Autozomálně dominantní polycystická choroba ledvin je geneticky podmíněné onemocnění, které u asi poloviny pacientů vede k chronickému renálnímu selhání. Jedná se o multisystémové onemocnění s predominancí postižení ledvin a výrazně zhoršující zdravotní stav nemocného. Kauzální terapie není známa. Mezi renální komplikace patří infekce močových cest, nefrolitiáza, hematurie, krvácení do cyst a další. Léčba ve fázi chronického renálního selhání zahrnuje náhradu funkce ledvin - hemodialýzu/peritoneální dialýzu a transplantaci ledviny. Kontroverzní otázkou je indikace, načasování a technika nefrektomie nativních polycystických ledvin. Vždy je třeba postupovat individuálně, žádný jednoznačný konsensus není. Rutinně se provádět nedoporučuje. Má také své chirurgické aspekty a implikace. K jejímu provedení se doporučují symptomatické ledviny, asymptomatické při potřebě získání místa k transplantaci ledviny a ledviny, kde je podezření na tumor. V tomto přehledu vyzdvihujeme důležité aspekty tohoto onemocnění v urologii.
Autosomal dominant polycystic kidney disease is a genetic disease that leads to chronic renal failure in about half of patients. It is a multisystem disease with a predominance of kidney affection and significantly worsening the patient's health. Causal therapy is not known. Renal complications include urinary tract infections, nephrolithiasis, hematuria, cyst bleeding and others. Treatment in chronic renal failure includes renal replacement therapy - hemodialysis/peritoneal dialysis and kidney transplantation. Indication, timing and technique of nephrectomy of native polycystic kidneys remains controversial. It is necessary to proceed individually, there is no clear consensus. It is not recommended to perform nephrectomy routinely. It also has its surgical aspects and implications. Native nephrectomy is recommended in symptomatic kidneys, in asymptomatic kidneys when it is necessary to obtain a place for kidney transplantation and in kidneys where a tumor is suspected. This review focuses on important aspects of ADPKD for urologists.

Kontroverzní otázkou je indikace, načasování a technika nefrektomie nativních polycystických ledvin. Metody: Retrospektivní observační studie zaměřená na chirurgické aspekty pacientů s ADPKD, kteří podstoupili na našem pracovišti nativní nefrektomii. Do souboru byli zařazeni pacienti operovaní v období 1. 1. 2000 – 31. 12. 2020. Celkem bylo zařazeno 115 pacientů s ADPKD (14,7 % ze všech transplantovaných). V souboru jsme u pacientů zhodnotili základní demografická data, typ operačního výkonu, indikace, komplikace. Výsledky: U 68 pacientů z celkových 115 (59 %) byla provedena nativní nefrektomie. U 22 pacientů (32 %) z nich byla provedena unilaterálně, u 46 (68 %) bilaterálně. Nejčastější indikace byly infekce (42 pacientů, 36 %), bolesti (31 pacientů, 27 %), hematurie (14 pacientů, 12 %), gastrointestinální důvody (1 pacient, 1 %), respirační důvody (1 pacient, 1 %), získání místa pro transplantaci (17 pacientů, 15 %) a podezření na tumor (5 pacientů, 4 %). Závěr: K provedení nativní nefrektomie se doporučují symptomatické ledviny, asymptomatické při potřebě získání místa k transplantaci ledviny a ledviny, kde je podezření na tumor.
Introduction: Autosomal dominant polycystic kidney disease (ADPKD) is a genetic disease that leads to chronic renal failure in about half of patients. It is a multisystemic disease with a predominance of kidney involvement, which significantly worsens the patient’s health. Controversial issues include the indication and the timing and technique of nephrectomy of native polycystic kidneys. Methods: A retrospective observational study focused on the surgical aspects of patients with ADPKD who underwent native nephrectomy at our institution. The group included patients operated on in the period 1/1/2000–31/12/2020. A total of 115 patients with ADPKD were enrolled (14.7% of all transplant recipients). We evaluated the basic demographic data, type of surgery, indications and complications in this group. Results: Native nephrectomy was performed in 68 out of a total of 115 (59%) patients. Unilateral nephrectomy was done in 22 (32%) patients and bilateral in 46 (68%). The most common indications were infections (42 patients, 36%), pain (31 patients, 27%), hematuria (14 patients, 12%), gastrointestinal reasons (1 patient, 1%), respiratory reasons (1 patient, 1%), obtaining a site for transplantation (17 patients, 15%) and suspected tumor (5 patients, 4%). Conclusion: Native nephrectomy is recommended in symptomatic kidneys, or in asymptomatic kidneys when it is necessary to obtain a place for kidney transplantation, and in kidneys where a tumor is suspected.
