Electronic Health Records (EHRs) are required for maintaining and using medical data. Many internationally well accepted EHR; EHR-S standards are available in the medical domain. There are various related health data standards as well. All these standards have specific advantages and disadvantages. Standard making process is an iterative process and the standards are still evolving. Though several important standards have been proposed during the last few decades, one major problem of these EHR standards is that they are not defined according to scope, scale and context. Computerized processes require specific definitions of medical terms and such terms are not very generic in respect of usage in different countries. There are also many aspects of evaluation of health data standards. In this paper, EHRs and other related standards, their usage, shortcomings etc. are studied in the context of data storage, data model and exchange of data formats. Suitability of the standards for smart environments is also explored with an objective to find a suitable presentation, organisation and storage of health data generated through smart health care applications.
The objective of this study is to provide an overview over the ethical issues relevant to the assessment, implementation, and use of smart-glasses. The purpose of the overview is to facilitate deliberation, decision making, and the formation of knowledge and norms for this emerging technology. An axiological question-based method for human cognitive enhancement including an extensive literature search on smart-glasses is used to identify relevant ethical issues. The search is supplemented with relevant ethical issues identified in the literature on human cognitive enhancement (in general) and in the study of the technical aspects of smart-glasses. Identified papers were subject to traditional content analysis: 739 references were identified of which 247 were regarded as relevant for full text examinations, and 155 were included in the study. A wide variety of ethical issues with smart-glasses have been identified, such as issues related to privacy, safety, justice, change in human agency, accountability, responsibility, social interaction, power and ideology. Smart-glasses are envisioned to change individual human identity and behavior as well as social interaction. Taking these issues into account appears to be relevant when developing, deliberating, deciding on, implementing, and using smart-glasses.
OBJECTIVES: Glucose is an important diagnostic biochemical marker of diabetes but also for organophosphates, carbamates, acetaminophens or salicylates poisoning. Hence, innovation of accurate and fast detection assay is still one of priorities in biomedical research. METHODS: Glucose sensor based on magnetic particles (MPs) with immobilized enzymes glucose oxidase (GOx) and horseradish peroxidase (HRP) was developed and the GOx catalyzed reaction was visualized by a smart-phone-integrated camera. RESULTS: Exponential decay concentration curve with correlation coefficient 0.997 and with limit of detection 0.4 mmol/l was achieved. Interfering and matrix substances were measured due to possibility of assay influencing and no effect of the tested substances was observed. Spiked plasma samples were also measured and no influence of plasma matrix on the assay was proved. CONCLUSIONS: The presented assay showed complying results with reference method (standard spectrophotometry based on enzymes glucose oxidase and peroxidase inside plastic cuvettes) with linear dependence and correlation coefficient 0.999 in concentration range between 0 and 4 mmol/l. On the grounds of measured results, method was considered as highly specific, accurate and fast assay for detection of glucose.

BACKGROUND: Despite the ever-increasing offering of SMART technologies (ie, computer-controlled devices acting intelligently and capable of monitoring, analyzing or reporting), a wide gap exists between the development of new technological innovations and their adoption in everyday care for older adults. OBJECTIVE: This study aims to explore the barriers and concerns related to the adoption of SMART technologies among different groups of stakeholders. METHODS: Data from 4 sources were used: semistructured in-person or internet-based interviews with professional caregivers (n=12), structured email interviews with experts in the area of aging (n=9), a web-based survey of older adults (>55 years) attending the Virtual University of the Third Age (n=369), and a case study on the adoption of new technology by an older adult care facility. RESULTS: Although all stakeholders noted the potential of SMART technologies to improve older adult care, multiple barriers to their adoption were identified. Caregivers perceived older adults as disinterested or incompetent in using technology, reported preferring known strategies over new technologies, and noted own fears of using technology. Experts viewed technologies as essential but expressed concerns about cost, low digital competency of older adults, and lack of support or willingness to implement technologies in older adult care. Older adults reported few concerns overall, but among the mentioned concerns were lack of ability or interest, misuse of data, and limited usefulness (in specific subgroups or situations). In addition, older adults' ratings of the usefulness of different technologies correlated with their self-rating of digital competency (r=0.258; P<.001). CONCLUSIONS: Older adults appeared to have more positive views of various technologies than professional caregivers; however, their concerns varied by the type of technology. Lack of competence and lack of support were among the common themes, suggesting that educationally oriented programs for both older adults and their caregivers should be pursued.

