V článku předkládáme kazuistiku pacienta, který vyhledal lékaře pro chronické bolesti varlete. V průběhu vyšetřování u něj byla nalezena nádorová duplicita – karcinom ledviny a prostaty. Oba nádory byly úspěšně odoperovány. V dalším průběhu mu musela být provedena oboustranná orchiektomie pro suspektní nález ve varlatech. Dle ultrazvuku a klinického nálezu bylo pravé varle zmenšené, uvnitř jeho parenchymu nejasný heteroechogenní nález. V levém varleti při jeho horním pólu byl nalezen na ultrazvuku suspektní hypoechogenní parenchym. Pro nález odpovídající sekundárnímu hypogonadismu, jsme po domluvě s pacientem, nasadili substituční terapii testosteronem (testosterone replacement therapy TRT). Vzhledem k diagnóze karcinomu prostaty je ovšem nutná dispenzarizace a pečlivé sledování včetně vyšetření prostaty přes konečník, odběrů prostatického specifického antigenu (PSA), testosteronu, jaterních testů, krevního obrazu, ultrazvukového vyšetření orgánu urogenitálního traktu a počítačové tomografie (CT). Substituce testosteronem u pacientů po léčbě pro karcinom prostaty je za určitých podmínek možná a bezpečná léčba sekundárního hypogonadismu.
We present a case report of a patient who visited a physician because of chronic testicular pain. During the investigation, he was found to have two synchronous tumors - carcinoma of the kidney and prostate. Both tumors were successfully removed. He had to undergo a bilateral orchiectomy in the follow-up due to a suspicious finding in his testicles. For secondary hypogonadism, we started testosterone replacement therapy after discussion with the patient. Due to his prostate cancer, ongoing medical care and close follow-up including DRE (digital rectal examination), PSA, testosterone, liver function tests, blood counts, ultrasounds and CT scans are necessary. Testosterone replacement in patients after treatment for prostate cancer is a possible and safe treatment of secondary hypogonadism under certain conditions.

Úvod: Chirurgické řešení iatrogenní léze distálního ureteru pomocí reimplantace event. včetně Boariho plastiky lalokem z močového měchýře se prováděla otevřeně či nověji laparoskopicky. Roboticky asistovaná laparoskopie se zdá optimální volbou zjednodušující miniinvazivní provedení. Video prezentuje robotickou reimplantaci močovodu s Boariho lalokovou plastikou. Soubor: V období od 1/2019 do 6/2020 bylo na pracovišti hlavního autora provedeno deset těchto reimplantací. Všechny léze byly následkem iatrogenního poškození během operací v malé pánvi. Reimplantace byly provedeny vždy kompletně minimálně invazivně bez nutnosti konverze. Je užíván čtyřramenný systém daVinci Xi. Kazuistika: Sedmdesátiletá žena po iatrogenní lézi pravého ureteru způsobené během laparoskopické hysterektomie s adnexektomií. Pooperačně měla bolesti v pravém boku, sonograficky nalezeno městnání. Nález byl řešen punkční nefrostomií a o tři měsíce později provedena reimplantace. Trendelenburgova poloha 20°, čtyři robotické porty 8 mm a asistentský port 12 mm. Nástroje 30° kamera, Prograsp forceps, Maryland bipolar forceps, Monopolar curved scissors, Large suture‑Cut Needle Driver. Rozrušeny operační srůsty, vypreparován pravý ureter a přerušen nad lézí. Na naplněném močovém měchýři vytvořen vpravo lalok ze stěny močového měchýře. Bez antirefluxní techniky provedena anastomóza nespatulovaného ureteru s lalokem měchýře na zavedeném double loop ureterálním stentu jednotlivými polyglactinovými stehy 5–0. Uzávěr defektu měchýře pokračujícím samokotvícím stehem z poliglecapronu 25. Délka operace – anestezie 115 min, čas na konzoli 60 min. Dimitována devátý pooperační den. Močový katétr byl odstraněn za 14 dní, stent za čtyři týdny. Opakované kontroly pomocí ultrasonografie jsou i za tři měsíce od výkonu bez dilatace dutého systému pravé ledviny. Závěr: Robotická reimplantace močovodu ev. i s lalokem z močového měchýře je preferovanou metodikou pracoviště. Zcela nahradila otevřený výrazně invazivnější přístup. Laparoskopii považují autoři za technicky výrazně obtížnější a nemají s ní vlastní zkušenosti.
