BACKGROUND: During electrophysiological mapping of tachycardias, putative target sites are often only truly confirmed to be vital after observing the effect of ablation. This lack of mapping specificity potentiates inadvertent ablation of innocent cardiac tissue not relevant to the arrhythmia. But if myocardial excitability could be transiently suppressed at critical regions, their suitability as targets could be conclusively determined before delivering tissue-destructive ablation lesions. We studied whether reversible pulsed electric fields (PFREV) could transiently suppress electrical conduction, thereby providing a means to dissect tachycardia circuits in vivo. METHODS: PFREV energy was delivered from a 9-mm lattice-tip catheter to the atria of 12 swine and 9 patients, followed by serial electrogram assessments. The effects on electrical conduction were explored in 5 additional animals by applying PFREV to the atrioventricular node: 17 low-dose (PFREV-LOW) and 10 high-dose (PFREV-HIGH) applications. Finally, in 3 patients manifesting spontaneous tachycardias, PFREV was applied at putative critical sites. RESULTS: In animals, the immediate post-PFREV electrogram amplitudes diminished by 74%, followed by 78% recovery by 5 minutes. Similarly, in patients, a 69.9% amplitude reduction was followed by 84% recovery by 3 minutes. Histology revealed only minimal to no focal, superficial fibrosis. PFREV-LOW at the atrioventricular node resulted in transient PR prolongation and transient AV block in 59% and 6%, while PFREV-HIGH caused transient PR prolongation and transient AV block in 30% and 50%, respectively. The 3 tachycardia patients had atypical atrial flutters (n=2) and atrioventricular nodal reentrant tachycardia. PFREV at putative critical sites reproducibly terminated the tachycardias; ablation rendered the tachycardias noninducible and without recurrence during 1-year follow-up. CONCLUSIONS: Reversible electroporation pulses can be applied to myocardial tissue to transiently block electrical conduction. This technique of pulsed field mapping may represent a novel electrophysiological tool to help identify the critical isthmus of tachycardia circuits.

AIMS: Interventional cardiology procedures may expose patients and staff to considerable radiation doses. We aimed to assess whether exposure to ionizing radiation during catheter ablation of supraventricular tachycardia (SVT) can be completely avoided. METHODS AND RESULTS: In this prospective randomized study, patients with SVT (atrioventricular re-entrant tachycardia n = 94, typical atrial flutter n = 29) were randomly assigned in a 1:1 ratio to catheter ablation with conventional fluoroscopic guidance (CF group) or with the EnSite Precision mapping system [zerofluoro (ZF) group]. Acute procedural parameters, increased stochastic risk of cancer incidence and 6-month follow-up data were assessed. Between May 2019 and August 2020, 123 patients were enrolled. Clinical parameters were comparable. Median procedural time was 60.0 and 58.0 min, median fluoroscopy time and estimated median effective dose were 240 s vs. 0 and 0.38 mSv vs. 0 and arrhythmia recurrence was 5% and 7.9% in the CF and ZF groups, respectively. The acute success rate was 98.4% in both groups. No procedure-related complications were reported. At an average age of 55.5 years and median radiation exposure of 0.38 mSv, the estimate of increased incidence was approximately 1 in 14 084. The estimated mortality rate was 1 per 17 857 exposed persons. CONCLUSIONS: The procedural safety and efficacy of the zero-fluoroscopic approach are similar to those of conventional fluoroscopy-based ablation for atrioventricular nodal re-entrant tachycardia and atrial flutter. Under the assumption of low radiation dose, the excessive lifetime risk of malignancy in the CF group due to electrophysiology procedure is reasonably small, whilst totally reduced in zero fluoroscopy procedures.

AIMS: Contemporary data from prospective multicentre registries on catheter ablation in pediatric patients are sparse. Aim of the European Pediatric Catheter Ablation Registry EUROPA was to contribute data to fill this gap of knowledge. METHODS AND RESULTS: From July 2012 to June 2017, data on catheter ablation in pediatric patients (≤18 years of age) including a 1-year follow-up from five European pediatric EP centres were collected prospectively. A total of 683 patients (mean age 12.4 ± 3.9 years, mean body weight 50.2 ± 19 kg) were enrolled. Target tachycardia was WPW/atrioventricular-nodal re-entrant tachycardia (AVRT) in 380 (55.7%) patients, AVNRT in 230 (33.8%) patients, ventricular tachycardia (VT) in 24 (3.5) patients, focal atrial tachycardia (FAT) in 20 (2.9%) patients, IART in 14 (2%) patients, and junctional ectopic tachycardia in 3 (0.45) patients. Overall procedural success was 95.6%. Compared with all other substrates, success was significantly lower in FAT patients (80%, n = 16, P = 0.001). Mean procedure duration was 136 ± 67 min and mean fluoroscopy time was 4.9 ± 6.8 min. Major complications occurred in 0.7% of the patients. No persisting AV block requiring permanent pacing was reported. At 1-year follow-up (605/683 patients, 95%), tachycardia recurrence was reported in 7.8% of patients. Recurrence after VT ablation (33%) was significantly higher (P = 0.001) than after ablation of all other substrates. CONCLUSION: The present study proves overall high efficacy and safety of catheter ablation of various tachycardia substrates in pediatric patients. Of note, complication rate was exceptionally low. Long-term success was high except for patients after VT ablation.

