UNLABELLED: Several mutations in this gene for the α subunit of the cardiac sodium channel have been identified in a heterogeneous subset of cardiac rhythm syndromes, including Brugada syndrome, progressive cardiac conduction defect, sick sinus node syndrome, atrial fibrillation and dilated cardiomyopathy. The aim of our study was to associate some SCN5A polymorphic variants directly with confirmed coronary stenoses in patients with non-LQTS ventricular fibrillation/flutter treated by an implantable cardioverter defibrillator. MATERIALS AND METHODS: A group of 32 unrelated individuals, aged 63 ± 12 years, was included in the study. All the patients were examined, diagnosed and treated with an implantable cardioverter defibrillator at the Department of Internal Cardiology Medicine, Faculty Hospital Brno. The control group included 87 persons of similar age without afflicted coronary circulation, which was confirmed coronagraphically. Genomic DNA was extracted from samples of peripheral blood according to the standard protocol. Two SCN5A polymorphisms-IVS9-3C/A (rs41312433) and A1673G (rs1805124, H558R)-were examined in association with coronary artery stenosis in the patients. RESULTS: In the case-control study, no significant differences in genotype distribution/allelic frequencies were observed for IVS9-3c>a and A1673G gene polymorphisms between patients with severe arrhythmias and healthy persons. The distribution of SCN5A double genotypes was not significantly different among different types of arrhythmias according to their ejection fraction in arrhythmic patients (p = 0.396). The ventricular arrhythmias with an ejection fraction below 40% were found to be 10.67 times more frequent in patients with multiple coronary stenosis with clinically valid sensitivity, specificity and power tests. In the genotype-phenotype study, we observed a significant association of both SCN5A polymorphisms with the stenosis of coronary vessels in the patients with severe arrhythmia. The double genotype of polymorphisms IVS9-3C/A together with A1673G (CCAA) as well as their simple genotypes were associated with significant multiple stenosis of coronary arteries (MVS) with high sensitivity and specificity (p = 0.05; OR = 5 (95% CI 0.99-23.34); sensitivity 0.70; specificity 0.682; power test 0.359) Moreover, when a concrete stenotic coronary artery was associated with SCN5A genotypes, the CCAA double genotype was observed to be five times more frequent in patients with significant stenosis in the right coronary artery (RCA) compared to those without affliction of this coronary artery (p = 0.05; OR = 5 (95% CI 0.99-23.34); sensitivity 0.682; specificity 0.700; power test 0.359). The CCAA genotype was also more frequent in patients without RCA affliction with MVS (p = 0.008); in patients with ACD affliction but without MVS (p = 0.008); and in patients with both ACD affliction and MVS compared to those without ACD affliction and MVS (p = 0.005). CONCLUSIONS: Our study presents a highly sensitive and specific association of two polymorphisms in SCN5A with significant coronary artery stenoses in patients with potentially fatal ventricular arrhythmias. At the same time, these polymorphisms were not associated with arrhythmias themselves. Thus, SCN5A gene polymorphic variants may form a part of germ cell gene predisposition to ischemia.

Genetika se v současné kardiologii uplatňuje zejména u kardiovaskulárních onemocnění s mendelovskou dědičností. Mezi tato onemocnění patří kardiomyopatie, dědičné formy arytmických syndromů, aortální syndromy a některé vrozené vývojové vady. Společným rizikem všech zmíněných skupin onemocnění je náhlá srdeční smrt, proto je důležitou součástí i posmrtné vyšetření a péče o příbuzné a pozůstalé. Cílem klinicko-genetického vyšetření a genetického poradenství v kardiologii je provést podrobný popis onemocnění - fenotypizaci, sestavení rodokmenu, kaskádový kardiologický screening v rodině a v indikovaných případech molekulárně-genetickou diagnostiku - genotypizaci. Znalost molekulární podstaty genetických onemocnění otvírá možnosti tzv. molekulárně genetické stratifikace onemocnění, jejímž cílem je personalizovaná péče o nemocné a jejich příbuzné. Kardiogenetické vyšetření je komplexní záležitost, kdy je třeba budovat mezioborové týmy. Komunikace mezi jednotlivými odborníky i týmy v národním měřítku je zcela zásadní pro zajištění péče o pacienty s často velmi vzácnou až raritní formou dědičného onemocnění.
In contemporary cardiology, genetics is mainly applied to cardiovascular diseases with Mendelian inheritance. These diseases include cardiomyopathy, hereditary forms of arrhythmic syndromes, aortic syndromes, and some congenital developmental defects. The common risk of all mentioned groups of diseases is sudden cardiac death, which is why post-mortem examination and care for relatives and survivors are an important part. The goal of clinical genetic examination and genetic counselling in cardiology is to carry out a detailed description of the disease - phenotyping, compiling a family tree, cascade cardiology screening in the family and, in indicated cases, molecular genetic diagnosis - genotyping.

