Radiační dermatitida je velmi častou reakcí při radioterapii, postihuje s různou intenzitou přibližně 95 % pacientů. Při závažném průběhu může mít významný dopad na kvalitu života. Vhodné použití klinicky prověřených produktů umožňuje ochraňovat pokožku pacienta a dosahovat velmi dobrých výsledků během léčby kožních reakcí. Praktičtí lékaři mohou významně přispět k naplnění potřeb pacientů, ujistit je, podpořit, edukovat případně doplnit informace. Existující vztahy praktických lékařů s jejich pacienty jsou dobrým předpokladem k tomu, aby se bez obav mohli doptat a poradit. Znalost aktuálních postupů pomůže jak konzistentním informacím pro pacienta a jeho edukaci, tak rozpoznání, kdy řešit obtíže a kdy doporučit pacienta ke specialistovi. Radiation dermatitis is a very common reaction in radiotherapy, affecting approximately 95 % of patients with varying intensity. In severe cases, it can significantly impact the quality of life. Appropriate use of clinically proven products protects the patient's skin and achieves excellent results during the treatment of skin reactions. General practitioners can significantly contribute to meeting patients' needs, reassuring them, supporting them, educating them, or supplementing information. Existing GP relationships with their patients are a good precondition for them to be able to inquire and advise without fear. Knowledge of current procedures will help provide consistent information for the patients and their education and recognize when to solve problems and when to recommend the patient to a specialist. The review article is a reduced version of the document Recommendation for preventive and therapeutic skin care of patients undergoing radiotherapy focused on the needs of primary care physicians.
Východiska: Onkologické rány a rány jiné etiologie u onkologických pacientů jsou sice vzácnou, ale pro pacienta závažnou komplikací, která významným způsobem negativně ovlivňuje kvalitu jeho života. Preventivní a léčebné intervence, edukce zdravotníků a pacienta snižují riziko jejich výskytu nebo rozsah následků. Pracovní skupina autorů z odborných skupin (Sekce podpůrné léčby a péče České onkologické společnosti ČLS JEP, Česká společnost pro léčbu rány, Společnost radiační onkologie, biologie a fyziky ČLS JEP, Česká asociace sester) připravila doporučení vhodné péče. Komplexní přístup k léčbě onkologické rány vč. symptomatické léčby s ní spojených komplikací hojení, prevence, včasný záchyt, mezioborová spolupráce a edukace jsou zásadní v přístupu k řešení ran souvisejících s aplikací chemoterapie, radioterapie a onkologické léčby obecně. Správná volba přípravků lokální péče a možnost doplnění onkologické léčby jsou základem v péči o rány při exulceraci tumoru. Cíl: Základní doporučení pro prevenci a léčbu onkologických ran pro denní praxi na základě doporučení odborných společností, výstupů studií a odborné literatury, ověřené praxe a shody názorů členů autorské skupiny. Doporučené postupy přispívají k redukci rozvoje, tíže a následků onkologických a jiných ran onkologických pacientů.
Background: Oncology wounds and wounds of other etiology are rare but serious complications, which significantly impair patients‘ quality of life. Preventive and curative interventions and education of healthcare personnel and patients reduce the risk of either their occurrence or their impact and consequences. A working group of authors from professional groups (the Supportive Care Group of the Czech Society for Oncology, the Czech Society for Wound Healing, the Society for Radiation Oncology, Biology and Physics, and the Czech Nurses Association) prepared recommendations for care. A comprehensive approach to the treatment of oncological wounds, including symptomatic treatment of associated healing complications, prevention, early detection, interdisciplinary cooperation and education are essential to deal with wounds related to chemotherapy administration, radiotherapy and oncological treatment in general. The proper choice of local care products and the eventuality of active oncological treatment are important elements of care in ulcerating tumors. Purpose: A basic summary of recommended interventions to prevent and treat oncology wounds in daily practice, defined based on expert societies guidelines, trials and literature data, proven practice and on the consensus opinions of the author‘s group members. The recommended procedures contribute to the reduction of the development, severity and consequences of oncological wounds and wounds of other etiology in oncological patients.

Velký podíl na zlepšení výsledků onkologické léčby má podpůrná péče. Dobře vedená podpůrná péče pomáhá lépe snášet onkologickou léčbu, zachovat či dokonce zlepšit kvalitu života a díky tomu i prodloužit přežití. Podpůrná péče zahrnuje léčbu bolesti, nevolnosti, zvracení, průjmů, infekčních komplikací, léčbu poklesu krvetvorby, dušnosti a únavy. U pacientů podstupujících radioterapii pro nádory hlavy a krku také péči o kůži při kožní toxicitě a péči o dutinu ústní kvůli orální mukozitidě. Nedílnou a velmi důležitou součástí podpůrné léčby je nutriční podpora, neboť malnutrice je nezávislým prediktivním i prognostickým faktorem. Článek shrnuje doporučení zejména Pracovní skupiny podpůrné péče v onkologii a Pracovní skupiny nutriční péče v onkologii (PSNPO), které pracují při České onkologické společnosti (ČOS ČSL JEP).
Supportive care plays an important role in cancer therapy. Good supportive care helps to control toxicity, maintain or improve quality of life and lenghten overall survival. Supportive care includes anagotherapy, therapy of nauzea, vomitus diarhoea, infectious complications, therapy of hematotoxicity, dyspnoea, fatigue and in radiotherapy of haed and neck cancers also therapy of skin toxicity and oral mucositis. Nutritional care is very important part of supportive care, because malnutrition is an independent prognostic and predictive factor. The artice concludes recomendation not only of two working groups of Czech oncology society - for supportive care in oncology and for nutritional care in oncology.

Purkyně, the Czech Wound Management Association, the Oncological Section of Czech Association of Nurses, and dermatologists. The document has been approved by the committees of these associations.
