Various medicinal plants find their use in cough treatment, based on traditions and long-term experience. Pharmacological principles of their action, however, are much less known. Herbal drugs usually contain a mixture of potentially active compounds, which can manifest diverse effects. Expectorant or antitussive effects, which can be accompanied by others, such as anti-inflammatory or antibacterial, are probably the most important in the treatment of coughs. The aim of this review is to summarize the current state of knowledge of the effects of medicinal plants or their constituents on cough, based on reliable pharmacological studies. First, a comprehensive description of each effect is provided in order to explain the possible mechanism of action in detail. Next, the results related to individual plants and substances are summarized and critically discussed based on pharmacological in vivo and in vitro investigation.

AIMS: Sufficient visibility of the mucosa during upper endoscopy is crucial for successful diagnosis, especially for early neoplastic lesions. Data documenting the effect of administration of mucolytic solution prior to gastroscopy in order to improve mucosal visibility are limited in Europe. The aim of the study was to assess the score of mucosal visibility in the upper gastrointestinal tract after administration of the mucolytic solution defined by us. PATIENTS AND METHODS: This is a monocentric, double-blind, randomized study involving 134 patients indicated for diagnostic upper endoscopy. Patients were randomly assigned to one of three arms, with mucolytic solution (100 mL water + 400 mg N-acetylcysteine + 20 mg simethicone), without the solution , and with 100 mL pure water. During the examination, 11 photographs were taken in defined areas. The visibility score was given by the sum of the score 0-5 from 5 defined localities evaluated by a blinded endoscopist and subsequently by two blinded endoscopists. Other parameters monitored were examination time and a semiquantitative evaluation of residual gastric fluid. RESULTS: The basic characteristics of the group (sex, age, indications for examination) were comparable between arms. The visibility score was similar in all arms - 17.4 ± 1.9 vs. 17.0 ± 2.0 vs. 17.6 ± 1.8 (P=0.32). The examination time and the amount of residual fluid in the stomach were comparable in all arms. CONCLUSIONS: Administration of the mucolytic solution in our study did not increase the mucosal visibility score in the esophagus, stomach and duodenum. OBJECTIVE: The aim of the study was to demonstrate that an administration of mucolytic solution with a maximum dose of simethicone and n -acetylcysteine before upper endoscopy improves mucosal visibility compared to a group without administration of mucolytic solution or water. METHODS: This study was a double-blind, randomized controlled trial. Patients were randomized into four groups, with the administration of 100 ml of water + 600 mg n -acetylcysteine + 400 mg simethicone, 100 ml of water + 400 mg n -acetylcysteine + 20 mg simethicone, 100 ml of water, and without any water or mucolytic solution. During the examination, a total of 10 images were taken in the defined areas. The overall visibility score was given by the sum of the 0-5 scores of the five areas and was assessed by the endoscopist performing the procedure and the blinded endoscopists using static images. RESULTS: A total of 129 patients were randomized. The group of patients did not differ in age, sex distribution, and indications significantly. The overall visibility score as assessed by the endoscopist performing the procedure was significantly higher in the group with the maximum dose of mucolytic solution compared to the group without solution or water (18.9 ± 2.9 vs. 16.6 ± 3.3, P = 0.023). This difference was not evident by the blinded evaluation of static photographs. CONCLUSION: Administration of mucolytic solution with a maximum dose of n -acetylcysteine and simethicone before upper endoscopy improved mucosal visibility in the upper gastrointestinal tract compared with the group without any preparation while evaluated by performing endoscopist.

