OBJECTIVES: To investigate the impact of intracerebral haematoma (ICH) on the outcomes and the factors related to an ICH in patients with aneurysmal subarachnoid haemorrhage (aSAH) in a low- and middle-income country. DESIGN: A multicentre prospective cohort study. SETTING: Three central hospitals in Hanoi, Vietnam. PARTICIPANTS: This study included all patients (≥18 years) presenting with aSAH to the three central hospitals within 4 days of ictus, from August 2019 to June 2021, and excluded patients for whom the admission Glasgow Coma Scale was unable to be scored or patients who became lost at 90 days of follow-up during the study. OUTCOME MEASURES: The primary outcome was ICH after aneurysm rupture, defined as ICH detected on an admission head CT scan. The secondary outcomes were 90-day poor outcomes and 90-day death. RESULTS: Of 415 patients, 217 (52.3%) were females, and the median age was 57.0 years (IQR: 48.0-67.0). ICH was present in 20.5% (85/415) of patients with aSAH. There was a significant difference in the 90-day poor outcomes (43.5% (37/85) and 29.1% (96/330); p=0.011) and 90-day mortality (36.5% (31/85) and 20.0% (66/330); p=0.001) between patients who had ICH and patients who did not have ICH. The multivariable regression analysis showed that systolic blood pressure (SBP) ≥140 mm Hg (adjusted odds ratio (AOR): 2.674; 95% CI: 1.372 to 5.214; p=0.004), World Federation of Neurosurgical Societies (WFNS) grades II (AOR: 3.683; 95% CI: 1.250 to 10.858; p=0.018) to V (AOR: 6.912; 95% CI: 2.553 to 18.709; p<0.001) and a ruptured middle cerebral artery (MCA) aneurysm (AOR: 3.717; 95% CI: 1.848 to 7.477; p<0.001) were independently associated with ICH on admission. CONCLUSIONS: In this study, ICH was present in a substantial proportion of patients with aSAH and contributed significantly to a high rate of poor outcomes and death. Higher SBP, worse WFNS grades and ruptured MCA aneurysms were independently associated with ICH on admission.

Aim. The aim of this study was to compare two compression devices after transradial coronary catheterization and intervention. Methods. Out of 280 consecutive patients who underwent cardiac catheterization and intervention (n = 74) as a part of a same-day discharge program, 140 patients were applied the TR Band (TB) compression device and 140 the Seal-One (SO) compression device. The time needed to achieve patent hemostasis, duration of compression and local complications were assessed. Results. In the TB group, patent hemostasis was achieved in 17.5 ± 10.3 min (min), in the SO group in 21.4 ± 10.5 min (p = NS). The duration of radial artery compression was 90.7 ± 38.4 min in the TB group and 64.0 ± 26.5 min in the SO group (p < 0.001). The incidence of hematomas ≥5 cm did not differ between the two groups (6.4% vs. 6.4%, p = NS), the incidence of hematomas larger than 10 cm was 0.7% in the TB group and 1.4% in the SO group (p = NS). No radial artery occlusion or other local complications were found. Conclusion. Postprocedural radial artery compression with TR Band and Seal-One devices is associated with early patent hemostasis and a short duration of compression. The use of the Seal-One device was related to a shorter mean compression time in this study. No radial artery occlusion at discharge, nor any other local complications occurred following the radial artery compression, except for several clinically insignificant hematomas.

Aims: Pocket haematoma is a common complication after defibrillator [implantable cardioverter defibrillator (ICD)] implantation, which is not only painful, but also increases the risk of device-related infection, and possibly embolic events. The present study seeks to evaluate the rate and predictors of clinically significant pocket haematoma. Methods and results: This study included 2500 patients receiving an ICD in the SIMPLE trial. A clinically significant pocket haematoma was defined as a haematoma that required re-operation or interruption of oral anticoagulation (OAC) therapy. Clinically significant pocket haematoma occurred in 56 of 2500 patients (2.2%) of which 6 (10.7%) developed device-related infection. Patients who developed pocket haematoma were older (mean age 67.6 ± 8.8 years vs. 62.7 ± 11.6 years, P < 0.001), were more likely to have permanent atrial fibrillation (30.4 vs. 6.7%, P < 0.001) and a history of stroke (17.9 vs. 6.7%, P = 0.004), or were more likely to receive peri-operative OAC (50.0 vs. 28.4%, P < 0.001), unfractionated heparin (16.1 vs. 5.2%, P = 0.003), or low-molecular-weight heparin (37.5 vs. 17.5%, P < 0.001). Independent predictors of wound haematoma on multivariable analysis included the use of heparin bridging (OR 2.65, 95% CI 1.48-4.73, P = 0.001), sub-pectoral location of ICD (OR 2.00, 95% CI 1.12-3.57, P =0.020), previous stroke (OR 2.47, 95% CI 1.20-5.10, P = 0.015), an upgrade from permanent pacemaker (OR 2.52, 95% CI 1.07-5.94, P = 0.035), and older age (OR 1.03, 95% CI 1.00-1.06, P = 0.049). Conclusion: Pocket haematoma remains an important complication of ICD implantation and is associated with a high risk of infection. Independent predictors of pocket haematoma include heparin bridging, prior stroke, sub-pectoral placement of ICD, older age, and upgrade from a pacemaker.

Pacienti starší 75 let tvoří většinu nemocných podstupujících trvalou kardiostimulaci. Nejenom vyšší věk, ale často i četná přidružená onemocnění z nich vytvářejí značně rizikovou skupinu, u které můžeme očekávat vyšší procento komplikací. Z onemocnění, která tyto pacienty nejčastěji kompromitují, se jedná o chronickou renální insuficienci, srdeční selhání, diabetes mellitus, autoimunní onemocnění a řadíme sem i nemocné s antikoagulační léčbou. Z peroperačních komplikací jsou u starších nemocných častější pneumotorax, perforace pravé síně nebo komory. Z dalších komplikací se u vyšší věkové skupiny nacházejí častěji než u mladších pacientů hematom, dislokace elektrody, dekubitus kapsy, infekce kapsy a infekční endokarditida.
The vast majority of patients undergoing permanent cardiac pacing, are patients older than 75 years. Not only higher age, but frequent associated diseases creates of them a significant risk group, with which we can expect a higher percentage of complications. These patiens often suffer from chronic renal failure, heart failure, diabetes mellitus, autoimmune disease and anticoagulant therapy. From the operational complications we more often find pneumothorax and perforation of the right atrium or the right chamber of the heart. From other complications in group of patients with higher age are more common hematoma, dislocation of electrode, decubitus or infection of the pocket and infectious endocarditis.

Catheter ablation of atrial fibrillation is a modern therapeutic method that effectively prevents arrhythmia recurrences. Because of the complexity nature of this procedure, it is not surprising that the rate of complications is higher compared with other types of catheter ablations. This review focuses on the most important complications, and discusses their prevention, diagnosis and therapy.
