OBJECTIVE: This study aimed to assess the association of bipolar disorder (BD) with risk of major adverse cardiac events (MACEs) after adjusting for established cardiovascular disease (CVD) risk factors. METHODS: We conducted a population-based historical cohort study using the Rochester Epidemiology Project. Patients older than 30 years with a clinical encounter from 1998 to 2000 with no prior MACE, atrial fibrillation, or heart failure were followed up through March 1, 2016. BD diagnosis was validated by chart review. Cox proportional hazards regression models were adjusted for established CVD risk factors, alcohol use disorder, other substance use disorders (SUDs), and major depressive disorder (MDD). RESULTS: The cohort included 288 individuals with BD (0.81%) and 35,326 individuals without BD as the reference group (Ref). Median (interquartile range) follow-up was 16.5 (14.6-17.5) years. A total of 5636 MACE events occurred (BD, 59; Ref, 5577). Survival analysis showed an association between BD and MACE (median event-free-survival rates: BD, 0.80; Ref, 0.86; log-rank p = .018). Multivariate regression adjusting for age and sex also yielded an association between BD and MACE (hazard ratio [HR] = 1.93; 95% confidence interval [CI] = 1.43-2.52; p < .001). The association remained significant after further adjusting for smoking, diabetes mellitus, hypertension, high-density lipoprotein cholesterol, and body mass index (HR = 1.66; 95% CI = 1.17-2.28; p = .006), and for alcohol use disorder, SUD, and MDD (HR = 1.56; 95% CI = 1.09-2.14; p = .010). CONCLUSIONS: In this study, BD was associated with an increased risk of MACE, which persisted after adjusting for established CVD risk factors, SUDs, and MDD. These results suggest that BD is an independent risk factor for major clinical cardiac disease outcomes.

Depresivní onemocnění je spojeno s významným funkčním postižením. Jednou z příčin narušení funkčních schopností jsou kognitivní poruchy. Kognice u deprese je narušena v menší míře než u demence nebo psychotických poruch, nicméně se zdá, že se nejedná pouze o příznak stavu (state marker), ale i o rys poruchy (trait marker). Poruchy kognitivních funkcí u unipolární deprese lze zaznamenat nejen během epizody jako důsledek depresivních příznaků, ale i u pacientů v remisi. Snížená výkonnost byla prokázána již u pacientů s první epizodou deprese. U depresivních pacientů jsou přítomny poruchy jak "chladné kognice" (nezávislé na emocích: exekutivní funkce, pozornost, pracovní paměť, celkové tempo a rychlost psychomotorického zpracování), tak i "horké kognice" (spojené s emocemi: sociální dezinterpretace, negativní bias). Kognitivní poruchy často tvoří reziduální příznaky, významně narušují každodenní fungování a přispívají k funkčnímu oslabení a selhávání pacientů. Pacienti sami subjektivně pociťují sníženou kognitivní výkonnost, k objektivní detekci a posuzování závažnosti slouží především neuropsychologické testy. V klinické praxi tyto testy nebývají k dispozici, alternativně lze využít jednoduché sebeposuzovací nástroje. V terapii kognitivního deficitu u deprese se používají nefarmakologické intervence (především kognitivní remediace) a farmakoterapie. Podle předběžných dat se zdá, že kombinace remediace a farmakoterapie může být efektivnější než samotné podávání léků. Zatím existuje jen několik randomizovaných studií s antidepresivy, kontrolovaných placebem nebo porovnávající dva preparáty vůči sobě, které primárně sledovaly prokognitivní účinky v negeriatrické populaci depresivních pacientů. Z dostupných farmak je k dispozici nejvíce důkazů o účinnosti duálních (duloxetin) a multimodálních (vortioxetin) antidepresiv. Ostatní látky (např. stimulancia, kognitiva, hormony, glutamátergní antagonisté, doplňky stravy) se zatím testují experimentálně jako adjuvantní terapie.
Depressive disorder results in significant functional impairment. Cognitive deficits are one of the causes of this disruption. There is a lower degree of impaired cognition in depression than in dementia or psychotic disorders; however, it is not just a state marker, but also trait marker. Cognitive dysfunctions in major depression can be observed not just as consequence of depressive symptoms during acute episode, but also in remitted patients. Impaired cognitive functioning has been found even in first-episode patients. Patients with depression have impairments of both "cold cognition" (emotion-independent: executive functions, attention, working memory, general and psychomotor processing speed) and "hot cognition" (emotion-laden: social misinterpretation, negative bias). Cognitive deficits are often residual symptoms, significantly interfere with everyday functioning, contribute to functional impairments. Patients themselves subjectively perceive low cognitive performance; the primary instruments for objective detection and assessment are neuropsychological tests. In clinical practice, these tests are not usually available, simple self-reported measures can be used instead. Both non-pharmacological (e.g., cognitive remediation) and pharmacological interventions are used for treatment of cognitive deficits in depression. Preliminary data indicate that a combination of remediation and pharmacotherapy can be more efficacious than drugs alone. So far, there is a limited number of randomized trials with antidepressants, either placebo-controlled or head-to-head comparisons, primarily testing procognitive effects in non-geriatric depressed patients. As for drug therapy, there is evidence that dual (duloxetine) and multimodal (vortioxetine) antidepressants are effective. Other agents (e.g., stimulants, cognitive drugs, hormones, glutamatergic antagonists, nutritional supplements) are being experimentally tested as adjuvant therapy.

