PURPOSE: The aim of this study was to determine the molecular genetic basis of an early-onset severe retinal dystrophy in three unrelated consecutive patients of Czech origin and to describe their ocular phenotype. METHODS: DNA samples from two probands were analyzed using a genotyping microarray (Asper) followed by either target analysis of 43 genes implicated in retinal disorders by next generation sequencing or whole-exome sequencing, respectively. The third proband underwent conventional Sanger sequencing of CRB1 based on her ocular findings. RESULTS: All three probands harboured a known disease-causing mutation c.2843G>A; p.(Cys948Tyr) in the CRB1 gene. One individual was homozygous for this mutation, while in the other two probands c.2308G>A; p.(Gly770Ser) and c.3121A>G; p.(Met1041Val) were also identified in the heterozygous state, respectively. Both variants were novel and evaluated by in silico analysis as pathogenic. A false-negative result was observed in one of the two samples examined by the genotyping microarray. Disease onset in all patients was before the age of 7 years. Hypermetropic refractive error, bilateral nummular retinal pigmentation, retinal thickening and cystoid spaces in the macula were observed in two probands, aged 6 and 7 years. The third proband, aged 28 years, had bone spicule-like pigmentary changes associated with increased retinal nerve fiber layer. CONCLUSIONS: The first study reporting on the molecular genetic cause of non-syndromic early-onset severe retinal dystrophy in Czech patients identified one homozygous and two compound heterozygote probands with CRB1 mutations. Retina nerve fibre layer measurements should be considered an integral part of the clinical evaluation of retinal dystrophies. Detailed clinical examination and imaging can both direct molecular screening and help to confirm or refute disease causation of identified variants.

BACKGROUND: Inversions are balanced structural chromosome rearrangements, which can influence gene expression and the risk of unbalanced chromosome constitution in offspring. Many examples of inversion polymorphisms exist in human, affecting both heterochromatic regions and euchromatin. RESULTS: We describe a novel, 15 Mb long paracentric inversion, inv(21)(q21.1q22.11), affecting more than a third of human 21q. Despite of its length, the inversion cannot be detected using karyotyping due to similar band patterns on the normal and inverted chromosomes, and is therefore likely to escape attention. Its identification was aided by the repeated observation of the same pair of 150 kb long duplications present in cis on chromosome 21 in three Czech families subjected to microarray analysis. The finding prompted us to hypothesise that this co-occurrence of two remote duplications could be associated with an inversion of the intervening segment, and this speculation turned out to be right. The inversion was confirmed in a series of FISH experiments which also showed that the second copy of each of the duplications was always located at the opposite end of the inversion. The presence of the same pair of duplications in additional individuals reported in public databases indicates that the inversion may also be present in other populations. Three out of the total of about 4000 chromosomes 21 examined in our sample carried the duplications and were inverted, corresponding to carrier frequency of about 1/660. Although the breakpoints affect protein-coding genes, the occurrence of the inversion in normal parents and siblings of our patients and the occurrence of the duplications in unaffected controls in databases indicate that this rare variant is rather non-pathogenic. The inverted segment carried an identical shared haplotype in the three families studied. The haplotypes, however, diverged very rapidly in the flanking regions, possibly pointing to an ancient founder event at the origin of the inversion. CONCLUSIONS: The identification of inv(21)(q21.1q22.11) supports the notion that paracentric inversions are the most common form of chromosomal variation and that some of them may still remain undetected.

Cíl: Popsat klinický nález a provést molekulárně genetickou analýzu u dvou probandů českého původu s diagnózou Stargardtovy choroby. Poskytnout souhrn praktických poznatků plynoucích z mutační analýzy genu ABCA4 a poukázat na některé problematické aspekty spojené se screeningem tohoto genu. Metody: U obou mužů ve věku 39 a 26 let jsme provedli oční vyšetření včetně optické koherenční tomografie se spektrální doménou (SD-OCT). DNA byla izolována z venózní krve. Identifikace patogenních sekvenčních variant byla provedena pomocí genotypovacího mikročipu pro gen ABCA4, verze 11.0 (Asper Ophthalmics, Estonsko) detekující 558 známých mutací a jednonukleotidových polymorfismů. Výsledky: U prvního probanda byla nejlepší korigovaná zraková ostrost obou očí 0,1, u druhého probanda 0,05. Klinické vyšetření prokázalo typický nález atrofie makuly s přítomností žlutavých skvrn na očním pozadí. Vyšetření SD-OCT odhalilo oboustranně chybění linie elipsoidů vnitřních segmentů fotoreceptorů, také v literatuře označovaná jako linie junkce vnitřních a zevních segmentů fotoreceptorů, ztenčení neuroretiny a nepravidelnosti pigmentového epitelu sítnice. V souladu s autozomálně recesivním typem přenosu byla rodinná anamnéza pro Stargardtovu chorobu v obou případech negativní. Molekulárně genetickým vyšetřením bylo zjištěno, že první proband je nositelem mutace c.4234C>T; p.(Gln1412*) v exonu 28 a c.5882G>A; p.(Gly1961Glu) v exonu 42. U druhého probanda byla detekována pouze jedna známá patogenní mutace c.1988G>A p.(Trp663*) v exonu 14. Závěr: Poprvé byly u českých pacientů zjištěny mutace zodpovědné za vznik Stargardtovy choroby. Nemožnost nalézt jednu nebo obě patogenní sekvenční varianty je u této choroby za použití současných metod poměrně častým jevem. Budoucí studie by se měly zaměřit na stanovení spektra a frekvence jednotlivých mutací v ABCA4 u rozsáhlejšího souboru pacientů českého původu. Znalost příčinných mutací zlepšuje klinické poradenství pacientům a v indikovaných případech umožňuje preimplantační diagnostiku.
Purpose: The aim of our study was to describe the phenotype and to perform molecular genetic investigation in two probands of Czech origin diagnosed with Stargardt disease (STGD). Methods: Both males underwent ocular examination including assessment by high-resolution spectral domain optical coherence tomography (SD-OCT). DNA was isolated from venous blood. Mutation detection was performed using the ABCA4 genotyping microarray (Asper Ophthalmics, Estonia). Results: The best corrected visual acuity in proband 1 (aged 39 years) was 0.1 bilaterally, and 0.05 in proband 2 (aged 26 years). Fundus examination showed typical multiple yellow-white lesions and macular atrophy. Alterations of retinal pigment epithelium, retinal thinning and disruption of the photoreceptor inner segment ellipsoid band were detected with an SD-OCT. Two known disease-causing mutations in ABCA4 were identified in proband 1; c.4234C>T, p.(Gln1412*) in exon 28; and c.5882G>A, p.(Gly1961Glu) in exon 42. Only one pathogenic change was detected in proband 2; c.1988G>A, p.(Trp663*) in exon 14. A second change, anticipated because of the recessive status of the disease, was not identified. Conclusion: The frequency and full spectrum of ABCA4 mutations in Czech patients with inherited retinal disorders is yet to be established. The inability to detect a second pathogenic change in ABCA4 coding sequences in proband 2 warrants further investigation.