BACKGROUND: The prophylactic administration of tranexamic acid has been shown to be appropriate for procedures with a high risk of perioperative bleeding in cardiac surgery and orthopaedics. In urology the ambiguous results have been reported. Our goal was to evaluate the effect of tranexamic acid administration in robotic-assisted radical prostatectomy (RARP). A pilot, prospective, double-blind, randomized study was conducted to evaluate this effect. METHODS: The study included 100 patients who received RARP in the period from April 2017 to January 2018. The patients were randomly assigned to study and control groups of 50 patients each. RESULTS: The median follow-up was 6 months. Lower haemoglobin level drop weighted for gram of operated prostate was observed in the study group when treating the dorsal vein complex (DVC) at the beginning of the procedure (p = 0.004 after 3 hours and p < 0.001 after 24 hours). There was no evidence of any serious side effect of tranexamic acid. CONCLUSION: We demonstrated the safety of tranexamic acid at RARP. In addition, we showed that administration of tranexamic acid at the beginning of RARP significantly reduces the decrease in haemoglobin after the procedure when treating the DVC at the beginning of the procedure.

AIMS: To retrospectively evaluate the technical features, efficacy, accuracy, appropriate complications, and relationships among monitored parameters of computed tomography-guided biopsies of the retroperitoneum-located processes. METHODS: From December 2008 to December 2018, 208 percutaneous biopsy procedures for tumors, sized 14 - 190 mm in diameter (median size 57.5 mm), were performed on patients with suspected retroperitoneal tumorous process on imaging examinations. The patients were men in 124 cases and women in 84 cases, aged 20 to 90 years (median age 63.2 years). Skin to lesion distance was variable; from 43 cm to 178 cm (median length 108.5 cm). RESULTS: In 202 cases (97.1%) results were true positive or true negative; only 6 interventions (2.9%) were histologically false negative and had to be confirmed surgically. DLBCL and metastatic disease to the retroperitoneal lymphatic nodes were the most common diagnoses (23.1% each); lymphoma types were verified in 40.9% of cases. 7 complications in total were revealed, 6 of which were minor hemorrhages, and in one case ureteral injury was detected. A statistically significant relationship between the hypervascular process and complication incidence (P=0.00166) and needle gauge (P=0.01427) was identified. CONCLUSION: Percutaneous CT-guided biopsy performed in patients with a suspected retroperitoneal tumorous process had a high accuracy in establishing the correct diagnosis including histological subtyping. Simultaneously, the complication rate was low.

Úvod. Radikální prostatektomie je nejúčinnější léčebnou metodou u karcinomu prostaty. Robotem asistovaný přístup je minimálně invazivní metoda, která je vysoce účinná a velmi vhodná také pro starší pacienty. Retrospektivně byl hodnocen soubor pacientů po robotem asistované radikální prostatektomii starších 65 let. Materiál a metodika. Hodnocený soubor měl 256 pacientů a všichni podstoupili robotem asistovanou radikální prostatektomii v období 2016–2018. Byly hodnoceny operační výsledky a komplikace. Dále byly zhodnoceny funkční a onkologické výsledky tohoto souboru. Výsledky. Medián doby sledování byl 18 měsíců. Operační čas byl 186 minut a krevní ztráty 150 ml. Nejčastější doba hospitalizace byla 3 nebo 4 dny. Pouze dva pacienti byli po operaci dočasně uloženi na JIP oddělení. Z dlouhodobých výsledků byla zaznamenána inkontinence moče u 10 % pacientů a striktura uretry u 0,7 %. Podle našich předběžných onkologických výsledků bylo celkové přežívání 100%, celková a karcinom-specifická mortalita byla nulová. Velmi závažné komplikace s nutností opakovaných reoperací byly pouze u jednoho pacienta. Závěr. Robotem asistovaná radikální prostatektomie je velmi vhodná i pro starší pacienty. Kalendářní věk pacienta by neměl být nejdůležitějším kritériem.