Introduction: Surgical treatment of an iatrogenic lesion of the distal ureter using reimplantation eventually including a Boari urinary bladder flap was performed openly or, more recently, laparoscopically. Robotically assisted laparoscopy seems to be the optimal choice for simplifying the mini-invasive design. This video presents a robotic reimplantation of the ureter with a Boari flap. File: In the period from 1/2019 to 6/2020, there were 10 of these surgeries (all iatrogenic lesions of ureter during pelvic surgery) at the department of the main author. All done without the need for conversion. The 4-arm daVinci Xi system is used. Case report: A 70 year old woman after iatrogenic injury of the right ureter during laparoscopic hysterectomy with adnexectomy. Postoperatively right side flank pain, and an ultrasound showed a right sided megaureter. The finding was resolved by puncture nephrostomy and reimplantation per‑ formed 3 months later. Trendelenburg position 20°, 4 robotic ports 8 mm and assistant port 12 mm. Tools 30° camera, Prograsp forceps, Maryland bipolar forceps, Monopolar curved scissors, Large suture-Cut Needle Driver. Surgical adhesions were freed up, right ureter dissected and cut above the lesion. On the filled bladder created a lobe from the right bladder on the right. Without antireflux technique, an anastomosis of an unspatulated ureter with a bladder lobe was performed on an inserted double loop ureteral stent with individual 5-0 polyglactin sutures. The defect of the urinary bladder was closed with running self-anchoring poliglecaprone 25 suture. Operation duration - general anaesthesia 115 min, console time 60 min. Discharged on the 9th postoperative day. Urinary catheter removed in 14 days, stent in 4 weeks. Regular ultrasonographic examinations are without dilatation of the upper urinary tract of the right kidney for 3 months. Conclusion: Robotic reimplantation of the ureter or even with a flap from the urinary bladder is the preferred method of our department. It completely replaced the open, significantly more invasive approach. Laparoscopy is considered by the authors to be technically significantly more difficult and they have no experience with it.

Úvod: Vezikovaginální píštěle (VVP) jsou jednou z obávaných gynekologických komplikací. Sama VVP neohrožuje pacientku na životě, ale vznikem močové inkontinence výrazně zhoršuje její kvalitu života. Rozsah a charakter úniku moči je závislý na velikosti VVP. Uzávěr VVP lze provádět jak vaginální cestou, tak abdominálně. V době otevřené chirurgie byl jako první krok užíván, pro svou mininvazivitu, vaginální přístup a k invazivnímu abdominálnímu přístupu cestou laparotomie bylo přistupováno až po selhání vaginálního přístupu. Zavedení laparoskopie snížilo invazivitu transabdominálního přístupu a pro větší efektivitu tohoto výkonu ve srovnání s vaginálním přístupem se laparoskopie stává metodou první volby na většině pracovišť. Následné zavedení roboticky asistované chirurgie pomáhá překonat některé překážky klasické laparoskopie, při ponechání všech výhod miniinvazivity (jedná se o vyšší stupeň laparoskopie). To je ještě více prohloubeno v případě užití jednoportové techniky