OBJECTIVES: Dental caries is a widespread multifactorial disease, caused by the demineralization of hard dental tissues. Susceptibility to dental caries is partially genetically conditioned; this study was aimed at finding an association of selected single nucleotide polymorphisms (SNPs) in genes encoding proteins involved in amelogenesis with this disease in children. MATERIALS AND METHODS: In this case-control study, 15 SNPs in ALOX15, AMBN, AMELX, KLK4, TFIP11, and TUFT1 genes were analyzed in 150 children with primary dentition and 611 children with permanent teeth with/without dental caries from the European Longitudinal Study of Pregnancy and Childhood (ELSPAC) cohort. RESULTS: Dental caries in primary dentition was associated with SNPs in AMELX (rs17878486) and KLK4 (rs198968, rs2242670), and dental caries in permanent dentition with SNPs in AMELX (rs17878486) and KLK4 (rs2235091, rs2242670, rs2978642), (p ≤ 0.05). No significant differences between cases and controls were observed in the allele or genotype frequencies of any of the selected SNPs in ALOX15, AMBN, TFIP11, and TUFT1 genes (p > 0.05). Some KLK4 haplotypes were associated with dental caries in permanent dentition (p ≤ 0.05). CONCLUSIONS: Based on this study, we found that although the SNPs in AMELX and KLK4 are localized in intronic regions and their functional significance has not yet been determined, they are associated with susceptibility to dental caries in children. CLINICAL RELEVANCE: AMELX and KLK4 variants could be considered in the risk assessment of dental caries, especially in permanent dentition, in the European Caucasian population.

BACKGROUND: Recurrent aphthous stomatitis is one of the most prevalent oral mucosal immunological diseases. A recent case-control study in the Egyptian population suggested that single nucleotide polymorphism Gly54Asp (rs1800450) of the mannose-binding lectin 2 gene might affect the mannose-binding lectin serum level and recurrent aphthous stomatitis development. The aim of this study was to determine the distribution of six functional mannose-binding lectin 2 gene polymorphisms and analyse their role in recurrent aphthous stomatitis susceptibility in the Czech population. METHODS: The study included 227 subjects; 137 healthy people and 90 patients with recurrent aphthous stomatitis. Six mannose-binding lectin 2 gene polymorphisms (rs11003125, rs7096206, rs7095891, rs5030737, rs1800450, rs1800451) were analysed by the SNaPshot assay method, mannose-binding lectin serum levels were determined by enzyme-linked immunosorbent assay (ELISA) method in a subgroup of subjects (N = 87). RESULTS: No significant differences in mean of mannose-binding lectin serum levels between healthy controls and patients with recurrent aphthous stomatitis were observed (383 ng/ml ± 249 standard deviation (SD) vs. 316 ng/ml ± 177 SD in remission phase vs. 343 ng/ml ± 254 SD in active phase; p > 0.05), also the allele and genotype frequencies of the studied mannose-binding lectin 2 polymorphisms did not differ significantly between the two groups (p > 0.05, odds ratio (OR): 0.75-1.23). Moreover, the distribution of mannose-binding lectin 2 haplotypes and haplogenotypes was similar in the healthy subjects and patients with recurrent aphthous stomatitis (p > 0.05, OR: 0.75-1.23). CONCLUSIONS: This study did not confirm the previously reported association of the mannose-binding lectin 2 Gly54Asp gene variant and low mannose-binding lectin serum level as the risk factors for susceptibility to recurrent aphthous stomatitis. In addition, no significant relationships between mannose-binding lectin 2 functional haplotypes or haplogenotypes and recurrent aphthous stomatitis were observed.

