OBJECTIVES: This study was designed to assess the risk of significant bleeding complications in patients receiving antiplatelet or anticoagulation medications at the time of implantable cardioverter-defibrillator (ICD) device implantation. BACKGROUND: Periprocedural management of antiplatelet or anticoagulation therapy at the time of device implantation remains controversial. METHODS: We performed a retrospective chart review of bleeding complications in all patients undergoing ICD or pacemaker implantation from August 2004 to August 2007. Aspirin or clopidogrel use was defined as taken within 5 days of the procedure. A significant bleeding complication was defined as need for pocket exploration or blood transfusion; hematoma requiring pressure dressing or change in anticoagulation therapy; or prolonged hospitalization. RESULTS: Of the 1,388 device implantations, 71 had bleeding complications (5.1%). Compared with controls not taking antiplatelet agents (n = 255), the combination of aspirin and clopidogrel (n = 139) significantly increased bleeding risk (7.2% vs. 1.6%; p = 0.004). In patients taking aspirin alone (n = 536), bleeding risk was marginally higher than it was for patients taking no antiplatelet agents (3.9% vs. 1.6%, p = 0.078). The use of periprocedural heparin (n = 154) markedly increased risk of bleeding when compared with holding warfarin until the international normalized ratio (INR) was normal (n = 258; 14.3% vs. 4.3%; p < 0.001) and compared with patients receiving no anticoagulation therapy (14.3% vs.1.6%; p < 0.0001). There was no statistical difference in bleeding risk between patients continued on warfarin with an INR > or =1.5 (n = 46) and patients who had warfarin withheld until the INR was normal (n = 258; 6.5% vs. 4.3%; p = 0.50). CONCLUSIONS: Dual antiplatelet therapy and periprocedural heparin significantly increase the risk of bleeding complications at the time of pacemaker or ICD implantation.

Katetrizační implantace aortální chlopně (TAVI) je nová metoda, která se osvědčila a již se stala rutinní součástí moderní intervenční kardiologie. Náš přehledový článek nejprve představí design dvou schválených a nejrozšířenějších chlopenních protéz. Poté následuje srovnání charakteristik pacientů a dosažených výsledků v počátcích této metody (studie Partner) a dnes (nedávno publikovaný registr Sentinel). V další části jsou uvedeny novinky v roce 2013, kde jistě stojí za zmínku význam CT vyšetření k přesnému měření aortálního anulu i výběru nejbezpečnější přístupové cesty. Méně obvyklé indikace představuje použití TAVI technologie v terapii degenerovaných bioprostetických náhrad v aortální pozici a také v terapii čisté aortální regurgitace; diskutujeme výsledky, ale také rizika. Stran dlouhodobých výsledků již máme první data z pětiletého sledování pacientů a funkce katetrizačně zavedených protéz se zatím jeví velmi optimisticky. Prezentujeme výsledky ekonomické analýzy studie Partner provedené v USA a orientační srovnání hospitalizačních nákladů TAVI a chirurgické náhrady aortální chlopně (AVR) v podmínkách českého zdravotnictví – zdá se, že rozdíl v ceně (po započtení nákladů na pooperační péči) není dramatický. Etickou otázku představuje pacient, který v rozporu s doporučením indikační komise sám preferuje TAVI před AVR. Má možnost si vybrat? Závěrem velmi stručně prezentujeme výsledky TAVI programu ve Fakultní nemocnici Královské Vinohrady.
Transcatheter aortic valve implantation (TAVI) as a new but now widely accepted method has become a routine part of modern interventional cardiology. Our review article will at first introduce the design of the two currently approved and dominant valve prostheses. We present a comparison of patient characteristics and results between the early phase (Partner study) and current situation (recently published in the Sentinel registry). In 2013, there are several new approaches and the importance of CT based evaluation of aortic annulus size and the safest vascular access are highlighted. There are two less common indications of TAVI technology – so-called valve-in‑valve for therapy of degenerated aortic bioprosthesis and the second indication is therapy of pure aortic regurgitation. We discuss the results as well as the risks. As far as long‑term results are concerned, preliminary data from a five‑year patient follow‑up and the results with this catheter based technology are very optimistic. We present the economic analysis of the Partner study performed in the USA and we have performed a preliminary comparison of in‑hospital costs between TAVI and surgical aortic valve replacement (AVR) within Czech healthcare – it seems that the cost difference (once the price of post‑operative care is included) is not dramatic. The patient who, despite the recommendation of Heart Team prefers TAVI to AVR, presents an ethical dilemma. Can the patient make this choice? Finally, we briefly present our results of the TAVI programme in University Hospital Královské Vinohrady.

