Klíšťata jsou vektorem a často současně rezervoárem virových i bakteriálních onemocnění. Člověk se obvykle nakazí po přisátí infikovaného klíštěte nebo jeho vývojového stadia jako náhodný článek jeho životního cyklu. Tato onemocnění mají vzhledem ke geografickým podmínkám v České republice značný epidemiologický význam a dopad na morbiditu. Předmětem sdělení je popis infekcí v našich podmínkách, jejich epidemiologie, klinický obraz a diagnostika se zaměřením na specifika dětského věku. Lymeská borrelióza, jako nejčastější z těchto onemocnění, je tématem samostatného sdělení.
Ticks are a vector and often a reservoir of viral and bacterial diseases. A person usually gets infected after the infestation of an infected tick or its developmental stage as a random link in its life cycle. Due to the geographical conditions in the Czech Republic, these diseases have a epidemiological significance and impact on morbidity. The communication subject describes infections, their epidemiology, clinical picture, and diagnostics, focusing on the specifics of childhood in our conditions. The subject of a separate communication is Lyme borreliosis, the most common of these presented diseases.

Česká republika je významnou endemickou oblastí lymeské borreliózy. Borrelie jsou přenášené klíšťaty a mají afinitu k řadě orgánů a tkání (kůže, nervový systém, klouby, myokard atd.). S tím souvisí rozmanité klinické projevy, u dětí se nejčastěji setkáváme s erythema migrans, neuroborreliózou a kloubním postižením. Základem laboratorní diagnostiky je v současné době průkaz specifických IgM a IgG protilátek, potvrzující kontakt s infekčním agens, nikoliv aktivitu infekce. Přímé metody mají omezené využití především pro nízkou senzitivitu (PCR) nebo nedostatek klinických dat (vyšetření specifické buněčné imunitní odpovědi). Základem stanovení diagnózy je pečlivé zhodnocení klinického obrazu podpořené výsledky laboratorních testů. V časných stadiích nemoci je efekt antibiotické léčby velmi dobrý. Nedostatečná klinická odpověď by měla vést k rozšíření diagnostického postupu o jiné, i neinfekční příčiny obtíží.
The Czech Republic is an important endemic area of Lyme borreliosis. Borrelia are transmitted by ticks and have an affinity for many organs and tissues (skin, nervous system, joints, myocardium, etc.). This is associated with a variety of clinical manifestations, with erythema migrans, neuroborreliosis and joint involvement being the most common in children. The basis of laboratory diagnosis is the detection of specific IgM and IgG antibodies, confirming contact with the infectious agent, not the activity of infection. Direct methods are of limited use mainly due to low sensitivity (PCR) or lack of clinical data (investigation of specific cellular immune response). A careful evaluation of the clinical picture, supported by laboratory test results, is the basis for diagnosis. In the early stages of the disease, the effect of antibiotic treatment is very good. An insufficient clinical response should lead to an extension of the diagnostic procedure to other, even non-infectious causes of the symptoms.
Cíl práce: Mezi roky 2017 a 2019 došlo ke globálnímu rozšíření viru a vzniku rozsáhlé epidemie spalniček, která náhle skončila na počátku roku 2020. Také v České republice (ČR) probíhaly epidemie spalniček jako součást celosvětového postižení. V současné komplikované epidemiologické situaci nabývá na významu molekulární surveillance, která je nezbytná pro potvrzení importu onemocnění a pro sledování cest přenosu viru. Umožňuje získat důkazy o přerušení endemického šíření a je nepostradatelná pro verifikaci eliminace spalniček. Cílem práce je zjistit, zda některý z genotypů viru spalniček cirkuloval v ČR déle než 12 měsíců, a tím se potvrdilo nebo vyvrátilo endemické šíření viru na našem území ve vztahu k nedávnému odebrání statusu země s eliminovanými spalničkami. Dalším cílem je zhodnocení současné laboratorní diagnostiky v souvislosti s epidemiemi spalniček posledních let a povinností zasílat vzorky ke konfirmaci a genotypizaci. Materiál a metodika: Celkem bylo molekulárně analyzováno 243 pozitivních nazofaryngeálních výtěrů od pacientů z ohnisek nákazy z celé ČR odebraných v letech 2018 a 2019. Molekulární analýza byla provedena sekvenováním nejvariabilnější části genomu viru spalniček – genu nukleoproteinu (N-450), podle metodiky Světové zdravotnické organizace (WHO). Sekvenční analýza byla provedena Sangerovou metodou s využitím sekvenátoru Applied Biosystems 3500, data byla analyzována pomocí bioinformatického softwaru Geneious. Výsledky: V ČR se během epidemie v letech 2018–2019 uplatnily pouze dva genotypy, majoritní D8 v osmi variantách a minoritní B3 v šesti variantách, u 8 vzorků byl detekován genotyp A. Dominující genotyp roku 2017 (D8, 4283) se v ČR poprvé objevil v lednu 2018 a po čtyřech měsících byl nahrazen genotypem D8, 4683, který se v ČR uplatňoval od března 2018 až do června 