BACKGROUND: Acromegaly is a disorder associated with hypersecretion of growth hormone, most usually caused by a pituitary adenoma. Dysmotility of the gastrointestinal tract has been reported in acromegalic patients. Achalasia is a disorder characterized by aperistalsis of the oesophagus with incomplete lower oesophageal sphincter relaxation and whose aetiology remains unknown. Mutations in some genes have previously been associated with the development of acromegaly or achalasia. The study aims were to analyse mutations in selected genes in a woman having both of these diseases, to identify their aetiological factors, and to suggest explanations for the co-incidence of acromegaly and achalasia. METHODS AND RESULTS: A female patient with acromegaly, achalasia, and a multinodular thyroid gland with hyperplastic colloid nodules underwent successful treatment of achalasia via laparoscopic Heller myotomy, a thyroidectomy was performed, and the pituitary macroadenoma was surgically excised via transnasal endoscopic extirpation. Germline DNA from the leukocytes was analysed by sequencing methods for a panel of genes. No pathogenic mutation in AAAS, AIP, MEN1, CDKN1B, PRKAR1A, SDHB, GPR101, and GNAS genes was found in germline DNA. The somatic mutation c.601C>T/p.R201C in the GNAS gene was identified in DNA extracted from a tissue sample of the pituitary macroadenoma. CONCLUSIONS: We here describe the first case report to our knowledge of a patient with both acromegaly and achalasia. Association of acromegaly and soft muscle tissue hypertrophy may contribute to achalasia's development. If one of these diagnoses is determined, the other also should be considered along with increased risk of oesophageal and colorectal malignancy.

The host immune system responds to pathogens through pattern recognition receptors (PRRs), such as Toll-like receptors (TLRs) and their co-receptor cluster of differentiation 14 (CD14). AIM: This study investigated the association between the functional polymorphism in the CD14 gene and the dental plaque microbiota in children with gingivitis. DESIGN: A total of 590 unrelated children (307 with plaque-induced gingivitis and 283 controls, aged 13-15 years) were enrolled in this case-control study. Dental plaque was processed using a ParoCheck® 20 detection kit. The CD14 -260C/T (rs2569190) polymorphism was determined with the PCR-RFLP method. RESULTS: Gingivitis was detected in 64.2% of boys and 35.8% of girls (P < .001). Children with gingivitis had a significantly higher occurrence of dental caries (P < .001). No significant differences in the CD14 -260C/T allele and genotype distribution among individuals with or without gingivitis in the whole cohort were found. Children with gingivitis and P gingivalis, however, were significantly more frequent carriers of the CT and TT genotypes than children with gingivitis without P gingivalis or healthy controls (P < .05). CONCLUSIONS: The CD14 -260C/T polymorphism acts in cooperation with P gingivalis to trigger plaque-induced gingivitis in Czech children.

Dental caries, a chronic multifactorial disease, is a major oral health problem worldwide. Its etiology involves a complex interplay of microbial, genetic, biochemical, social, physical environmental, and health-influencing behavioral factors. The project is designed as a combination of prospective monitoring of selected cohort and a cross-sectional studies [children with caries vs. healthy controls or children with early childhood caries (ECC)], and in vitro studies addressing (i) in vitro cultivation of cariogenic bacterial strains in the presence of substrates with various cariogenic potential, (ii) their transriptome/metabolome analysis, (iii) multivariate analyses of clinical, biochemical, microbial, genetic and environmental factors, and (iv) education of mothers and children–behavioral intervention. Implementation of obtained knowledge might reduce costs on health care in connection with caries. The assessment of the individual risk could rationalize health care in terms of "personalized medicine" (particularly the disease diagnosis and prevention).
Zubní kaz je chronické multifaktoriální onemocnění, které postihuje značnou část světové populace. Jeho etiologie je komplexní a zahrnuje interakci mikrobiálních, genetických, biochemických, sociálních a behaviorálních faktorů. Projekt je koncipován jako kombinace prospektivního sledování vybrané kohorty a průřezových studií [dětí s kazem vs. zdravých kontrol nebo dětí s kazem raného dětství (ECC)] a in vitro studií, které se budou zabývat (i) in vitro kultivací kariogenních bakteriálních kmenů za přítomnosti substrátů s různým kariogenním potenciálem, (ii) analýzou jejich transkriptomu/metabolomu, (iii) multivariační analýzou klinických, biochemických, mikrobiálních, genetických faktorů a vlivů zevního prostředí a (iv) edukací matek dětí – behaviorální intervencí. Začlenění získaných poznatků do praxe by mohlo přispět k redukci nákladů na zdravotní péči související s léčbou kazu. Zhodnocení individuálního rizika rozvoje kazu by umožnilo přechod k “personalizované medicíně“ (zejména v rámci diagnostiky a prevence onemocnění).

