Riziko lékových interakcí je u antiepileptik vysoké. Výsledek může být buď prospěšný, kdy dojde ke zvýšení terapeutického účinku, nebo škodlivý, jestliže se zvýší riziko nežádoucího účinku nebo se sníží terapeutický efekt léčiva. Skutečný vliv lékové interakce je velice obížné predikovat vzhledem k významné interindividuální variabilitě mezi pacienty. Terapeutické monitorování léčiv je metoda používaná jako nástroj k dosažení optimálního dávkovacího režimu u konkrétního pacienta s epilepsií.
The propensity of drug-drug interactions in antiepileptic drugs is high. The outcome either may be beneficial, if the therapeutic potency of the drug is enhanced, or harmful if the interaction causes an increase in the adverse effects of the drug or if a reduction in efficacy occurs. It is difficult to predict the full extent of drug interactions because of the large interindividual variation between patients. Therapeutic drug monitoring has been used as a tool to optimize therapy regimens for individual patient with epilepsy.
Mono-methylindoles (MMI) were described as agonists and/or antagonists of the human aryl hydrocarbon receptor (AhR). Here, we investigated the effects of MMI on AhR-CYP1A pathway in human hepatocytes and HepaRG cells derived from human progenitor hepatic cells. All MMI, except of 2-methylindole, strongly induced CYP1A1 and CYP1A2 mRNAs in HepaRG cells. Induction of CYP1A genes was absent in AhR-knock-out HepaRG cells. Consistently, CYP1A1 and CYP1A2 mRNAs and proteins were induced by all MMIs (except 2-methylindole), in human hepatocytes. The enzyme activity of CYP1A1 was inhibited by MMIs in human hepatocytes and LS180 colon cancer cells in a concentration-dependent manner (IC50 values from 1.2 μM to 23.8 μM and from 3.4 μM to 11.4 μM, respectively). Inhibition of CYP1A1 activity by MMI in human liver microsomes was much weaker as compared to that in intact cells. Incubation of parental MMI with human hepatocytes either diminished (4-methylindole, 6-methylindole) or enhanced (7-methylindole) their agonist effects on AhR in AZ-AHR reporter cells. In conclusion, overall effects of MMI on AhR-CYP1A pathway in human cells comprise the induction of CYP1A genes through AhR, the inhibition of CYP1A catalytic activity and possibly the metabolic transformation causing loss or gain of AhR agonist activity of parental compounds.

Apigenin is a natural flavonoid compound that has antioxidative, anti-inflammatory, and hepatoprotective effects, but the underlying mechanisms are still unclear. In this study, the effects of apigenin on ethanol-induced oxidative stress and lipopolysaccharide (LPS)-induced inflammatory cytokine production were examined in cultured rat hepatocytes. Following pretreatment of ethanol-stimulated hepatocytes with apigenin 6–24 mM for 2 h, the levels of cytochrome P450 2E1 (CYP2E1) protein expression and supernatant alanine aminotransferase and malondialdehyde were reduced (P < 0.05 or P < 0.01), while the activities of glutathione reductase and glutathione peroxidase were increased (P < 0.05 or P < 0.01). Likewise, the pretreatment of LPS-stimulated hepatocytes with the same concentrations of apigenin could decrease the levels of nuclear factor-κB protein expression and supernatant tumor necrosis factor-α and interleukin-6 (P < 0.05 or P < 0.01), and increase the level of IκB-α protein expression (P < 0.05 or P < 0.01). In all of these results, the concentration of 24 μM was the most effective. These findings demonstrate that apigenin may exert an inhibitory effect on ethanol-induced oxidative stress and LPS-induced inflammation in the cultured hepatocytes, and its mechanisms may be related to the reduction of CYP2E1 expression, increment of antioxidative ability, and regulation of inflammatory gene expression.

