V terapii chronické lymfocytární leukemie (CLL) došlo v posledním desetiletí k zásadním změnám s výrazným zlepšením léčebných výsledků a delším přežitím pacientů s tímto onemocněním. Revoluční změnou byl příchod cílených léků, a to zejména inhibitorů Brutonovy tyrozinkinázy (akalabrutinib, ibrutinib, zanubrutinib) a Bcl2 inhibitorů (venetoklax). Tyto malé molekuly již i v běžné praxi prakticky nahradily chemoterapii, a to nejdříve v relapsech, a v poslední době i v první linii léčby CLL. Mezi hlavní výhody cílené terapie patří lepší tolerance léčby a vysoká účinnost, a to i u skupiny pacientů s prognosticky nepříznivou aberací genu TP53, kteří zpravidla byli refrakterní na chemoterapii. Tento článek shrnuje aktuální možnosti cílené terapie a hlavní trendy vývoje léčby CLL.
The treatment of chronic lymphocytic leukemia (CLL) has undergone profound changes in the last decade with significant improvements in therapeutic outcomes and longer survival of patients with this disease. A major change has been the advent of targeted drugs, especially Bruton's tyrosine kinase inhibitors (acalabrutinib, ibrutinib, zanubrutinib) and Bcl2 inhibitors (venetoclax). These small molecules have already practically replaced chemotherapy in routine practice, first in relapse setting and more recently in first-line treatment of CLL. The main advantages of targeted therapy include better tolerance to treatment and high efficacy, including in the group of patients with prognostically unfavourable TP53 gene aberrations, who were usually refractory to chemotherapy. This article summarizes the current options for targeted therapy and the main trends in CLL treatment.

Aktuálně dostupná data z klinických studií jednoznačně dokládají benefit cílené terapie oproti chemoimunoterapii v léčbě relabované či refrakterní chronické lymfocytární leukemie (R/R CLL). Jednou z dominantně používaných modalit cílené léčby je kombinace antiCD20 protilátky rituximabu a inhibitoru antiapoptotického proteinu BCL-2 venetoklaxu, jejíž efektivita a bezpečnost byly prokázány v rámci studie Murano. Hlavními výhodami tohoto režimu je časově limitovaná terapie, vysoké procento dosažených dlouhotrvajících léčebných odpovědí a vysoké procento hlubokých remisí na úrovni měřitelné reziduální choroby (MRD). Časově limitovaná terapie také umožňuje diskuzi o možnosti opakování léčby v případě laboratorního či klinického relapsu. Robustní data podporující tento přístup však zatím chybí. Venetoklax navíc dosahuje velmi dobrých výsledků i ve skupině vysoce rizikových pacientů s mutací TP53. Z nežádoucích účinků je třeba jmenovat především syndrom z nádorového rozpadu během ramp-up periody a cytopenie vyskytující se v průběhu léčby. Naše kazuistika dokumentuje případ pacienta s chronickou lymfocytární leukemií, včetně prokázané mutace TP53, u kterého byla ve druhé linii léčby použita kombinace rituximab-venetoklax (R-Ven) s velmi dobrou klinickou odpovědí.
Currently available data from clinical trials clearly demonstrate the benefit of targeted therapy over chemoimmunotherapy in the treatment of relapsed or refractory chronic lymphocytic leukemia (R/R CLL). One of the predominantly used modalities of targeted therapy is the combination of the antiCD20 antibody rituximab and the BCL-2 antiapoptotic protein inhibitor venetoclax, whose efficacy and safety were confirmed in the Murano trial. The main advantages of this regimen are the time-limited therapy, the high percentage of long-lasting treatment responses and the high percentage of deep remissions at the measurable residual disease (MRD) level. In addition, the time-limited therapy allows discussion of the possibility of repeating the treatment in case of laboratory or clinical relapse. However, robust data supporting this approach are still lacking. Venetoclax also achieves very good results in the group of high-risk patients with TP53 mutation. Among the side effects, it is important to mention tumor-lysis syndrome during the ramp-up period and cytopenias occurring in the course of treatment. Our case report documents the case of a patient with chronic lymphocytic leukemia, including a proven TP53 mutation, in whom the rituximab-venetoclax combination (R-Ven) was used as a second-line treatment with a very good clinical response.