Posturální stabilita stoje je v klinické praxi nejčastěji testována pomocí technik využívající jednoduché funkční testy nebo pomocí komplexnějších škál a baterií, které hodnotí nejen rovnováhu stoje, ale jsou často spojovány s dalšími funkčními testy stoje, změn poloh a chůze. Pro objektivní přístrojové hodnocení posturální stability jsou nejčastěji využívány silové plošiny, které slouží k měření reakčních sil působících od podložky, anebo tlakové plošiny určené pro měření tlaku způsobeného kontaktu nohou s podložkou. V článku je popisováno využití systému hodnotícího dynamickou posturální stabilitu pomocí dynamické počítačové posturografie NeuroCom SMART EquiTest System, který je nejčastěji využíván pro hodnocení klinického stavu pacientů s poruchami rovnováhy. Ze zkušeností získaných při řešení studií využívající výše uvedený systém vyplývá, že číselné hodnoty normovaných dat uváděných výrobcem Natus Medical Incorporated nejsou, s ohledem na jejich nastavení k diagnostice poruch rovnováhy, dostatečně citlivé pro hodnocení parametrů dynamické posturální stabilizace běžné mladé populace. Studie je proto věnována prezentaci zjištěných referenčních hodnot vybraných parametrů dynamické posturální stabilizace přístroje NeuroCom SMART EquiTest System pro věkovou skupinu mezi 20-30 lety. U záměrně vybrané skupiny 50 zdravých probandů (muži n=25, ženy n=25) ve věku 20-30 let (±25,22) byly za shodně dodržeých podmínek testování naměřeny parametry testovacích protokolů, nabízených NeuroCom SMART Equitest System: Sensory Organization Testu (SOT), Motor Control Testu (MCT) a Adaptation Testu (ADT). Výsledky byly vyhodnoceny stejnou metodikou, jakou byly získány normativní hodnoty publikované v manuálech testovacího systému. Získaná výsledná data některých parametrů popsaných testů jsou prezentována jako referenční hodnoty (REF 20-30) pro skupinu mladých zdravých jedinců ve věku 20-30 let žijících v ČR a jeví se jako výrazně citlivější pro hodnocení dynamické posturální stability běžné mladé a aktivně sportující populace.
In clinical practice, postural stability of standing position is most frequently tested by basic techniques using simple functional tests or by means of more complex scales and batteries, which do not only evaluate balance in the standing position, but at also associated with other functional tests in the standing position, changes in position and walking. For the objective evaluation of postural stability, the most frequently used are force workout platforms, which serve for measuring of reaction forces acting from the platform, or pressure platforms meant for measuring pressure caused by the contact of feet with the platform. The paper describes the use of the system evaluating dynamic postural stability by means of the dynamic computing posturography NeuroCom SMART EquiTest System, which is most frequently used for evaluating clinical conditions of the patients with balance disorders. From the experience obtained in processing the studies using the above-mentioned system in came out that numeric values of and standardized data, presented by the produced Natus Medical Incorporated, considering their adjustment are not for the diagnostics of balance disorders, sufficiently sensitive form evaluating parameters of dynamic postural stabilization in a common young population. The study is therefore devoted for presenting the determined reference values of selected parameters of dynamic postural stabilization of NeuroCom SMART EquiTest System for the age group between 20 – 30 years of age. In the purposely selected group of 50 healthy probands (25 men and 25 women) at the age of 20 to 30 years (25±22), the equally maintained testing conditions provided parameters of testing protocols offered by NeuroCom SMART Equitest System: Sensory Organization Test (SOT), Motor Control Test (MCT) and the Adaptation Test (ADT). The results were evaluated by the same methodology, as the normative values published in manuals of the testing system. The resulting data of several parameters of the described tests obtained, resented as reference values (REF 20-30) for the group of young healthy individuals at the age of 20-30 years, who live in the Czech Republic and appear be markedly more sensitive for evaluation dynamic postural stability of a common population actively performing sports.

