ETHNOPHARMACOLOGICAL RELEVANCE: Ferns form an important part of the human diet. Young fern fiddleheads are mostly consumed as vegetables, while the rhizomes are often extracted for starch. These edible ferns are also often employed in traditional medicine, where all parts of the plant are used, mostly to prepare extracts. These extracts are applied either externally as lotions and baths or internally as potions, decoctions and teas. Ailments traditionally treated with ferns include coughs, colds, fevers, pain, burns and wounds, asthma, rheumatism, diarrhoea, or skin diseases (eczema, rashes, itching, leprosy). AIM OF THE REVIEW: This review aims to compile the worldwide knowledge on the traditional medicinal uses of edible fern species correlating to reported biological activities and isolated bioactive compounds. MATERIALS AND METHODS: The articles and books published on edible fern species were searched through the online databases Web of Science, Pubmed and Google Scholar, with critical evaluation of the hits. The time period up to the end of 2022 was included. RESULTS: First, the edible fern species were identified based on the literature data. A total of 90 fern species were identified that are eaten around the world and are also used in traditional medicine. Ailments treated are often associated with inflammation or bacterial infection. However, only the most common and well-known fern species, were investigated for their biological activity. The most studied species are Blechnum orientale L., Cibotium barometz (L.) J. Sm., Diplazium esculentum (Retz.) Sw., Marsilea minuta L., Osmunda japonica Thunb., Polypodium vulgare L., and Stenochlaena palustris (Burm.) Bedd. Most of the fern extracts have been studied for their antioxidant, anti-inflammatory and antimicrobial activities. Not surprisingly, antioxidant capacity has been the most studied, with results reported for 28 edible fern species. Ferns have been found to be very rich sources of flavonoids, polyphenols, polyunsaturated fatty acids, carotenoids, terpenoids and steroids and most of these compounds are remarkable free radical scavengers responsible for the outstanding antioxidant capacity of fern extracts. As far as clinical trials are concerned, extracts from only three edible fern species have been evaluated. CONCLUSIONS: The extracts of edible fern species exert antioxidant anti-inflammatory and related biological activities, which is consistent with their traditional medicinal use in the treatment of wounds, burns, colds, coughs, skin diseases and intestinal diseases. However, studies to prove pharmacological activities are scarce, and require chemical-biological standardization. Furthermore, correct botanical classification needs to be included in publications to simplify data acquisition. Finally, more in-depth phytochemical studies, allowing the linking of traditional use to pharmacological relevance are needed to be done in a standardized way.

Znalost faktorů a mechanismů, které ovlivňují vstřebávání Levotyroxinu a jeho další využití v organismu, je důležitou podmínkou při snaze o optimalizaci léčby. Mezi ně patří stravovací návyky, interní onemocnění spojená s poruchami vstřebávání u střevních a jiných onemocnění, léky a látky v přírodě zasahující do mechanismu tyreoidálních hormonů - tzv. tyreoidální disruptory. Jejich vlivem může být léčba porušené funkce štítné žlázy neuspokojivá i přes dodržení pravidel substituční léčby.
Thyroid medications are among the most commonly prescribed drugs in our country and worldwide. Knowledge of the factors and mechanisms that influence the absorption of levothyroxine and its further utilization in the body is an important condition in the effort to optimize treatment. These include dietary habits, internal diseases associated with absorption disorders in intestinal and other diseases, drugs and substances in nature interfering with the mechanism of thyroid hormones - the so-called thyroid disruptors. Due to their influence, the treatment of impaired thyroid function may be unsatisfactory despite compliance with the rules of replacement therapy.
Přibližně u 10 % dětí, které se narodily malé na svůj gestační věk (SGA; small for gestational age) s porodní hmotností a/nebo délkou nižší než −2 SD, nedojde po narození k růstovému výšvihu. Tyto děti zůstávají malé po celé dětství (s výškovým skóre směrodatné odchylky [ht-SDS] pod −2,5 SDS) a skončí jako malí dospělí. Označujeme je jako SGA-SS (small for gestational age with short stature). Evropská léková agentura schválila již v roce 2003 SGA-SS jako indikaci pro léčbu růstovým hormonem. Z 1502 dětí SGA-SS evidovaných v české národní databázi příjemců růstového hormonu REPAR již 397 dětí (221 chlapců) ukončilo růst a dosáhlo téměř finální dospělé tělesné výšky. Léčba růstovým hormonem v dávce 0,034 mg/kg/den (medián) byla zahájena u chlapců ve věku 8,5 roku (medián; P5-95: 3,2–15,0), u dívek ve věku 7,3 roku (3,9–13,5). Průměr ht-SDS při zahájení léčby byl −3,21 (SD: 0,91). Léčba byla ukončena u chlapců ve věku 16,2 roku (medián; P5-95: 14,0–18,7 roku), u dívek ve věku 14,3 roku (12,4–17,3). Léčba vedla do okamžiku ukončení léčby k průměrnému přírůstku ht-SDS o 1,59 a k dosažení téměř dospělé výšky (near-final height) ht-SDS −1,62 (SD: 0,97; p < 0,0001 proti výšce před léčbou). Při ukončení léčby tedy většina pacientů dosáhla výšky nad −2 SDS, v rozmezí hodnot běžné populace. Po adjustaci na ht-SDS v 18 letech věku tato výška odpovídá ht-SDS −2,30 (SD: 1,09; p < 0,0001 proti výšce před léčbou). I po této adjustaci, která efekt léčby mírně podhodnocuje, byla léčba růstovým hormonem úspěšná a vedla ke zlepšení v průměru o 0,91 ht-SDS. Výsledky analýzy z databáze REPAR prokazují u dětí SGA-SS účinnost léčby růstovým hormonem a její potenciál významně zlepšit dospělou tělesnou výšku.
