combinational approach with other synergistic drugs is needed. In the case of multiple myeloma, immunomodulatory agents together with corticosteroids represent the most promising combinations. Among myeloid malignancies, the anti-PD-1 monoclonal antibodies are examined dominantly in acute myeloid leukaemia and myelodysplastic syndromes in combination with potentially synergistic hypomethylating drugs such as 5-azacitidine, resulting in promising outcomes that warrant further investigation. We have described all available clinical results of PD-1/PD-L1 inhibitors in haematological malignancies and discussed related toxicities, as well as highlighted crucial preclinical studies in this review.

AIMS: Some types of monoclonal gammopathies are typified by a very limited availability of aberrant cells. Modern research use high throughput technologies and an integrated approach for detailed characterisation of abnormal cells. This strategy requires relatively high amounts of starting material which cannot be obtained from every diagnosis without causing inconvenience to the patient. The aim of this methodological paper is to reflect our long experience with laboratory work and describe the best protocols for sample collection, sorting and further preprocessing in terms of the available number of cells and intended downstream application in monoclonal gammopathies research. Potential pitfalls are also discussed. METHODS: Comparison and optimisation of freezing and sorting protocols for plasma cells in monoclonal gammopathies, followed by testing of various nucleic acid isolation and amplification techniques to establish a guideline for sample processing in haemato-oncology research. RESULTS: We show the average numbers of aberrant cells that can be obtained from various monoclonal gammopathies (monoclonal gammopathy of undetermined significance/light chain amyloidosis/multiple myeloma (MM)/MM circulating plasma cells/ minimal residual disease MM-10 123/22 846/305 501/68 641/4000 aberrant plasma cells of 48/30/10/16/37×106 bone marrow mononuclear cells) and the expected yield of nucleic acids provided from multiple isolation kits (DNA/RNA yield from 1 to 200×103 cells was 2.14-427/0.12-123 ng). CONCLUSIONS: Tested kits for parallel isolation deliver outputs comparable with kits specialised for just one type of molecule. We also present our positive experience with the whole genome amplification method, which can serve as a very powerful tool to gain complex information from a very small cell population.

Minimal residual disease (MRD) by multiparameter flow cytometry (MFC) is the most effective tool to define a deep response in multiple myeloma (MM). We conducted an MRD correlative study of the EMN02/HO95 MM phase III trial in newly diagnosed MM patients achieving a suspected complete response before maintenance and every 6 months during maintenance. Patients received high-dose melphalan (HDM) versus bortezomib-melphalan-prednisone (VMP) intensification, followed by bortezomib-lenalidomide-dexamethasone (VRd) versus no consolidation, and lenalidomide maintenance. Bone marrow (BM) samples were processed in three European laboratories, applying EuroFlow-based MFC protocols (eight colors, two tubes) with 10-4-10-5 sensitivity. At enrollment in the MRD correlative study, 76% (244/321) of patients were MRD-negative. In the intention-to-treat analysis, after a median follow-up of 75 months, 5-year progression-free survival was 66% in MRD-negative versus 31% in MRD-positive patients (HR 0.39; p < 0.001), 5-year overall survival was 86% versus 69%, respectively (HR 0.41; p < 0.001). MRD negativity was associated with reduced risk of progression or death in all subgroups, including ISS-III (HR 0.37) and high-risk fluorescence in situ hybridization (FISH) patients (HR 0.38;). In the 1-year maintenance MRD population, 42% of MRD-positive patients at pre-maintenance became MRD-negative after lenalidomide exposure. In conclusion, MRD by MFC is a strong prognostic factor. Lenalidomide maintenance further improved MRD-negativity rate.
