OBJECTIVE: Hand osteoarthritis (OA) is a frequent musculoskeletal disorder with an increasing prevalence during ageing. This study aimed to evaluate circulating microRNAs (miRNAs) in the plasma of patients with hand OA compared with age- and sex-matched healthy control subjects. METHODS: In total, 238 participants (96 with erosive and 73 with non-erosive hand OA patients and 69 healthy control subjects) were included in this study. All patients underwent clinical examinations, including self-reported measures (AUSCAN and Algofunctional index). Radiographs of both hands were scored with the Kallman scale. The profile of miRNAs in plasma was screened using TaqManTM Low-Density Array, and candidate miRNAs were validated on two quantitative real-time PCR (qRT-PCR) systems (QuantStudio and SmartChip). RESULTS: Of all the 754 miRNAs, 40 miRNAs were different between hand OA patients and healthy control subjects in the screening cohort. Following the two-phase validation process, three miRNAs (miR-23a-3p, miR-146a-5p, and miR-652-3p) were increased in patients with hand OA compared with healthy control subjects and were associated with the AUSCAN sum score and AUSCAN pain. Furthermore, an inverse correlation of miR-222-3p with the Kallman radiographic score was found. The expression of miRNAs did not differ between erosive and non-erosive hand OA. CONCLUSION: The profile of circulating miRNAs could unveil candidate biomarkers associated with hand OA symptoms. Longitudinal studies are required to determine the role of miRNAs in hand OA.

The aim of the study was to examine the relation between polymorphisms and serum levels of selected cytokines (IL-6, IL-13 and IL-15), production of autoantibodies and factors describing rheumatoid arthritis (RA), such as DAS28 and Total Sharp Score. A total of 156 patients with RA according to the ACR criteria, and 200 control subjects were recruited into the study. The measurements of CRP, anti-CCP, the presence of rheumatoid factors (RFs), radiographs of both hands with calculation of Total Sharp Score (TSS) and DAS28 were obtained from all patients with RA. In total, five polymorphisms in genes coding cytokines (IL-6, IL-13 and IL-15) were detected. The levels of these selected cytokines were measured in serum using ELISA method. A significant difference in allele frequencies between patients with RA and controls was observed for IL-15 -267C/T polymorphism. A higher prevalence of heterozygote variants of IL-15 polymorphisms (14035A/T and -267C/T) in the RF IgG- and RF IgA-negative subgroups was observed. Furthermore, the association of polymorphisms in gene for IL-15 with circulating level of IL-15 (14035A/T and 367G/A) and with total RF and Ig-specific RFs (-267C/T) was found. The relation of IL-15 to RFs IgA, IgM, IgG and the measure of DAS28 was proved. The frequency of the T allele of the IL-13 polymorphism -1112C/T was higher in subgroup with faster progression of the disease (TSS/month ≥ 0.1). In conclusion, we present an association of IL-15 gene polymorphisms with the RFs including subtypes (RF, IgG, IgA) underlined by the relation of increased IL-15 levels in circulation to RFs.