Of children born small for gestational age (SGA) with birth weight and/or birth length below −2 SD, about 10% fail to catch-up after birth. These children remain short over childhood (with height standard deviation score [ht-SDS] below −2.5 SDS) and end up as short adults. They are assigned as SGA-SS (small for gestational age with short stature). European Medicine Agency approved growth hormone therapy in SGA-SS already in 2003. Out of 1502 SGA-SS children that are registered in the Czech national database of growth hormone recipients REPAR, 397 children (221 boys) already completed growth and achieved near-final height. Growth hormone therapy (median dose 0.034 mg/kg/day) was initiated at age 8.5 years in boys (median; P5-95: 3.2-15.0) and 7.3 years in girls (3.9-13.5). The mean ht-SDS at treatment initiation was −3.21 (SD: 0.91). Treatment was terminated at age 16.2 years in boys (median; P5-95: 14.0-18.7) and 14.3 years in girls (12.4-17.3). By treatment termination, the patients have had gained 1.59 ht-SDS (mean) and have achieved near-final height −1.62 ht-SDS (SD: 0.97; p<0.0001 if compared to pre-treatment). Thus, majority of patients exceeded −2 SDS at treatment completion and achieved height within normal limits. After adjusting for ht-SDS at age 18 years, their height corresponded to ht-SDS −2.30 (SD: 1.09; p < 0.0001 if compared to pre-treatment). Growth hormone therapy was successful even after this adjustment that mildly underestimates the treatment effect, and led to mean height increase of 0.91 ht-SDS. Analysis results of the REPAR database confirm efficacy of growth hormone therapy in SGA-SS children and its potential to significantly improve the adult height.
O úspěchu léčby růstovým hormonem a dosažení optimální dospělé výšky rozhoduje jednak růst před zahájením puberty a jednak vydatnost pubertálního růstového výšvihu. U kohorty 397 dětí (221 chlapců) z české národní databáze příjemců růstového hormonu REPAR, které se narodily malé na svůj gestační věk, měly malou výšku v dětství (SGA-SS) a již dosáhly své téměř dospělé výšky, jsme vyhodnotili vliv prepubertální a pubertální složky růstu na celkový efekt léčby růstovým hormonem. Odděleně jsme posoudili děti také podle výšky rodičů (oba nad −2 SD výšky, jeden pod −2 SD výšky, oba pod −2 SD výšky) a podle porodních parametrů – nízké porodní hmotnosti, nízké porodní délky či kombinace obojího. Léčba růstovým hormonem vedla do okamžiku ukončení léčby k průměrnému přírůstku ht-SDS o 1,59, z toho za prepubertální období o 1,57. Během puberty se již výška při pokračující léčbě nezlepšila. Po adjustaci na ht-SDS v 18 letech věku stoupla výška ve srovnání se stavem při zahájení léčby o 0,91 ht-SDS (všechny tři parametry p < 0,0001 proti výšce před léčbou). Významně více vyrostly děti s oběma rodiči vyššími než −2 SD a také děti, které měly sníženou výlučně porodní hmotnost, nikoliv porodní délku. Výsledky analýzy z databáze REPAR prokazují účinnost léčby růstovým hormonem u dětí SGA-SS. O efektu léčby rozhoduje především prepubertální složka růstu, proto důsledné sledování růstu dětí narozených SGA a včasné zahájení léčby v časném dětství má pro ně klíčový význam.
