Perianal fistulas adversely affect the patients' quality of life. The effect of the current conservative or surgical treatment is low especially in patients with Crohn's disease. The introduction of mesenchymal stem cells therapy in to the algorithm of fistulas management by their systemic or topical application could increase the chance of long-term healing. These procedures must be verified in an experimental study, due to limited number of patients. Coecostomy in rat is an ideal model for exploring this new treatment technique and also sufficient to get statistically evaluable intervention groups. The results of the experiment will create the scientific background for introducing this method into the clinical practice.
Perianální píštěle významně negativně ovlivňují kvalitu života nemocných. Efekt současné konzervativní i chirurgické léčby je nízký zvláště u pacientů s Crohnovou chorobou. Zavedení terapie mezenchymálními kmenovými buňkami pomocí jejich systémového či lokálního podání do algoritmu terapie píštělí by mohlo zvýšit šanci na dlouhodobé zhojení a zlepšit kvalitu jejich života. Tyto postupy je vzhledem k limitovanému počtu pacientů nutné ověřit v experimentální studii. Model cékostomie u laboratorního potkana je ideální pro zkoumání této nové léčby a vytvoření statisticky hodnotitelné sestavy. Výsledky experimentu budou podkladem k zavedení této metody do klinické praxe.

Východiska: Cílem práce je porovnat glomerulární filtraci měřenou izotopovou metodu (measured glomarular filtration rate – mGFR DTPA) a odhadovanou glomerulární filtraci (estimated GFR – eGFR). Glomerulární filtrace (GFR) je odhadovaná ze sérového kreatininu (eGFRcreatinine), sérového cystatinu C (eGFRcystatin C) a pomocí kombinované rovnice (eGFRcreatinine + cystatin C). Studie se zaměřuje na onkologické pacienty zvažované pro léčbu cis‑diamindichlorplatinou (cisplatin). Hodnotili jsme dopad různých GFR metod na redukci dávky cisplatiny. Materiál a metody: Studovaná populace byla tvořena 112 po sobě jdoucími pacienty z onkologického centra ve Zlíně v České republice, kteří byli zvažováni pro léčbu cisplatinou. mGFR DTPA byla provedena dynamickou renální scintigrafií využívající kyselinu diethyltriaminopentaaoctovou (DTPA) značenou izotopem technecia 99mTc. Kreatinin a cystatin C byly stanoveny nově standardizovanými metodami. Odhad GFR byl počítán podle The Chronic Kidney Disease Epidemiology (CKD‑EPI) rovnic, které byly vytvořeny v letech 2009 a 2012. Výsledky: Medián (mezikvartilové rozpětí) mGFR DTPA byl 1,335 ml/s/1,73 m2 (1,070–1,725). Medián eGFRcystatin C 1,195 ml/s/1,73 m2 (0,885–1,625) byl nižší než medián mGFR DTPA (p < 0,05). Medián eGFRcreatinine 1,460 ml/s/1,73 m2 (1,210–1,660) byl vyšší než mGFR DTPA (p < 0,05). Korelační analýza a Bland‑Altmanův rozdílový graf ukazují velké individuální rozdíly mezi mGFR DTPA a všemi eGFR. Závěr: Onkologičtí pacienti jsou specifická skupina pacientů, která se liší od všeobecné populace. Byly nalezeny významné individuální rozdíly mezi mGFR DTPA a všemi eGFR. To má velký dopad na detekci pacientů s CKD a potenciální úpravu dávky léků. Background: The aim of the study is to compare measured glomerular filtration rate by technetium radiolabled diethylene tiamine pentaacetic acid (mGFR DTPA) to estimated GFR (eGFR). Glomerular filtration rate (GFR) is estimated from serum creatinine (eGFRcreatinine), serum cystatin C (eGFRcystatin C) and by combined equation (eGFRcreatinine + cystatin C). This study focuses on oncology patients considered for treatment with cis‑diamminedichloroplatinum (cisplatin). We evaluated the impact of different GFR methods on the reduction of cisplatin dose. Patients and Methods: The study population consisted of 112 consecutive oncology patients from oncology center treated in the town of Zlin in the Czech Republic, who were considered for cisplatin treatment. mGFR DTPA was performed by dynamic renal 99mTc scintigraphy method using diethyltriaminepentaacetic acid. Creatinine and cystatin C were determined by newly standardized tests. Estimation of GFR was calculated using The Chronic Kidney Disease Epidemiology (CKD‑EPI) equations which were established in 2009 and 2012. Results: The median (interquartile range) of mGFR DTPA was 1.335 ml/s/1.73 m2 (1.070–1.725). The median of eGFRcystatin C 1.195 ml/s/1.73 m2 (0.885–1.625) was lower than mGFR DTPA (p < 0.05). The median of eGFRcreatinine 1.460 ml/s/1.73 m2 (1.210–1.660) was higher than mGFR DTPA (p < 0.05). Correlation analysis and Bland‑Altman plots show high individual differences between mGFR DTPA and all eGFR‘s. Conclusions: Oncology patients are a very special group of patients who differ from general population. There are significant individual differences between mGFR DTPA and all eGFR‘s, impacting detection rate of CKD and potential drug dosage adjustment.

