Chirurgické výkony na kolenním kloubu a ošetření úrazů v této lokalizaci jsou doménou ortopedie a traumatologic. Pečují o zachování a obnovu tvaru a funkce lokomočního aparátu od dětství po vysoké stáří, a to všech jeho struktur, na základě diagnostiky, terapie a výzkumu onemocnění, vad a úrazů těchto struktur. Medicínskou disciplínou, která se na výsledcích těchto čiímostí významně podílí, je rehabilitace. Bez rehabilitace si nemůžeme představit úspěšnou léčbu a doléčení těchto patologických stavů.
Surgical operation on knee-joint and injury treatment in this region belong to orthopedics and traumatology. These interventions support preservation and restoration of locomotor apparatus function from children age to senescence, of all their functions. on the ground of diagnostics, therapy and research of diseases, disorders and injuries of these structures. Rehabilitation is being the principal medical discipline dealing with results of these activities. We could not imagine successful treatment and after-care of these pathological conditions without rehabilitation.
This report describes the case of a young male who had been followed-up between the ages of 14-21 years at different health facilities for symptoms initially considered to be caused by tumor, then by chronic osteomyelitis or ankylosing spondylitis and finally diagnosed as the SAPHO syndrome. Musculoskeletal symptoms of the SAPHO syndrome include focal, probably aseptic chronic osteomyelitis, synovitis and formation of hyperostoses. Therapy of the SAPHO syndrome is predominantly conservative. However, in this case we had to employ arthroscopic synovectomy first, for severe synovitis resistant to any conservative therapy approach. Later, left total hip replacement has been performed for advanced hip joint damage accompanied by intense pain and significant range of motion reduction. At the present time, the patient is 51/2 years after synovectomy of the knee and 5 years after the hip joint replacement. The knee joint is without effusion or functional limitations, with intermittent pains only. The hip prosthesis in the risk area is fully integrated without signs of component loosening, with very good functional outcome. While synovectomy can be obviously fully recommended in cases like this, the total joint replacement should be considered unique and indicated only rarely after careful consideration of all circumstances.We consider this approach absolutely exceptional also because we have not found reference to similar case in any national or international professional literature available.

Infection of the total joint replacement is the most severe complication seen in prosthetics. Besides a number of other factors, the rise of infection is also influenced by the material applied, not only by its chemical composition but even by the surface texture. This, together with the properties of the contaminating bacteria, has effects on the development of biofilm formation on the implant surface. A set of 91 infective complications of the total hip replacement was used to evaluate clinically the influence of implanted materials on the infection development. The relation between cemented, hybrid, and non-cemented substitutions, particularly the relation of various metals used for implantation and especially the effects of polyethylene were assessed. The time factor of the infection development was also evaluated. Statistical significance was found between the sets in all surveyed parameters. The implants with polymer materials were revealed to become contaminated more easily. Non-cemented implants with porous surfaces are infected less often; here infection appears earlier than in cemented implants with polished metal surfaces. This report aims at better understanding the problems of biomaterial surfaces and, thus, at decreasing the incidence of infections in total joint replacements.
