Článek pojednává o opětovném nasazení klozapinu poté, co byl z nějakého důvodu (většinou kvůli nežádoucím účinkům) vysazen. Zvlášť v případě vysazení kvůli závažným až potenciálně život ohrožujícím nežádoucím účinkům je opětovné nasazení klozapinu téma kontroverzní. Kdy je znovunasazení možné a jaký by měl být postup? Snažíme se přinést přehled zásadních informací, které jsou dostupné zejména z posledních let v literatuře nebo v terapeutických doporučeních.
The article deals with the reinitiation of clozapine treatment after it was discontinued for a reason (mostly because of side effects). Reinitiation is a controversial topic, especially after severe to life-threatening side effects. When is the reiniation possible and how should this be done? We try to provide a summary of relevant information which is available in literature of recent years or in therapeutic guidelines.

Článek shrnuje některé nežádoucí účinky při léčbě antipsychotiky, které nejsou v praxi časté, ačkoliv jde o léčbu běžně užívanými antipsychotiky. Výběr z literatury je doplněn kazuistikami pacientů z naší praxe. Zaměřili jsme se na trombózy a anticholinergní deliria a doplnili ostatními. Příčiny a okolnosti rozvoje vedlejších účinků jsou doplněny klinickým managementem těchto situací.
The article summarizes some of the side effects of antipsychotics that are not common in everyday clinical practice, although the patients were treated by commonly used antipsychotics. The information from literature is supplemented by cases of the patients treated by us. We focused on thromboses and anticholinergic deliria, but some other side effects are mentioned as well. The causes, circumstances of initiation and clinical management of these side effects are disscused.

Introduction: Therapeutic drug monitoring (TDM) of clozapine is a very useful method for verifying both the correct intake and the interindividual variability of its metabolism, thereby avoiding the risk of toxicity. The purposes of this paper were to discover how many patients using clozapine in common clinical practice have clozapine plasma concentration (PC) levels in the proposed reference range and to identify factors that influence clozapine PC levels. Methods: Our study included 100 inpatients (diagnosed with schizophrenia or schizoaffective disorder) taking standard doses of clozapine (100-700 mg/day). Clozapine concentration was measured by high-performance liquid chromatography. Correlations between doses and PC levels and the influence of smoking and gender on clozapine PC levels were calculated. Results: A large number of the patients (67%) had PC levels outside the proposed reference range. The clozapine PC levels were influenced by dose, gender, and cigarette smoking. Conclusion: The correlations between dose, gender, and cigarette smoking and clozapine PC levels highlighted by our study overlap other research. It was surprising to find such a large number of patients with clozapine PC levels outside the therapeutic range. This result suggests the importance of clozapine TDM due to misunderstood inter- and/or intraindividual variability or misestimated partial therapeutic compliance.

Článek shrnuje dosavadní zásadní literaturu a doporučení týkající se monitorování plazmatických koncentrací (therapeutic drug monitoring = TDM) antipsychotik, se zaměřením na olanzapin a klozapin. Vysvětluje pojem TDM, referenční rozmezí, shrnuje indikace k provádění TDM. Zabývá se faktory které mohou plazmatické koncentrace (PC = plasmatic concetration) ovlivnit, a naopak faktory, které mohou být PC ovlivněny. Uvádí, kde a jak se dá v České republice TDM provádět.
The article summarizes existing basic literature and recommendations concerning therapeutic drug monitoring (TDM) of antipsychotics. It is focused on olanzapine and clozapine. Moreover, the terms "therapeutic drug monitoring" and "therapeutic reference range" are explained and indications for TDM are summarized. The factors that can influence plasma levels and that can be influenced by plasma levels are also mentioned. The article reports, where and how TDM can be done in the Czech Republic.