Poruchy srdečního rytmu jsou heterogenní skupinou onemocnění vyskytující se v každém věku. Klinická závažnost je dána konkrétním typem arytmie - od jednotek spíše benigních (velká část supraventrikulárních tachykardií) po arytmie komorové s různě vysokým rizikem náhlé srdeční smrti. Lepší než dlouhodobá antiarytmická profylaxe je moderní a často i kauzální terapií elektrofyziologické vyšetření srdce s možností endo- a/nebo epikardiální katetrové ablace. U indikovaných pacientů přistupujeme k implantaci kardiostimulátoru či kardioverter-defibrilátoru z primární nebo sekundární prevence.
Cardiac arrhythmias are a heterogeneous group of disorders which are common in every age group. The clinical importance depends on the specific type of arrhythmia - from benign forms (a majority of supraventricular tachycardias) to ventricular arrhythmias with a risk of a sudden cardiac death. Rather than a long-term antiarrhythmic prophylaxis, a cardiac electrophysiological examination with endo and/or epicardial catheter ablation is a modern, and in many cases a causal therapy for heart rhythm disorders. Implantation of a cardiac pacemaker or cardioverter-defibrillator is indicated in some selected groups of patients in primary or secondary prevention.

Wolffův-Parkinsonův-Whiteův (WPW) syndrom je definován přítomností akcesorní dráhy a s ní souvisejících symptomů. Typickým EKG obrazem preexcitace je krátký interval PQ, přítomnost vlny delta a rozšíření komplexu QRS na povrchovém EKG. Podstatou syndromu preexcitace je existence akcesorní dráhy, která umožňuje převod vlny depolarizace ze síní na komory mimo atrioventrikulární (AV) uzel a predisponuje ke vzniku arytmií a náhlé smrti. Nejčastěji se vyskytující arytmií u pacientů s WPW syndromem je atrioventrikulární reentry tachykardie. Není přítomna u všech pacientů s preexcitací [1-4]. Až u jedné třetiny pacientů s atrioventrikulární reentry tachykardií (AVRT) se vyskytuje fibrilace síní, která může být díky rychlému převodu spojkou převedena na svalovinu komor a vést k rozvoji život ohrožující fibrilace komor. Nejúčinnější léčbou WPW syndromu je radiofrekvenční ablace akcesorní dráhy [2,5-8]. Článek popisuje kasuistiku 40leté ženy po kardiopulmonální resuscitaci pro fibrilaci komor, která byla primomanifestací WPW syndromu u této pacientky. Zároveň uvádí patofyziologii, klinický obraz a možnosti léčby pacientů s WPW syndromem.
Wolff-Parkinson-White syndrome (WPW) is defined as a condition involving an accessory pathway associated with symptoms. A typical ECG pattern of a pre-excitation shows a short PQ interval, presence of delta wave and a broad QRS complex on surface ECG. The underlying mechanism involves an accessory pathway, which enables conduction of a depolarization wave from atria to ventricles bypassing the AV node and predisposes to arrhythmias and sudden cardiac death. The most common arrhythmia in patients with WPW syndrome is atrioventricular reentrant tachycardia. However, it is not present in all patients with pre-excitation [1-4]. Up to 1/3 of patients with AVRT experience atrial fibrillation, which may be conducted to ventricular myocardium via the accessory pathway and lead to a life-threatening ventricular fibrillation. The most effective treatment of the WPW syndrome is a radiofrequency catheter ablation [2,5-8]. This paper describes a case of a 40-year-old woman after a cardiopulmonary resuscitation for ventricular fibrillation, which was a primary manifestation of the WPW syndrome. It focuses on pathophysiology, clinical pattern and treatment possibilities of patients with WPW syndrome.

