Úspěch kloubních náhrad závisí na více faktorech, mezi něž patří výběr pacienta, design endoprotézy, způsob fixace ke kostnímu lůžku, kvalita kosti a měkkých tkání, pečlivá operační technika, pooperační péče. Reverzní náhrada ramena se jeví jako vhodné operační řešení akutních i chronických postižení ramenního kloubu ve správně indikovaných případech. Její implantace je považována většinou chirurgů za obtížnější výkon než náhrady jiných kloubů. V práci autoři shrnují vlastní operační techniku ve dvou hlavních indikačních skupinách. Přehledně jsou popsány rozdíly v operační technice, včetně výhod a nevýhod v jednotlivých skupinách, a zdůrazněny důležité momenty operace. V každé skupině je popsáno polohování pacienta na operačním stole, vlastní technika, ale i pooperační péče. Závěrem jsou shrnuty v bodech zásady správné implantace reverzní náhrady ramena, zásadní kroky operace a čeho se vyvarovat, specifika implantace z traumatologické indikace.
The success of joint replacements depends on several factors, including patient selection, design of the prosthesis and the method of fixation to bone bed, quality of bone and soft tissue, meticulous surgical technique and postoperative care. Reverse shoulder replacement seems to be reliable surgical management of acute and chronic disability of the shoulder joint in appropriately selected cases. Its implantation is considered by most surgeons to be more difficult performance than those other joints. The authors summarize the surgical technique in two main indication groups. Clearly explains the differences in surgical technique, including advantages and disadvantages in each group and highlights important moments of surgery. In each group described positioning of the patient on the operating table, operative technique itself, but also post-operative care. In the conclusion summarizes the principles of implantation reverse shoulder replacement, essential steps in surgery and what to avoid, specificity implantation in trauma indication.

V práci autoři rozdělují endoprotézy ramenního kloubu podle technických kritérií a specifikují jednotlivé indikace k jejich použití. Od jednotlivých typů náhrad se odvíjející specifické komplikace. Komplikace jsou rozděleny na obecné a typické pro jednotlivé typy náhrad. U každé komplikace jsou zmíněny příčiny a jejich technické řešení. Jednotlivé komplikace jsou doplněny rentgenologickou a obrazovou dokumentací.
The authors grouping the shoulder joint replacement according to technical criteria and specify the indications for their use. Since various design patterns specific complications occur. Complications are divided into general and specific to each type of prosthesis. For each complication are mentioned causes and their technical solutions. Each group of complications is complemented by X-ray and visual documentation.

PURPOSE: Fractures of the scapular, or glenoid, neck are strongly debated in the relevant literature. Analysis of the respective studies, however, revealed a considerable confusion in terms of definition, terminology and diagnosis. In addition, there are few studies, primarily case reviews, dealing in detail with these fractures. The aim of this article is to present detailed information about fractures of the scapular neck, based on the analysis of our own experience and of the published literature. MATERIAL AND METHODS: Our group of 17 fractures of the scapular neck comprised 14 men and three women, with a mean age of 38 years (range, 24-55). The authors diagnosed four fractures of the anatomical neck, nine fractures of the surgical neck and five trans-spinous fractures of the neck. Of these 17 scapular neck fractures, six fractures were treated non-operatively, and 11 fractures were operated on via the posterior Judet approach. The mean follow-up was 4.9 years (range, 1-11). RESULTS: Fractures in all six non-operatively treated patients healed without complications in the anatomical position and with full function of the shoulder. In 11 operatively treated patients, all fractures of the scapula and clavicle healed within three months. In nine cases the function of the shoulder was subjectively and objectively assessed as normal, and in the remaining two cases as fair. CONCLUSION: The term "scapular, or glenoid, neck" covers three different types of fractures, i.e., fracture of the anatomical neck, fracture of the surgical neck and trans-spinous neck fracture. Fractures of the surgical neck are divided into stable, fractures with rotational instability and fully unstable fractures, depending on the integrity of the coracocacromial and coracoclavicular ligaments. Accurate diagnosis of fractures of the scapular neck requires 3D CT reconstructions. Undisplaced or minimally displaced fractures may be treated non-operatively, markedly displaced fractures constitute an indication for osteosynthesis via the Judet approach.