pomocí Single site daVinci Xi technologie. Indikace a operační postup jednoportového roboticky asistovaného uzávěru vezikovaginální píštěle: Jednalo se o padesátiletou pacientku po laparoskopicky asistované vaginální hysterektomii s pravostrannou adnexektomií. Výkon jsme provedli v celkové anestezii v Trendelenburgově poloze 31°. Tuto polohu používáme standardně u tohoto typu operací. Obloučkovitou incizí v hraně pupku délky 25 mm a opatrnou preparací jednotlivých vrstev jsme otevřeli dutinu břišní (DB). Po vyšetření DB, které bylo bez patologického nálezu, jsme zavedli touto incizí složený pracovní disk "Single site“, který se po rozevření sám fixuje v ráně. Tímto diskem jsme v prefabrikovaných otvorech zavedli optický port pro 8 mm standardní robotickou 3D kameru s šikmou optikou a dva zahnuté pracovní porty 5 mm pro robotické flexibilní nástroje. Ještě je v disku jedno místo na asistentský port, který může být 10 mm či 8 mm, my využili 8 mm, a dokončili dokaci robota. Transvezikálně jsme ozřejmili lokalizaci píštěle a vztah k ústím ureterů. Poté jsme provedli resekci kanálu píštěle a odpreparování pochvy od stěny močového měchýře, který mobilizujeme, co nejdále od stěny pochvy s píštělí. Uzavření pochvy a močového měchýře jsme provedli vstřebatelnými V‑LOC 90R stehy a stehy StratafixTM na permanentním močovém katétru Ch 20 Foley, který jsme ponechali 21 dní na spád. Na našem pracovišti je odstranění permanentního močového katétru za tři týdny standardní dobou. Dle literatury (1, 2, 4, 5) možno odstranit katétr i dříve (7–10 dní). Břišní drén byl odstraněn druhý pooperační den. Celkový čas operace (skin to skin) byl 135 minut, z toho vlastní délka výkonu na robotické konzoli byla 65 minut. Výsledek: Pooperační průběh byl nekomplikovaný. Dvacátýprvní den od operace byl vytažen permanentní katétr a pacientka od té doby zcela bez urologických obtíží a jizva v pupku pevná a zhojena tak, že není na první pohled patrná. Diskuze: V případě, že je indikován abdominální přístup řešení VVF, doporučujeme zvážit užití robotického nebo laparoskopického přístupu, abychom minimalizovali morbiditu spojenou s provedením velké incize. První případ roboticky asistované korekce VVF popsali Melamud et al. v roce 2005 (1). Od této doby byla uveřejněna celá řada malých studií a kazuistik (2, 3, 4, 5, 6). Nemáme sice k dispozici přímá srovnání korekce VVF pomocí roboticky asistovaného vs. otevřeného vs. laparoskopického přístupu, benefit robota v podobě lepšího zvětšení, trojrozměrného zobrazení, lepší manipulace a kratší učební křivky oproti laparoskopii může být při operaci VVF neocenitelný (2, 4, 7, 8, 9). Korekci VVF pomocí laparoskopické operace jedním portem (LESS) poprvé popsali v roce 2011 Abdel‑Karim et al. Autoři užili zařízení Triport a další 5 mm port pro suturu (10). O využití této technologie při operaci VVF máme zatím k dispozici pouze minimum údajů (11). Závěr: Jednoportové roboticky asistované uzavření vezikovaginální píštěle může u vybrané skupiny pacientek nahradit více portový přístup laparoskopický či robotický při stejném kurativním výsledku s podstatně lepším kosmetickým efektem. A o srovnání s eventuálním laparotomickým přístupem ani nemluvě.