2019. Vzorků s tímto genotypem bylo 170/243, tj. 70 %. Mezi první a další detekcí tohoto genotypu byly tři měsíce s nulovým záchytem, to ovšem neznamená, že by během těchto měsíců v populaci necirkuloval. Analýza sedmi vzorků z roku 2017 spolupracující Regionální referenční laboratoří v Institutu Roberta Kocha v Berlíně (RRL RKI) prokázala u pěti vzorků z Ostravy příslušnost ke genotypu B3 a dvě různé varianty genotypu D8 (Praha, Liberec). V laboratorní diagnostice byl zaznamenán posun v typu zasílaných klinických materiálů, neboť mezi lety 2017 a 2019 vzrostl podíl vzorků k přímému průkazu viru z 18 % na 43 %. Zasílání vzorků k sekvenaci do Národní referenční laboratoře v Praze (NRL) probíhalo podle nastavených legislativních pravidel. Laboratoře za rok 2018 a 2019 zaslaly celkem 424 vzorků. Sekvenace byla úspěšná u 243 z nich, tj. u téměř 60 %. U ostatních vzorků se sekvenace nepodařila pro nízkou virovou nálož. Závěry: V ČR byla zavedena sekvenace viru spalniček jako nezbytná součást molekulární surveillance, podařilo se analyzovat téměř 60 % pozitivních vzorků. Na základě provedené sekvenční analýzy bylo prokázáno endemické šíření viru spalniček, neboť mezi roky 2018 a 2019 cirkuloval nepřetržitě virus genotypu D8, 4683 MVs/Gir Somnath.IND/42.16 na území České republiky po dobu 16 měsíců. Laboratorní diagnostika se více zaměřuje na přímý průkaz viru, což společně s rozšířením genotypizace o další úsek genomu umožní zkvalitnění molekulární surveillance.
Objective: Between 2017 and 2019, measles virus spread globally, causing a large measles epidemic that suddenly ended in 2020. Measles outbreaks also occurred in the Czech Republic (CR) as part of the global public health problem. In the recent alarming epidemiological situation, molecular surveillance is becoming increasingly important as it plays a vital role in the identification of imported cases and in the monitoring of virus transmission. Molecular surveillance makes it possible to obtain evidence of the discontinuation of the endemic spread and is indispensable for the verification of measles elimination. The study aim is to find out whether any of measles virus genotypes circulated in the CR for more than 12 months in order to either confirm or refute the endemic spread of measles virus in the country in relation to the recent loss of the measles elimination status. Another aim is to assess the current laboratory diagnosis from the perspective of recent measles outbreaks and the obligation to refer samples for confirmation and genotyping. Material and methods: In total, 243 positive nasopharyngeal swabs collected from outbreak patients from all over the CR in 2018 and 2019 were analysed by molecular methods. The most variable part of the measles virus genome, the nucleoprotein gene (N-450), was sequenced according to the WHO protocol. The sequence analysis was performed by Sanger method using the Applied Biosystems 3 500 sequencer, and sequence data were analysed by the bioinformatics programe Geneious. Results: In the CR, only two genotypes were found in measles outbreaks in 2018–2019, eight variants of the dominant D8 and six B3 variants, while genotype A was detected in eight samples. The dominant genotype of 2017 (D8, 4283) was identified for the first time in the CR in January 2018. Four months later, it was replaced by genotype D8, 4683, occurring in the CR from March 2018 to June 2019. This genotype was identified in 170 of 243 samples (70%). There was a 3-month window between the first and the second detection of this genotype, which does not imply that in the meantime the virus did not circulate in the population. The analysis of seven samples from 2017 conducted by the collaborating Regional Reference Laboratory at the Robert Koch Institute (RRL RKI) in Berlin assigned five samples from Ostrava to genotype B3 and detected two variants of genotype D8 (Praha, Liberec). Laboratory diagnosis was facilitated by a higher proportion of clinical specimens available for direct detection of the virus, which increased from 18% in 2017 to 43% in 2019. Samples were referred to the National Reference Laboratory (NRL) in Prague for sequencing in accordance with the set legal rules. Between 2018 – 2019, laboratories sent 424 samples. Two hundred and forty-three samples (60%) were successfully sequenced, while the sequencing of the remaining samples failed due to low viral load. Conclusions: Measles virus sequencing was introduced in the Czech Republic as a necessary part of molecular surveillance, and almost 60% of positive samples were analysed. The sequencing analysis confirmed the endemic spread of measles virus, with genotype D8, 4683 MVs/GirSomnath.IND/42.16 found to circulate in the CR for 16 months between 2018 and 2019. Laboratory diagnosis is recently focusing more on direct detection of the virus, which along with genotyping extended to include another part of the genome will improve molecular surveillance.
