Léková preskripce u seniorů je zatížena velkým množstvím preskripčních chyb, jejichž častým důsledkem je vysoký podíl nežádoucích polékových reakcí spojených s vyšší morbiditou, spotřebou zdravotní péče a zvýšenými náklady. Dosud nejrozšířenější metodu pro identifikaci rizikové, potenciálně nevhodné preskripce u seniorů představují revidovaná Beersova kritéria z roku 2003 zahrnující seznam rizikových léčiv a relativních kontraindikací lék-nemoc, jejichž předepisování ve stáří by mělo být omezeno. Pro posouzení nedostatečné preskripce potenciálně přínosných léčiv dosud chyběla standardizovaná metoda. Mezi limity Beersových kritérií patří zařazení některých starších léčiv a léčiv, která nejsou v Evropě užívána, zpochybněno bylo i zařazení několika léčiv, pro něž existují i ve stáří specifické indikace, kritizováno bylo též nezahrnutí lékových interakcí a duplicit a celkově jejich malá uživatelská přátelskost. Tyto důvody vedly v roce 2007 k vypracování nových kritérií STOPP a START, která představují screeningový nástroj pro komplexní posouzení bezpečnosti a kvality preskripce u pacientů nad 65 let. V přehledovém článku je poprvé v českém písemnictví představena česká verze obou kritérií. S pomocí STOPP kritérií jsou identifikovány v lékovém režimu potenciálně nevhodné léky, které mohou být buď zcela vysazeny, nebo nahrazeny bezpečnější alternativou. Současné užití kritérií START umožní lékaři zvážit přínos nasazení léčby ve vybraných častých klinických situacích, z nichž mohou starší nemocní profitovat. Oba screeningové nástroje představují novou metodu pro zlepšení kvality geriatrické preskripce v klinické praxi.
Drug prescribing in the old age is burdened by a significant number of prescribing errors often resulting in high rate of adverse drug events associated with increased morbidity, health care utilization and health costs. The revised 2003 Beers' Criteria represent the most widely used method for identification of high risk ("potentially inappropriate") medication in elderly persons. A standardized method for evaluating omission of potentially beneficial drugs has been lacking. The Beers' Criteria consist of the list of selected drugs with high potential of adverse drug events in old age and clinical conditions with relative contraindications of selected drugs (drug-disease interactions). Prescribing of these drugs should be avoided in older patients. However, several limitations prevent wider use of Beers' Criteria: several outdated drugs or drugs unavailable in Europe are listed, some controversial drugs with specific indications are on the list, drug-drug interaction and drug class duplications are not mentioned, and last but not least low user friendliness was criticized. To overcome these limitations, new STOPP and START Criteria were developed in 2007 to serve as a screening tool for comprehensive assessment of safety and quality of prescription in patients 65 years and older. In the current review article Czech version of both criteria are presented for the first time in the Czech literature. Using STOPP Criteria potentially inappropriate drugs are identified in drug regimen which could be stopped altogether or replaced by a safer drug alternative. Concomitant use of START Criteria will help the prescribing physician to consider the benefit of starting new drugs in selected clinical situations. Both screening tools represent a new method for improving quality of geriatric prescribing in clinical practice.
Přítomnost multimorbidity u seniorů vede k užívání vyššího počtu druhů medikace.Vysoká prevalence polyfarmakoterapie může vést k zvyšujícímu se riziku užívání nevhodných léčiv, podužívaní správné léčby, chybám v užívání léků, poklesu compliance, interakcím lék-lék nebo lék-nemoc a výskytu nežádoucích polékových reakcí. Lékařům k správné preskripci léčiv pomáhají různé screeningové nástroje a algoritmy. Mezi nejznámější patří Beersova kritéria Americké geriatrické společnosti pro potencionálně nevhodné medikamenty u seniorů. Správné klinické hodnocení, porozumění celkovému stavu pacienta a stanovení individuálních léčebných cílů zůstávají klíčové k poskytnutí vysoce kvalitní individualizované péče a ke správnému užívaní medikace.
The presence of multimorbidity in the elderly lead to the use of multiple medication. The high prevalence of polypharmacy with aging may lead to an increased risk of inappropriate drug use, under-use of effective treatments, medication errors, poor adherence, drug-drug and drug-disease interactions and adverse drug reactions. Several tools and algorithms are available to aid clinicians in prescribing of adequate treatment. One of the best known tools are The American Geriatrics Society Beers Criteria for Potentially Inappropriate Medication Use in Older Adults. Good clinical assessment, an understanding of the patient's overall condition and individual treatment goals remain pivotal to provide high quality care and lead to right use of medication.

Demografické změny a velmi výrazné stárnutí populace v následujících letech s sebou přináší potřebu adaptace zdravotních a sociálních služeb a obecně veřejných i soukromých služeb. Vlivem stárnutí se mění klinický obraz onemocnění a manifestují se geriatrické syndromy. Nejvýznamnějším z nich je křehkost (frailty). Geriatričtí pacienti potřebují specifickou modifikaci zdravotní péče, a to ve složce akutní, následné i dlouhodobé. Důraz by měl být kladen na evaluaci funkčního stavu a podporu soběstačnosti, která je důležitá pro zachování kvality života. Mezi nejčastější příčiny zhoršení zdravotního stavu a hospitalizace geriatrických pacientů patří nežádoucí účinky léků a následky nesprávné preskripce. V evropském prostředí vznikly skríninkové nástroje k posouzení adekvátnosti a účelnosti preskripce (STOPP a START kritéria). Individualizovaná farmakoterapie je důležitou součástí péče o geriatrického pacienta.
Demographic changes pose unprecedented challenges for the system of health and social care and ageing of the population will continue significantly in the following years. The elderly population is very heterogeneous. Geriatric patients, particularly fragile and dependent senior citizens, require a specific modification of health care, including acute, subsequent, and long-term care. The emphasis in care should be placed on evaluation of the functional status and support of self-sufficiency that is important for maintaining the quality of life in older age.

