Úvod: Močová exkrece magnezia je důležitý preventivní faktor chránící před vznikem nefrolitiázy, a to inhibicí několika klíčových dějů v její patogenezi. Pacienti s Crohnovou chorobou (CD) mají riziko litiázy přibližně 2krát vyšší, zejména pacienti po ileokolické (IC) resekci. Cílem je stanovit hladinu magnesurie u těchto pacientů a porovnat ji s pacienty s IC postižením bez resekce a poté obě skupiny se zdravými kontrolami. Sekundárním cílem bylo posouzení kalciurie a dalších modifikujících faktorů. Metody: Do studie byli zařazeni pacienti s CD starší 18 let s IC resekcí (skupina 1) a s postižením terminálního ilea bez resekce (skupina 2) a jako kontrolní skupina osoby bez známého střevního onemocnění (skupina 3). Vylučovacími kritérii byla terapie citráty, závažná renální insuficience (GFR < 30 ml/min/1,73 m2), více než dvě střevní resekce, ileostomie, kolektomie, syndrom krátkého střeva, akutní infekce močových cest a známky relapsu CD. Anamnestické údaje byly zjišťovány dotazníkem, byl odebrán vzorek krve a čerstvé moči, bylo provedeno ultrazvukové vyšetření ledvin a žlučníku na přítomnost litiázy a u pacientů byl proveden 24hodinový sběr moči ke stanovení oxalurie, citraturie, magnesurie a kalciurie. Výsledky: Do studie bylo zařazeno 107 osob, z toho 34 pacientů po resekci IC, 42 s CD bez resekce a 31 zdravých kontrol. Ženy tvořily 43 %, průměrný věk byl 38 ± 11,5 let. V hodnotách magnesurie byl prokázán signifikantní rozdíl mezi skupinou s resekcí a bez resekce (medián 2,28 vs. 3,97 mmol/l; p = 0,047) a zejména mezi skupinou s resekcí a zdravými kontrolami (medián 2,28 vs. 4,31 mmol/l; p = 0,0003). Skupina bez resekce proti zdravým kontrolám nedosáhla signifikantního rozdílu (medián 3,97 vs. 4,31 mmol/l; p = 0,455). Hodnoty kalciurie se mezi skupinami významně nelišily (medián 3,75 vs. 4,6 vs. 4,3 mmol/l; p = 0,293). Závěr: Hodnoty magnesurie pacientů s CD po IC resekci jsou významně nižší proti skupině pacientů s CD s IC postižením bez provedené resekce i od zdravých kontrol. Skupina bez resekce dosáhla výsledků srovnatelných s kontrolami. Hodnoty kalciurie se v naší práci mezi skupinami významně nelišily. Předpokládáme, že pacienti s CD po IC resekci s vyšším rizikem vzniku urolitiázy by mohli profitovat ze suplementace magnezia v rámci prevence tvorby konkrementů. Potvrzení této teze bude ovšem vyžadovat ověření dalším výzkumem.
Urinary excretion of magnesium is an important preventive factor against nephrolithiasis by inhibiting several key processes in its pathogenesis. Patients with Crohn’s disease (CD) have an approximately 2-fold higher risk of lithiasis, especially those after ileocolic (IC) resection. The aim is to determine the magnesuria level in these patients and compare it with patients with CD and IC involvement without resection and then both groups with healthy controls. The secondary objective was to assess calciuria and other modifying factors. Methods: CD patients aged 18 years or older with IC resection (group 1) and terminal ileal involvement without resection (group 2) were enrolled in the study, with subjects without known bowel disease as controls (group 3). Exclusion criteria were citrate therapy, severe renal insufficiency (GFR < 30 ml/min/1.73 m2), more than two bowel resections, ileostomy, colectomy, short bowel syndrome, acute urinary tract infection and evidence of CD relapse. Anamnestic data were collected by questionnaire, blood and fresh urine samples were collected, renal and gallbladder ultrasound was performed for the presence of lithiasis, and patients underwent 24-hour urine collection to determine oxaluria, citraturia, magnesuria and calciuria. Results: 107 subjects were included in the study, including 34 patients with IC resection, 42 with CD without resection and 31 healthy controls. 43% were women, mean age was 38 ± 11.5 years. There was a significant difference in magnesuria values between the resection and non-resection group (median 2.28 vs. 3.97 mmol/l; P = 0.047) and especially between the resection group and healthy controls (median 2.28 vs. 4.31 mmol/l; P = 0.0003). The group without resection vs. healthy controls did not reach a significant difference (median 3.97 vs. 4.31 mmol/l; P = 0.455). Calciuria values did not differ significantly between groups (median 3.75 vs. 4.6 vs. 4.3 mmol/l; P = 0.293). Conclusion: Magnesuria values of CD patients after IC resection were significantly lower compared to the group of CD patients with IC involvement without resection and healthy controls. The group without resection achieved results comparable to controls. Calciuria values were not significantly different between groups in our study. We hypothesize that patients with CD after IC resection at higher risk of urolithiasis might benefit from Mg supplementation to prevent concretion formation. However, confirmation of this thesis will require verification by further research.
OBJECTIVE: Crohn's disease (CD) can be associated with a wide range of extraintestinal manifestations (EIMs), including neurological ones. Published studies differ in their conclusions about the epidemiology and etiopathogenesis of neurological EIMs. The aims of this study were to demonstrate the presence and find risk factors of peripheral (somatic and autonomic) neuropathy patients with severe CD on anti-TNFα biological therapy. MATERIAL AND METHODS: A clinical examination focusing on detection of peripheral sensor-motor nervous dysfunction (including Sudoscan) and examination of autonomic nervous system dysfunction (using Ewing ́s battery tests and spectral analysis) together with laboratory tests and collection of demographic data followed by administration of questionnaires were performed on a total of 30 neurologically asymptomatic outpatients with severe CD on anti-TNFα biological therapy. RESULTS: Peripheral sensor-motor nervous function via clinical neurological examination was pathological in 36.7% and Sudoscan in 33.3% of cases. Statistically significant associations between vibration perception test and age, CD and biological therapy duration, body mass index and Crohn's Disease Activity Index were proved while statistically significant associations between temperature perception test and age and BMI were proved as well. Additionally, a decrease of total protein in a patient ́s serum below the physiological cut-off in the 6 months prior to measurement was associated with a pathological result of a Sudoscan. Cardiovascular autonomic neuropathy based on Ewing ́s battery tests was present in 56.7% of patients, no statistically significant risk factors were found. Our peripheral neuropathy questionnaire correlated with the results of the Sudoscan test and some tests of the clinical examination of peripheral sensor-motor nervous function (discriminatory contact perception test, temperature perception test). CONCLUSIONS: This study demonstrated a relatively high prevalence of peripheral (especially autonomic) neuropathy and verified some risk factors for the development of peripheral somatic neuropathy in asymptomatic patients with severe form of CD on anti-TNFα biological therapy.

