Recurrent aphthous stomatitis (RAS) is the most common disease of the oral mucosa, and it has been recently associated with bacterial and fungal dysbiosis. To study this link further, we investigated microbial shifts during RAS manifestation at an ulcer site, in its surroundings, and at an unaffected site, compared with healed mucosa in RAS patients and healthy controls. We sampled microbes from five distinct sites in the oral cavity. The one site with the most pronounced differences in microbial alpha and beta diversity between RAS patients and healthy controls was the lower labial mucosa. Detailed analysis of this particular oral site revealed strict association of the genus Selenomonas with healed mucosa of RAS patients, whereas the class Clostridia and genera Lachnoanaerobaculum, Cardiobacterium, Leptotrichia, and Fusobacterium were associated with the presence of an active ulcer. Furthermore, active ulcers were dominated by Malassezia, which were negatively correlated with Streptococcus and Haemophilus and positively correlated with Porphyromonas species. In addition, RAS patients showed increased serum levels of IgG against Mogibacteriumtimidum compared with healthy controls. Our study demonstrates that the composition of bacteria and fungi colonizing healthy oral mucosa is changed in active RAS ulcers, and that this alteration persists to some extent even after the ulcer is healed.

Patients with unipolar depressive disorder and in the depressive phase of bipolar disorder often manifest psychological distress and cognitive deficits, notably in executive control. We used computerized cognitive training in an attempt to reduce psychological affliction, improve everyday coping, and cognitive function. We asked one group of patients (intervention group) to engage in cognitive training three times a week, for 20 min each time, for eight consecutive weeks. A second group of patients (control group) received standard care only. Before the onset of training we administered to all patients self-report questionnaires of mood, mental and psychological health, and everyday coping. We also assessed executive control using a broad computerized neurocognitive battery of tests which yielded, among others, scores in Working Memory, Shifting, Inhibition, Visuomotor Vigilance, Divided Attention, Memory Span, and a Global Executive Function score. All questionnaires and tests were re-administered to the patients who adhered to the study at the end of training. When we compared the groups (between-group comparisons) on the amount of change that had taken place from baseline to post-training, we found significantly reduced depression level for the intervention group. This group also displayed significant improvements in Shifting, Divided Attention, and in the Global executive control score. Further exploration of the data showed that the cognitive improvement did not predict the improvements in mood. Single-group data (within-group comparisons) show that patients in the intervention group were reporting fewer cognitive failures, fewer dysexecutive incidents, and less difficulty in everyday coping. This group had also improved significantly on the six executive control tests and on the Global executive control score. By contrast, the control group improved only on the reports of cognitive failure and on working memory.

BACKGROUND: Methamphetamine is a neurotoxic agent. Its chronic abuse may result in cognitive impairment with negative consequences for patients' treatment and rehabilitation. The aim of the study was to compare Wisconsin Card Sorting Test profiles of Czech subjects dependent on methamphetamine with healthy individuals. SUBJECTS AND METHODS: Forty-three hospitalized Czech Caucasian patients including twenty-seven men at the average age of 25.3±5.2 years dependent on methamphetamine for 6.2±3.3 years were assessed by the Wisconsin Card Sorting Test. We used the same neurocognitive test for the comparison group of healthy controls with the same ethnicity (N=52, men N=28, average age of 38.7±12.1 years). We applied the Chi-Square Test, Two-Sample T Test, Mann-Whitney U Test and Kolmogorov-Smirnov Test to compare methamphetamine dependent patients with healthy volunteers. RESULTS: All recorded Wisconsin Card Sorting Test parameters were significantly different in the group of methamphetamine dependent patients versus healthy volunteers (P=0.04-0.006; Mann-Whitney U Test, Two-Sample T Test). The results showed a higher error rate and a smaller achievement quality in the patients as against healthy subjects. We ascertained a significant cognitive deterioration in the patients as compared to healthy volunteers even if the average patients' achievements were in the normal range according to the test norms. CONCLUSIONS: A cognitive impairment was present in the group of patients as compared to healthy controls. Better understanding of neurocognitive symptoms in methamphetamine dependent subjects should help to generate modern therapeutic approaches, both pharmacological and psychosocial, to prevent or attenuate the long-term negative consequences of methamphetamine use disorders.

