BACKGROUND: Clinical parameters linked to a low benefit of ICD implantation and increased mortality risks are needed for an individualized assessment of potential benefits and risks of ICD implantation. METHODS: Analysis of a prospective registry of all patients hospitalized from 2009 to 2019 in a single centre for a first implantation of any type of ICD. RESULTS: A total of 2,681 patients were included in the registry. Until the end of follow-up (38.4 ± 29.1 months), 682 (25.4%) patients died. The one-year mortality in all patients, the one-year CV mortality, the three-year mortality in all patients, and the three-year CV mortality were 7.8%, 5.7%, 20.6%, and 14.8%, respectively. There was a statistically significant difference when the subgroups were compared according to the type of cardiomyopathy. No significant difference was found between primary and secondary prevention and between the types of devices. Male gender, age ≥ 75 years, diabetes mellitus, and atrial fibrillation were associated with a significantly increased mortality risk. CONCLUSION: In an analysis of a long-term follow-up of 2,681 ICD patients, we found no mortality difference between patients with ischemic or non-ischemic cardiomyopathy and in the device type. A higher mortality risk was found in men, patients older than 75 years, diabetics, and those with atrial fibrillation.

AIM: Retrospective national sub-analysis of antidiabetic pharmacotherapy in patients with diabetes mellitus (DM) and heart failure (HF) based on data reported to the National Register of Paid Health Services in the Czech Republic between 2012-2018. METHODOLOGY AND RESULTS: In 2012, there were 75,022 patients with HF and DM (i.e. 42.5% of patients with HF), 6 years later 117,265 (i.e. 41.0% of HF patients in 2018). The most represented antidiabetic drug was metformin (45.6%). Of the insulins and analogues, glargine showed the largest positive trend (5.8% 2012; 14.8% 2018). Empagliflozin was the most prescribed SGLT-2 inhibitor (1.8% in 2018). A decrease in prescribing was observed for saxagliptin (0.5% 2012; 0.1% 2018) and for sulfonylurea derivates - gliclazide (13.0% 2012; 10.3% in 2018) and glimepiride (12.9% 2012; 9.0% 2018). Linagliptin was the most prescribed dipeptidyl peptidase inhibitor (0.7% 2012; 6.8% 2018). CONCLUSION: In the Czech Republic, between 2012 and 2008, there was an increase in prevalence of patients with heart failure and concomitant diabetes mellitus, their proportion being similar. In correspondence with other registries, metformin was used mostly. A positive trend was observed in prescription of DDP-4 and SGLT-2 inhibitors, while there was a significant decrease in patients taking sulfonylureas.

Data získaná z Národního registru hrazených zdravotních služeb (NRHZS) o vývoji poskytované péče diabetology, internisty a praktickými lékaři v letech 2010 až 2021 dokládají narůstající počet i procentuální zastoupení pacientů s diabetem léčených zejména neinzulinovými antidiabetiky praktickými lékaři (vzestup z 4,7 % na 22,8 % diabetiků). Zároveň data poskytují informaci o stagnaci počtu diabetiků léčených jen inzulinem. Registrovaný vývojový trend v poskytování péče lékaři podněcuje diskuzi o dalším směrování včetně potřeby zajistit kvalitní strukturovaný a multidisciplinární přístup k pacientům s diabetem v následujících letech.
Data obtained from the National register of covered medical services (NHRZS) on provided care developed by diabetologists, general practitioners and internists since 2010 to 2021 confirm an increased number and percentage of diabetic patients treated especially by non-insulin antidiabetic drugs by general practitioners (an increase from 4.7 % to 22,8 % diabetic patients). At the same time data offer an information on stagnation of diabetic patients treated by insulin only. The trend registered in provided care stimulates a discussion on future way including the needs for high quality of structured and multidisciplinary approach offered to diabetic patients in future years.

Se zvyšující se nadějí dožití se zvyšuje prevalence diabetes mellitus (DM). Specifická problematika péče o nemocné seniory vyžaduje v terapii DM ve vyšším věku vytvoření doporučeného postupu, který vznikne na základě mezioborové spolupráce.
As life expectancy increases, the prevalence of DM increases. The specific issue of caring for sick seniors requires the creation of a recommended procedure in DM therapy at an older age, which will be created on the basis of interdisciplinary cooperation.

Cíl: Cílem studie je srovnání dostupných dat o výskytu dekubitů z národních databází s daty konkrétního poskytovatele zdravotních služeb – Fakultní nemocnice Ostrava, identifikace rozdílů a jejich příčin. Soubor a metodika: Retrospektivní observační studie. Soubor zahrnoval data hospitalizací pacientů s dekubitem z Národního registru hrazených zdravotních služeb (NRHZS) a Nemocničního informačního systému FN Ostrava (NIS FNO) v období 2018–2020. Výsledky: V letech 2018–2020 bylo do NRHZS nahlášeno celkem 294 hospitalizací pacientů s dekubitem, zatímco v NIS FNO jich bylo evidováno 2 231. Počet hospitalizací s dekubitem byl v NIS FNO až 8x vyšší, než lze detekovat z vykazování diagnózy L89 dle MKN-10 pro zdravotní pojišťovny. Na jednu hospitalizaci připadá průměrně 1,6 dekubitu. Strukturální rozdíly výskytu dekubitů v NIS FNO a NRHZS (větší podíl nižších kategorií dekubitů, nižší věkový průměr a vyšší mortalita pacientů s dekubitem) jsou pravděpodobně důsledkem toho, že v NRHZS je pacient zaznamenán jednou bez ohledu na počet dekubitů, a protože dekubity nižší kategorie nejsou vykazovány, neboť většinou nevyžadují výkon identifikovatelný v sazebníku. Závěr: Studie prokázala, že skutečná prevalence dekubitálních lézí může být významně vyšší, než ukazují dostupné celostátní statistiky NRHZS. Výsledky podporují význam a nezbytnost výzkumu v oblasti nákladovosti prevence a léčby dekubitů.
Aim: The study aims to compare the available data on the prevalence of pressure ulcers from national databases with the data of a specific provider of health services – University Hospital Ostrava, analysis of the differences found and identification of their causes. Materials and methods: Retrospective observational study. The file included data on hospitalisations of patients with pressure ulcers from the National Register of Reimbursed Health Services (NRRHS) and the Hospital Information System of the University Hospital Ostrava (HIS UHO) from 2018 to 2020. Results: Between 2018 and 2020, a total of 294 hospitalisations of patients with pressure ulcers were reported to the NRRHS, while 2,231 were registered in the HIS UHO. The number of hospitalisations with pressure ulcers was up to eight times higher in the analysed period in the HIS UHO than can be detected from the diagnosis with the code L89 according to ICD-10 for health insurance companies. There is an average of 1.6 pressure ulcers per hospitalisation. The structural differences in the prevalence of pressure ulcers between HIS UHO and NRRHS (higher proportion of lower pressure ulcer categories, lower average age of patients with pressure ulcers and higher mortality of patients with pressure ulcers) are probably because a patient appears once in NRRHS regardless of the number of pressure ulcers and because lower pressure ulcer categories are not reported because they usually do not require an identifiable procedure from the list of medical procedures. Conclusion: The study showed that the actual prevalence of pressure ulcers could be significantly higher than the available national NRRHS statistics. The study results also support the importance and necessity of research on the cost-effectiveness of the prevention and treatment of pressure ulcers.
