PURPOSE OF THE STUDY To compare the treatment outcome of scaphoid facture fixation with one versus two Herbert screws (HBS). MATERIAL AND METHODS 72 patients underwent open reduction internal fixation (ORIF) following acute scaphoid fracture, and were followed prospectively by one surgeon. All fractures were Herbert & Fisher classification type B, the most common fracture lines being oblique (n=38) and transverse (n=34). Fractures with similar fracture lines were randomly assigned into two groups; fractures stabilized with one HBS (n=42) and fractures stabilized with two HBS (n=30). A specific methodology was developed for placement of two HBS; in the case of transverse fractures, screws were introduced perpendicular to the fracture line, for oblique fractures the first screw was placed perpendicular to the fracture line and the second screw was placed along the longitudinal axis of the scaphoid. RESULTS Patients were followed for a total 24 months, no patients were lost to follow-up. Outcome measures included bone healing, duration to bone healing, carpal geometry, range of motion (ROM), grip strength, and the Mayo Wrist Score. Patient rated outcomes were measured using DASH. Bone healing was radiographically and clinically confirmed in 70 patients. There were two non-unions after fixation with one HBS. Radiographic angles in both groups did not differ significantly from the physiological values. The mean duration to bone union was 1.8 months for one HBS and 1.5 months for two HBS. Mean grip strength was 47 kg in the group with one HBS (16-70 kg), 94 % of the unaffected hand, and 49 kg in the group with two HBS, 97% unaffected hand. The average Visual Analog Scale (VAS) score for the group with one HBS was 2.5, while for the group with two HBS was 2.0. Both groups had excellent and good results. For the group with two HBS, they are more. (100% for those fixed with two HBS and for those fixed with one HBS = 95% excellent and good and 5% bad results). DISCUSSION A review of the literature confirms that the addition of the second screw increases the stability in the scaphoid fractures by offering added resistance to torque forces. Most authors propose the parallel placing of both screws in all cases. In our study we offer an algorithm for the placement of screws depending of the type of fracture line. For transverse fractures screws are placed parallel and perpendicular to the fracture line, for oblique fractures the first screw is placed perpendicular to the fracture line, and the second screw is placed along the longitudinal axis of the scaphoid. This algorithm covers the main laboratory requirements for maximal fracture compression depending of the fracture line. CONCLUSIONS This study of 72 patients in whom patients with similar fracture geometry were separated into two groups fixed by one HBS and fixed by two HBS. Analysis of the results demonstrate that osteosynthesis with two HBS creates greater fracture stability. The proposed algorithm for fixation of acute scaphoid fractures using two HBS is achieved by simultaneously placing the screw along the axial axis and perpendicular to the fracture line. The stability is improved by the equal distribution of the compression force on the entire fracture surface. Key words: scaphoid fractures, Herbert screw, two screws fixation.