Avaskulární osteonekróza je závažným onemocněním převážně středního věku. Je definována jako odumření buněk kosti a kostní dřeně s následným kolapsem kosti. V pozadí tohoto procesu stojí porucha krevního zásobení subchondrální kosti. V klinické praxi je rozvoj netraumatické osteonekrózy asociován s celou řadou stavů, mezi něž patří také systémové podávání glukokortikoidů. Patogeneze kortikoidy navozené osteonekrózy zatím není zcela objasněna. Na základě experimentálních výsledků se zdá, že významnou roli hrají změny lipidového metabolismu, alterace hemokoagulačních, fibrinolytických a mikrocirkulačních poměrů či přímé ovlivnění buněk kosti. Podkladem změn je nespíše ovlivnění genové transkripce kortikoidy na lokální úrovni. Jednoznačná kauzalita mezi podáním kortikoidu a následnou osteonekrózou v humánní medicíně zatím prokázána nebyla a ani být nemohla. Důvodem je fakt, že samotná onemocnění, pro která jsou glukokortikoidy podávány, mohou být predispozicí k osteonekróze. Asociace mezi podáváním steroidů a výskytem osteonekrózy je však vysoká, a to zejména v případě mnohočetné osteonekrózy (až 91 %). Osteonekrózou jsou více ohroženi ti nemocní, kteří jsou léčeni intravenózními pulzy metylprednisolonu a kterým je na počátku léčby podávána vysoká iniciální denní dávka perorálního kortikoidu. Výše kumulativní nebo průměrné denní udržovací dávky hraje pravděpodobně menší roli. Osteonekróza je postupný proces, který se rozvíjí několik měsíců až let asymptomaticky. Díky moderním zobrazovacím metodám (magnetická rezonance) lze časná stadia osteonekrózy diagnostikovat již za dva až tři měsíce po začátku expozice kortikoidům. V počátečních fázích nekrotického procesu probíhají reparativní děje a proces může být reverzibilní. Pokud stav progreduje, dochází postupně ke kolapsu kosti a proces se stává nevratným. Pro další prognózu je tedy stěžejní detekovat onemocnění v časném, reparabilním, většinou asymptomatickém stadiu. Vzácnějším, ale svou závažností významným fenomenem je multifokální osteonekróza, které autoři věnují zvláštní pozornost. Celá problematika mnohočetné a s glukokortikoidy asociované osteonekrózy je pak dokumentována dvěma kazuistikami.
Avascular osteonecrosis is a serious disorder affecting mostly middle-aged individuals. It is characterized as death of bone cells and bone marrow cells with subsequent bone collapse. Alteration of subchondral bone blood supply is responsible for this process. Development of atraumatic osteonecrosis is associated with wide range of conditions including systemic use of glucocorticoids. Pathogenesis of glucocorticoid induced osteonecrosis is not fully understood yet. On the basis of experimental data, it seems that altered lipid metabolism, changes in coagulation, fibrinolysis, and microcirculation or direct affection of bone cells may play an important role. It is probably based on local alteration of gene transcription induced by glucocorticoids. Causal relationship between use of glucocorticoids and subsequent osteonecrosis in human medicine was not and even could not have been demonstrated yet. It is based on the fact that the diseases alone could also predispose to osteonecrosis. An association between the use of glucocorticoids and occurrence of osteonecrosis is rather high, particularly in the cases of multiple osteonecrosis (up to 91 %). Patients treated with intravenous pulses of methylprednisolone and with initial high daily doses of oral glucocorticoids are most susceptible for the development of osteonecrosis. Cumulative or mean daily doses play probably less important role. Osteonecrosis is slowly progressive process that may develop asymptomatically during several months or years. Thanks to modern imaging methods (magnetic resonance), it is possible to diagnose early phase of osteonecrosis within two to three months after glucocorticoids exposure. Reparative changes occur in the beginning of the necrotic process, which may be reversible at this stage. If it progresses, subsequent bone collapse makes the process irreversible. For further prognosis, it is crucial to diagnose the disease early, in reparative and mostly asymptomatic phase. Uncommon, however due to its severe course, significant phenomenon represents multiple osteonecrosis that is mostly discussed in this overview. The whole problem of multiple osteonecrosis and osteonecrosis associated with glucocorticoids is documented with two case reports.

V práci autoři upozorňují na možnost koincidence dvou nezávisle na sobě probíhajících revmatických onemocnění, jejichž příznaky byly delší dobu podceňovány, proto byla opožděně diagnostikována a následně vedla k destruktivním změnám kloubů dolních končetin. V tomto případě se jednalo o 29letou pacientku s diabetem mellitem I. typu od 11 let věku na inzulinoterapii s multiorgánovými komplikacemi, s kolísavou glykemií, s dvouletou anamnézou artritidy levého hlezna a nově vzniklou pravostrannou gonitidou. V anamnéze posledních 3 let dále uváděla recidivující průjmy, které byly hodnoceny jako průjmy při diabetické neuropatii. V laboratoři byly přítomny vysoké reaktanty akutní fáze, hypochromní anémie, proteinurie a pozitivní antigen HLA B27. Na rentgenovém snímku byl obraz těžké destrukce talu a tibie s podezřením na neuropatický kloub, který byl potvrzen magnetickou rezonancí. Dodatečné gastroenterologické vyšetření prokázalo diagnózu Crohnovy choroby v akutním stadiu v oblasti terminálního ilea. Onemocnění bylo tedy přehodnoceno na enteropatickou artritidu při morbus Crohn s rozvojem Charcotova kloubu v oblasti levého hlezna při diabetické polyneuropatii. Po nasazení adekvátní terapie ve spolupráci s gastroenterologem, ortopedem a podiatrem došlo ke zmírnění až vymizení subjektivních příznaků, normalizaci hladin glykemie, ústupu průjmů i artritidy a zastavení progrese destrukce levého hlezenného kloubu.
The authors refer here a rare coincidence of two independent rheumatic diseases. The symptoms of both conditions have been underestimated for a long time Therefore the correct diagnosis was delayed and disease lead to destructive changes of lower extremity joints. This case describes 29-year-old patient with insulin-dependent diabetes mellitus type 1 diagnosed at the age of 11 years with multiorgan complications, fluctuating glycaemia levels, two year’s history of left ankle arthritis, and newly appeared arthritis of the right knee. She mentioned recurrent diarrhoea during last 3 years that has been assessed as a part of diabetic neuropathy. High acute phase reactants, hypochromic anemia, proteinuria, and HLA B27 positivity have been detected in laboratory tests. X-ray showed a severe talus and tibia destruction and neuropathic joint was confirmed by magnetic resonance. Subsequent gastroenterological examination diagnosed acute Crohn’s disease of the terminal ileum. Afterwards, the disease was reevaluated as an enteropathic arthritis due to Crohn’s disease with the development of Charcot joint in the region of left ankle due to diabetic polyneuropathy. When sufficient treatment in cooperation with gastroenterologist, orthopedic surgeon, and podiatric physician was introduced, most of the signs and symptoms of arthritis, diarrhea, and diabetes mellitus have improved, including the situation of the left ankle.

