INTRODUCTION: The terminology for female pelvic floor prolapse (POP) should be defined and organized in a clinically-based consensus Report. METHODS: This Report combines the input of members of two International Organizations, the International Urogynecological Association (IUGA) and the International Continence Society (ICS), assisted at intervals by external referees. Appropriate core clinical categories and a sub-classification were developed to give a coding to definitions. An extensive process of fourteen rounds of internal and external review was involved to exhaustively examine each definition, with decision-making by collective opinion (consensus). RESULTS: A Terminology Report for female POP, encompassing over 230 separate definitions, has been developed. It is clinically-based with the most common diagnoses defined. Clarity and user-friendliness have been key aims to make it interpretable by practitioners and trainees in all the different specialty groups involved in female pelvic floor dysfunction and POP. Female-specific imaging (ultrasound, radiology and MRI) and conservative and surgical managements are major additions and appropriate figures have been included to supplement and clarify the text. Emerging concepts and measurements, in use in the literature and offering further research potential, but requiring further validation, have been included as an appendix. Interval (5-10 year) review is anticipated to keep the document updated and as widely acceptable as possible. CONCLUSION: A consensus-based Terminology Report for female POP has been produced to aid clinical practice and research.

INTRODUCTION: In Spain, the offer of prevention programmes is very varied and structured, but today the challenge remains how to put evidence into practice. European Commission define a best practice as “a relevant policy or intervention implemented in a real-life setting which has been favourably assessed in terms of adequacy (ethics and evidence) and equity as well as effectiveness and efficiency related to process and outcomes”. The aim of the study was to describe the Spanish Project of Best Practice Portal in Drug Demand Reduction and Behavioural Addictions (the BBPPA portal). METHODS: The study design was a descriptive narrative essay on the Spanish BBPPA portal. RESULTS: The main results were: 1) design the Spanish BBPPA portal; 2) design a programme evaluation system in the field of drug demand reduction and behavioural addictions; 3) identification, evaluation and dissemination of programmes as best practices in drug demand reduction and behavioural addictions, 4) dissemination and regular updating of the available evidence; 5) training of professionals aimed at improving the quality of addiction prevention programmes. CONCLUSIONS: This project shows the feasibility of working on the quality of prevention programmes, applying the European Drug Prevention Quality Standards adapted to the context of the evaluation of best practices in Spain
BACKGROUND: Studies of correlates of active transport to and from school (ATS) focus mainly on children, have a limited conceptualisation of ATS trips, lack heterogeneity in built environments, and rarely consider effect modifiers. This study aimed to estimate associations of parent-perceived neighbourhood environment characteristics with self-reported ATS among adolescents from 14 countries, and whether associations differ by sex, city/region, and distance to school. METHODS: Observational cross-sectional design. Data were from the International Physical activity and Environment Network (IPEN) Adolescent study and included 6302 adolescents (mean age 14.5 ± 1.7 years, 54% girls) and a caretaker from 16 diverse sites. Adolescents self-reported usual travel to and from school by walking and bicycling (days/week) and time it would take to walk. Parents completed the Neighbourhood Environment Walkability Scale for Youth (13 scores computed). Generalised additive mixed models estimated associations of parent neighbourhood perceptions with 1) any active transport to/from school, 2) regular walking (5-10 times/week), 3) regular cycling to/from school, and 4) profiles of ATS generated using latent profile analyses. Interactions were also explored. RESULTS: Overall, 58.7% reported any ATS, 39.9% regularly walked, 7.7% regularly cycled, and four profiles of ATS were identified: walk to and from school; walk from school; cycle to and from school; no ATS. Distance to school was negatively associated with all outcomes, though evidence was weak for regular cycling to/from school. Land use mix - diversity was positively related to all ATS outcomes except those related to cycling. Accessibility and walking facilities were associated with higher odds of any ATS, regular walking to/from school, and the profile walking to and from school. Residential density was negatively related to regular cycling to/from school. Positive associations were observed between traffic safety and any ATS, and between safety from crime, aesthetics, and odds of regular cycling to/from school. Distance to school, adolescent sex, and city moderated several associations. CONCLUSIONS: Parent perceptions of compact, mixed-use development, walking facilities, and both traffic and crime-related safety were important supportive correlates of a range of ATS outcomes among adolescents in high- and low-middle-income countries. Policies that achieve these attributes should be prioritised to support more widespread ATS.