Hodnocení dvanáctisvodového elektrokardiografického záznamu je rutinní součástí základního vyšetření u pacientů s přítom- ností klinických příznaků svědčících pro kardiovaskulární onemocnění v rámci prvního kontaktu. Interpretace nálezu ve svodu aVR bývá často opomíjeno. V článku autoři poskytují přehled situací, kdy je hodnocení elektrokardiografických změn ve svodu aVR velmi přínosné a může zásadně ovlivnit plán dalšího diagnosticko-léčebného postupu. Jedná se především o akutní koro- nární syndrom, akutní perikarditidu, pravidelnou tachykardii s širokými nebo štíhlými QRS komplexy plicní embolii a intoxikaci tricyklickými antidepresivy.
Evaluation of twelve-lead electrocardiogram is a routine part of initial emergency assessment of patients presenting with severe cardiovascular symptoms. However, aVR lead is traditionally neglected in this process. The authors provide an overview of clinical situations where aVR lead assessment is beneficial and can have a significant impact on further management. These include clinical setting of acute coronary syndrome, acute pericarditis, regular wide complex tachycardia and narrow complex tachycardia, pulmonary embolism and tricyclic antidepressant overdose.

BACKGROUND: Activation of the immune system plays a pathogenic role in the process of myocardial remodeling in patients with supraventricular arrhythmias. The intensity of this process is associated with the effectiveness of electrical cardioversion and radiofrequency catheter ablation (RFA). The aim of this study was to test the ability of the biochip microarray to detect immune parameters in patients with supraventricular arrhythmias undergoing RFA treatment. METHODS: We used a biochip-based microarray system to determine multiple immune parameters in a group of 35 patients who had undergone RFA for atrioventricular nodal reentry tachycardia (AVNRT), atrial flutter (AFL) and atrial fibrillation (AF). RESULTS: Before the procedure, serum IL-6 and VEGF levels were significantly increased in patients with atrial fibrillation compared to patients with AVNRT (IL-6: 6.4±6.3 ng/L vs. 1.5±0.7 ng/L, P < 0.01; VEGF: 132.4±74 ng/L vs. 88.5±56.4 ng/L, P < 0.01). After the procedure, serum IL-6, VEGF, IFN-γ and MCP-1 levels significantly increased compared to baseline (IL-6: 5.2±4.8 ng/L vs. 2.9±2.1 ng/L, P < 0.01; VEGF: 195.8±160 ng/L vs. 119.8± 110 ng/L, P < 0.05; IFN-γ: 3.1±1.2 ng/L vs. 2.3±0.6 ng/L, P < 0.05; MCP-1: 104.1±84.5 ng/L vs. 54.5±50 ng/L, P < 0.05). Serum IL-6 and IFN-γ were associated with the number of RFA applications (IL-6: r = 0.56, n 33; IFN-γ: r = 0.47, n 33). CONCLUSIONS: This study showed that biochip-based microarray can be useful in the detection of immune activation in patients with arrhythmias and can detect myocardial injury after RF procedures.

Při preparticipačním skríningu sportovců stoupá záchyt asymptomatických dětí a mladých dospělých s obrazem preexcitace charakteru Wolff-Parkinson-White (WPW) na klidovém EKG. Tito jedinci jsou, kromě možného vzniku epizod atrioventrikulární reentry tachykardie, potenciálně ohroženi náhlou srdeční smrtí v důsledku rychlého převodu náhle vzniklé fibrilace síní na komory přes přídatnou síňokomorovou spojku. Komplexní zhodnocení nálezu a stanovení terapeutického postupu je v rukách kardiologa, v managementu onemocnění se ale zásadním způsobem uplatňuje zátěžová diagnostika, a to k neinvazivnímu odhadu vlastností atrioventrikulární spojky, zejména její schopnosti antegrádního (ze síní na komory) vedení při vzrůstající tepové frekvenci.
The number of asymptomatic children and young adults diagnosed with Wolff-Parkinson- White (WPW) preexcitation on resting ECG is arising, in particular after the implementation of the pre-participation screening of young competitive athletes. Besides symptoms of atriventricular re-entrant tachycardia, these patients are at increased risk of sudden cardiac death (SCD) due to rapid antegrade conduction of atrial fibrillation over the accessory pathway (AP). Individual risk assessment of SCD in affected persons is generally complex and both diagnostics and therapy are led by cardiologist, exercise stress testing however plays a crucial role in noninvasive assessment of the AP properties, namely of its capacity for antegrade (atrioventricular) conduction.