BACKGROUND: The clinical effects of smartphone-based interventions for bipolar disorder (BD) have yet to be established. OBJECTIVES: To examine the efficacy of smartphone-based interventions in BD and how the included studies reported user-engagement indicators. METHODS: We conducted a systematic search on January 24, 2022, in PubMed, Scopus, Embase, APA PsycINFO, and Web of Science. We used random-effects meta-analysis to calculate the standardized difference (Hedges' g) in pre-post change scores between smartphone intervention and control conditions. The study was pre-registered with PROSPERO (CRD42021226668). RESULTS: The literature search identified 6034 studies. Thirteen articles fulfilled the selection criteria. We included seven RCTs and performed meta-analyses comparing the pre-post change in depressive and (hypo)manic symptom severity, functioning, quality of life, and perceived stress between smartphone interventions and control conditions. There was significant heterogeneity among studies and no meta-analysis reached statistical significance. Results were also inconclusive regarding affective relapses and psychiatric readmissions. All studies reported positive user-engagement indicators. CONCLUSION: We did not find evidence to support that smartphone interventions may reduce the severity of depressive or manic symptoms in BD. The high heterogeneity of studies supports the need for expert consensus to establish ideally how studies should be designed and the use of more sensitive outcomes, such as affective relapses and psychiatric hospitalizations, as well as the quantification of mood instability. The ISBD Big Data Task Force provides preliminary recommendations to reduce the heterogeneity and achieve more valid evidence in the field.

OBJECTIVES: There are many mobile health applications (apps) now available and some that use in some way laboratory medicine data. Among them, patient-oriented are of the lowest content quality. The aim of this study was to compare the opinions of non-laboratory medicine professionals (NLMP) with those of laboratory medicine specialists (LMS) and define the benchmarks for quality assessment of laboratory medicine apps. METHODS: Twenty-five volunteers from six European countries evaluated 16 selected patient-oriented apps. Participants were 20-60 years old, 44% were females, with different educational degrees, and no professional involvement in laboratory medicine. Each participant completed a questionnaire based on the Mobile Application Rating Scale (MARS) and the System Usability Scale, as previously used for rating the app quality by LMS. The responses from the two groups were compared using the Mann-Whitney U test and Spearman correlation. RESULTS: The median total score of NLMP app evaluation was 2.73 out of 5 (IQR 0.95) compared to 3.78 (IQR 1.05) by the LMS. All scores were statistically significantly lower in the NLMP group (p<0.05), except for the item Information quality (p=0.1631). The suggested benchmarks for a useful appear: increasing awareness of the importance and delivering an understanding of persons' own laboratory test results; understandable terminology; easy to use; appropriate graphic design, and trustworthy information. CONCLUSIONS: NLMP' evaluation confirmed the low utility of currently available laboratory medicine apps. A reliable app should contain trustworthy and understandable information. The appearance of an app should be fit for purpose and easy to use.

Cíl studie: Zjistit, kolik pacientů podstupujících předanestetické vyšetření (PAV) má chytrý telefon a jaký je podíl pacientů ochotných podstoupit PAV nebo jeho část s využitím telefonu nebo jiné formy elektronické komunikace. Typ studie: Observační prospektivní multicentrická cross-sectional. Typ pracoviště: Anesteziologické ambulance pěti fakultních/krajských nemocnic. Materiál a metoda: Do studie byli zařazováni pacienti podstupující PAV před plánovaným operačním výkonem s požadavkem anesteziologické péče. U každého pacienta byly dotazníkovou formou zaznamenány údaje: věk, pohlaví, nejvyšší dosažené vzdělání a odpověď na otázky – „používáte internet?“, „máte přístup na e-mail?“, „máte chytrý telefon?“, „pokud by byla možnost provedení PAV přes internet a nemusel jste jít kvůli tomu do zdravotnického zařízení, využil/a byste dané možnosti?“. Byly použity deskriptivní statistické metody. Výsledky: Byla analyzována data od 1685 pacientů. Vlastnictví chytrého telefonu udáválo 957 pacientů (57 %), dostupnost internetu 1330 pacientů (79 %), vlastnictví e-mailového účtu 1260 pacientů (75 %). Ochotu absolvovat PAV s využitím internetu udávalo 536 pacientů (32 %). Ochota k provedení PAV přes internet byla významně vyšší u pacientů s VŠ vzděláním (164 z 343, 48 %). Závěr: Vybavenost mobilními technologiemi a přístup k internetu byly zjištěny u více než poloviny dotazovaných pacientů. Ochota k PAV bez osobního kontaktu s lékařem byla vyšší u pacientů s vysokoškolským vzděláním.