Introduction: Vesicovaginal fistulas (VVPs) are among much‑feared gynaecological complications. A VVP in itself is not life‑threatening for the patient, but the resulting urinary incontinence significantly impairs her quality of life. The extent and nature of urine leakage depend on the size of the VVP. A VVP closure can be performed via both the vaginal and abdominal routes. In the era of open surgery, the vaginal approach was used as the first step due to its minimal invasiveness, and an invasive abdominal approach with laparotomy was only adopted after a failure of the vaginal approach. The introduction of laparoscopy reduced the invasiveness of the transabdominal approach, and, due to a higher effectiveness of this procedure in comparison with the vaginal approach, laparoscopy has become the method of first choice in most centres. The subsequent introduction of robot‑assisted surgery has helped to overcome some of the obstacles of classic laparoscopy, while maintaining all the advantages of minimal invasiveness (it is a higher‑grade laparoscopy). This is even more enhanced when the single‑port technique of the da Vinci Xi Single‑Site technology is used. Indications for and the operating procedure of a single‑port robot‑assisted closure of a vesicovaginal fistula: We report a 50-year‑old patient who underwent laparoscopy‑assisted vaginal hysterectomy with right adnexectomy. The procedure was carried out under general anaesthesia in the Trendelenburg position of 31°. We use this position by default in this type of surgery. The abdominal cavity was opened with a 25-mm semi‑lunar incision at the umbilical margin and by carefully dissecting the individual layers. Once the abdominal cavity was inspected with no pathology found, the single‑site working disk was deployed into the abdomen through the incision in its folded configuration, and then unfolded and self‑secured in the wound. Using this disk, an optical port for a standard 8-mm robotic 3D camera with oblique optics and two curved 5-mm working ports for flexible robotic instruments were placed through prefabricated openings. There is one more site in the disk for an 8-mm or 10-mm assistant port; we used the 8-mm one, and completed robot docking. The fistula location and its relation to the ureteric orifices were identified transvesically. Next, the fistula canal was resected and the vaginal wall was dissected from the bladder which was mobilized as far as possible from the vaginal wall with the fistula. Closure of the vagina and bladder was performed with non‑absorbable V‑LOC 90R sutures and StratafixTM sutures on an indwelling Foley catheter Ch 20 that was kept in situ for 21 days. At our centre, removing an indwelling urinary catheter in 3 weeks’ time is a standard procedure. According to the literature (1, 2, 4, 5), a catheter can be removed earlier than that (in 7 to 10 days). The abdominal drain was removed on postoperative day 2. The total duration of surgery (skin to skin) was 135 minutes, with the robotic console time being 65 minutes. Result: The postoperative course was uneventful. On day 21 after surgery, the indwelling catheter was removed, and since then, the patient has been completely free from urological complaints, with the umbilical scar being firm and healed to such an extent that it is unnoticeable at first glance. Discussion: When the abdominal approach is indicated in the management of VVF, we recommend considering the use of robotic or laparoscopic approach in order to minimize the morbidity associated with major incisions. The first case of robot‑assisted repair of VVF was reported by Melamud et al in 2005 (1). Since then, a number of small studies and case reports have been published (2, 3, 4, 5, 6). Although no direct comparisons of VVF repair using robot‑assisted vs. open vs. laparoscopic approach are available, the benefits of robot‑assisted surgery in terms of increased magnification, three‑dimensional visualization, superior manipulation, and a shorter learning curve compared with laparoscopy can all be invaluable in the surgical management of VVF (2, 4, 7, 8, 9). The repair of a VVF using laparoendoscopic single‑site surgery (LESS) was first described by Abdel‑Karim et al. in 2011. The authors used the TriPort system and another 5-mm port for the suture (10). Only scarce data are available on the use of this technology in VVF surgery (11). Conclusion: In a selected group of patients, a single‑port robot‑assisted closure of a vesicovaginal fistula may replace multiple‑port laparoscopic or robotic approach with the same curative outcome and a significantly superior cosmetic effect, with a possible laparotomic approach bearing no comparison to the above.

Autoři v článku popisují vzácnou komplikaci zavedení ureterálního stentu do močovodu. Jedná se o kazuistiku pacientky, která byla přijata pro městnání v dutém systému obou ledvin při zevním útlaku močovodů tumorózní masou v retroperitoneu. Dle histologie se potvrdil B-lymfom retroperitonea, který byl příčinou potíží pacientky. Při zavedení ureterálního stentu do močovodu došlo k jeho perforaci a konec splintu byl zaveden do dolní duté žíly. Pacientka byla asymptomatická, tato komplikace byla objevena až 12. den na kontrolním CT. Samotná extrakce stentu proběhla bez komplikací a bez krvácení. Nicméně pacientka následně zemřela nejspíše na sepsi.
The authors describe a rare complication of ureteral stenting is the case study of a patient admitted to the hospital for congestion in the outlet system of both kidneys due to external ureteral compression by tumorous mass in the retroperitoneum. Histology confirmed the B-lymphoma of the retroperitoneum as a cause of the patient‘s problems. The ureteral stent was perforated in the course of inserting the stent into the ureter and the end of the splint was introduced into the inferior vena cava. The patient was asymptomatic, and this complication was detected as late as on day 12 on the follow-up CT scan. Stent extraction was without complications and without bleeding.