Úvod: Shrnuli jsme výsledky našeho sledování 32 případů pakloubů člunkové kosti léčených na našem ortopedickém oddělení Nemocnice Třebíč v letech 1998–2008 metodou Matti – modifikace Russe. Metoda a materiál: Operovali jsme 28 mužů a 4 ženy v průměrném věku 27,4 roku v intervalu od 8 měsíců po 12 let po úrazu. 80 % pacientů bylo operováno do 4 let po zlomenině skafoidea. Všichni pacienti byli léčeni na spádových chirurgických či ortopedických ambulancích a následně operačně na našem pracovišti. Pacienti byli sledováni v délce od 1,5 do 11 let po operaci s průměrnou dobou léčení 4,5 měsíce. Hodnotili jsme subjektivní výsledky, objektivní nález a rtg nález. Subjektivně dobrého výsledku jsme dosáhli u 22 pacientů (70 %), uspokojivého u 6 pacientů (19 %) a špatného u 4 pacientů (13 %). Výsledky: Objektivní nález jako dobrý byl hodnocen u 23 pacientů (73 %), jako uspokojivý u 7 operantů (22 %) a jako špatný u 2 pacientů (6 %). Rentgenologicky dobrý nález jsme zaznamenali u 21 pacientů (67 %), jako uspokojivý u 8 pacientů (26 %) a jako špatný u 3 pacientů (10 %). Diskuse: Analýza naší studie potvrzuje dobré výsledky léčení pakloubu navikulární kosti pomocí metody Matti, resp. modifikaci podle Russe. Převážná většina nemocných je výrazně zlepšena, bez přítomnosti instability či kolapsu karpu, a proto považujeme tuto metodu v případě správné indikace za vhodnou a úspěšnou.
Introduction: We have summarized the results of our study 32 cases of scaphoid nonunion treated at the Orthopaedic Department of the Třebíč hospital from 1998 to 2008 using the Matti method - with the Russe modification. Methods and material: We have operated on 28 men and 4 women with a mean age of 27.4 years at the time of 8 months to 12 years after the initial accident. 80 % of the patients were operated on within 4 years after scaphoid fracture. All patients were treated at the regional surgical or orthopaedic clinics and subsequently referred to surgery at our department. Patients were observed for 1.5 to 11 years after the surgery, with an average duration of treatment being 4.5 months. We evaluated the results of subjective and objective findings and X-ray findings. Subjectively, good results were achieved in 22 patients (70%), satisfactory in 6 patients (19%) and bad in 4 patients (13%). Results: Objective finding was assessed as good in 23 patients (73%), fair in 7 patients (22%) and poor in 2 patients (6%). Good radiographic findings were noted in 21 patients (67%), fair in 8 patients (26%) and poor in 3 patients (10%). Discussion: Analysis of our study confirms the good results of the treatment of navicular bone nonunion using the Matti method, and specifically its modification by Russe. The majority of patients saw significant improvement, without instability or carpal collapse and therefore we consider this method if to be eligible and successful in proper indications.
AIMS: To compare the functional results and complications associated with palmar percutaneous and dorsal limited approaches in the surgical treatment of nondisplaced or minimally displaced scaphoid fractures type B2. METHODS: A total of 76 patients with acute nondisplaced or minimally displaced type B2 scaphoid fractures were included in a prospective randomised study. The assignment patients to groups according to type of operative approach was based on systematic sampling. Clinical follow-up, X-ray or CT imaging were performed at four, eight and twelve weeks and one year after the surgery. Patient satisfaction and the results of DASH form were evaluated as well. The differences were statistically tested. RESULTS: We found significantly better flexion and grip strength in the group of palmar percutaneous aprroach during the follow-up examination at eight weeks after the surgery. Significantly better flexion of the same group persisted at twelve-week follow-up examination. No differences in results or complications were statistically significant up to one year follow up. CONCLUSIONS: We found no advantage to the palmar percutaneous approach in the treatment of nondisplaced and minimally displaced scaphoid fractures type B2 compared to dorsal limited approach.

Na bolestech v oblasti zápěstí mají vliv kromě postižení nervů stavy úrazové, jako jsou poranění kostí a vazů. Kostní poranění bývají diagnostikována, ale vazivová jsou často přehlédnuta a vedou ke vzniku nestabilit, které častou vedou k obtížím. Pokud nejsou nestability správně ošetřeny, jsou podkladem rozvoje degenerativních změn artritidy v oblasti zápěstí. Autoři prezentují nejčastější postižení zápěstí z pohledu ortopeda, představují klinické obtíže a možnosti ošetření.

Karpální nestability představují heterogenní skupinu poranění zápěstí s ohledem na rozsah poranění, stupeň poškození i variabilitou své symptomatologie. Často mohou být primárně nerozpoznány – například při léčbě současné zlomeniny distálního konce vřetenní kosti. Často to může být i fyzioterapeut, který první upozorní na potíže manifestující se v průběhu rehabilitace pacientů po zlomenině distálního radia, člunkové kosti nebo léčených pro vágní diagnózu „distorze zápěstí“, které jsou způsobeny nestabilitou zápěstí. Pro diagnostiku je možné využít klinické testy, radiologické vyšetření, včetně stresových snímků, magnetickou rezonanci nebo artroskopii zápěstí. Časný záchyt a přesná klasifikace poranění nám umožní zvolit vhodný léčebný postup, a tím zvýšit šanci na příznivý klinický výsledek.
Carpal instability represents a heterogenic group of wrist injuries considering the extent of the injury, degree of damage and variability of symptomatology. They can be often unrecognited at the beginning – e.g. in the therapy of simultaneous fracture of distant end of radius. It is often the physiotherapist who primarily draws attention to complaints that become manifest in the course of the patient’s rehabilitation after fracture of the distal radius, secaphoid, or treated for vague diagnosis of the wrist distortion which are caused by wrist instability. The establishment of diagnosis may be based on clinical tests, radiologic examination including stress images, magnetic resonance or wrist arthroscopy. An early and precise classification of the injury makes it possible to select a suitable therapeutic procedure and accordingly improve the chance for favorable clinical results.
