Kanagliflozin a jeho postavení v léčbě diabetů 2. typu

KanagLiflozin je inhibitor sodíko-giukózového kotransportéru 2 (sodium glucose co-transporter 2, SGLT2), s mírným inhibičním vlivem také na SGLT1. Primárně byl vyvinut k léčbě diabetes meLLitus 2. typu a podobně jako další Látky z této skupiny vede prostřednictvím inhibice SGLT2 ke zvýšení vylučování glukózy močí a tím ke zlepšení kompenzace diabetu, poklesu krevního tlaku a snížení hmotnosti. Provedené studie však prokázaly také jeho významný přínos při snížení kardiovaskulárních komplikací, zpomalení progrese diabetického onemocnění Ledvin a prevence a zlepšení srdečního selhání. Na rozdíl od dalších inhibitorů SGLT2 je v případě kanagLiflozinu možná titrace dávky ze 100 mg na 300 mg s dalším zvýšením antidiabetické účinnosti. V tomto čLánku shrnujeme jednak mechanismus působení kanagLiflozinu a jeho farmakokinetiku a Lékové interakce, jednak specifické vLastnosti pLynoucí z jeho částečné inhibice SGLT1. Podrobně také diskutujeme účinky zvýšení dávky kanagLiflizinu na 300 mg, které přináší daLší zLepšení kompenzace diabetu i snížení hmotnosti a krevního tLaku. KanagLiflozin představuje zajímavou možnost Léčby diabetu 2. typu s ceLou řadou daLších benefitů nad rámec zLepšení kompenzace diabetu.
Canagliflozin is an inhibitor of sodium-glucose cotransporter 2 (SGLT2), with additional mild inhibitory effect on SGLT1. It was primarily developed for the treatment of type 2 diabetes and, like other agents in this group, it increases urinary glucose excretion through SGLT2 inhibition and improves glucose control, decreases blood pressure and body weight. Several trials have also shown its significant benefit in reducing cardiovascular complications, slowing the progression of diabetic kidney disease and preventing and improving heart failure. In contrast to other SGLT2 inhibitors, dose titration from 100 mg to 300 mg is possible with canagliflozin with a further increase in its antidiabetic efficacy. In this article, we summarize both the mechanism of action of canagliflozin and its pharmacokinetics and drug interactions, and the specific properties resulting from its partial inhibition of the SGLT1 transporter. We also discuss in detail the effects of increasing the dose of canagliflozin to 300 mg, which yields further improvements in diabetes compensation as well as weight and blood pressure reduction. Canagliflozin represents an interesting treatment option for type 2 diabetes with a number of additional benefits beyond improving glucose control.
