Rhaponticum carthamoides, maral root, is being used, namely in Orient, for centuries. The focus of interest of medicinal chemists has recently been pointed to its chemical components, namely 20-hydroxyecdysone, a herbal drug with very low toxicity. It is registered in the EU to cure, e.g., Duchenne muscular dystrophy. Moreover, being recognised as an adaptogen, it has been in use to cure prediabetes and can be classified as a natural substance, for which intensive research into cancer and leukaemia treatment is running, in some cases together with bufadienolides. Ecdysteroids also have the ability to affect drug resistance in the treatment of tumours. The paper brings the digest of the most interesting information, mainly from the human health research.

The male accessory glands (AG) and gonoducts of moths develop during metamorphosis and are essential for successful fertilization of females. We found that these reproductive organs are innervated by a sex-specific cluster of peptidergic neurons in the posterior 9th neuromere of the terminal abdominal ganglion (TAG). This cluster of ~20 neurons differentiate during metamorphosis to innervate the accessory glands and sperm ducts. Using immunohistochemistry and in situ hybridization (ISH) we showed that these neurons express four neuropeptide precursors encoding calcitonin-like diuretic hormone (CT-DH), allatotropin (AT) and AT-like peptides (ATLI-III), allatostatin C (AST-C), and myoinhibitory peptides (MIPs). We used contraction bioassay in vitro to determine roles of these neuropeptides in the gonoduct and accessory gland activity. Spontaneous contractions of the seminal vesicle and AG were stimulated in a dose depended manner by CT-DH and AT, whereas AST-C and MIP elicited dose dependent inhibition. Using quantitative RT-PCR we confirmed expression of receptors for these neuropeptides in organs innervated by the male specific cluster of neurons. Our results suggest a role of these neuropeptides in regulation of seminal fluid movements during copulation.

BACKGROUND AND AIMS: Obstructive sleep apnoea is a potentially serious sleep disorder associated with the risk of cardiovascular disease. It is treated with continuous airway pressure (CPAP) but this is not always successful. Unsuccessful cases should be treated by bilevel positive airway pressure (BiPAP). The aim of this study was to determine whether common respiratory parameters and/or body mass index (BMI) can be used to predict the probability CPAP failure and hence start such patients on BiPAP from the outset. METHODS: A sample of patients treated by CPAP for OSAS was evaluated a retrospective cohort study. The data measured in sleep monitoring of the successfully treated group and of the group where CPAP had failed were compared. Subsequently, the predictive abilities of BMI, Apnoea Index (AI), Apnoea-Hypopnea Index (AHI), percentage of sleep time in less than 90% oxygen saturation (T90), average oxygen saturation over the duration of sleep (SaO2) and average desaturation per hour of sleep (ODI) were assessed with respect to CPAP failure, both individually and in combination. RESULTS: A sample of 479 patients was included in the study. All of the recorded variables except AI were significantly associated with failure of CPAP and their ability to predict the failure ranged from poor to moderate. Since there was significant correlation among all the variables measured a two-variable prediction model combining T90 and BMI produced no significant improvement in the quality of CPAP failure prediction. CONCLUSIONS: BMI was a significant predictor of CPAP failure although it was slightly less predictive than T90. The set of monitored variables included in our study does not allow for CPAP failure to be predicted with clinically relevant reliability.

