BACKGROUND: In recent decades, magnetic resonance imaging (MRI) has gained prominence as a standard diagnostic method for preoperative assessment in patients with anorectal malformations and a colostomy, with the potential to replace the classic fluoroscopic distal pressure colostogram (FDPC). Three MRI techniques are available: MRI-distal pressure colostogram with gadolinium (MRI-DPCG) or saline (MRI-DPCS) instillation into the colostomy and native MRI without colostomy instillation. OBJECTIVE: To evaluate and compare the diagnostic accuracy of MRI (native MRI, MRI-DPCG and MRI-DPCS) in the preoperative workup of boys with an anorectal malformation and a colostomy and to compare it to FDPC. MATERIALS AND METHODS: Sixty-two boys with preoperative MRI using one of the three approaches and 43 with FDPC met the inclusion criteria for this retrospective study. The presence and localization of rectal fistulas according to the Krickenbeck classification were evaluated and compared with intraoperative findings. RESULTS: The accuracy of fistula detection for MRI in general (regardless of the technique), MRI-DPCS, MRI-DPCG, native MRI and FDPC was 95% (59/62, P<0.001), 100% (12/12, P=0.03), 100% (30/30, P<0.001), 85% (17/20, P=0.41) and 72% (31/43, P=0.82), respectively. The accuracy of describing fistula type in patients with a correctly detected fistula using these methods was 96% (45/47, P<0.001), 100% (9/9, P<0.001), 100% (23/23, P<0.001), 87% (13/15, P<0.001) and 67% (13/21, P=0.002), respectively. CONCLUSION: MRI is a reliable method for detecting and classifying fistulas in boys with an anorectal malformation and a colostomy and can be considered the modality of first choice for preoperative workup.

Předkládáme kazuistiku mladé ženy s komplikovaným průběhem idiopatického střevního zánětu s těžkým luminálním a perianálním postižením. Pacientka byla nejprve léčena pro ulcerózní kolitidu, pro refrakterní průběh podstoupila proktokolektomii s konstrukcí ileopouchanální anastomózy. Přibližně rok po uzavření pojistné stomie se na počátku gravidity manifestovaly perianální projevy, které vedly ke změně diagnózy na Crohnovu chorobu. Během těhotenství pacientka podstoupila opakované drenáže perianálních píštělí, chirurgickou revizi pro ileus a byla zahájena léčba ustekinumabem. V listopadu 2022 pacientka porodila ve 38. týdnu zdravého chlapce.
We present a case report of a young female patient with complicated course of inflammatory bowel disease with severe luminal, as well as perianal symptoms. The patient was initially diagnosed with ulcerative colitis and underwent proctocolectomy with ileopouch-anal anastomosis. One year after oostomy closure she got pregnant and, at the same time, she developed perianal abscess and fistula. The diagnosis was thus changed to Crohn’s disease. During pregnancy, she underwent repeated perianal fistula drainages, surgical revision of ileus and treatment with ustekinumab was started. The patient delivered a healthy boy in November 2022 at 38th gestational week.

Úvod: Análna fistula je častým ochorením s incidenciou 5,6 na 100 000 žien a 12,3 mužov. Najčastejšie je kryptoglandulárneho pôvodu. Cieľom tejto štúdie je zhodnotiť naše skúseností s pacientmi liečenými pre komplexné análne fistuly vlastným komplexným operačným výkonom s posuvným endorektálnym lalokom. Metódy: Do štúdie sme zaradili 524 pacientov s komplexými análnymi fistulami, ktorí boli odoslaní na našu chirurgickú kliniku od januára 2005 do konca decembra 2022. Diagnózu sme stanovili detekciou traktu fistuly fyzikálnym vyšetrením, anoskopiou, sondážou traktu fistuly a endorektálnou ultrasonografiou. Komplexný operačný výkon spolu s konštrukciou Posuvného endorektálneho laloku vo vlastnej modifkácii sme realizovali u všetkých pacientov. Výsledky: Primárna operačná intervencia v súbore 326 pacientov s komplexnými análnymi fistulami (bez pacientov s morbus Crohn) bola úspešná u 283 (87 %) pacientov. Defekt posuvného endorektálneho laloku v pooperačnom období sme identifikovali u 17 (5,2 %) pacientov, soiling u 16 (4,9 %) a inkontinenciu pre flatus u 9 (2,7 %) pacientov. V súbore 120 pacientov po viacerých operačných výkonoch (bez pacientov s morbus Crohn) bol náš operačný postup úspešný u 92 (76,6 %) pacientov. Defekt posuvného endorektálneho laloku sme v pooperačnom období identifikovali u 6 (5 %) pacientov, soiling u 8 (6,6 %) a inkontinenciu pre flatus u 3 (2,5 %) pacientov. Záver: Konštrukcia posuvného endorektálneho laloku bola kuratívna a bez ovplyvnenia úrovne análnej kontinencie u 87 % pacientov po primárnej operačnej intervencii a u 76,6 % po viacerých operačných výkonoch. Komplexný operačný výkon s konštrukciou posuvného endorektálneho laloku podľa nášho postupu zachováva funkciu sfinkterov a má relatívne nízke percento recidív. Počet predchádzajúcich operačných intervencií nemal žiadny vplyv na úroveň análnej kontinencie.