Analysis of the proteome of malignant breast tumors. Determination of novel prognostic and predictive factors.The validity of the classical prognostic factors versus molecular prognostic factors will be determined. Statistical analysis of breast cancer patients using GUHA method. General Unary Hypotheses Automaton method of exploration data analysis.
Analýza proteomu zhoubných nádorů prsu. Zjišťování nových faktorů efektivity léčby a selhání léčby s cílem zlepšit dosavadní léčebné výsledky. Vztah nových faktorů a stávajících prognostických a prediktivních faktorů. Kultivace nádorů in vitro. Data získaná v rámci projektu budou zpracována metodou explorační analýzy dat GUHA. Tato metoda umožňuje automatickou tvorbu a testování hypotéz o skutečnostech podporovaných daty.

The aim of the project is to evaluate predictive significance of biological factors, including tumor phenotype, parameters of angiogenesis or immune response, in breast cancer patients treated by neoadjuvant chemotherapy. Immunohistochemical analysis ofselected parameters will be performed before and after therapy in a retrospective cohort, and data will also be collected prospectively. Expression of oncogenes will be evaluated along with leukocyte infiltration and parameters of neoangiogenesis. Prognostic and predictive significance of the evaluated parameters will be studied.
Cílem projektu je zhodnocení prediktivního významu biologických faktorů, včetně fenotypu nádoru, ukazatelů angiogeneze a imunitní odpovědi, u nemocných léčených neoadjuvantní chemoterapií pro karcinom prsu. Bude provedeno imunohistochemické vyšetření sledovaných parametrů před léčbou a po léčbě u retrospektivní kohorty i prospektivní sběr dat. Bude sledována exprese onkogenů, infiltrace nádoru leukocyty i parametry ukazující novotvorbu cév. Bude zhodnocen prognostický a prediktivní význam sledovaných parametrů.

Benefit of adjuvant trastuzumab in breast cancer has been reported in four randomized trials of phase III, and these results are consistent in showing improvement in disease-free survival (DFS). Current evidence for homogeneity of this DFS benefit in subgroups of patients with the different size of the primary HER2-positive tumor treated according to the HERA trial is reviewed. It is evident that current published evidence is insufficient to rule out that there is a cohort of patients with HER2-positive disease who do not achieve a reduction in the risk of recurrence by adjuvant treatment with trastuzumab after completion of previous adjuvant chemo- and radiotherapy. An alternative interpretation of results of the HERA trial currently available in two primary reports (1-year, and 2-year median follow- up, respectively) is discussed. The risk factors of central nervous system (CNS) metastases in breast cancer and problem of CNS metastases in HER2-positive tumors are briefly reviewed. A hypothesis on the relations between brain metastases, their risk factors, the size of the primary tumor, and their impact on the DFS in patients with HER2-positive tumors treated with adjuvant trastuzumab is proposed based on the results of the HERA trial. Altogether, some direct evidence is presented here based on the published results of the HERA trial, and still more indirect evidence based on the information on related topics in literature, to show that current clinical practice of adjuvant trastuzumab in mono-therapy, which is based on assumption that there is a homogeneous benefit as for disease-free survival for all sizes of primary HER2-positive tumors above 1 cm, may not be based on such firm evidence as is commonly presented.

A 73-year-old male with a history of bladder carcinoma presented with conjunctival melanoma that was excised. Multiple lung metastases were detected during diagnostic work-up. Because of uncertainty about the histology of the metastatic lung tumor, a biopsy was performed that revealed a third primary non-small cell carcinoma of the lung. This case illustrates the need to obtain histological verification in patients with conjunctival melanoma and distant metastases, especially if metastases are detected early in the follow-up.