UNLABELLED: The aim of our case-control study was to compare selected psychological and social characteristics between diabetic patients with and without the DF (controls). METHODS: 104 patients with and 48 without DF were included into our study. Both study groups were compared in terms of selected psychosocial characteristics. RESULTS: Compared to controls, patients with DF had a significantly worse quality of life in the area of health and standard of living as shown by lower physical health domain (12.7 ± 2.8 versus 14.7 ± 2.5; P < 0.001) and environment domain (14.1 ± 2.2 versus 15 ± 1.8; P < 0.01) that negatively correlated with diabetes duration (r = -0.061; P = 0.003). Patients with DF subjectively felt more depressed in contrast to controls (24.5 versus 7.3%; P < 0.05); however, the depressive tuning was objectively proven in higher percentage in both study groups (83.2 versus 89.6; NS). We observed a significantly lower level of achieved education (P < 0.01), more patients with disability pensions (P < 0.01), and low self-support (P < 0.001) in patients with the DF compared to controls. In the subgroup of patients with a previous major amputation and DF (n = 6), there were significantly worse outcomes as in the environment domain (P < 0.01), employment status, and stress readaptation (P < 0.01) in contrast to the main study groups. CONCLUSIONS: Patients with DF had a predominantly worse standard of living. In contrast to our expectations, patients with DF appeared to have good stress tolerability and mental health (with the exception of patients with previous major amputation) and did not reveal severe forms of depression or any associated consequences.

Prace se zabývá extrapolací hlavních výsledků subprojektu CNS zaměřeného na zjištění kognitivních funkcí u pacientů s unipolární depresivní poruchou v remisi do oblasti kognitivního tréninku. Zjistili jsme (Preiss et al., 2009 a 2009), že stav kognitivních funkcí je u této skupiny pacientů jednak poměrně dobrý a odpovídající normě vzhledem k věku a vzdělání, jednak mírně snížen oproti osobám bez depresivní poruchy v anamnéze. V této práci navrhujeme možnosti pozitivního ovlivnění kognitivních deficitu formou kognitivního tréninku (Cognifit). Popjpisuieme hioznosti této metody, přednosti oproti konkurenčním metodám, probíhající pilotní studii a plány do budoucnosti. Domníváme se, že forma individualizovaného kognitivního tréninku on-line může být vhodným doplňkem již existujících služeb v této oblasti.
This paper extrapolates main results of CNS subproject focused on cognitive functioning in patients with unipolar depressive di sorder into area of cognitive training. We found (Preiss et al., 2009 a 2009) that cognitive functioning in these patients is relatively we ll and within the norms according to age and education, but also slightly below the performance of controls without the history of depressive disorder. In this paper we suggest possibilities for influence of cognitive functioning through the Cognifit programme. We describe possibilities of this approach, advan- tages in comparison with competetive methods, ongoing pilot study and plans for the future. We summarized that this form of ind ividualized on-line cognitive training could be appropriate complement to existing services in this area.
AIMS: The aim of the present study was to compile a specific algorithm of prevention of venous thromboembolism in hospitalized psychiatric patients because this specific issue has not been addressed sufficiently in the literature.

Studie zkoumá kognitivní výkon ve verbální paměti během remise unipolámí depresivní poruchy. Testovali jsme hypotézu, že ambulantní medikovaní i nemedikovaní pacienti žijící ve svém běžném prostředí se nebudou lišit od kontrol v Paměťovém testu učení. K 97 pacientům jsme přiřadili 97 kontrol podle vzdělání, věku a pohlaví a porovnávali je v Paměťovém testu učení. Pacienti měli horší výkony ve srovnání s kontrolami. 1 po korekci pro subjektivní míru deprese měřenou BDI zůstal deficit v oddáleném vybavení signifikantní. Výsledky naznačují, že deficit ve verbálni paměti by mohl být považován u unipolární depresivní poruchy v remisi za "trait marker".
The present study investigates performance of verbal memory during remission from unipolar depressive episodes. We tested a hy- pothesis that outpatients do not differ in cognitive variables from matched controls, in a well defined outpatient sample, cons isting of medicated and unmedicated patients, with a history of MDD, living in their natural environments. Ninety-seven fully remitted pa tients with MDD were compared to 97 healthy control subjects matched for education, age and gender on Auditory Verbal Learning Test (AVLT). Patients with remitted MDD comparing to controls were impaired on verbal memory tasks. After correction for subjective level of depression, deficits in the delayed verbal recall (AVLT) remained significant. These findings suggest deficits in delayed ve rbal recall can serve as a trait marker for MDD out of hospitalization in unipolar depression.