Objective: The aim of our study was to find out whether patients scheduled for elective surgery owned a smartphone, had access to internet/e-mail and whether they would be willing to undergo preoperative assessment (POA) using internet/mobile devices. Design: Observational, prospective, multicentre, cross-sectional study. Setting: Preoperative clinics at tertiary care hospitals in the Czech Republic. Materials and methods: The study was based on a simple questionnaire which the patients had to answer at the end of their POA appointment. The recorded data included age, gender, level of education, ownership of a smartphone, access to internet/e-mail and the patients' willingness to undergo remote POA. The obtained data were analysed using descriptive statistical methods. Results: We analysed data from 1685 patients. Total 957 (57%) of them owned a smartphone, 1330 (79%) had access to the internet and 1260 (75%) used e-mail. Total 536 patients (32%) were willing to undergo remote POA. Patients with a university degree (164 of 343, 48%) were more likely to be willing to have remote POA. Conclusion: More than half of the patients undergoing POA owned a smartphone, had internet connection and used e-mail. Overall one third of the patients expressed their willingness to undergo remote POA; this proportion was higher in patents with a degree.
Dynamický počítačový posturograf NeuroCom SMART EquiTest System umožňuje kromě analýzy dynamické posturální stability také trénink posturální stability s využitím vizuálního biofeedbacku na stabilní či nestabilní ploše a ve stabilním nebo nestabilním prostředí (22). Systém nabízí řadu možností tréninku, jejichž cílem je zlepšit dynamickou rovnováhu a mobilitu (8). V článku je popsána případová studie, která pilotně sledovala klinický efekt individuálně sestaveného pětitýdenního tréninkového programu (s frekvencí cvičení 2x týdně) využívající počítačovou dynamometrii u pacienta po unilaterální transfemorální amputaci dolní končetiny. Vyšetření hodnotící dynamickou posturální stabilitu s využitím „core assesments“ - Sensory Organization Testu (SOT), Motor Control Testu (MCT) a testu Limits of Stability (LOS), stejně jako všechna další vyšetření, byla provedena před a po intervenci. Schopnost funkční mobility a rovnováhy byla testována pomocí funkčního testu Timed Up and Go (TUG). Četnost pádů byla zjištěna v rámci odebrání anamnézy pacienta. Subjektivní pocit rovnováhy byl zjišťován pomocí dotazníku Activities Specific Balance Confidence Scale (ABC). Výsledky intervence byly kvalitativně posuzovány a byla stanovena i klinická významnost testů. Výsledky ukázaly, že tréninkový program, využívající vizuální biofeedback, měl u prezentovaného pacienta vliv na zlepšení posturálních funkcí a na snížení četnosti pádů. Ke klinicky významnému zlepšení nedošlo u schopnosti funkční mobility a rovnováhy měřené pomocí Timed Up and Go testu, ale u subjektivního pocitu rovnováhy, měřeného pomocí Activities Specific Balance Confidence Scale. Ze získaných výsledků lze usuzovat, že se NeuroCom SMART EquiTest Systém, využívaný pro trénink posturálních funkcí, se ukazuje jako slibná, systematická a objektivní metoda, která může vést ke snížení rizika pádů a zlepšení ve funkčních rovnovážných aktivitách prováděných při stoji na zemi u pacientů s amputacemi.
The Dynamic computing posturograph NeuroCom SMART EquiTest System enables, in addition to analysis of dynamic postural stability, also the training of postural stability with using visual biofeedback for stable or unstable plane and in stable or unstable environment (22). The system offers various possibilities of training aimed at improvement of dynamic balance and mobility and resembling functional activities, which may occur during everyday life (8). The article described the case study following clinical effects of individually adapted five-week training program (with the exercise frequency twice weekly), using computing dynamometry in a patient after unilateral transfemoral amputation of lower extremity. The examination assessing dynamic postural stability used the core assessments – Sensory Organization Test (SOT), Motor Control Test (MCT) and the Limits of Stability Test (LOS), as well as all other examinations before and after the intervention. The ability of functional mobility and balance was tested by means of the functional test Timed Up and Go (TUG). The frequency of falling down was detected within the framework of taking anamnesis of the patient. The subjective feeling of balance was determined by the Activities Specific Balance Confidence Scale (ABC) questionnaire. The results of intervention were qualitatively evaluated and clinical important of the tests was established. The results made it clear that the training program, using visual feedback, favorably influenced postural functions and decreased the frequency of falling down. There was not any clinical improvement in a capability of functional mobility and balance measured by means of the Timed Up and Go Test, but in the subjective feeling of balance measured by Activities Specific Balance Confidence Scale. The results obtained make it possible to conclude that NeuroCom SMART EquiTest System, used in training of postural functions seems to be a promising, systemic and objective method, which may decrease the risk of falling down and improve functional balance activities performed in standing position on the ground in patients with amputations.