Chronic periodontitis (CP), an infectious disease of the supporting structures of teeth which may lead to their elimination, effects about half of the adult population. Connection to systemic diseases such as cardiovascular diseases, diabetes mellitus, low birth weight and others has been shown in several studies. Resolvins and lipoxins are low-weight lipid mediators which terminate the inflammatory response. During healing of periodontal defects granulation tissue (GT) was traditionally removed during surgical procedure. If exposed to optimal environment GT could change to a functional tissue. By using biotechnology granulation tissue modified by resolvins and lipoxins represents an innovative approach to the treatment of CP. In the experimental part, effectiveness of the termination of periodontal inflammation on the animal model will be accessed. If positive results would be achieved, clinical part of the project will follow with the aim to analyze the techniques and procedures in correlation to standard treatment of CP.
Chronická parodontitida (CP), zánětlivé onemocnění závěsného aparátu zubů, které může vést k jejich ztrátě, postihuje přibližně polovinu populace. Řada studií prokázala souvislost CP s některými systémovými chorobami, např. kardiovaskulárními onemocněními, diabetem mellitem, nízkou porodní váhou novorozenců a dalšími. Na ukončení zánětu se podílí řada mediátorů, mezi jinými také lipidové mediátory resolviny a lipoxiny. Během hojení tkání parodontu dochází k tvorbě granulační tkáně (GT), která je při chirurgickém zákroku běžně odstraněna. Pokud by se však GT nacházela ve vhodných podmínkách, je schopna se přeměnit na funkční tkáň. Pomocí biotechnologií připravení a využití resolvinů a lipoxinů při modifikaci granulační tkáně a následné chirurgické léčbě by mohlo představovat inovativní přístup k terapii CP. V experimentální části bude na animálním modelu ověřena efektivnost použitého postupu při hojení parodontálního zánětu. V navazující klinické části budou při pozitivních výsledcích předchozích experimentů použité techniky a postupy analyzovány ve vztahu ke standardní léčbě CP.

Úvod a cíl práce: Zubní kaz v časném dětství (ECC) je definován jako přítomnost jedné nebo více kariézních lézí (nekavitovaných či kavitovaných) nebo výplně na kterémkoliv dočasném zubu nebo nepřítomnost zubu chybějícího pro kaz a jeho komplikace u dětí mladších než šest let. Na rozvoji ECC se podílejí kariogenní bakterie, mezi které řadíme Streptococcus mutans, Lactobacillus spp. a také druhy Actinomyces. Počáteční fáze vzniku zubního kazu je spojena s přítomností viditelného zubního plaku na povrchu zubu a S. mutans v dutině ústní, přičemž pro adhezi bakterie S. mutans je nejrizikovější období mezi 19. a 31. měsícem – první infekční okno. Cílem naší práce bylo identifikovat kariogenní bakterie Streptococcus mutans, Lactobacillus spp. a Actinomyces spp. u dětí postižených ECC a u jejich matek. Metodika: Do studie bylo zahrnuto 80 párů matka/dítě (průměrný věk dítěte 3,8 let ± 1,14 SD). Děti byly vyšetřeny běžným způsobem na stomatologickém křesle za pomoci sondy, zrcátka a dobrého osvětlení. Stanovili jsme u nich kpe index zubů. U matek jsme zjišťovali věk, přítomnost povlaku, počet zubních kazů, výplní a extrahovaných zubů (má/nemá), preventivní prohlídky (chodí/nechodí) a to, zda u krmení olizuje dítěti lžičku nebo dudlík, který spadl na zem. Materiál pro vyšetření bakterií S. mutans, Lactobacillus spp. a Actinomyces spp. jsme získávali u dětí stěrem z povrchu kariézních zubů, u matek byl povlak stírán z povrchu všech zubů, vždy pomocí vatové sterilní štětičky. Odebrané vzorky byly vyhodnoceny pomocí testu CarioGene® (Protean s.r.o., Dobrá