Verukózní útvar na plosce – verukózní karcinom plosky. Stručný přehled Autoři předkládají případ 64letého muže s 20 let trvajícím verukózním útvarem na pravé plosce. Histopatologické vyšetření potvrdilo diagnózu dlaždicobuněčného karcinomu charakteru verukózního karcinomu. Práce poskytuje přehled současných poznatků o tomto onemocnění.
Verrucous Tumor of the Sole – Verrucous Carcinoma of the Sole. Minireview The authors describe a case of a 64-year-old man with a verrucous lesion on the right sole, that had been present for 20 years. The biopsy confirmed the diagnosis of squamous cell carcinoma of verrucous type. The paper provides an overview of current knowledge of this disease.

Obrovskobuněčný fibroblastom je vzácný lokálně agresivní nádor podkožních měkkých tkání. Vyskytuje se ponejvíce na trupu u mladých jedinců nejčastěji v první dekádě života. Nádor má tendenci lokálně recidivovat, vzdálené metastázy popsány nebyly. Vzhledem k poměrně časté kombinaci s dermatofibrosarcoma protuberans (DFSP), některým podobným morfologickým znakům, stejnému imunohistochemickému profilu a shodné genové fúzi t(17;22) COL1A1-PDGFB, byl obrovskobuněčný fibroblastom označen jako juvenilní forma DFSP. V této práci popisujeme případ mladého muže s pomalu rostoucím podkožním tumorem třísla, který byl excidován. Histologické vyšetření identifikovalo mezenchymální tumor s kolísající celularitou, účast obrovských vícejaderných buněk a pleomorfních vřetenitých buněk, které místy lemovaly tzv. pseudovaskulární či angiektoidní prostory. Imunohistochemie prokázala silnou CD34 pozitivitu všech těchto buněk, přičemž ERG byl pozitivní jen v endoteliích pravých kapilár. Nález vedl k suspekci na obrovskobuněčný fibroblastom, vzhledem k jeho semimaligní povaze a dosahu nádoru do linie excize byla léze opakovaně reexcidována. Molekulárně genetické vyšetření prokázalo transkripční produkt fúzního genu COL1A1-PDGFB, což vedlo k definitivnímu potvrzení diagnózy. Práce obsahuje rešerši literatury a stručný historický přehled vývoje konceptu obrovskobuněčného fibroblastomu jako samostatné jednotky.
Giant cell fibroblastoma is a rare locally aggressive tumor of subcutaneous mesenchymal tissue, occurring mostly on the trunk in young individuals with maximal incidence in the first decade of life. Local recurrences of giant cell fibroblastoma are common if marginally excised, however, distant metastases do not occur. Giant cell fibroblastoma was labelled as a juvenile variant of dermatofibrosarcoma protuberans (DFSP) due to quite frequent combination of both lesions, morphological similarities, identical immunoprofile, and shared gene fusion t(17;22) COL1A1-PDGFB. In this paper, we report a case of a young man with a slowly growing subcutaneous tumor in the groin. The tumor was excised and histological examination identified a mesenchymal tumor with variable cellularity, presence of multinucleated giant cells and pleomorphic spindle cells, which lined pseudovascular or angiectoid spaces. The CD34 immunohistochemistry showed strong positivity in all of these cells, whereas ERG was positive only in endothelial cells in true vessels. These findings led to a suspicion on giant cell fibroblastoma. Because of its borderline malignant behaviour and positive surgical margins, the lesion was subsequently reexcised. The molecular analysis identified the transcription product of gene fusion COL1A1-PDGFB and thus, final diagnosis was confirmed. The article includes review of the literature and brief historical overview of giant cell fibroblastoma concept as an unique entity.