BACKGROUND: Patients with chronic lymphocytic leukemia (CLL) have a higher risk of developing other malignancies (OMs) compared to the general population. However, the impact of CLL-related risk factors and CLL-directed treatment is still unclear and represents the focus of this work. METHODS: We conducted a retrospective international multicenter study to assess the incidence of OMs and detect potential risk factors in 19,705 patients with CLL, small lymphocytic lymphoma, or high-count CLL-like monoclonal B-cell lymphocytosis, diagnosed between 2000 and 2016. Data collection took place between October 2020 and March 2022. FINDINGS: In 129,254 years of follow-up after CLL diagnosis, 3513 OMs were diagnosed (27.2 OMs/1000 person-years). The most common hematological OMs were Richter transformation, myelodysplastic syndrome (MDS) and acute myeloid leukemia (AML). Non-melanoma skin (NMSC) and prostate cancers were the most common solid tumors (STs).The only predictor for MDS and AML development was treatment with fludarabine and cyclophosphamide with/without rituximab (FC ± R) (OR = 3.7; 95% CI = 2.79-4.91; p < 0.001). STs were more frequent in males and patients with unmutated immunoglobulin heavy variable genes (OR = 1.77; 95% CI = 1.49-2.11; p < 0.001/OR = 1.89; 95% CI = 1.6-2.24; p < 0.001).CLL-directed treatment was associated with non-melanoma skin and prostate cancers (OR = 1.8; 95% CI = 1.36-2.41; p < 0.001/OR = 2.11; 95% CI = 1.12-3.97; p = 0.021). In contrast, breast cancers were more frequent in untreated patients (OR = 0.17; 95% CI = 0.08-0.33; p < 0.001).Patients with CLL and an OM had inferior overall survival (OS) than those without. AML and MDS conferred the worst OS (p < 0.001). INTERPRETATION: OMs in CLL impact on OS. Treatment for CLL increased the risk for AML/MDS, prostate cancer, and NMSC. This is the first large-scale cross-country analysis of patients with chronic lymphocytic leukemia (CLL) aimed to evaluate the incidence, types, and key prognostic factors of secondary malignancies, and to assess the impact on overall survival based on retrospective claims data from three Central European countries. We analyzed 25,814 newly diagnosed CLL patients from Czechia, Hungary, and Poland; 10,312 (39.9%) patients were treated for CLL in study periods between 2004 and 2016. Out of the treated patients, 1986 (19.3%) received the FCR therapy in the first line and 779 (7.6%) received FCR in subsequent lines. We observed that 33.7% of treated patients developed secondary malignancies during the study. Based on country estimates, the probability to develop a secondary malignancy within 4 years since starting the first-line FCR therapy ranged between 28.0% and 36.8%. We found the age at diagnosis, male gender, any malignancy prior to the CLL diagnosis, and the CLL treatment to be the key risk factors for developing secondary malignancies. Specifically, the FCR therapy was a statistically significant (p < 0.001) prognostic factor for risk increase with the hazard ratio between 1.46 and 1.60. Across the three Central European countries, we observed consistent results indicating FCR increased the risk of secondary malignancies in CLL patients. We conclude that secondary malignancies are clearly an undervalued burden for CLL patients, caregivers, and the healthcare system. When evaluating new therapies in regulatory and reimbursement decision making, the factor of secondary malignancies deserves deeper considerations.

Richterův syndrom (RS) je závažnou komplikací chronické lymfocytární leukemie, která se ročně vyskytuje ve 2-10 % případů. I přes nové modality léčby na poli hematoonkologie zůstává RS zásadní klinickou výzvou. Tato kazuistika prezentuje případ pacienta, u kterého i přes využití různých novodobých léčebných přístupů nedošlo ke kontrole onemocnění. Tento případ tak demonstruje krajně nepříznivou prognózu pacientů s RS a klinickou naléhavost vývoje nových terapeutických přístupů.
Richter ́s syndrome (RS) is a serious complication of chronic lymphocytic leukemia, occurring yearly in 2-10 % of cases. Despite new treatment modalities in the field of haematooncology, RS remains a major clinical challenge. This case report presents a patient in whom disease control was not achieved despite the use of various novel therapeutic approaches. This case thus demonstrates the extremely unfavorable prognosis of RS patients and the clinical urgency of developing new therapeutic approaches.