Úvod: Předchozí studie u pacientů s unipolární depresí ukázaly, že časný pokles EEG kordance (nová metoda kvantitativního EEG – QEEG) může predikovat odpověď na antidepresivní léčbu. Parametr kordance kombinuje komplementární informace absolutního a relativního výkonového EEG spektra. Současné studie prokázaly, že hodnota kordance koreluje s perfuzí mozku. Autoři diskutují vztah kordance a aktivity předního cingula. Cílem studie bylo prokázat, zda časný pokles EEG kordance pøedstavuje obecný fenomé n, který souvisí s odpovědí na léčbu u pacientů s rezistentní depresí. Metoda: Do studie bylo zaøazeno 24 pacientù trpících rezistentní depresí. Jedná se o doplněný soubor 17 pacientů, jehož analýza již byla publikována. Klinický stav a odpověď na léčbu byly hodnoceny před zahájením léčby, po jejím 1. a 4. týdnu. EEG kordance byla vypočtena ze 3 frontálních elektrod ve frekvenčním pásmu theta před zahájením a po 1. týdnu léčby. Výsledky: Všech 24 pacientů dokonèilo 4týdenní studii. U všech 6 respondérů byl zaznamenán pokles v prefrontální kordanci po 1. týdnu léčby. Ve skupině nonrespondérů to bylo pouze u 4 pacientů z 18. Pokles prefrontální kordan ce u respondérů, stejně jako její nárůst u nonrespondérů po 1. týdnu, byl statisticky významný (p=0,03 v obou skupinách) a rozdíl ve změně kordance se mezi oběma skupinami lišil (p = 0,001). Závěr: Naše výsledky naznačují, že QEEG kordance může být slibným nástrojem v predikci odpovědi na antidepresivní léčbu.
Introduction: Previous studies of patients with unipolar depression have shown that early decreases of EEG cordance (a new quanti- tative EEG method) can predict clinical response. Cordance combines complementary informations, from absolute and relative powe r of EEG spectra. Recent studies have shown that cordance correlates with cortical perfusion. The authors discuss connection betw een cordance and activity of anterior cingulum. The aim of study was to examine whether early QEEG decrease represents a phenomenon associated with response to treatment with different antidepressants in patients with treatment resistant depression. Method: The subjects were 24 inpatients with treatment resistant depression. It is extended sample (17 inpatients) from our work, which was pub- lished previously. Clinical status and response to treatment were monitored at baseline and after 1 and 4 weeks on an antidepre ssant treatment. QEEG cordance was computed at 3 frontal electrodes in theta frequency band in baseline and after week 1. Results: All 24 patients completed the 4-week study. All six responders showed decreases in prefrontal cordance after the first week of trea tment. Only 4 of the 18 nonresponders showed early prefrontal cordance decrease. The decrease of prefrontal QEEG cordance after week 1 in responders as well as the increase in nonresponders were both statistically significant (p-value 0.03 in both groups) and the c hanges of prefrontal cordance values were different between both groups (p-value 0.001). Conclusion: Our results suggest that QEEG cordance may become a useful tool in the prediction of response to antidepressants.

Kognitivní funkce jsou u unipolární deprese často postiženy, a to i v případech, kdy odezní akutní epizoda, v období tzv. remise či úplné úzdravy. Dřívější chápání kognitivního deficitu jako pseudodemence, tj. přechodného zhoršení kognitivních funkcí, nahrazuje představa o dlouhodobé nedostačivosti některých složek kognitivních funkcí u přinejmenším některých pacientů, které nemusí být závislé jen na míře a hloubce deprese samotné. Sledování dynamiky kognitivního deficitu, vztahů ke klinickým a demografickým proměnným má význam pro pochopení proměnlivosti či trvanlivosti deficitu a možná v budoucnu i k cílenější terapeutické intervenci u nemocných. Projekt podporuje sledování kognitivního deficitu u širšího souboru osob s unipolární depresivní poruchou pomocí podrobného neuropsychologického vyšetření.
Cognitive functions in unipolar depression are affected even after succesful treatment of depressive episode, during remission or fully recovery. Earlier understanding of cognitive deficits as pseudodementia, e.g. transition worsening of cognitive functions is at present replace by the idea of a long-term insufficiency of some aspects of cognitive functioning at least in some patients, which could not be dependent on extent and depths of illness alone. Following changes of cognitive deficits, relationships to clinical and demographical variables could improve our understanding of variability or endurance of cognitive deficits and maybe, in the future, to support more directed terapeutical interventions to the patients. The project support monitoring of cognitive deficits at larger sample of patiens with unipolar depression through detailed neuropsychological assessment.