Cíl: Cílem výzkumné studie bylo zjistit, zda lze využít robotickou rehabilitaci chůze k pozitivnímu ovlivnění psychosomatických a sociálních potíží dětí s poruchou na úrovni lehké mentální retardace. V případě mentální retardace a léčby souvisejících kauzálních neurologických poruch a onemocnění patří k aktuálním trendům stimulace a případný rozvoj nepostižených nebo zachovaných schopností jedince a zaměření rehabilitace na integraci jedince do společnosti. K výzkumné studii nás inspirovaly výsledky studií, které se týkají účinnosti robotické rehabilitace chůze v případě dětské mozkové obrny, která je často s mentální retardací spojena. Soubor a metodika: Šetřený soubor o celkovém počtu 65 pacientů byl rozdělen do tří skupin. Vyšetřeno bylo 24 dětí s dětskou mozkovou obrnou, 21 s teratogenní etiologií a 20 s postnatální etiologií u lehké mentální retardace. Sledováno bylo 42 chlapců a 23 dívek ve věku 3–6 let na začátku léčby a po 6 týdnech. Sledovány byly psychosomatické indikátory. Jako metodologická východiska byly použity materiály Mezinárodní klasifikace funkčních schopností (MKF) a Hodnocení funkční nezávislosti (FIM). Výsledky: Statisticky významný pozitivní rozdíl byl zaznamenán u somatických indikátorů v případě dětské mozkové obrny (p < 0,05). V případě teratogenní etiologie bylo zaznamenáno, ač statisticky nevýznamné, ale také zlepšení u somatických ukazatelů. O pozitivním posunu u všech sledovaných skupin můžeme hovořit v případě sociální adaptability. V případě dětí s dětskou mozkovou obrnu bylo zjištění opět statisticky významné (p < 0,05). Závěr: Výsledky šetření ukazují, že robotická terapie chůzí může být vhodnou rehabilitační i podpůrnou metodou také u mentálně retardovaných osob. Při jejím využití můžeme doporučit zohlednění původní etiologii poruchy. Jednoznačně lze doporučit preferenci této metody u mentálně retardovaných osob s dětskou mozkovou obrnou po hypoxii.
Aim: The aim of study was to find out the effect of robotic gait rehabilitation on psychosomatic indicators at people with different etiology of mental retardation. A stimulation and eventual development of unaffected or preserved abilities of individuals belong among current trends in the case of mental retardation and the treatment to related causal neurological disorders and diseases. The rehabilitation focused on environmental factors with aim to integrate individual into society is another current trend. For a decision to realize this research study we have been inspired by results of numerous studies, reported, for example, about the results of influence of robotic rehabilitation on psychosomatically health in the case of cerebral palsy. Material and methods: A total of 65 children in age from 3 to 6 years (42 boys, 23 girls) were observed in the beginning of rehabilitation and then after 6 weeks. Investigated patients were selected into three groups. 21 patients were with cerebral palsy, 21 with teratogenic and 20 with postnatal etiology. Psychosomatic indicators were monitored. The International Classification of Functioning, Disability and Health (ICF), and Functional Independence Measure were used as a methodological background. Results: We observed statistically significant positive differences in somatic indicators in the cause of cerebral palsy. In the case of teratogenic etiology was observed, although not statistically significant, but also an evident improvement of somatic indicators. In the case of all observed groups of patients we can talk about positive improvement in social adaptability. Statistically significant (p < 0.05) were findings again in case of cerebral palsy. Conclusions: Results of the research indicate that the robotic gait rehabilitation may be usable as a rehabilitative and supportive method also in the treatment to mentally retarded persons. In the case of its use it is appropriate to take into account of the origin etiology of the disorder. Definitely we can recommend a preference for this method for mentally retarded persons with cerebral palsy after hypoxia.

V posledních 10 letech se objevila možnost využít k odstranění vhodně lokalizovaných T1 a T2 spinocelulárních karcinomů orofaryngu, zejména kořene jazyka, tonzily a supraglotis, miniinvazivní robotický přístup skrze ústní dutinu - TORS (TransOralRoboticSurgery). Lokalizací a tvarem jsou pro TORS nejlépe vhodné povrchově uložené a ohraničené spinocelulární karcinomy, dostupné pro robotické nástroje. Rozhodující úlohu při indikaci k TORS hraje zobrazení karcinomů hlavy a krku pomocí magnetické rezonance.
In the last 10 years, it became possible to be used by appropriately localized T1 and T2 squamous cell carcinomas of oropharynx and supraglottis robotic minimally invasive approach trough oral cavity – TORS (TransOralRoboticSurgery). Localization and shape best suited tumors for TORS are surface stored and bounded squamous cell carcinomas available for robotic tools. Decisive role in indication for removal of cancers by the TORS plays magnetic resonance imaging and computer tomography of the head and neck.