Mezi inhibitory receptoru pro epidermální růstový faktor (epidermal growth factor receptor, EGFR) používané při terapii pacientů s pokročilým nemalobuněčným karcinomem plic patří v České republice registrované gefitinib, erlotinib, afatinib a osimertinib. Tato léčiva mohou vyvolávat řadu lékových interakcí různého klinického významu. Rozsah a závažnost lékových interakcí jsou dány vlastnostmi jednotlivých léčiv z hlediska jejich biotransformace, transportu a rozpustnosti ve vodě při pH vyšším než 6. S pomocí údajů obsažených v Databázi lékových interakcí DrugAgency a dalších informací publikovaných v odborné literatuře v letech 2002–2017 bylo prokázáno, že nejvyšší interakční potenciál, tj. schopnost vyvolávat klinicky významné lékové interakce, má erlotinib, mírně menší interakční potenciál pak gefitinib a významně nižší interakční potenciál má osimertinib a zejména afatinib. Ten také lze z hlediska lékových interakcí považovat za nejbezpečnější inhibitor EGFR.
Inhibitors of epidermal growth factor receptor used in the therapy of patients with advanced non‑small‑cell lung carcinoma include gefitinib, erlotinib, afatinib and osimertinib approved in the Czech Republic. These drugs can cause a range of drug interactions of variable clinical significance. The extent and severity of drug interactions are defined by characteristics of the individual drugs, regarding their biotransformation, transport and solubility in water while pH is higher than 6. Based on the data in the drug interaction database DrugAgency and further information published in medical literature in 2002–2017, it has been confirmed that the highest interaction potential, i.e., the ability to cause clinically relevant drug interactions, is associated with erlotinib and gefitinib has slightly lower interaction potential whereas osimertinib and particularly afatinib show significantly lower interaction potential. With respect to drug interactions, afatinib can be considered as the safest EGFR inhibitor.

Aryl hydrocarbon receptor (AhR) is a transcription factor, the activity of which is modulated by hormones including glucocorticoids and estrogens. In this study, we examined the effects of triiodothyronine (T3), a ligand and activator of thyroid hormone receptor (TR), on transcriptional activity of AhR and the expression of its target gene CYP1A1. Study was carried out in human hepatocellular carcinoma cells HepG2 and primary cultures of human hepatocytes (HH). Gene reporter assay in stably transfected AZ-AhR cells revealed that T3 dose-dependently augmented 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD)-inducible AhR-dependent luciferase activity. In contrast, T3 had no effect on TCDD-inducible expression of CYP1A1 mRNA, protein and catalytic activity. Incubation of human hepatocytes with T3 had modulatory and inter-individual (7 cell cultures from 7 different liver donors) effects on both basal and dioxin-inducible CYP1A1/2. Since there was no correlation between T3 effects on CYP1A expression and T3-dependent expression of Spot14 mRNA, the involvement of additional factors besides TR is supposed. Overall, the co-incubation of normal and cancer human hepatic cells with TCDD and T3 suggested transcriptional cross-talk between AhR and TR, which may have physiological and toxicological implications.

Natural polyphenol resveratrol (trihydroxystilbene) is a partial agonist of human aryl hydrocarbon receptor AhR, thereby, displaying a plethora of biological effects. Biological activities of metoxylated and hydroxylated stilbenes were studied in the past. The aim of the current study was to describe the effects of 13 different hydroxy- and methoxystilbenes, including their cis/trans isomers on the transcriptional activity of AhR and the expression of CYP1A genes in hepatic cancer cells HepG2 and in primary human hepatocytes. Techniques of gene reporter assays, qRT-PCR, Simple Western blotting by Sally Sue™ and electrophoretic mobility shift assay EMSA were employed. All compounds activated AhR, but their efficacies, potencies and dose-response profiles differed substantially. The strongest activators of AhR and inducers of CYP1A1 in HepG2 cells were DMU-212 ((E)-3,4,5,4´-tetramethoxystilbene), trans-piceatannol, cis-piceatannol, trans-trismethoxyresveratrol and trans-pinostilbene. While DMU-212 and trans-trismethoxyresveratrol also induced CYP1A1 and CYP1A2 in primary human hepatocytes, the effects of trans-piceatannol, cis-piceatannol and trans-pinostilbene weaned off. On the other hand, trans-4-methoxystilbene was strong CYP1A inducer in hepatocytes but not in HepG2 cells. Differences between effects of stilbenes in HepG2 cells and human hepatocytes are probably due to the extensive phase I and phase II xenobiotic metabolism in human hepatocytes. The data obtained may be of toxicological relevance.

Článek shrnuje dosavadní zásadní literaturu a doporučení týkající se monitorování plazmatických koncentrací (therapeutic drug monitoring = TDM) antipsychotik, se zaměřením na olanzapin a klozapin. Vysvětluje pojem TDM, referenční rozmezí, shrnuje indikace k provádění TDM. Zabývá se faktory které mohou plazmatické koncentrace (PC = plasmatic concetration) ovlivnit, a naopak faktory, které mohou být PC ovlivněny. Uvádí, kde a jak se dá v České republice TDM provádět.
The article summarizes existing basic literature and recommendations concerning therapeutic drug monitoring (TDM) of antipsychotics. It is focused on olanzapine and clozapine. Moreover, the terms "therapeutic drug monitoring" and "therapeutic reference range" are explained and indications for TDM are summarized. The factors that can influence plasma levels and that can be influenced by plasma levels are also mentioned. The article reports, where and how TDM can be done in the Czech Republic.
