Ezetimib je nové hypolipidemikum, které v tenkém střevě inhibuje absorpci cholesterolu pocházejícího z potravy. Podstatně snižuje koncentraci LDL cholesterolu (low-density lipoprotein cholesterol) v monoterapii, i v kombinaci se statiny. Nicméně jeho vliv na kardiovaskulární mortalitu zůstává nejasný. Zhodnotili jsme relevantní publikovaná data zabývající se působením ezetimibu na různé fáze aterosklerotického procesu. K nalezení relevantních abstrakt a článků v anglickém jazyce srovnávajících terapii ezetimibem a statiny za účelem ovlivnění aterosklerotického procesu jsme použili databáze MEDLINE, EMBASE, BIOSIS a jiné. Na základě dostupných dat se zdá, že ezetimib vede k redukci zánětu, pokud je podáván v kombinaci se statiny, ale jeho vliv na funkci endotelu je nejasný. Účinek na progresi ischemické choroby srdeční nebo prevenci kardiovaskulárních příhod není dosud znám. Dle dostupných dat je užití ezetimibu jako léku druhé či třetí volby vhodné k dosažení cílových hodnot LDL cholesterolu.