Both pre-pubertal growth and magnitude of the pubertal growth spurt are the prerequisites of potential of growth hormone therapy to optimize final height. We analyzed impact of pre-pubertal and pubertal growth components on the total effect of growth hormone therapy in a cohort of 397 children (221 boys) registered in the Czech national database of growth hormone recipients REPAR. All were born small for gestational age, had persistent short stature in childhood (SGA-SS) and already achieved their near-final height. We separately evaluated children according to parental heights (with both parents over −2 ht-SDS, one below −2 ht-SDS, both below −2 ht-SDS) and according to their size at birth – low birth weight, low birth length, low both parameters. Up to treatment completion, treatment led to a height gain of 1,59 ht-SDS; of this, the pre-pubertal gain was 1,57 ht-SDS. Thus, ht-SDS remained unchanged within puberty, despite continuous growth hormone administration. The final ht-SDS gain adjusted for age 18 years was 0,91 (all three parameters p<0.0001 if compared to pre-treatment). Children with both parents taller than −2 SD and children with exclusively low birth weight had significantly better treatment outcomes. Analysis results of the REPAR database confirm efficacy of growth hormone therapy in SGA-SS children. The treatment success is based on pre-pubertal growth component. Therefore, a careful follow-up of children born SGA and a timely initiation of growth hormone therapy in early childhood are of crucial importance.
BACKGROUND: Inborn errors of immunity are genetic disorders characterized by various degrees of immune dysregulation that can manifest as immune deficiency, autoimmunity, or autoinflammation. The routine use of next-generation sequencing in the clinic has facilitated the identification of an ever-increasing number of inborn errors of immunity, revealing the roles of immunologically important genes in human pathologies. However, despite this progress, treatment is still extremely challenging. OBJECTIVE: We sought to report a new monogenic autoinflammatory disorder caused by a de novo activating mutation, p.Tyr515∗, in hematopoietic cell kinase (HCK). The disease is characterized by cutaneous vasculitis and chronic pulmonary inflammation that progresses to fibrosis. METHODS: Whole-exome sequencing, Sanger sequencing, mass spectrometry, and western blotting were performed to identify and characterize the pathogenic HCK mutation. Dysregulation of mutant HCK was confirmed ex vivo in primary cells and in vitro in transduced cell lines. RESULTS: Mutant HCK lacking the C-terminal inhibitory tyrosine Tyr522 exhibited increased kinase activity and enhanced myeloid cell priming, migration and effector functions, such as production of the inflammatory cytokines IL-1β, IL-6, IL-8, and TNF-α, and production of reactive oxygen species. These aberrant functions were reflected by inflammatory leukocyte infiltration of the lungs and skin. Moreover, an overview of the clinical course of the disease, including therapies, provides evidence for the therapeutic efficacy of the Janus kinase 1/2 inhibitor ruxolitinib in inflammatory lung disease. CONCLUSIONS: We propose HCK-driven pulmonary and cutaneous vasculitis as a novel autoinflammatory disorder of inborn errors of immunity.
Selective cyclooxygenase-1 (COX-1) inhibition has got into the spotlight with the discovery of COX-1 upregulation in various cancers and the cardioprotective role of COX-1 in control of thrombocyte aggregation. Yet, COX-1-selective inhibitors are poorly explored. Thus, three series of quinazoline derivatives were prepared and tested for their potential inhibitory activity toward COX-1 and COX-2. Of the prepared compounds, 11 exhibited interesting COX-1 selectivity, with 8 compounds being totally COX-1-selective. The IC50 value of the best quinazoline inhibitor was 64 nM. The structural features ensuring COX-1 selectivity were elucidated using in silico modeling.

Familial hypocalciuric hypercalcemia (FHH) type 1, caused by a heterozygous inactivating mutation of the gene encoding the calcium-sensing receptor (CaSR), is characterized by mild to moderate hypercalcemia, hypocalciuria and inappropriately normal or elevated parathyroid hormone (PTH). FHH must be differentiated from primary hyperparathyroidism (PHPT) because parathyroidectomy is ineffective in the former. Herein, we report a 39-year-old male patient with a 13-year history of asymptomatic PTH-dependent hypercalcemia (mean calcium of 2.88 mmol/l; reference range 2.15-2.55 mmol/l) and calcium-to-creatinine clearance ratio (Ca/Cr) ranging from 0.007 to 0.0198, which is consistent with either FHH or PHPT. Although a family history of hypercalcemia was negative, and PET-CT with fluorocholine was suggestive of a parathyroid adenoma, genetic analysis of the CaSR gene identified a heterozygous inactivating mutation NM_000388.4:c.1670G>A p. (Gly557Glu) in exon 6 and a polymorphism NM_000388.4:c.1192G>A p. (Asp398Asn) in exon 4. The G557E mutation has been previously reported in a Japanese family in which all family members with the mutation had Ca/Cr below 0.01 consistent with FHH. The biochemical profile of FHH and PHPT may overlap. Our FHH patient with a G557E CaSR mutation illustrates that the differential diagnosis can be difficult in an index case with no family history, (false) positive parathyroid imaging and higher calciuria than expected for FHH. Calcium intake, vitamin D status and bone resorption might have contributed to the Ca/Cr variations over a 13-year clinical follow up. This case thus emphasizes the irreplaceable role of genetic testing of the CaSR gene when clinical evaluation is inconclusive.