Článek obsahuje souhrn poznatků o patogenezi, epidemiologii, klinických projevech, prognóze a terapii infekce bakterií Campylobacter jejuni. Je zdůrazněn rozdíl mezi charakterem reaktivní artritidy navozené tímto patogenním agens oproti charakteru reaktivní artritidy navozené C. trachomatis. Z imunogenetického hlediska je to slabší asociace s antigenem HLA-B27, z klinického pohledu častá polyartritida v oblasti rukou, která může vést v úvodu choroby k záměně s virovou či revmatoidní artritidou. Popsány jsou i jiné orgánové projevy campylobacterové infekce s důrazem na závažné neurologické komplikace. Poukázáno je i na možnost dlouhodobého negativního dopadu na celkový zdravotní stav nemocného. Součástí práce je stručný popis případu nemocné s chronickým průběhem onemocnění.
The article reviews pathogenesis, epidemiology, clinical manifestations, prognosis, and treatment of Campylobacter jejuni infection. The difference between characteristics of Campylobacter jejuni – and C. trachomatis – induced reactive arthritis is particularly emphasized. With respect to immunogenetic and clinical aspects, Campylobacter jejuni reactive arthritis is less frequently associated with HLA-B27 and polyarthritis of the hands is more often seen. The clinical manifestations at the onset of the disease may be confused with viral or rheumatoid arthritis. Other organ manifestations of Campylobacter infections, emphasizing serious neurological complications, are also described. Potential longstanding negative impact of the infection on the general patient’s health status is mentioned. The article describes in detail a case report on the patient suffering from chronic course of Campylobacter jejuni infection and associated reactive arthritis.

Erythema nodosum představuje nejčastější formu panikulitidy. Může se vyskytovat izolovaně nebo může být součástí celé řady chorob. Klinický obraz spočívá v akutním výsevu bolestivých zarudlých uzlů, které jsou lokalizovány většinou na přední straně bérců a bývají symetricky rozloženy. Uzly postupně mění zbarvení v nažloutlé až nazelenalé. Výsev může být provázen febrilním stavem či jinými celkovými příznaky. K odlišení erythema nodosum od jiných panikulitid či kožních ekzantémů jiné etiologie pomáhá skutečnost, že erythema nodosum se vždy zhojí ad intergrum, nezanechává kožní atrofie, jizvy ani hyperpigmentace. Pokud není klinický obraz jednoznačný, přispěje k diagnóze kožní biopsie, kde lze histologickým vyšetřením prokázat septální panikulitidu bez účasti vaskulitidy. U lehčích případů izolovaného erythema nodosum jsou lékem volby nesteroidní antirevmatika Systémové podání glukokortikoidů je indikováno zřídka a to jen u těžších forem. Před jejich aplikací je nutné vyloučit infekční příčinu výsevu. V případě, že je erythema nodosum součástí jiného onemocnění (probíhající nespecifická nebo specifická infekce, reaktivní artritida, sarkoidóza, idiopatický střevní zánět apod.) léčíme základní onemocnění.
Erythema nodosum represents the most frequent form of panniculitis. It may occur separately or as a part of wide range of many diseases. The clinical manifestation consists of an acute onset of painful erythematous nodules, which are predominantly localized symmetrically on a foreside of lower extremities. The nodules subsequently change the color from merely yellow to green. Onset of the disease may be accompanied by febrile status or other systemic manifestations. To differentiate erythema nodosum from other panniculitis or skin lesions of other etiology, the fact that erythema nodosum restores completely and skin atrophy, scars, or hyperpigmentation do not persist is helpful. Skin biopsy and histological finding of septal panniculitis without vasculitis may be of benefit, when the clinical picture is inconclusive. Nonsteroidal anti-inflammatory drugs are used in mild cases of a self-limited erythema nodosum. Systemic administration of glucocorticoids is indicated rarely and obviously in the most severe course of the disease. It is necessary to eliminate infection cause of the onset prior to administration of glucocorticoids. If the erythema nodosum occurs in an association with other diseases (non-specific or specific infections, reactive arthritis, sarcoidosis, idiopathic bowel inflammation, etc.), we treat the underlying disease.

Krční páteř u revmatoidní artritídy je druhou nejčastěji postiženou lokalitou. Vertikální posun je nejméiié častým, ale zcela nejzávažnějším typem postižení v oblasti Cl-2. Radiologický průkaz negaB potenciály mohou pomoci při určení sublďinické leze, ale pokud zatím nedošlo k zřetelné strukturální dysfunkci, mohou být zcela fyziologické. Na kazuistickém sdělení je demonstrován případ vertikáliiího posunu s rozvojem nebezpečné symptomatologie zadní jámy lební, Úspěšně řešené stabilizací.
The defect of cervical spine in rheumatoid arthritis (RA) is the second most frequent localization. Vertical shift is the least frequent, but most severe type of defect in the area of C1-C2. Radiologic demonstration of vertical shift may be negative in the beginning of the defect as well as in positive clinical findings. Evoked potentials may help in the determination of subclinical lesion, but may be completely physiological if a distinct structural dysfunction is not yet present. The case report demonstrates a vertical shift with the development of dangerous symptomatology of posterior cranial fossa, which was successfully treated by stabilization.