Lékárna je zdravotnickým zařízením, do kterého pacienti přicházejí ještě před samotnou návštěvou lékaře, a to s mírnými akutními či chronickými obtížemi spojenými s kašlem. Role farmaceuta při poradenské činnosti v rámci samoléčby je klíčová, neboť bez správného zhodnocení stavu pacienta je obtížné zvolit správnou léčbu a režimová opatření. Na trhu je v současné době k dispozici široká paleta volně prodejných léčivých přípravků, o nichž by měl mít farmaceut i farmaceutický asistent přehled a ovládat jejich indikace, kontraindikace, dávkování a potenciální rizika. Cílem článku je uvést možnosti zhodnocení pacientova stavu a vytvořit aktuální přehled volně prodejných léčivých přípravků, jejich lékových forem a dávkovacích schémat, které lze pro symptomatologickou léčbu suchého dráždivého a produktivního kašle v lékárně zvolit.
A pharmacy is a medical facility to which patients come even before the doctor's visit, either with mild acute or chronic cough-related problems. The pharmacist's role in self-medication counseling is crucial, because without proper evaluation of the patient's condition, it is difficult to choose the right treatment and regimen measures. There is currently a wide variety of over-the-counter medicinal products available on the market and the pharmacist and pharmaceutical assistant should be aware of their indications, contraindications, dosage regiment and potential risks. The aim of the article is to present the possibilities of evaluating the patient's condition and to create an up-to-date overview of over-the-counter medicinal products, their dosage forms and dosage schemes, which can be chosen in the pharmacy for the symptomatic treatment of dry and productive cough.

Erdostein patří mezi běžně používaná mukolytika. Mechanismus účinku této látky spočívá v ovlivnění viskozity hlenu, jeho tvorby a také mukociliárního transportu. Zvláštností erdosteinu je ale také antioxidační, protizánětlivé či antibakteriální působení. Článek zmíněné charakteristiky diskutuje v kontextu aktuálních informací s cílem zaměřit se i na dětskou populaci.
Erdosteine belongs to the commonly used mucolytic. The mechanism of action of this substance lies in regulating the mucus viscosity, its formation and also mucociliary transport. However, the peculiarity of erdosteine consists in its antioxidant, anti-inflammatory and antibacterial effect. The article discusses these characteristics in the context of current information with a focus also on the paediatric population.

Odborné poradenství u nezávažných akutních respiračních infekcí patří do častých kompetencí lékárníka. Respirační infekce jsou jedno z nejčastějších infekčních onemocnění postihující jak děti, tak i dospělé. Akutní kašel pravidelně v prvních dnech provází respirační infekční onemocnění. Paletu vhodných léčiv pro samoléčení kašle v posledních letech rozšířil i erdostein v lékové formě granulátu pro přípravu suspenze. Erdostein má širokou škálu zajímavých farmakologických vlastností významných pro léčbu onemocnění dýchacích cest. Součástí racionálního samoléčení je i doporučení imunomodulačních postupů.
Expert advice on non-serious acute respiratory infections is a common competence of a pharmacist. Respiratory infections are one of the most common infectious diseases affecting both children and adults. An acute cough is regularly accompanied by a respiratory infectious disease in the first days. In recent years, the palette of suitable medicines for self-treatment of cough has been expanded by erdosteine in the pharmaceutical form of granules for the preparation of a suspension. Erdosteine has a wide range of interesting pharmacological properties relevant for the treatment of respiratory diseases. Part of rational self-treatment is also the recommendation of immunomodulating procedures.
Infekce horních cest dýchacích patří mezi nejčastější onemocnění, se kterými přichází pacienti do lékárny. Lékárník či farmaceutický asistent by měl na základě dostupných a získaných informací o pacientovi zhodnotit jeho stav a posoudit vhodnost samoléčení. V případě samoléčení by měl pacientovi doporučit vhodné přípravky zvolené individuálně dle jednotlivých symptomů nemoci a následně vysvětlit, jak s doporučenými přípravky zacházet a jakými opatřeními maximalizovat jejich účinek a případně též minimalizovat jejich rizika.
Upper respiratory tract infections belong to the most common diseases that patients come to the pharmacy with. Based on the available and gathered information, the pharmacist or pharmaceutical assistant (PA) should assess the patient's condition and risk factors and determine the suitability for self-treatment. In that case, the pharmacist or PA should recommend an appropriate medicine based on the individual patient's symptoms. He or she should also explain how to administer the medication in order to maximize its effect or minimize its potential risks.