BACKGROUND: ADNP is essential for embryonic development. As such, de novo ADNP mutations lead to an intractable autism/intellectual disability syndrome requiring investigation. METHODS: Mimicking humans, CRISPR (clustered regularly interspaced short palindromic repeats)-Cas9 editing produced mice carrying heterozygous Adnp p.Tyr718∗ (Tyr), a paralog of the most common ADNP syndrome mutation. Phenotypic rescue was validated by treatment with the microtubule/autophagy-protective ADNP fragment NAPVSIPQ (NAP). RESULTS: RNA sequencing of spleens, representing a peripheral biomarker source, revealed Tyr-specific sex differences (e.g., cell cycle), accentuated in females (with significant effects on antigen processing and cellular senescence) and corrected by NAP. Differentially expressed, NAP-correctable transcripts, including the autophagy and microbiome resilience-linked FOXO3, were also deregulated in human patient-derived ADNP-mutated lymphoblastoid cells. There were also Tyr sex-specific microbiota signatures. Phenotypically, Tyr mice, similar to patients with ADNP syndrome, exhibited delayed development coupled with sex-dependent gait defects. Speech acquisition delays paralleled sex-specific mouse syntax abnormalities. Anatomically, dendritic spine densities/morphologies were decreased with NAP amelioration. These findings were replicated in the Adnp+/- mouse, including Foxo3 deregulation, required for dendritic spine formation. Grooming duration and nociception threshold (autistic traits) were significantly affected only in males. Early-onset tauopathy was accentuated in males (hippocampus and visual cortex), mimicking humans, and was paralleled by impaired visual evoked potentials and correction by acute NAP treatment. CONCLUSIONS: Tyr mice model ADNP syndrome pathology. The newly discovered ADNP/NAP target FOXO3 controls the autophagy initiator LC3 (microtubule-associated protein 1 light chain 3), with known ADNP binding to LC3 augmented by NAP, protecting against tauopathy. NAP amelioration attests to specificity, with potential for drug development targeting accessible biomarkers.

V situaci, kdy se nám nebo našemu blízkému obtížně či dlouhodobě hojí rána, je třeba si uvědomit, že správná výživa proces hojení podpoří či přímo umožní. Tělo potřebuje dodat jak základní stavební kameny pro hojení, tak ostatní nezbytné součásti. V dnešní době se klade velký důraz na moderní obvazové materiály, jichž je nepřeberné množství. Pomáhají likvidovat infekci či odumřelou tkáň, udržují v ráně vlhké prostředí příznivé pro nově se tvořící buňky. Hojení ran je dnes celá věda. Zkoumají se růstové i zánětlivé faktory, enzymy, různé složité mechanismy vzniku ran (například proleženin), možnosti, jak jim předejít, i možnosti jejich hojení. Přestože se vyskytnou i rány nezhojitelné, ve značném počtu zaznamenáváme úspěchy. Jakýkoli obvazový materiál či postup bude fungovat lépe při současné dobré výživě. Hojení rány není záležitost pouze lokálního ošetřování, ale vždy celkové péče o nemocného poskytované odborníky více oborů. Podporu výživy je třeba zahájit včas, neboť se jedná o klíčovou oblast umožňující dobré hojení.
AIMS: To report the histological findings in two AlphaCor artificial corneas explanted following complications,to compare the mechanism of biointegration and/or failure of the device and the correlation with clinical histories. METHODS: The first patient was diagnosed with optic deposits and corneal melting during the late postoperative period and required the removal of the device. The second patient experienced injury with penetration of the device 5 months after implantation. The explanted devices embedded within the corneal tissues were processed for pathological analysis at the John A. Moran Eye Center, University of Utah, Salt Lake City, USA. Histologic findings were compared with earlier histological results from animal studies and human case series and correlated with clinical histories. RESULTS: Both devices demonstrated normal skirt porosity. Light microscopic examination of the specimens disclosed adequate biointegration - colonization of the peripheral skirt with cellular elements present within the pores of the skirt. In the first patient (specimen 1) epithelial ingrowth was observed between the peripheral skirt and the posterior surface of the specimen, with partial extrusion of the device in that area. In the second case (specimen 2) the central, optic component of the device was missing. CONCLUSION: Histologic findings of the AlphaCor skirt were consistent with earlier animal studies and human cases series reports. These findings confirm the occurrence of biointegration. Epithelial ingrowth and scattered chronic mononuclear inflammatory cells were observed in the area of partial extrusion in the patient with a history of melting prior to explantation.

Gyrodactylus orecchiae sp. n. (Monogenea, Gyrodactylidae) is described from the skin, fins, eyes and gills of juvenile Sparus aurata L. (gilthead seabream) following two outbreaks of gyrodactylosis amongst stocks held in inshore floating cages on the Adriatic coast of Albania and Croatia. Fish were heavily infected (1000+ gyrodactylids/fish) with G. orecchiae which reportedly resulted in approximately 2-10% mortality amongst the infected stock. Morphologically, the haptoral hooks of G. orecchiae most closely resemble those of Gyrodactylus arcuatus Bychowsky, 1933 in the approximate shape of the ventral bar with its pronounced ventral bar processes and marginal hook sickles which possess a square line to the inner edge of the sickle blade and large rounded heels. The marginal hooks are also morphologically similar to those of Gyrodactylus quadratidigitus Longshaw, Pursglove et Shinn, 2003 and Gyrodactylus colemanensis Mizelle et Kritsky, 1967, but G. orecchiae can be readily discriminated from all three species by the characteristic infolding of the hamuli roots and the shape of the marginal hook sickle. Molecular sequencing of the ITS1, 5.8S, ITS2 regions (513+157+404 bp, respectively) of G. orecchiae and alignment with other gyrodactylids for which these same genomic regions have been determined, suggests that this is a new species. No similarities were found when the ITS1 region of G. orecchiae was compared with 84 species of Gyrodactylus available on GenBank.
