Záměrem sdělení je popsat stručný přehled soudobého vědeckého pojetí selektivního mutismu. Od první průkopnické práce doktora Kussmaula z roku 1877 a pozdější práce dětského psychiatra Tramera z roku 1934 doznalo povědomí o selektivním mutismu významných změn. Tato přehledová studie nabízí souhrn moderních vědeckých poznatků o SM. Hlavním cílem je upozornění na skutečnost, že současné studie a analýzy nabízí zařazení SM mezi neurovývojové poruchy, jelikož je selektivní mutismus heterogenní, komplexní a mnohostranná porucha. Zařazení mezi úzkostné poruchy se nejeví jako zcela přiléhavé. Text zároveň nabízí návrh subtypů pro SM, ze kterého vyplývá nutnost multimodální intervence. V popředí vědeckého zkoumání je nyní snaha odhalit vztah mezi úzkostí a selektivním mutismem: zda jde o etiologický mechanismus, či symptom. Vyjasnění této problematiky by pravděpodobně vedlo k reklasifikaci této relativně vzácné úzkostné poruchy.
The aim of this article is to describe a brief overview of the current scientific concept of selective mutism. Professional awareness of selective mutism has undergone significant changes since the first pioneering work of Dr. Kussmaul in 1877 and the later work of child psychiatrist Tramer in 1934. This review study provides a summary of the current scientific knowledge on selective mutism. The main goal is to draw attention to the fact that current studies and analyzes offer the classification of selective mutism among neurodevelopmental disorders, as selective mutism is a heterogeneous, complex and multifaceted disorder. Classification among anxiety disorders does not appear to be entirely appropriate. At the same time, the text offers a proposal of subtypes for selective mutism, which implies the necessity of a multimodal intervention. At the forefront of scientific research is now the effort to uncover the relationship between anxiety and selective mutism – whether it is an etiological mechanism or a symptom. Clarification of this issue would probably lead to the reclassification of this relatively rare anxiety disorder.
Selektivní mutismus je klasifikován jako méně častá úzkostná porucha. Tento typ poruchy je vysoce rušivý a limituje kvalitu života. Včasné odhalení a vhodně zvolená terapie je proto zásadní. Terapie by měla stát na dvou základních lidských potřebách: VZTAH a BEZPEČÍ. V současnosti se dominantně využívá terapie se zapojením více modalit. Vědecky je potvrzená účinnost zejména behaviorální terapie a využití „nesoustředěné“ komunikace (Oerbeck et al., 2014; Driessen et al., 2020). Multimodální přístup jsme v našem případě rozšířili o senzomotorickou arteterapeutickou metodu Práce v hliněném poli®. Jde o komplexní metodu zaměřenou na tělo a bazální smysly. Jde o tzv. „bottom-up“ terapii. Při jejím využití sledujeme na našem pracovišti významné zmírnění symptomatologie selektivního mutismu u dětí předškolního věku v krátkodobém horizontu. Práce v hliněném poli® by tedy mohla být dalším uznaným terapeutickým přístupem u dětí se selektivním mutismem

V tomto článku představujeme postupy, které byly uplatněny ve spolupráci s logopedem (SLP) a manželským a rodinným terapeutem (MFT) při léčbě osmiletého bilingvního dítěte s diagnózou selektivního mutismu. Zabýváme se popisem charakteristik dětí, u nichž je selektivní mutismus diagnostikován, a poskytujeme přehled různých lékařských přístupů.

Autorka článku provedla výzkumné dotazníkové šetření, které probíhalo od října 2009 do ledna 2010 v Pardubickém kraji. Hlavním cílem byla analýza současného stavu vzdělanosti a erudice pediatrů v oblasti logopedické péče a prevence. Výsledky výzkumného šetření ukázaly, že pediatři mají zájem o logopedickou osvětu, ale také by uvítali materiály, které by zprostředkovávaly rodičům logopedickou problematiku. Proto předkládáme Čtení o logopedii, které souhrnně, stručně a přehledně charakterizuje obor klinické logopedie. Čtení o logopedii vysvětluje pediatrům organizaci logopedické péče v České republice, popisuje ontogenezi řeči a podrobně se věnuje deseti okruhům narušené komunikační schopnosti. Každý okruh je terminologicky vymezen, jsou uvedeny etiologické faktory, klasifikace, symptomatologie, diagnostika a možnosti preventivních opatření. V závěru se odkazuje na dostupnou a citovanou literaturu. Efektivní vzdělávání rodičů v oblasti vývoje řeči a jazyka a jeho narušení je umožněno elektronickou cestou – www.logopediecr.eu. Internetové stránky odpovídají svou náročností, terminologií i obsahovým vymezením charakteru uživatele – pro rodiče je zvolen vhodný a srozumitelný slovník, přitažlivý stručnými a srozumitelnými výrazy, doplněný tabulkami a odkazy na doporučené pomůcky.
The author performed research questionnaire survey, which from October 2009 to January 2010 in the Pardubice Region. The main goal was to analyze present state of erudition pediatricians in the area of logopedic intervention and prevention. The results of the research survey revealed that pediatricians are interested in logopedic health education, but they would also welcome materials, which mediated the logopedic problems to parents. Therefore we present here the Reading about Speech and Language Therapy, which briefly and comprehensively summarized and characterized the branch of clinical speech and language therapy. The Reading about Speech and Language Therapy explains to pediatricians the organization of logopedic care in the Czech Republic, speech ontogenesis and deals in detail with ten areas of disturbed communication ability. Each term is terminologically specified etiologic factors, classification, symptomatology, diagnostics and possibilities of preventive measures are outlined. In the conclusion the author refers to available and cited literature. Effective education of parents in the area of speech and language evolution and its disruption is made possible by electronic communication – www.logopediecr.eu. Internet pages correspond, in its demandingness, terminology and contents of the user’s character – a suitable and understandable vocabulary was selected for parents – attractive by brief and comprehensible expression, supplemented with tables and references to recommended tools.