BACKGROUND: Sentinel node biopsy in vulvar cancer is associated with much less morbidity than inguinofemoral node dissection. Our study focused on describing the morphology of superficial lymphatic drainage of the vulva and its relationship to regional nodes, which may facilitate orientation during surgery. MATERIALS AND METHODS: In 24 female cadavers, injections of patent blue (at various localisations medially, unilaterally and bilaterally) were used to visualise the lymphatic drainage of the vulva. After dissection of lymphatic vessels and nodes, their course was documented by photograph and then analysed. Subsequently, a map of vulvar superficial lymphatics was created. RESULTS: The cutaneous and subcutaneous tissue of the vulva primarily drained to superficial inguinal nodes. There was no evidence of a solitary lymph node that drained the unilateral vulva. Each area of the vulva drained to its own lymph node, which was variably localised in the subcutaneous groin around the great saphenous vein. Anastomoses between individual inguinal superficial lymph nodes are likely. Right-left symmetry in the course of lymphatic collectors was not detected. Natural drainage of the medial and paramedial areas to contralateral inguinal nodes was also not detected. The drainage pattern to ipsilateral inguinal nodes was consistent in cadavers without evidence of vulvar disease and may be applicable in the early stages of vulvar cancer. CONCLUSIONS: There was no evidence of a solitary node that drained the unilateral vulva. Each part of the vulva may drain to a corresponding lymph node in a different localisation of the groin. The surgeon should take this variability into account.

Autoři prezentují kazuistiku mladé ženy, která se dostavila na pohotovost s nově vzniklým zduřením v pravém třísle. Ačkoliv byla historie tohoto zduření typická pro tříselnou, resp. stehenní kýlu, byly zde jisté diskrepance s klinickým nálezem. Pacientka byla proto poslána na sonografické vyšetření, které potvrdilo synoviální cystu vycházející z pravé kyčle. Toto je vzácný případ prezentace synoviální cysty, který byl v dostupné literatuře jen minimálně zdokumentován.
We are pleased to introduce this interesting case report. A female patient presented to emergency department with a new lump in her right groin. Although there was quite convincing history for femoral hernia, the clinical findings were not correlating with this completely. The patient underwent ultrasound examination which showed synovial cyst originating from the right hip. This is a rare presentation of hip synovial cyst and we have therefore decided to publish this case report.

Karcinom z Merkelových buněk je vzácným maligním kožním nádorem s agresivním chováním, který ve většině případů bývá asociovaný s infekcí polyomavirem karcinomu z Merkelových buněk (MCPyV). Ve výjimečných případech se nádor manifestuje jako metastáza do lymfatické uzliny bez jednoznačného průkazu primárního kožního ložiska. V této práci prezentujeme případ 42letého pacienta s tříselnou lymfadenopatií, s histologickým nálezem karcinomu z Merkelových buněk. Imunohistochemicky byla v nádoru prokázána exprese chromograninu-A, synaptofyzinu a tečkovitá pozitivita cytokeratinu 20. Molekulárně genetickým vyšetřením byla pomocí metody real-time PCR detekována přítomnost genomu MCPyV v nádorových buňkách. Dermatologické vyšetření ani staging pomocí PET CT ložisko primárního nádoru v jiné lokalizaci u pacienta neprokázalo.
Merkel cell carcinoma is a rare cutaneous tumor with an aggressive clinical course. In most cases it is associated with Merkel cell polyomavirus infection. Exceptionally, the tumor can present as a lymph node metastasis without a discernible cutaneous primary. In this report we present the case of a 42 year-old man with inguinal lymphadenopathy, histologically consistent with Merkel cell carcinoma. Tumor cells expressed immunohistochemically chromogranin-A, synaptophysin and displayed dot-like positivity for cytokeratin 20. The genome of MCPyV in neoplastic cells was detected using real-time PCR. A cutaneous primary has not been identified neither during the dermatologic examination, nor with PET CT scan.

Endometriosis is a common clinical presentation for gynaecologists. Occasionally it can present to general surgeons as a swelling in the groin or abdominal wall. This condition should be included in the differential diagnosis in female patients. A 32-year-old woman with a 2-year history of a painful persistent lump in her right groin was referred to the general surgeons by her general practitioner. She was referred with a diagnosis of a suspected inguinal hernia. MRI excluded a hernia and exploration of the groin and subsequent histology confirmed the lesion to be an endometrial deposit.