voda u Českých Budějovic, Česká republika) pro DNA diagnostiku kariogenních bakterií a byly statisticky zpracovány. Výsledky: Přítomnost bakterie S. mutans byla zjištěna u 27 párů matka/dítě, negativní nález u obou byl prokázán ve 32 případech. Nepřítomnost Lactobacillus spp. u dítěte i u jeho matky byla zjištěna u 13 dvojic, pozitivní nález u obou byl ve 46 případech. Bakterie Actinomyces spp. byla detekována u matky i u jejího dítěte v 67 případech. Statisticky byla prokázána významná souvislost mezi přítomností bakterií S. mutans a Lactobacillus spp. u matky a jejího dítěte (p < 0,0001 a p < 0,00001). Po rozdělení na skupinu matek olizujících/ neolizujících lžičku při krmení byly výsledky signifikantní pouze u matek z první skupiny (p < 0,001). V případě bakterie Actinomyces spp. nebyly výsledky statisticky významné (p > 0,05). Závěr: Výsledky práce nasvědčují tomu, že zdrojem kariogenní infekce u dítěte je jeho matka. Za potenciální cesty přenosu infekce je pokládáno hlavně olizování lžičky při krmení dítěte.
Aim: Early childhood caries (ECC) is defined as the presence of one or more carious lesions (noncavitated or cavitated) or filled tooth surfaces in any primary tooth or missing teeth (due to caries) in children below six years of age. Cariogenic oral bacteria Streptococcus mutans, Lactobacillus spp. and Actinomyces spp. were found to be involved in the caries development. The early phase of the caries development is associated with the presence of visible plaque on the teeth surface and S. mutans occurrence in the oral cavity, with the most critical period for S. mutans colonization from 19–31 months of age. The aim of this study was to identify the cariogenic bacteria Streptococcus mutans, Lactobacillus spp., and Actinomyces spp. in children with early childhood caries and their mothers. Methods: A total of 80 mother/child pairs (child mean age 3.8 years ± 1.14 SD) were included in the study. Children were orally examined in a dental chair using a probe, mirror and a good light source. In children, the dmft index was identified, in mothers, the age, the presence of plaque, the number of caries, fillings, extracted teeth, whether they visit their dentists regularly and whether they lick their children´s spoon or pacifier were determined. The dental plaque samples for the assessment of the presence of S. mutans, Lactobacillus spp. and Actinomyces spp. were taken from the surface of carious teeth in children and surface of all teeth in mothers, always using sterile cotton swabs. The presence of cariogenic bacteria was identified by the CarioGene® test (Protean s.r.o., Dobrá voda, Czech Republic) and the results were statistically evaluated. Results: The presence of S. mutans was identified in 27 mother/child pairs, negative finding in both mother and child was found in 32 cases. The absence of Lactobacillus spp. in the children and their mothers was detected in 13 pairs, the positive finding in both in 46 cases. Actinomyces spp. was detected in both the child and mother in 67 cases. We demonstrated a significant relationship between the presence of S. mutans and Lactobacillus spp. in mothers and their children (p < 0.0001 and p < 0.00001). When mothers were split into the group who licked the spoon at feeding their and the group who did not, the results were significant only in mothers from the first group (p < 0.001). However, the results of Actinomyces spp. were not statistically significant (p > 0.05). Conclusion: The results of the study demonstrated that the source of acquiring cariogenic bacteria in children suffering from ECC might be through mothers. The potential way of transmission may be licking the infant´s spoon.
