An ankle bone consisting of several fragments is a rare anatomical variant of the talus, in the literature described as talus partitus or frontal split. On radiographs or CT scans it presents as two or more relatively large fragments. The term talus partitus is sometimes incorrectly used for an accessory bone that occasionally develops behind the ankle bone and is called os trigonum. talus partitus is an unusual developmental anomaly in which the talus is partitioned although it ossifies from a single centre. The cause of its split has not been explained although this may also be due to a previous unrecognized injury. In the Slovak medical literature no report of this anomaly has been found. Here we resent the case of a 30-year-old man in whom the ankle bone split was an incidental finding. This patient also had ipsilateral developmental dysplasia of the hip. This is the first report of a patient with the two concurrently found anomalies.

UNLABELLED: Talus bipartitus is a rare skeletal variation. Several causative factors have been proposed, but none of them seem to be convincing. We hypothesize that talus bipartitus may result from ossification disruption in neonatal period caused possibly by an infection. The observations supporting this link are discussed. The hypothesis is supported by two cases with symptomatic talus bipartitus illustrating the postulated connection. KEY WORDS: ankle, pain etiology, talus abnormalities, talus bipartitus.

Avascular necrosis (AVN) of the talus in a posttraumatic setting describes a condition of temporary or permanent bone death of a different extent, initiated by a circulatory disturbance. It is estimated that about 75% of all talar AVNs are caused by a prior trauma. The incidence of posttraumatic AVN rises with higher energy of injury, severity of talar body or neck displacement, fracture comminution, and injury to the soft tissues and major vessels in the lower leg and ankle region. These conditions are often seen in open fractures, fracture dislocations, and pure dislocations.

The talus is the key articular segment linking the leg and foot, and as such, is subject to complex loads and may occasionally fracture. Fracture patterns provide clues to the underlying pathomechanics and energy of the injury, both of which can help guide treatment and suggest prognosis. Talus fractures have a wide variety of presentation from low-energy avulsion fractures of the lateral or posterior processes, to high-energy comminuted talar body fractures. Appropriate, expedient treatment provides the patient the best chance of obtaining a good functional outcome. Treatment relies on appropriate diagnosis, which hinges on clinical suspicion provided by the patient's account of pathomechanics, clinical examination, and radiological workup. This current concepts review discusses the pathomechanics, presentation, workup, treatment, and prognosis of fractures of the talar head, neck, body, lateral process, posterior process, and talar extrusions.