Introduction: Anal fistula is a common disease with incidence of 5.6 per 100,000 women and 12.3 men. It is most often of cryptoglandular origin. The aim of this study is to evaluate our experience with patients treated for complex anal fistula with our own complex surgical procedure with advancement endorectal flap. Methods: 524 patients with complex anal fistulas who were sent to our surgical clinic from January 2005 to the end of December 2022 were in- cluded in the study. We established the diagnosis by detecting the fistula tract by physical examination, anoscopy, probing the fistula tract and endorectal ultrasonography. We performed a complex operation together with the construction of the advancement endorectal flap in our own modification for all patients. Results: Primary surgical intervention in a group of 326 patients with complex anal fistulas (excluding patients with Crohn‘s disease) was successful in 283 (87%) patients. We identified advancement endorectal flap defect in the postoperative period in 17 (5.2%) patients, soiling in 16 (4.9%) and flatus incontinence in 9 (2.7%) patients. In a group of 120 patients after multiple surgeries (excluding patients with Crohn‘s disease), our surgical procedure was successful in 92 (76.6%) patients. In the postoperative period, we identified a advancement endorectal flap defect in 6 (5%) patients, soiling in 8 (6.6%) and flatus incontinence in 3 (2.5%) patients. Conclusions: The construction of the advacement endorectal flap was curative and without affecting the level of anal continence in 87% of patients after primary surgical intervention and in 76.6% after multiple surgical procedures. Complex surgery with the construction of the advancement endorectal flap according to our procedure preserves the function of the sphincters and has a relatively low percentage of recurrences. The number of previous surgical interventions had no affect on the level of anal continence.

Úvod: Idiopatické střevní záněty jsou onemocnění, jejichž incidence celosvětově stoupá. Navzdory pokroku v konzervativní i chirurgické léčbě významná část pacientů nedosáhne kompletní remise a musí podstoupit opakované operace. Léčba kmenovými buňkami (MSC) představuje novou alternativu imunomodulační léčby. Metody: Provedli jsme systematický přehled literatury zabývající se použitím mezenchymálních kmenových buněk k léčbě IBD. K jejich vyhledávání byla použita databáze PubMed, zařazeny byly původní studie publikované mezi lety 2015 a 2021. Výsledky: Nalezeno bylo celkem 80 citací, z nich po výběru bylo do přehledu zahrnuto 21 klinických studií a 47 experimentálních studií. Závěr: Účinnost MSC v léčbě perianální Crohnovy choroby je přinejmenším srovnatelná se současnými léčebnými metodami. Pozitivní efekt byl rovněž prokázán na zvířecích modelech luminální choroby nebo enterokutánní píštěle. Výsledky po systémovém podání u IBD pacientů jsou zatím nejednoznačné.