BACKGROUND: Maintaining of remission early in the disease course of Crohn's disease (CD) is essential and has major impact on the future prognosis. This study aimed to identify baseline predictors to develop model allowing stratification of patients who will not benefit from long-term azathioprine (AZA) treatment and will require more intensive therapy. METHODS: This study was designed to develop clinical prediction rule using retrospective data analysis of pediatric CD patients included in prospective inception cohort. Clinical relapse was defined as necessity of re-induction of remission. Sequence of Cox models was fitted to predict risk of relapse. RESULTS: Out of 1190 CD patients from 13 European centers, 441 were included, 50.3% patients did not experience clinical relapse within 2 years of AZA treatment initiation. Median time to relapse was 2.11 (CI 1.59-2.46) years. Of all the tested parameters available at diagnosis, six were significant in multivariate analyses: C-reactive protein (p = 0.038), body mass index Z-score >0.8 SD (p = 0.002), abnormal sigmoid imaging (p = 0.039), abnormal esophageal endoscopy (p = 0.005), ileocolonic localization (p = 0.023), AZA dose in specific age category (p = 0.031). CONCLUSIONS: Although the possibility of predicting relapse on AZA treatment appears limited, we developed predictive model based on six baseline parameters potentially helpful in clinical decision. IMPACT: The possibility of predicting relapse on AZA treatment appears to be possible but limited. We identified six independent predictors available at diagnosis of early AZA/6-MP treatment failure in pediatric CD patients. Using combination of these factors, a model applicable to clinical practice was created. A web-based tool, allowing estimation of individual relapse risk in pediatric CD patients on a particular therapeutic regimen, has been developed.

Následující kazuistika prezentuje pacienta, u něhož byl diagnostikován adenokarcinom tenkého střeva asociovaný s onemocněním morbus Crohn. Karcinom střeva je obávanou a vzácnou komplikací idiopatických střevních zánětů. Mezi nejdůležitější faktory ke vzniku dysplazie patří extenzivní postižení střeva a dlouhodobý průběh zánětu.
This case report presents a patient diagnosed with small intestine adenocarcinoma associated with Crohn´s disease. Intestinal cancer is a feared and rare complication of idiopathic bowel diseases. The most important factors for dysplasia include extensive involvement of the intestine and a long-term inflammatory process.