BACKGROUND AND AIM: Polypharmacy and potentially inappropriate medication (PIM) use in older populations (65+ years) have not yet been investigated by meta-analyses in developing countries. This systematic literature review and meta-analysis aimed to investigate the prevalence of polypharmacy and PIM use and major risk factors associated with PIM prescribing in older adults in Ethiopia. METHODS: We searched PubMed/MEDLINE, Scopus, Embase, and Google Scholar databases to identify relevant studies published between January 1990 and October 2020. Observational studies reporting the prevalence and association of risk factors with polypharmacy and PIM use in the older population were meta-analyzed. A multilevel meta-analysis was conducted to pool the prevalence estimates, and the risk of PIM use was reported as a relative risk (RR) with a 95% confidence interval (CI). RESULTS: We identified by systematic literature review 404 articles. Of those, 8 studies fulfilled inclusion criteria, comprising a total sample of 2,608 participants. The overall prevalence of polypharmacy and PIM use pooled by meta-analysis in the Ethiopian older population was 33 and 37%, respectively. The risk factors of PIM use were analyzed in the meta-analysis (particularly polymorbidity, polypharmacy, gender, and older age), and only older age of 65+ (RR: 1.71, 95% CI: 1.16-2.51) was significantly associated with PIM use. CONCLUSION: This first meta-analysis from a developing country revealed a high prevalence of polypharmacy and PIM use in the Ethiopian older population. There was no awareness about the risk of PIMs in patients with polypharmacy and polymorbidity, and older age significantly predicted PIM use. Interventions ensuring rational geriatric pharmacotherapy are essential in developing countries in order to reduce the expected burden of PIM-related geriatric morbidity, higher costs, and mortality.

PURPOSE: The aim of this study was to explore patterns and long-term development in prescribing potentially inappropriate medication (PIM) according to the EU(7)-PIM list to elderly patients in Germany. METHODS: We analysed anonymized German claims data. The study population comprised 6.0 million insured individuals at least 65 years old, including all their prescriptions reimbursed in 2019. For the analysis of long-term development, we used data for the years 2009-2019. Factors associated with PIM prescribing were considered from two perspectives: patient-oriented analysis was performed with logistic regression and prescriber-oriented analysis was performed with multiple linear regression. RESULTS: EU(7)-PIM prevalence was reduced from 56.9% in 2009 to 45.1% in 2019. Average annual volume (DDDs/insured) decreased from 145 in 2009 to 121 in 2019. These figures are substantially greater than those for the older PRISCUS list. The majority of investigated ATC level 2 groups with the highest EU(7)-PIM DDD volume exhibited substantial decreases; moderate increases were found for antihypertensive and urological drugs. Antithrombotics increased strongly with the introduction of direct oral anticoagulants. The most prevalent EU(7)-PIM medication was diclofenac; however, in the age group 85+ years, apixaban was twice as prevalent as diclofenac. Polypharmacy, female sex, age < 90 years, need for nursing care and living in Eastern regions were identified as risk factors. Prescriber specialty was the most marked factor in the prescriber-oriented analysis. CONCLUSION: Although the use of EU(7)-PIMs has been declining, regional differences indicate considerable room for improvement. The comparison with PRISCUS highlights the necessity of regular updates of PIM lists.

V přehledovém článku autoři zdůrazňují specifický rys starší populace – nárůst počtu chorob s následným nárůstem počtu druhů užívaných medikamentů, vyšší pravděpodobností výskytu nežádoucích účinků léků a lékových interakcí a chyb v užívání medikace. Daná situace vyžaduje tvorbu jednoduchého a bezpečného konečného lékového schématu a při pochybnostech o schopnosti staršího nemocného užívat medikamenty bez chyb i zajištění asistence. Dále je zmíněn vývoj systémů kritérií pro zvýšení bezpečnosti terapie starších nemocných od původních Beersových kritérií až po současně užívaná STOPP-START kritéria. Pozornost je věnována nejčastěji užívaným lékovým skupinám – inhibitorům angiotenzin konvertujícího enzymu, betablokátorům, blokátorům kalciového kanálu, diuretikům a statinům. Závěrečná část přehledu je věnována vztahu ztráty soběstačnosti a vyplývajícím rizikům pro bezpečnost farmakoterapie. Článek je zakončen tabulkou s formulací zásad preskripce pro starší nemocné.
The authors stress the specific characteristics of the older population – increasing number of diseases and increasing number of medications used, higher risk of unwanted side effects and drug interactions. Higher number of medicaments usually means lower adherence by patients. This situation needs a simple and safe final medication scheme. If the ability of older patients to take the medications correctly decreases, it is necessary to provide them with assistance. The development of safety criteria is mentioned – starting with Beers criteria up to the currently-used STOPP-START criteria. The most frequently used medicaments are discussed – angiotensin-converting enzyme inhibitors, beta-blockers, calcium channel blockers, diuretics and statins. The final part of the article deals with the relationship between the loss of self-sufficiency and the resulting risks for the safety of pharmacotherapy. The article is completed with a table containing basic principles of drug prescription for elderly patients.