Introduction. Radical prostatectomy is the most effective treatment method for prostate cancer. A robot-assisted approach is a minimally invasive method that is highly effective and also suitable for elderly patients. The cohort of patients who underwent robot-assisted radical prostatectomy over 65 years of age was retrospectively evaluated. Material and Methodology. The evaluated group had 256 patients, and all underwent robot-assisted radical prostatectomy in the period 2016- 2018. Surgical outcomes and complications were evaluated. Furthermore, the functional and preliminary oncologic results of this group were analysed. Results. Median follow-up time was 18 months. The operating time was 186 minutes and the blood losses were 150 ml. The most common length of hospitalization was 3 or 4 days. Only two patients were temporarily transferred to the ICU after surgery. Urinary incontinence was recorded in 10% patients and urethral stricture in 0.7%. According to our preliminary oncologic results, overall survival was 100% and overall and cancer-specific mortality was zero. Only one patient had serious complications with the need for repeated reoperations. Conclusion. Robot-assisted radical prostatectomy is also very suitable for elderly patients. The patient’s age only would not be the most important criterion.
Stoupající počty urologických výkonů a rostoucí věk a nemocnost urologických pacientů zvyšuje rizika perioperačních krvácivých komplikací. Neexistuje jednoduché doporučení k prevenci krvácení. Postup by měl vycházet z aktuálního stavu pacienta a povahy konkrétního urologického výkonu. Nezřídka je třeba konzultace ambulantního specialisty – hematologa či angiologa. V práci představujeme přehled současných možností farmakologické profylaxe krvácení při urologických výkonech.
The rising number of urological procedures and the increasing age and morbidity of urologic patients increase the risk of perioperative bleeding complications. There is no simple recommendation to prevent bleeding. It should be based on the current state of the patient, who undergo the specific urological procedure. Often, consultation with a specialist (hematologist or angiologist) is useful. In this paper we present an overview of the current possibilities of pharmacological prophylaxis of bleeding during urological procedures.

Cíl práce: Prezentujeme krátké video z roboticky asistované radikální cystektomie, hysterektomie, adnexotomie a profylaktické appendektomie s intrakorporálně provedenou Brickerovou derivací. Materiál a metody: Pacientka s BMI 36,6 byla indikována k výše uvedenému výkonu pro invazivní uroteliální karcinom - pT2N0M0 - high grade s dlaždicobuněčnou diferenciací. Příprava k výkonu a peri- a pooperační péče probíhala dle protokolu ERAS (Early Recovery After Surgery). Při výkonu byly nejprve vypreparovány oba močovody do pánve, kde byly zaklipovány a následně byly uvolněny laterální stěny měchýře až na endopelvickou fascii. Poté bylo otevřeno parietální peritoneum v Douglasově prostoru a otevřena pochva pod děložním čípkem, skeletizovány a přerušeny pedikly močového měchýře. Preparát byl uvolněn přerušením zaklipované uretry. Byla provedena bilaterální pánevní lymfadenektomie odstraněním lipolymfatické tkáně v okolí zevních ilických cév a z obturatorní fossy. Všechny preparáty byly extrahovány pochvou, která byla následně sešita pokračujícím Vloc stehem. Pod sigmatem byl protažen levý močovod doprava a vytvořena ureterální ploténka dle Wallace. Byla provedena profylaktická appendektomie a exkludována 15cm klička preterminálního ilea, kterou byly protaženy stentové ureterální cévky a byla napojena ureterální ploténka. Kontinuita střeva byla obnovena dvěma endostaplery. Uretrou byl do pánve zaveden permanentní katétr jako drén. Ranou po asistentském portu byl vyveden distální konec konduitu a zhotovena urostomie. Výsledky: K výkonu byl použit systém Da Vinci Xi, rozmístění robotických 8mm portů v transverzální linii cca 3cm nad pupkem (viz výsledný snímek na konci videa) a 12mm asistentský v místě předpokládané stomie. Délka výkonu byla 5,5 hodiny, krevní ztráta do 100ml, doba na JIP čtyři dny, střevní peristaltika byla obnovena druhý