Karcinom z prstenčitých buněk (signet ring cell carcinoma – SRCC) je vzácný typ vysoce maligního adenokarcinomu, který produkuje mucin a postihuje převážně žaludek, vaječníky a tenké střevo. Primární lokalizace SRCC v tračníku je raritní a prognóza tohoto typu nádoru je vysoce nepříznivá. Prezentujeme kazuistiku 44leté ženy léčené pro Crohnovou chorobou od roku 2000 a poslední tři roky na biologické terapii ustekinumabem s rozvojem klinické, laboratorní a endoskopické remise. Pacientka byla v počátkem roku 2021 došetřována pro nově vzniklé pasážové obtíže, koloskopicky bylo zjištěno stenotické postižení tračníku (sigmatu) a z histologického vzorku verifikován primární kolorektální karcinom z prstenčitých buněk, který v tomto případě klinicky imitoval aktivitu Crohnovy choroby. Vzhledem k metastatickému charakteru postižení již v době diagnózy (játra, pobřišnice) a k přetrvávajícím symptomům poruchy střevní pasáže bylo přistoupeno k ileostomii jako definitivnímu chirurgickému řešení a byla zahájena paliativní chemoterapie. Prognóza pacientky byla infaustní a nemocná zemřela do 5 měsíců od stanovení diagnózy v hospicové péči.
Signet ring cell carcinoma (SRCC) is a rare type of highly malignant adenocarcinoma that produces mucin and predominantly affects the stomach, ovaries, and the small intestine. Primary localisation in the colon is rare, and the prognosis of this type of adenocarcinoma is very poor. We present a case report of a 44-year-old woman who suffered from Crohn‘s disease beginning in the year 2000. During the last three years of her treatment, she was treated using biological therapy with ustekinumab. The treatment led to clinical, laboratory, and endoscopic remission. In February 2021, she had widely metastatic primary signet ring cell carcinoma of the colon, which mimicked the IBD activity in the case of her colon obstruction. Because of the extensive infiltration of her liver and peritoneum at the time of the diagnosis, in addition to the symptoms of the passage disorders she was experiencing, she proceeded with an end ileostomy and palliative treatment was started. The patient‘s prognosis was unfavourable, and she died within 5 months of the diagnosis in hospice care.

Předkládáme kazuistiku mladé ženy s komplikovaným průběhem idiopatického střevního zánětu s těžkým luminálním a perianálním postižením. Pacientka byla nejprve léčena pro ulcerózní kolitidu, pro refrakterní průběh podstoupila proktokolektomii s konstrukcí ileopouchanální anastomózy. Přibližně rok po uzavření pojistné stomie se na počátku gravidity manifestovaly perianální projevy, které vedly ke změně diagnózy na Crohnovu chorobu. Během těhotenství pacientka podstoupila opakované drenáže perianálních píštělí, chirurgickou revizi pro ileus a byla zahájena léčba ustekinumabem. V listopadu 2022 pacientka porodila ve 38. týdnu zdravého chlapce.
We present a case report of a young female patient with complicated course of inflammatory bowel disease with severe luminal, as well as perianal symptoms. The patient was initially diagnosed with ulcerative colitis and underwent proctocolectomy with ileopouch-anal anastomosis. One year after oostomy closure she got pregnant and, at the same time, she developed perianal abscess and fistula. The diagnosis was thus changed to Crohn’s disease. During pregnancy, she underwent repeated perianal fistula drainages, surgical revision of ileus and treatment with ustekinumab was started. The patient delivered a healthy boy in November 2022 at 38th gestational week.

AIMS: i] To evaluate the evolution of pregnancies and offspring after inflammatory bowel disease [IBD] surgery during pregnancy; and ii] to describe the indications, the surgical techniques, and the frequency of caesarean section concomitant with surgery. METHODS: Patients operated on due to IBD during pregnancy after 1998 were included. Participating clinicians were asked to review their databases to identify cases. Data on patients' demographics, IBD characteristics, medical treatments, IBD activity, pregnancy outcomes, surgery, delivery, and foetal and maternal outcomes, were recorded. RESULTS: In all, 44 IBD patients were included, of whom 75% had Crohn's disease; 18% of the surgeries were performed in the first trimester, 55% in the second, and 27% in the third trimester. One patient had complications during surgery, and 27% had postsurgical complications. No patient died. Of deliveries, 70% were carried out by caesarean section. There were 40 newborns alive. There were four miscarriages/stillbirths [one in the first, two in the second, and one in the third trimester]; two occurred during surgery, and another two occurred 2 weeks after surgery; 14% of the surgeries during the second trimester and 64% of those in the third trimester ended up with a simultaneous caesarean section or vaginal delivery. Of the 40 newborns, 61% were premature and 47% had low birth weight; 42% of newborns needed hospitalisation [25% in the intensive care unit]. CONCLUSIONS: IBD surgery during pregnancy remains an extremely serious situation. Therefore, surgical management should be performed in a multidisciplinary team, involving gastroenterologists, colorectal surgeons, obstetricians, and neonatal specialists.
