Poorly differentiated chordoma is a newly recognized entity in the recent World Health Organization (WHO) classification of tumors of soft tissue and bone. Slightly over 60 such cases have been documented. Herein, we present a clinicopathological profile, including radiological features, of nine cases, which occurred in five males and four females, with age varying from 1 to 29 years (median = 43), in the cervical spine (n = 2), skull base (n = 2), clivus (n = 2), thoracic spine (n = 1) lumbar spine (n = 1) and coccyx (n = 1) Average tumor size was 4.8 cm. None of the 6-referral cases was diagnosed as a poorly differentiated chordoma at the referring laboratory. Histopathologically, all cases displayed a cellular tumor comprising polygonal cells (n = 9) displaying moderate to marked nuclear pleomorphism with prominent nucleoli (n = 7), eosinophilic (n = 9) to vacuolated cytoplasm (n = 7), rhabdoid morphology (n = 4), interspersed mitotic figures (n = 5), focal necrosis (n = 6) and inflammatory cells (n = 9). A single tumor displayed areas resembling classic chordoma, transitioning into poorly differentiated areas. There were multinucleate giant cells and physaliphorous cells in two tumors, each, respectively. Immunohistochemically, tumor cells were positive for AE1/AE3 (7/7), EMA (7/7), cytokeratin (CK) MNF116 (1/1), OSCAR (1/1), brachyury (9/9, diffusely), S100P (4/7, mostly focally), and glypican 3(2/4). SMARCB1/INI1 was completely lost in all nine tumors. A single case tested by FISH showed homozygous deletion of the SMARCB1 gene. Therapeutically (n = 7), all patients were treated with surgical resection (invariably incomplete) (n = 5), followed by adjuvant radiation therapy (n = 4) and chemotherapy (n = 4). While a single patient partially responded to treatment and another patient is alive with no evidence of disease after 23 years, three patients died of disease, six, eight, and 11 months post-diagnosis, despite adjuvant treatments. A single patient presented with a metastatic lung nodule, while another developed widespread metastasis. Poorly differentiated chordomas display a spectrum of features, are associated with a lower index of suspicion for a diagnosis, and display aggressive outcomes. Critical analysis of radiological and histopathological features, including necessary immunostains (brachyury and SMARCB1/INI1), is necessary for their timely diagnosis. These tumors show loss of SMARCB1/INI1 immunostaining and homozygous deletion of INI1/SMARCB1 gene.

We present a case of chordoma in a patient who had been previously treated for ductal carcinoma of the breast. The initial clinical findings and radiological studies suggested a possibility of metastases. However, the findings also adhered to the classical presentations and findings of the chordoma of the base of skull. It was only after the surgical resection and immunohistochemical confirmation that the diagnosis of chordoma could be established. Here, we discuss chordoma with the analysis of our clinical intrigue.

Selární oblast patří v oblasti neurokrania k regionům, které se vyznačují velmi vysokým stupněm anatomické, embryologické, neurologické a endokrinologické komplexnosti. Přestože se jedná o anatomicky velmi malou oblast, je z těchto důvodů množství patologických stavů, se kterými se zde můžeme setkat, značné. Článek podává souhrn diferenciálně diagnostických postupů při stanovování diagnózy v této oblasti pomocí výpočetní tomografie a magnetické rezonance. Uvedena jsou jak běžná onemocnění v této oblasti, tak i onemocnění vzácná, která mohou činit diferenciálně diagnostické obtíže.
Sellar region belongs to the intracranial areas with the highest degree of anatomical, embryological, neurological, and endocrinological complexity. Despite the fact it covers only a small anatomical area, the number of pathologic conditions that can be encountered here is substantial. The article gives an overview of differential diagnostic approaches to pathologic conditions occuring in that area by means of computed tomography and magnetic resonance imaging. Listed are both common as well as rare findings potentially causing diagnostic difficulties.

Předkládá základní diferenciálně diagnostickou rozvahu a rozebírá přínos jednotlivých typů vyšetření u jednotlivých postižení.
This review article summarises radiological features of most common tumorous lesions of spine on X-ray, CT and MR imaging. The main differential diagnostic abnormalities are presented and the value of the individual imaging methods is noted.

PURPOSE OF THE STUDY The aim of the study was to evaluate the group of patients operated on for chordoma at our department. We present diagnostic and surgical management options relevant to the localisation of chordoma in the spine, and evaluate the results in terms of post-operative complications, tumour recurrence and patient survival. MATERIAL AND METHODS Between 1989 and 2002 a total of 20 patients diagnosed with chordoma were treated. There were 15 men and five women with an average age of 53 years. The cervical spine was affected in 20 %, thoracic in 15%, lumbar in 25 % and sacral in 40 % of the patients. All of them suffered from back pain, and nine patients (45 %) had neurological symptoms. The average time from the onset of complaints till disease diagnosis was 7.2 months. RESULTS The average survival time was 63 months, with eight patients (40 %) surviving for more than five years. Of the patients with chordoma of the mobile spine, 66 % were treated by a combined antero-posterior procedure involving somatectomy, vertebral body replacement and posterior stabilisation; for sacral spine chordoma, a dorsal approach was always used. Of 11 patients (55 %) who required repeat surgery, eight had recurrent tumour and three had wound infection. DISCUSSION Chordomas are rare, slow-growing tumours usually diagnosed with a delay, particularly when localised in the sacral spine. At present magnetic resonance imaging is the essential diagnostic method allowing us to plan the appropriate surgical management. When the mobile spine is affected, a combined antero-posterior procedure including somatectomy, vertebral body replacement with a graft or implant and posterior stabilisation is used. When the sacral spine is involved, some authors prefer en bloc resection from the posterior approach, others use a combined antero-posterior procedure. Chordomas are known to have a high risk of local recurrence. Post-operative radiotherapy, which makes the disease-free interval longer, is recommended, chemotherapy has no effect. CONCLUSIONS Chordomas are associated with serious diagnostic and therapeutic problems, with frequent local recurrence. Prognosis is good if early diagnosis is made, and en bloc resection is performed.

Chordomas are locally invasive, malignant bone tumors that rarely occur in the cervical spine. En bloc resection or even fully resecting the tumor along its margin offers improved patient survival and a potential disease cure. Complete resection of tumors involving the upper cervical vertebrae requires a combined anterior-posterior approach but is complicated by the presence of vertebral arteries (VAs). In addition, reconstruction of the postresection defect may be prone to failure. The authors present a case of a chordoma involving the axis that was treated using a single-stage total intralesional C-2 spondylectomy with preservation of both VAs because the patient did not tolerate a preoperative occlusion test. A three-column reconstruction technique is also presented.