Úspěch kloubních náhrad závisí na více faktorech, mezi něž patří výběr pacienta, design endoprotézy, způsob fixace ke kostnímu lůžku, kvalita kosti a měkkých tkání, pečlivá operační technika, pooperační péče. Reverzní náhrada ramena se jeví jako vhodné operační řešení akutních i chronických postižení ramenního kloubu ve správně indikovaných případech. Její implantace je považována většinou chirurgů za obtížnější výkon než náhrady jiných kloubů. V práci autoři shrnují vlastní operační techniku ve dvou hlavních indikačních skupinách. Přehledně jsou popsány rozdíly v operační technice, včetně výhod a nevýhod v jednotlivých skupinách, a zdůrazněny důležité momenty operace. V každé skupině je popsáno polohování pacienta na operačním stole, vlastní technika, ale i pooperační péče. Závěrem jsou shrnuty v bodech zásady správné implantace reverzní náhrady ramena, zásadní kroky operace a čeho se vyvarovat, specifika implantace z traumatologické indikace.
The success of joint replacements depends on several factors, including patient selection, design of the prosthesis and the method of fixation to bone bed, quality of bone and soft tissue, meticulous surgical technique and postoperative care. Reverse shoulder replacement seems to be reliable surgical management of acute and chronic disability of the shoulder joint in appropriately selected cases. Its implantation is considered by most surgeons to be more difficult performance than those other joints. The authors summarize the surgical technique in two main indication groups. Clearly explains the differences in surgical technique, including advantages and disadvantages in each group and highlights important moments of surgery. In each group described positioning of the patient on the operating table, operative technique itself, but also post-operative care. In the conclusion summarizes the principles of implantation reverse shoulder replacement, essential steps in surgery and what to avoid, specificity implantation in trauma indication.

PURPOSE: This study was carried out to quantify the effect of an alkali-modified surface on implant stability during healing using an animal model. MATERIALS AND METHODS: A total of 24 screw-shaped, self-tapping, commercially pure titanium dental implants, divided into a test group (implants with an alkali-modified surface or "biosurface") and a control group (implants with a turned, machined surface) were inserted without pretapping in the tibiae of 3 beagle dogs. The resonance frequency analysis method was used to measure the implant stability quotient (ISQ) 0, 1, 3, 9, and 12 weeks after implantation. The animals were sacrificed after 2, 5, and 12 weeks, and the bone-implant contact (BIC%) was evaluated histomorphometrically. RESULTS: The difference in the osseointegration rates (deltaISQ/deltahealing time) between the implants with alkali-modified surface (biosurface) and those with a turned, machined surface was evaluated as a mean of 0.843 ISQ/week within the first 9 weeks of healing. The mean increase in the secondary implant stability was found to be proportional to the mean increase in the BIC at healing period earlier than 5 weeks. DISCUSSION: The characteristics that differed between the implant surfaces, ie, specific surface area, contact angle, and hydroxylation/hydration, may represent factors that influence the rate of osseointegration and the secondary implant stability. CONCLUSION: The alkali-treated surface enhances the secondary stability in the early stages of healing compared to the turned, machined surface, as a consequence of faster BIC formation.
