PURPOSE: In the phase III CheckMate 238 study, adjuvant nivolumab significantly improved recurrence-free survival (RFS) and distant metastasis-free survival versus ipilimumab in patients with resected stage IIIB-C or stage IV melanoma, with benefit sustained at 4 years. We report updated 5-year efficacy and biomarker findings. PATIENTS AND METHODS: Patients with resected stage IIIB-C/IV melanoma were stratified by stage and baseline programmed death cell ligand 1 (PD-L1) expression and received nivolumab 3 mg/kg every 2 weeks or ipilimumab 10 mg/kg every 3 weeks for four doses and then every 12 weeks, both intravenously for 1 year until disease recurrence, unacceptable toxicity, or withdrawal of consent. The primary endpoint was RFS. RESULTS: At a minimum follow-up of 62 months, RFS with nivolumab remained superior to ipilimumab (HR = 0.72; 95% confidence interval, 0.60-0.86; 5-year rates of 50% vs. 39%). Five-year distant metastasis-free survival (DMFS) rates were 58% with nivolumab versus 51% with ipilimumab. Five-year overall survival (OS) rates were 76% with nivolumab and 72% with ipilimumab (75% data maturity: 228 of 302 planned events). Higher levels of tumor mutational burden (TMB), tumor PD-L1, intratumoral CD8+ T cells and IFNγ-associated gene expression signature, and lower levels of peripheral serum C-reactive protein were associated with improved RFS and OS with both nivolumab and ipilimumab, albeit with limited clinically meaningful predictive value. CONCLUSIONS: Nivolumab is a proven adjuvant treatment for resected melanoma at high risk of recurrence, with sustained, long-term improvement in RFS and DMFS compared with ipilimumab and high OS rates. Identification of additional biomarkers is needed to better predict treatment outcome. See related commentary by Augustin and Luke, p. 3253.

Současně prokazuje významnou prevalenci souběžných onemocnění a rizikových faktorů a dlouhé období nedostatečné léčby před nasazením cílené léčby.

As of 31 December 2022, there were a total of 443 patients with hidradenitis suppurativa in the registry of mean age 44.6 years and 56.13% were men. The mean age at the time of diagnosis was 33.0 years and the age at first biological treatment was 42.2 years. The average time from diagnosis to the start of the biological treatment was 9.3 years and patients were treated for an average of 2.4 years. In 2022, a total of 94 patients started the biological treatment. At the end of 2022, a total of 367 patients were treated and the average duration of treatment was 2.3 years, the most common medication was Humira (84.7%). At the end of 2022, there were a total of 808 patients with atopic dermatitis in the registry of mean age 36.2 years, 53.5% were men. The mean age at the time of diagnosis was 5.0 years and the age at first targeted treatment was 35.2 years. The average time from diagnosis to the start of the targeted treatment was 30.2 years and patients were treated for an average of 1.5 years. A total of 64.1% of patients had concomitant allergic rhinitis, 57.7% food allergy and 44.4% allergic asthma. Ocular comorbidities occurred in 28.7% of patients, the most common were atopic conjunctivitis (15.3%). At the start of treatment and at the last visit, the mean EASI was 31.6 and 4.4, respectively, mean BSA was 56.4% and 8.4%, respectively, and mean DLQI 18.4 and 3.8 respectively. In 2022, a total of 306 patients started the biological treatment. At the end of 2022, a total of 757 patients were treated and the average duration of treatment was 1.4 years, the most common medication was Dupixent (91.8%). Conclusion: BIOREP is the first registry of patients with psoriasis treated with biologics in Central and Eastern Europe. The analysis confirmed the high activity of diseases according to objective evaluation and the significant negative impact on the quality of life before initiation of targeted treatment, with a decrease in the evaluated scores during treatment. At the same time, it demonstrates a high prevalence of comorbidities and risk factors and a long diseases duration before the introduction of targeted treatment.

Mycosis fungoides is a T-cell lymphoma with characteristic skin involvement. In early stages, the diagnosis making could be chalenging, however, advanced stages has their specific signature both in skin and blood. Recently, therapeutical options increased significantly and patients are experiencing long-term disease free interval together with extended overall survival.

Text prošel Pracovní skupinou odborníků obou odborných společností a schválily ho výbory České dermatologické společnosti (ČDS) ČLS JEP a České společnosti alergologie a klinické imunologie (ČSAKI).
Chronic spontaneous urticaria is relatively common disease with considerable impact on patient´s quality of life, impairing the performance at school, work and in daily life. Early diagnostics and early appropriate treatment are crucial for getting the disease and it’s symptoms under control. The update and revision of international guidelines for the treatment of chronic urticaria (2021) as a joint initiative of The European Academy of Allergy and Clinical Immunology (EAACI), Global Allergy and Asthma European Network (GA2LEN), European Dermatology Forum (EDF) and The Asia Pacific Association of Allergy, Asthma and Clinical Immunology (APAAACI) represent a suitable tool for clinical practice in the Czech Republic. Proven non-sedative 2nd generation antihistamines are the first line of therapy, even in increased dosage. These can be administered even by general practitioners as they are often the first physicians seen by the patient. Omalizumab is added to the antihistamine as a second line, and now also at higher doses. In the third line, cyclosporin A is in combination with an antihistamine. This treatment is in the competence of specialists in specialized centers. Due to the interdisciplinary character of chronic urticaria, it is effective and beneficial to manage the treatment of patients with chronic urticaria not only by dermatologists, but also by allergologists and clinical immunologist, including higher-line treatments such as biologics and immunosuppressants and kinase inhibitors and other drugs in the future. The article was revised by the Expert Working Group of both professional societies and approved by the committees of The Czech Dermatovenerological Society and The Czech Society for Allergology and Clinical Immunology.