Introduction: The incidence of inflammatory bowel disease is increasing worldwide and, despite advances in conservative and surgical treatment, a significant proportion of patients do not achieve complete remission and have to undergo repeated surgical procedures. Stem cell therapy is a new alternative to immunomodulatory therapy. Methods: We performed a systematic review of the literature dealing with the use of mesenchymal stem cells for the treatment of IBD. The PubMed database was searched and original studies published between 2015 and 2021 were included. Results: A total of 80 citations were found, of which 21 clinical studies and 47 experimental studies were included in the review. Conclusion: The efficacy of MSCs in the treatment of perianal Crohn‘s disease is at least comparable to current therapies. A positive effect of MSC administration has also been demonstrated in animal models of luminal disease or enterocutaneous fistula. The results after systemic administration in IBD patients are still ambiguous. Key words: inflammatory bowel disease,
Biliární ileus je vzácná komplikace žlučových kamenů a je unikátní příčinou střevní obstrukce. Spočívá ve vzniku cholecystoenterické píštěle, přes kterou se žlučový kámen dostane do zažívacího traktu. Spontánní úprava střevní obstrukce při biliárním ileu je extrémně vzácná. Popisujeme případ 71leté pacientky vyšetřené pro čtyřměsíční bolest v pravém hypochondriu. Udávala třídenní anamnézu úplné zácpy a abdominální distenze před dvěma měsíci, která se spontánně upravila. CT vyšetření popsalo pneumobilii. Při explorativní laparoskopii byla zjištěná cholecystoduodenální píštěl, která se ošetřila přerušením, ústí v první části duodena se uzavřelo a byla provedena úspěšná cholecystektomie. Přítomnost pneumobilie u pacienta se žlučovými kameny by mělo vést k podezření na bilioenterickou píštěl. Ve vzácných případech se střevní obstrukce při biliárním ileu vyřeší spontánně. Primární laparoskopické ošetření cholecystoduodenální píštěle je bezpečné a proveditelné v přítomnosti zkušeného laparoskopického chirurga.
Gallstone ileus is a rare complication of gallstones and is a comparatively unique cause of intestinal obstruction. It involves the development of a cholecysto-enteric fistula through which a gallstone can pass into the gastrointestinal tract. Spontaneous resolution of intestinal obstruction in gallstone ileus is extremely rare. We report a 71-year-old patient who presented with right hypochondrial pain for four months. She had a three-day history of absolute constipation and abdominal distention two months before presentation that resolved spontaneously. Computed tomography revealed pneumobilia. Laparoscopic exploration showed a cholecysto-duodenal fistula that was divided, the opening in the first part duodenum was closed and cholecystectomy was completed successfully. The presence of pneumobilia in a patient with gallstones should raise the suspicion of bilio-enteric fistula. The obstruction component of gallstone ileus can resolve spontaneously in rare occasions. Single stage laparoscopic management of cholecysto-duodenal fistula is safe and feasible in the presence of an experienced laparoscopic surgeon.

BACKGROUND: Injection of autologous adipose tissue (AT) has recently been demonstrated to be an effective and safe treatment for anal fistulas. AT mesenchymal stem cells (AT-MSCs) mediate the healing process, but the relationship between molecular characteristics of AT-MSCs of the injected AT and fistula healing has not been adequately studied. Thus we aimed to characterize the molecular and functional properties of AT-MSCs isolated from autologous AT injected as a treatment of cryptogenic high transsphincteric perianal fistulas and correlate these findings to the healing process. METHODS: 27 patients (age 45 ± 2 years) diagnosed with perianal fistula were enrolled in the study and treated with autologous AT injected around the anal fistula tract. AT-MSCs were isolated for cellular and molecular analyses. The fistula healing was evaluated by MRI scanning after 6 months of treatment. AT-MSC phenotype was compared between responders and non-responders with respect to fistula healing. RESULTS: 52% of all patients exhibited clinical healing of the fistulas as evaluated 6 months after last injection. Cultured AT-MSCs in the responder group had a lower short-term proliferation rate and higher osteoblast differentiation potential compared to non-responder AT-MSCs. On the other hand, adipocyte differentiation potential of AT-MSCs was higher in non-responder group. Interestingly, AT-MSCs of responders exhibited lower expression of inflammatory and senescence associated genes such as IL1B, NFKB, CDKN2A, TPB3,TGFB1. CONCLUSION: Our data suggest that cellular quality of the injected AT-MSCs including cell proliferation, differentiation capacity and secretion of proinflammatory molecules may provide a possible mechanism underlying fistula healing.