Cíl studie: Metoda SNP array (využívající k diagnostice polymorfismy v 1 nukleotidu – single nucleotide polymorphism) umožňuje odhalit v karyotypu nedetekovatelné submikroskopické změny (mikrodelece, mikroduplikace), jež mohou mít kauzální souvislost s patologickým ultrazvukovým nálezem u plodu. V článku je stručně popsán princip, výhody, nevýhody a možnosti použití metody SNP array v prenatální diagnostice. Jsou prezentovány zkušenosti za 10 měsíců používání této metody v prenatální diagnostice. Typ studie: Prospektivní studie. Pracoviště: Gennet, Praha. Metodika: Vyšetření SNP array bylo v období duben 2010 až leden 2011 provedeno u 110 vzorků DNA získané z kultivovaných nebo nekultivovaných buněk plodů po biopsii choria (CVS, n=14), amniocentéze (AMC, n=88), kordocentéze (n=1) a z tkáně z abortů (n=7). Vyšetření byla indikována na základě patologického nálezu na ultrazvuku s normálním karyotypem plodu, případně k dořešení patologického cytogenetického nálezu vzniklého de novo. Zpracování DNA pro chip Illumina InfiniumHD HumanCytoSNP-12v2.1, jeho příprava a skenování byly provedeny podle standardních protokolů firmy Illumina. Data byla analyzována pomocí softwaru Illumina KaryoStudio a GenomeStudio. Výsledky: Metodou SNP array bylo úspěšně vyhodnoceno 108 vzorků (neúspěch vyšetření byl pouze ve 2 případech, 1,8 %). Odchylky od normálního profilu (CNV-Copy Number Variation) byly zachyceny u 29 případů (29/108 = 27 %), z nichž 16 bylo klinicky významných (16/108=15 %). Jako pravděpodobně nepatogenní byly CNV u 8 případů a u 5 případů nebyl zatím původ nalezené CNV ověřen u rodičů. Po odečtení případů ověřujících de novo patologii v karyotypu plodu byla klinicky významná CNV nalezena v 9 případech (9/94 = 10 %). Nález klinicky významné CNV byl nejčastěji v kategorii ultrazvukových nálezů srdečních vad, vad centrálního nervového systému (CNS) a mnohočetných anomálií. Ve všech 14 případech de novo chromozomální přestavby v karyotypu byla jasně specifikována závažnost přestavby (patologie u 7/14 = 50 %). Závěr: Výsledky studie ukazují, že vedle jednoznačného přínosu při posuzování patogenity de novo chromozomálních aberací prokazuje SNP array i jednoznačný přínos pro záchyt dosud skrytých submikroskopických změn.
Objectives: SNP array (array method using Single Nucleotide Polymorphisms) enables to detect cytogenetically undetectable submicroscopic alterations (microdeletions, microduplications), which could be also causative for ultrasonographic anomalies of fetus. This article describes the principle, advantages, disadvantages and application possibilities of the SNP array method in prenatal diagnosis. The ten month experience with SNP array use in prenatal diagnosis is presented. Design: Prospective study. Settings: Gennet, Prague. Material and methods: During the period from April 2010 to January 2011 we performed 110 SNP array analyses of fetal DNA: 14 chorionic villi samples (CVS), 88 amniotic fluid samples (AMC), 1 cord blood sample and 7 miscarriage samples. Laboratory tests were carried out on DNA from both cultured and uncultured fetal cells. Examinations were performed in fetuses with sonographic abnormal findings having normal karyotype. In addition 14 fetal cytogenetic abnormalities were solved. SNP array analysis was performed using Illumina InfiniumHD HumanCytoSNP-12 chip. All data were analysed by Illumina KaryoStudio and GenomeStudio software. Results: SNP array analysis was performed in 108 fetuses (only 2 examination failures, 1.8%). In total, we detected CNV (copy number variation) in 29 samples (29/108 = 27%). 15% (16/108) of fetuses with abnormal ultrasound findings were found to carry clinically relevant CNV. Probably benign CNVs were found in 8 samples (8/108 = 7%) and in additional 5 CNVs parental samples have not been analysed yet. Excluding karyotypically abnormal cases clinically relevant CNVs were found in 10% of fetuses (9/94). In all cases with de novo chromosomal aberration the clinical relevancy was clarified (imbalances in 50%). Conclusion: Our data suggest that SNP array analysis is a relevant and useful technique in prenatal diagnosis.
