BACKGROUND: Clinically silent uterine rupture with complete fetal expulsion into the abdominal cavity is an extremely rare complication. Diagnosis can be difficult and the risk to the mother and fetus is high. Conservative management has been described only in a few cases of partial expulsion of the fetus so far. CASE PRESENTATION: We present a case of 43-year-old tercigravida with a history of previous laparotomic myomectomy and subsequent cesarean section. The subsequent pregnancy was complicated by uterine wall loosening and rupture at the site of the previous uterine scar after myomectomy and complete fetal expulsion into the abdominal cavity. The diagnosis was made at 24 + 6 weeks of gestation. Considering the absence of clinical symptomatology and the good condition of the fetus, a conservative approach was chosen with intensive monitoring of the maternal and fetal conditions. The pregnancy ended by elective cesarean section and hysterectomy at 28 + 0 weeks of gestation. The postpartum course was uneventful and the newborn was discharged to home care 63 days after delivery. CONCLUSIONS: Fetal expulsion into the abdominal cavity after silent uterine rupture of the scarred uterus may be accompanied by minimal symptomatology making early diagnosis difficult. This rare complication must be considered in the differential diagnosis in women after major uterine surgery. In selected cases and under conditions of intensive maternal and fetal monitoring, conservative management may be chosen to reduce the risks associated with prematurity.

This work presents a case series of four children diagnosed with severe cerebrovascular disease in association with recent severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, yet no patient from the group met typical diagnostic criteria for multisystem inflammatory syndrome in children. Our aim was to highlight the possible vascular involvement and coagulopathies associated with SARS-CoV-2 infection in the pediatric population. Further data are needed to better understand the pathophysiological basis of this condition in children and to ensure its optimal management.

Kazuistika popisuje případ 17leté dívky opakovaně vyšetřované pro dlouhodobé bolesti břicha. Její potíže splňovaly kritéria pro funkční zácpu dle IV. revize Římské klasifikace, a proto jí bylo doporučeno užívání osmotických laxativ, režimová opatření a rehabilitace. Tato terapie však neměla dostatečný efekt. Pro přetrvávání potíží a progresi bolestí břicha při defekaci bylo doplněno proktologické vyšetření. To odhalilo možnou příčinu obtíží pacientky, a to anus ventralis - vrozenou anomálii řazenou ke skupině anorektálních malformací. Anus ventralis je stav, při kterém se na hrázi nachází anální otvor normálního vzhledu, ovšem v různé míře dislokovaný ventrálním směrem. Tento posun může být velmi mírný, a unikat tak odhalení, jak tomu bylo přes opakovaná vyšetření i v případě naší pacientky. Obvyklou manifestací této anomálie je nástup obstipace brzy po porodu, nicméně k rozvoji chronických potíží může dojít i později. Operace v tomto případě přinesla jejich řešení. Cílem sdělení je rozšířit povědomí o této méně obvyklé, avšak nikoli vzácné, organické příčině zácpy a seznámit čtenáře s možnostmi její diagnostiky a terapie.
We describe the case of a 17year old girl who was repeatedly examined for chronic abdominal pain. Her symptoms met the Rome IV. criteria for functional obstipation and therefore she was recommended the use of osmotic laxatives, régime measures and physiotherapy. However, this therapy brought insufficient effect. For ongoing symptomatology, further diagnostic steps were performed including proctological examination by which anus ventralis was diagnosed. This anorectal malformation represents a condition in which the anal orifice of a normal appearance is shifted in the ventral direction. Clinical symptoms depend on the level of the anal orifice shift. The usual manifestation of this anomaly is the onset of obstipation soon after birth, but if only slight shift is present, chronic problems may develop over time. Surgical reconstruction brought permanent improvement of the symptoms. The aim of this case report is to raise awareness of this less common, but not rare, organic cause of chronic obstipation and to describe its therapeutic and diagnostic modalities.

Východiska: Zánětlivý myofibroblastický nádor jícnu je vzácná mezenchymální neoplazie středně maligního potenciálu. Přestože se nejčastěji vyskytuje v plicích, může se objevit v mnoha jiných anatomických lokalizacích vč. gastrointestinálního traktu, avšak postižení jícnu je velmi vzácné. Myofibroblastický nádor jícnu se nejčastěji manifestuje u dětí a mladých dospělých. Základním terapeutickým postupem je chirurgická resekce. Kazuistika: V této kazuistice prezentujeme případ 13letého chlapce s myofibroblastickým nádorem jícnu, který podstoupil chirurgickou resekci v roce 2013 a od té doby je v trvalé remisi. Závěr: Chirurgická léčba je nejvíce upřednostňovanou metodou léčby. V případě kompletní resekce je riziko recidivy nízké. Nicméně každého pacienta po resekci je třeba pečlivě dispenzarizovat.
Background: Inflammatory myofibroblastic tumor (IMT) is a rare mesenchymal neoplasm with intermediate malignant potential. Although most often seen in the lungs, it can occur at multiple anatomical locations, including the gastrointestinal tract. An esophageal lesion is extremely rare, however. IMTs present most commonly in children and young adults. The main therapeutic approach is surgical resection. Case report: We report on the follow-up of a case in a 13-year-old boy with IMT in the esophagus. He underwent surgical resection in 2013 and is free of disease to date. Conclusion: Surgical resection is the most preferred therapy. If the resection is complete, the risk of recurrence is low. Nevertheless, every patient should be carefully followed up after the resection.