pooperační den, drén byl ponechán 48 hodin, ureterální cévky 14 dní. Pooperační průběh byl komplikován prolongovanou serózní sekrecí z pochvy, která spontánně ustoupila 18. pooperační den. Histologicky nebyl zachycen v celém močovém měchýři vitální tumor. Závěr: Roboticky asistovaná radikální cystektomie je výkon bezpečný s celou řadou výhod pro pacienta. Vynikající přehled v omezeném prostoru pánve umožňuje precizní preparaci tkání a redukci krevních ztrát. Extrakce preparátu pochvou umožňuje provedení výkonu bez minilaparotomie. Tím učinně brání sekundárnímu hojení rány vlivem zvýšené lymfatické sekrece z přerušených lymfatik po pánevní lymfadenektomii. Intrakorporání konstrukce derivace umožňuje výkon provést bez traumatizace střeva otevřením peritoneální dutiny a účinně tak brání rozvoji pooperačního paralytického ileu. Vyvedení drénu uretrou se naopak ukazuje jako faktor disponující k prolongované serózní sekreci z uretry.
Purpose: A short video is presented of robotic‑assisted radical cystectomy, hysterectomy, adnexectomy, and prophylactic appendectomy with intracorporeal Bricker diversion. Material and methods: A patient with a BMI of 36.6 to under went the above procedure for high grade invasive urothelial carcinoma – pT2N0M0 – with squamous cell differentiation. Presurgical preparation and peri- as well as postoperative care were preformed according to the ERAS (enhancedearly recovery after surgery) protocol. During the procedure, both ureters were dissected into the pelvis where they were clipped, and, subsequently, the lateral bladder walls were released up to the endopelvic fascia. Next, the parietal peritoneum in the Pouch of Douglas was opened, the vagina was opened below the cervix, and the bladder pedicles were skeletalized and transected. The specimen was released by transecting the urethra secured with clips. Bilateral pelvic lymph node dissection was performed by removing lipolymphatic tissues around the external iliac vessels and from the obturator fossa. All the specimens were removed through the vagina which was subsequently closed with a continuous V‑Loc suture. Below the sigmoid, the left ureter was drawn to the right and a Wallace ureteral plate was created. Prophylactic appendectomy was performed, and a 15-cm preterminal ileal loop was excluded through which ureteral stent catheters were passed and the ureteral plate was anastomosed Bowel continuity was restored using two endostaplers A permanent catheter was inserted via the urethra into the pelvis as a drain The distal end of the conduit was pulled out through the assistant port incision, and urostomy was performed Results: The surgery was carried out using the Da Vinci Xi system, with robotic 8‑mm ports placed in the transverse line approximately 3 cm above the umbilicus (see the resulting image at the end of the video) and a 12‑mm assistant port in the anticipated stoma site The duration of the procedure was 5 5 hours, with a blood loss of no more than 100 mL and an ICU stay of 4 days; bowel motility was restored on postoperative day 2; the drain was left in place for 48 hours and ureteral catheters for 14 days The postoperative course was complicated by prolonged serous secretion from the vagina that resolved spontaneously on postoperative day 18 Histologically, no viable tumour was detected in the entire bladder Conclusion: Robotic ‑assisted radical cystectomy is a safe procedure with a number of benefits for the patient An excellent view of the confined pelvic space allows for a precise tissue dissection and blood loss reduction Specimen removal through the vagina makes it possible for the procedure to be performed without the use of minilaparotomy This effectively inhibits secondary wound healing due to increased lymphatic flow from the interrupted lymphatics after pelvic lymphadenectomy Intracorporeal construction of the diversion allows for the procedure to be done without traumatizing the bowel by opening the peritoneal cavity, thus effectively preventing the development of postoperative paralytic ileus On the contrary, a drain passed through the urethra proved to be a factor predisposing to prolonged serous secretion from the urethra.