Práce se zabývá sebeposouzením výsledků subjektivně měřené efektivity tréninkového cyklu programu Cognifit. Porovnávali jsme před a po tréninku soubor pacientů (N = 19) s afektivními poruchami (F31, F32 a F 33), který v domácich podmínkách trénoval po dobu nejméně 8 týdnů (24 tréninkových sezení), s podobným klinickým souborem (N = 17), který intervenci nedostával. Generalizace efektu tréninku do oblastí každodenního života pacientů byla sledována pomocí tří dotazníkových metod zaměřených na kognitivní funkce, dvou na kvalitu života a dvou pro osoby blízké. V experimentální skupině došlo k subjektivním pozitivním změnám (v 5 ze 6 použitých proměnných) oproti menším změnám, ke kterým došlo v kontrolní skupině (1 proměnná ze 6). Při další analýze, která vzala v úvahu najednou experimentální i kontrolní skupinu, přetrvaly rozdíly pouze u 2 proměnných ze 6 (Beckův dotazník depresivity. Dotazník zaměřený na každodenní projevy paměti, EMQ). Cognifit má pozitivní efekt na subjektivní vnímání kognitivních funkcí (každodenní paměťové schopnosti). Efekt tréninku se neprojevil V posouzení osobami blízkými.
This article summarizes results of subjective evaluation of Cognifit program Experimental group of patients (N = 19) with affe ctive disor- ders (F31, F32 and F33 accoring to ICD-10) training at home for at least 8 weeks (24 sessions) was compared with also patient c ontrol group (N = 17). Generalization into everyday life was followed with three questionnaire methods oriented to evaluation of subjectivel y perceived co- gnitive status (DEX, CFQ, EMQ), questionnaires on quality of life (SQUALA), well being (SOS-10), depression (BDI-II), and two q uestionaires for relatives (DEX-SO, CFQ-SO). In the experimental group, positive changes were found in 5 from 6 variables, in the control gr oup in 1 from 6 variables. In further analysis, which took into account both groups, experimental and control, improvement in 2 variables (BD I-II and EMQ) persisted. The Cognifit program has positive effect on perceived level of depression and everyday memory functioning. The train ing effect in evaluation from the relatives was not found.

Prace se zabývá extrapolací hlavních výsledků subprojektu CNS zaměřeného na zjištění kognitivních funkcí u pacientů s unipolární depresivní poruchou v remisi do oblasti kognitivního tréninku. Zjistili jsme (Preiss et al., 2009 a 2009), že stav kognitivních funkcí je u této skupiny pacientů jednak poměrně dobrý a odpovídající normě vzhledem k věku a vzdělání, jednak mírně snížen oproti osobám bez depresivní poruchy v anamnéze. V této práci navrhujeme možnosti pozitivního ovlivnění kognitivních deficitu formou kognitivního tréninku (Cognifit). Popjpisuieme hioznosti této metody, přednosti oproti konkurenčním metodám, probíhající pilotní studii a plány do budoucnosti. Domníváme se, že forma individualizovaného kognitivního tréninku on-line může být vhodným doplňkem již existujících služeb v této oblasti.
This paper extrapolates main results of CNS subproject focused on cognitive functioning in patients with unipolar depressive di sorder into area of cognitive training. We found (Preiss et al., 2009 a 2009) that cognitive functioning in these patients is relatively we ll and within the norms according to age and education, but also slightly below the performance of controls without the history of depressive disorder. In this paper we suggest possibilities for influence of cognitive functioning through the Cognifit programme. We describe possibilities of this approach, advan- tages in comparison with competetive methods, ongoing pilot study and plans for the future. We summarized that this form of ind ividualized on-line cognitive training could be appropriate complement to existing services in this area.
Efektivita kognitivní remediace na kognitivní deficit u schizofrenie je opakovaně prokazována jak na úrovni psychometrické, tak i na úrovni fyziologické. Kognitivní remediace pomocí speciálně vyvinutých počítačových programů vykazuje signifikantně pozitivní výsledky především v parciálních kognitivních oblastech u pacientů se schizofrenií. Tato práce je součástí podprojektu Centra neuropsychiatrických studií (CNS), jehož cílem je vytvořit registr neuropsychiatrických pacientů s prokázaným kognitivním deficitem, prospektivně je sledovat a zároveň ověřit M krátkodobé výsledky, klinickou změnu stavu po kognitivní remediaci při laboratorním (trénink kognitivních funkcí pomocí počítačů) a při klinickém postupu (trénink kognitivních funkcí v rámci komplexního rehabilitačního programu) u pacientů se schizofrenním onemocněním. Výsledky našeho výzkumu potvrzují pozitivní krátkodobý parciální vliv laboratorního přístupu na kognitivní deficit. Laboratorní přístup pomocí počítačového programu prokázal navíc generalizační efekt v rámci kognitivní domény pracovní paměti. Komplexní přístup se prokázal efektivní při generalizovaných deficitech.
Efficacy of cognitive remediation in schizophrenia cognitive deficit has been demonstrated at both psychometric and physiologic al level. Cognitive remediation with computer-aided specialized programs has showed consistently significant positive results, particular ly in specific cognitive domains of schizophrenia patients. Our study is a part of a CNS project aimed to create a register of neuropsychiatri c patients with confirmed cognitive deficit, to follow them prospectively and simultaneously to investigate non-pharmacological interventions i n cognitive remediation and prevention of their deterioration. We studied and compared short-term results, a clinical change after a cognit ive remediation in laboratory (computer-aided cognitive training), and clinical settings (training of cognitive functions as a part of a comple x psychiatric rehabi- litation) in schizophrenia patients. Our results confirmed a positive short-term partial effect of laboratory intervention on c ognition. Moreover, the laboratory intervention demonstrated effect of generalization in a single cognitive domain. Complex program showed to be mo re effective in generalized deficits.