INTRODUCTION: Reperfusion therapy by primary percutaneous coronary intervention (PPCI) is generally indicated in patients suffering from acute myocardial infarction (MI) with ST-segment elevation (STEMI). Prior to hospital admission, full ST-segment resolution (fSTR) may occur. Optimal management of such patients with transient STEMI (TSTEMI) is potentially challenging. Our aim was to evaluate the hypothesis that in TSTEMI patients, patency of infarct related artery (IRA) is achieved before PPCI, and to compare the outcome of TSTEMI and STEMI patients during a prolonged follow-up. MATERIAL AND METHODS: Three hundred consecutive adult STEMI patients were referred to catheterization laboratory. In all patients, standard 12 lead ECGs were obtained both at the time of the first medical contact, and on catheterization laboratory admission. RESULTS: TSTEMI occurred in 20 patients (6.7%). Despite fSTR (isoelectric ST segment), occluded IRA was found in 5 of these patients (25%). Pre-PPCI TIMI flow grade 2 was found in 6 TSTEMI patients (30%). Troponin T value at 24 h after symptom onset was lower in the TSTEMI group (1.8±2.5 mg/L vs. 3.6±3.5 mg/L, P=0.008). These patients also had a lower value of brain natriuretic peptide (156.3±119.5 ng/L vs. 438.5±429.0 ng/L, P<0.001) and higher left ventricular ejection fraction (59.9±6.3% vs. 51.6±10.2%, P<0.001). All patients were followed for a median of 5.6 years during which the overall survival did not differ between the TSTEMI and STEMI groups. CONCLUSION: Primary PCI is strongly recommended in TSTEMI patients because of a relatively high incidence of occluded infarct related arteries. The rate of patients with TSTEMI is relatively low.

AIMS: Fragmented QRS complex with visible notching on standard 12-lead electrocardiogram (ECG) is understood to represent depolarization abnormalities and to signify risk of cardiac events. Depolarization abnormalities with similar prognostic implications likely exist beyond visual recognition but no technology is presently suitable for quantification of such invisible ECG abnormalities. We present such a technology. METHODS AND RESULTS: A signal processing method projects all ECG leads of the QRS complex into optimized three perpendicular dimensions, reconstructs the ECG back from this three-dimensional projection, and quantifies the difference (QRS 'micro'-fragmentation, QRS-μf) between the original and reconstructed signals. QRS 'micro'-fragmentation was assessed in three different populations: cardiac patients with automatic implantable cardioverter-defibrillators, cardiac patients with severe abnormalities, and general public. The predictive value of QRS-μf for mortality was investigated both univariably and in multivariable comparisons with other risk factors including visible QRS 'macro'-fragmentation, QRS-Mf. The analysis was made in a total of 7779 subjects of whom 504 have not survived the first 5 years of follow-up. In all three populations, QRS-μf was strongly predictive of survival (P < 0.001 univariably, and P < 0.001 to P = 0.024 in multivariable regression analyses). A similar strong association with outcome was found when dichotomizing QRS-μf prospectively at 3.5%. When QRS-μf was used in multivariable analyses, QRS-Mf and QRS duration lost their predictive value. CONCLUSION: In three populations with different clinical characteristics, QRS-μf was a powerful mortality risk factor independent of several previously established risk indices. Electrophysiologic abnormalities that contribute to increased QRS-μf values are likely responsible for the predictive power of visible QRS-Mf.