Inhibitory Brutonovy kinázy (BTKi) jsou první skupinou cílených léčiv, která výrazně zlepšila prognózu i kvalitu života nemocných s chronickou lymfocytární leukemií (CLL). Specifické nežádoucí účinky BTKi, zejména kardiovaskulární (KV) a krvácivé, však mohou limitovat jejich dlouhodobé užívání. Ibrutinib, BTKi první generace, byl pro nežádoucí účinky přerušen až u 26 % pacientů léčených v rámci klinických studií. I z těchto důvodů pokračují snahy o vývoj dalších generací BTKi, které by měly bezpečnější profil při zachovaném protinádorovém účinku. Patří mezi ně kovalentně se vázající BTKi akalabrutinib a zanubrutinib a reverzibilní, nekovalentní BTK inhibitory pirtobrutinib a nemtabrutinib. V České republice je v současnosti z veřejného zdravotního pojištění hrazen akalabrutinib. V rámci prevence KV komplikací je zásadní stanovení KV rizika u konkrétního pacienta; pro tyto potřeby lze využít jednoduché skórovací systémy typu Framinghamského skóre nebo tabulky SCORE. U nemocných s vysokým KV rizikem je doporučována léčba BTKi 2. generace nebo léky ze skupiny bcl-2 inhibitorů, případně inhibitorů fosfatidylinositol-3-kinázy. Pokud dojde k rozvoji fibrilace síní nebo srdečního selhání, je nezbytná spolupráce s kardiologem. Riziko závažného krvácení je, na rozdíl od KV komplikací, podobné u obou dostupných preparátů (2–9 % u ibrutinibu vs. 2–5 % u akalabrutinibu). V prevenci klinicky manifestního krvácení je zásadní přerušení léčby BTKi s dostatečným předstihem před každým invazivním výkonem a znovuzahájení terapie až ve chvíli, kdy je riziko krvácení již minimální. Důležitým faktorem pro snížení rizika rozvoje jakékoli komplikace je znalost stávající medikace pacienta a potenciálních lékových interakcí s BTKi. Tato práce shrnuje mechanizmus vniku kardiovaskulárních a krvácivých komplikací, jejich incidenci ve vybraných klinických studiích a doporučení pro prevenci a léčbu těchto nežádoucích účinků v běžné klinické praxi.
Bruton’s tyrosine kinase (BTK) inhibitors have altered the treatment landscape of chronic lymphocytic leukaemia (CLL). These highly eff ective drugs improve not only the prognosis but also the quality of life of CLL patients. Long-term BTK inhibitor treatment can be limited by specifi c adverse events (AEs) such as cardiovascular (CV) complications or bleeding. Ibrutinib, the fi rst-in-class BTK inhibitor, was discontinued in up to 26% of patients in clinical trials due to AEs. Therefore, there are continuing eff orts to develop BTK inhibitors with the same eff ectivity but better safety profi le, such as covalent BTKi acalabrutinib and zanubrutinib and non-covalent BTKi pirtobrutinib and nemtabrutinib. The pre-treatment workup for all patients should include CV risk level assessment using scoring systems, e. g., Framingham risk score or SCORE. In patients with high CV risk levels, next-generation BTK inhibitors or other targeted drugs (venetoclax or idelalisib) are generally preferred over ibrutinib. Patients who experience CV toxicity, particularly atrial fi brillation or heart failure, should be consulted with a cardiologist to defi ne the best treatment algorithm. In contrast to CV toxicity, the risk of major bleeding events is equal for both ibrutinib and acalabrutinib (2–9% vs. 2–5%) based on data from clinical trials. Regarding prevention of bleeding events, BTK inhibitor treatment should be appropriately held prior to any invasive procedure and cannot be restarted until the risk of bleeding is minimal. Good knowledge of the patient’s current medication and potential interactions is crucial in the prevention of any adverse event. This review describes the mechanisms of pathogenesis of cardiovascular complications and bleeding in BTK inhibitor-treated patients. It summarises their incidence in selected clinical trials and provides recommendations for managing these AEs in clinical practice.

In this retrospective international multicenter study, we describe the clinical characteristics and outcomes of patients with chronic lymphocytic leukemia (CLL) and related disorders (small lymphocytic lymphoma and high-count monoclonal B lymphocytosis) infected by SARS-CoV-2, including the development of post-COVID condition. Data from 1540 patients with CLL infected by SARS-CoV-2 from January 2020 to May 2022 were included in the analysis and assigned to four phases based on cases disposition and SARS-CoV-2 variants emergence. Post-COVID condition was defined according to the WHO criteria. Patients infected during the most recent phases of the pandemic, though carrying a higher comorbidity burden, were less often hospitalized, rarely needed intensive care unit admission, or died compared to patients infected during the initial phases. The 4-month overall survival (OS) improved through the phases, from 68% to 83%, p = .0015. Age, comorbidity, CLL-directed treatment, but not vaccination status, emerged as risk factors for mortality. Among survivors, 6.65% patients had a reinfection, usually milder than the initial one, and 16.5% developed post-COVID condition. The latter was characterized by fatigue, dyspnea, lasting cough, and impaired concentration. Infection severity was the only risk factor for developing post-COVID. The median time to resolution of the post-COVID condition was 4.7 months. OS in patients with CLL improved during the different phases of the pandemic, likely due to the improvement of prophylactic and therapeutic measures against SARS-CoV-2 as well as the emergence of milder variants. However, mortality remained relevant and a significant number of patients developed post-COVID conditions, warranting further investigations.