Popisujeme možnosti nového úhlově stabilního implantátu NCB® při operačním léčení periprotetických zlomenin femuru. NCB® dlahy kombinují možnost konvenční dlahové techniky s polyaxiálním umístěním šroubů a úhlovou stabilitou. V naší sestavě došlo pouze k jedné únavové zlomenině. Popisujeme technické detaily a možnosti použití. Tato dlaha přináší slibné výsledky, zvláště u starých pacientů s osteoporózou.
We report on the application of the new fixed angle NCB® implant, in treatment of periprosthetic femoral fractures. The NCB® splint combines conventional splinting technique with polyaxial placement of screws and angular stability. In our group there was only one fatigue breakage of the splint. We describe the technical details and the possibilities for use of the NCB® splint. This implant provides promising results, particularly in elderly patients with osteoporosis.

PURPOSE OF THE STUDY: The aim of this prospective study was to investigate whether female gender and revision surgery were significant risk factors for intra-operative periprosthetic fractures during total hip arthroplasty (THA). MATERIAL AND METHODS: The group investigated comprised the patients who, in the period 1995-2009, sustained an intra-operative periprosthetic fracture during primary or revision THA. The patients were treated by a therapeutic procedure based on the Vancouver system. The results were related to the total number of patients undergoing THA in that period, Statistical analysis was performed using Pearson's x2 test at the 5% significance level. RESULTS: Intra-operative periprosthetic fractures occurred in 110 patients (89 women, 21 men). The average age of the patients was 69 years (70 in women and 62 in men). The women significantly outnumbered the men (p < 0.001). In the period under study, 2936 primary and 791 revision THAs were performed; the incidence of all intra-operative fractures was 3%. Intra-operative fractures during primary THA were recorded in 95 patients (3.2%; range in individual years, 0.4 to 5.9%); fractures during revision THA were found in 15 patients (1.9%; range, 0 to 8.0%). This difference was slightly above the set significance level (p = 0.057). The majority of intra-operative fractures were minimal Vancouver type-A fractures in the greater trochanter region. They were recorded in 95 of the 110 patients (86%) with either primary or revision THA. Of the 15 type-B fractures, eight were shown by a detailed evaluation to occur during revision THA. An independent analysis of type-B fractures in relation to all replacements showed that their occurrence was significantly higher in revision than in primary THA (p = 0.006). DISCUSSION Intra-operative periprosthetic fractures have primarily been studied in terms of their frequency and the cause of their occurrence. Revision surgery and female gender are regarded as risk and predisposing factors. Our results, in accordance with other relevant data, confirmed that serious intra-operative type-B fracture occurred more frequently during revision THA than during primary surgery. It further showed a significantly higher number of periprosthetic fractures in women than in men. However, this finding is affected by the fact that women in general undergo more THAs than men and that the female median life span is longer. The authors consider a careful pre-operative planning and thorough evaluation of all risk factors related to surgery as basic preventive measures. CONCLUSIONS This continuous 15-year study on patients with intra-operative periprosthetic fractures of the hip has allowed us to conclude that these fractures occur more frequently in women than in men, and that more serious fractures (Vancouver type-B) are significantly more frequent during revision than primary THA. The higher incidence in women is, to a great part, accounted for by osteoporosis of the skeleton in elderly people. In revision THA, poor bone quality plays a role as well as osteolysis due to polyethylene granuloma which may be present.

Anatomical neck fractures of the scapula are rare. The authors have found in the literature only four radiologically documented fractures of the anatomical neck of the scapula. Two of them were published by Hardegger et al., the third case was published by Arts and Louette. The last case, in fact only a radiograph and a rather poor 3D CT reconstruction of a fracture of the anatomical neck of scapula, was published by Jeong and Zuckerman. Together with author's two patients, the group of radiologically verified anatomical neck fractures of the scapula comprises six cases in total (four men, one woman, one gender unspecified). Analysis of the radiographs showed that in all these cases, the fracture line separated only the glenoid fossa from the scapular body, with a short spike of the lateral border of the scapula. The proximal part of the vertical fracture line ran into the coracoglenoid notch, between the upper border of the glenoid and the base of the coracoid process. The distal part of the fracture line crossed the lateral border of the scapular body 2-4 cm distal to the inferior pole of the glenoid fossa. The gleniod fragment was always formed by the glenoid fossa and a short spike of the lateral border of the scapular body. In five cases, the glenoid fragment, together with the humeral head, was displaced distally and the humeral head came to lie below the level of the coracoid process. At the same time, the glenoid fragment rotated into a valgus position. Only in one case, did the radiographs fail to show valgus displacement and the fracture was angulated in the transverse plane. In all six cases, the subacromial space between the acromion and the humeral head was widened. All fractures were operated on via a Judet posterior approach. In five cases, the outcome of the operation was assessed at 3, 5, 12, 21 and 120 months after surgery, three-being rated as excellent or very good, one as good and one as poor.