Mutations in genes encoding cardiac ionic channels and related proteins are identified in ~50% families with the long QT syndrome. Each family is usually characterized by its own mutation; the same mutation found in unrelated families living in the same region may represent the founder mutation. In our database, the same KCNQ1 mutation (c.926C>T; p.T309I-Kv7.1) was present in 5 putatively unrelated LQTS families. This project is aimed at verification of the hypothesis that T309I-Kv7.1 mutation is the founder mutation in our region. Using pedigree extension and genetic screening in T309I-Kv7.1 families, new mutation carriers will be identified and clinically investigated. The functional effect of the mutation will be revealed using biophysical analysis in wild type and mutant human IKs channels expressed in CHO cells, and mathematical simulations in a human ventricular cell model. These data will allow us to provide genotype and phenotype-guided therapeutic measures to prevent malignant arrhythmias and sudden cardiac death even in asymptomatic mutation carriers.
U ~50% rodin se syndromem dlouhého intervalu QT jsou nacházeny mutace v genech kódujících srdeční iontové kanály a související proteiny. Každá rodina má obvykle svou “vlastní” mutaci. Stejná mutace nacházená v nepříbuzných rodinách ze stejného regionu může představovat tzv. „founder mutation“. V naší databázi je stejná mutace c.926C>T; p.T309I-Kv7.1 genu KCNQ1přítomna v 5 nepříbuzných rodinách. Cílem projektu je potvrzení hypotézy, že mutace T309I-Kv7.1 představuje v našem regionu „founder mutation“. Rozšířením rodokmenů a genetickým screeningem v T309I-Kv7.1 rodinách budou identifikováni a klinicky vyšetření noví nosiči mutace. Funkční efekt mutace bude hodnocen biofyzikální analýzou u „wild type“ a mutovaného lidského IKs kanálu exprimovaného v CHO buňkách a matematickými simulacemi na lidskému modelu srdeční komorové buňky. Tato data umožní zavést genotypem a fenotypem řízená terapeutická opatření k prevenci maligních komorových arytmií a náhlé srdeční smrti i u asymptomatických nosičů mutace.

Tento přehledový článek shrnuje a porovnává metodiku a výsledky dostupných originálních prací zabývajících se hodnocením anatomických variant plicních žil u pacientů s fibrilací síní publikovaných v posledních dvou desetiletích. Existující studie ukazují, že znalost anatomických variant plicních žil je důležitým předpokladem pro úspěšně provedenou katétrovou ablaci fibrilace síní. V textu se věnujeme analýze a vzájemnému srovnání metodiky a výsledků použitých při hodnocení původních autorů.
This review summarizes and compares the methods and results of available original papers on the evaluation of anatomical variants of pulmonary veins in patients with atrial fibrillation published in the last two decades. The studies included show that an understanding of anatomical variants of pulmonary veins is an important prerequisite for successful catheter ablation of atrial fibrillation. The present text deals with an analysis and mutual comparison of the methods and results used in various evaluations of the original authors.
Syndrom Brugadových (BrS) je relativně vzácný hereditární arytmický syndrom, který je charakterizován typickým elektrokardiografickým obrazem ve svodech z pravého prekordia a rizikem maligních komorových arytmií. Popisujeme případ arytmické bouře u pacienta s BrS. Cílem kazuistiky je připomenout nutnost volby nezvyklé farmakoterapie izoprenalinem. Jedná se sice o situaci vzácnou, avšak život ohrožující, kdy tato znalost může být pro záchranu pacienta rozhodující.
The Brugada syndrome (BrS) is a relatively rare hereditary arrhythmic syndrome. It is characterized by typical electrocardiographic pattern in the right precordial leads and increased risk of malignant ventricular arrhythmias. We are describing a case of an arrhythmic storm in a patient with BrS. The aim of this report is to stress the necessary choice of uncommon pharmacotherapy with isoproterenol. In this rare, nevertheless life threatening situation, such knowledge could be substantial for patient rescue.

Přehledová práce se věnuje problematice arytmií v těhotenství. Je zaměřena prakticky na konkrétní aspekty diagnostiky a léčby podle nejnovějších verzí doporučených postupů. Řešení arytmií v těhotenství s sebou nese řadu specifických problémů, především nutnost zvažování možného vlivu léčby na plod. Nicméně v život ohrožujících situacích nemá toto hledisko ovlivňovat podání léku matce.
The review is dedicated to the topic of arrhythmias in pregnancy. A practical approach is emphasised and particular aspects of diagnostics and treatment are mentioned according to the most recent guidelines. The treatment of arrhythmias during pregnancy carries a number of specific problems, especially the need to consider the possible effect on the foetus. However, in life-threatening situations, this aspect should not influence the administration of the drug to the mother.
