PURPOSE OF THE STUDY The purpose of our retrospective study is to evaluate 5-year functional and radiological outcomes in patients following corrective osteotomy of the distal radius and ulnar osteotomy for malposition after a distal radius fracture, to identify differences in the outcomes of corrective osteotomies depending on the type of the original fracture according to the AO classification, the grade of arthritis of radiocarpal (RC) joint, surgical approach and the way of stabilisation of the osteotomy. MATERIAL AND METHODS The followed-up group of 22 patients (8 men and 14 women) underwent osteotomy for malposition of distal radius in the period 2007-2011. The age of patients in the followed-up group ranged from 21 to 72 years, with the mean age of 51 years at the time of surgery. The indications for corrective osteotomy due to distal radius deformity were the following: functional limitation, pain and radiological parameters confirming the deformity. The opening wedge osteotomy of distal radius performed through volar or dorsal approach, isolated shortening osteotomy of ulna and a combination of both the methods were used. The observations included: the original type of fracture according to AO/OTA classification, functional outcomes - Disabilities of the Arm Shoulder and Hand (DASH) score, Mayo Wrist Score (MWS), range of motion, grade of wrist arthritis and specific complications. The follow-up period was 5 years. RESULTS Average results were obtained in the group of patients before/after the osteotomy: DASH score- 35/14, MWS- 54/77, flexion- 44°/64°, dorsiflexion- 48°/61°, supination- 75°/79°, pronation- 72°/83°, ulnar duction- 20°/23°, radial duction- 9°/16°, grip strength in percentage- 59%/83%. After osteotomy, a statistically significant increase was observed in flexion, dorsiflexion, supination, pronation, radial duction, ulnar duction, grip strength in percentage, MWS, while in the DASH score a statistically significant decrease was reported. Based on the statistical evaluation of differences in functional outcomes after osteotomy, in patients with type A and C original distal radius fractures no difference in range of motion parameters was found after osteotomy. As to the mean values, the flexion and dorsiflexion range of motion was greater by 10° in patients after extra-articular osteotomy for malposition following the type A original fracture as compared to the type C fracture. By measuring the Joint Space High (JSH) ratio, no statistically significant changes were found regarding the progression of arthrosis of the radiolunate and radioscaphoid part of the RC joint as against the arthrosis in patients up to 5 years after corrective osteotomy of the distal radius. In patients with distal radius malposition and RC joint grade 1-2 arthritis according to the Knirk and Jupiter classification, better functional outcomes were achieved than in the limited and total wrist arthrodesis. In our patients, at 5 years after osteotomy no worsening was observed of the existing wrist arthritis and no arthritis was newly diagnosed. Specific complications were found in 4 cases (18.2 %). In 2 patients after radial osteotomy from dorsal approach (extensor tendon irritation, rupture of the long extensor tendon of the thumb), removal of osteosynthesis material was necessary in both the patients. In one patient after the isolated ulnar shortening osteotomy, an intraoperative fissure of distal ulnar fragment was detected, which healed without any further complications. In one patient an iatrogenic fracture of anterior superior iliac spine was observed after harvesting the corticospongious graft from the ala of the ilium. DISCUSSION Corrective osteotomy is a well-established method for treating distal radius deformities following a fracture. Even at present, there are various opinions regarding the indications, contraindications, timing of the surgery, osteotomy technique and the need to use a bone graft. Limited or total wrist arthrodeses in the area of wrist arthrosis and deformities bring good results with respect to the pain relief, but a limited range of motion occurs mainly in younger patients. Bearing this in mind, in grade 1-2 wrist arthritis in patients with distal radius deformities, a better functional outcome can be achieved by osteotomy. By using dorsal or volar approach, comparable outcomes can be obtained, but with the dorsal approach there is higher frequency of complications and the need to remove the osteosynthesis material. CONCLUSIONS Corrective osteotomies resulted in an improved functional outcome in the treatment of deformities after a distal radius fracture. Apart from deformity correction, the treatment has been affected also by the RC joint arthritis. The study confirmed neither statistically significant differences in the osteotomy outcomes in patients with the original type A or type C distal radius fracture according to the AO classification, nor the statistical significance of the RC joint arthritis. Our results have proven better functional outcomes of deformity correction achieved by osteotomy in case of grade 1 and 2 arthritis than by the limited wrist arthrodesis. In patients showing arthritic changes, no progression was reported within five years after the osteotomy. Key words: distal radius fracture, distal radius deformity, osteotomy, functional results.

Although literature surrounding handedness and cerebellar asymmetry is limited, many researchers have suggested that a relationship exists (e.g., A. A. Beaton, 2003; L. Jäncke, K. Specht, S. Mirzazade, & M. Peters, 1999; I. C. McManus & K. M. Cornish, 1997; M. Peters, 1995; P. J. Snyder, R. M. Bilder, H. Wu, B. Bogerts, & J. A. Lieberman, 1995). For example, J. Tichy and J. Belacek ( 2008 , 2009 ) identified a link between cerebellar dominance and hand preference. The authors aimed to assess the relationship between cerebellar dominance and handedness, in 8-10-year-olds (N = 157 right-handers) as assessed with hand performance tests. Articular joint passivity in the wrist and performance differences between the hands were used as a means of assessing cerebellar dominance, where a link to skilled hand performance tests was revealed. Specifically, significant correlations between articular joint passivity and all measurements of handedness (p < .001) were observed. Greater hypotonia was seen in the left wrist of 95% of right-handers. This result supports the assumption that the preferred and nonpreferred hand could be controlled by the cerebellum in a different ways.

Názor na chirurgickou léčbu izolované scapho-trapezio-trapezoidní artrózy není doposud jednotný a ani etiologie vzniku izolované artrózy tohoto kloubu není dostatečně objasněna. Nejčastěji užívaná léčebná metoda u symptomatické scapho-trapezio-trapezoidní artrózy je artrodéza, která způsobuje omezení pohybu zápěstí a je zatížena komplikacemi především v podobě vzniku bolestivého pakloubu a rozvojem radioscaphoidního impingementu. Další metodou operačního řešení je resekce distálního pólu scaphoidea, která často způsobí zvýraznění midkarpální malpozice mezi proximální a distální karpální řadou a v některých případech dochází k progresivním změnám charakteru midkarpální destrukce kloubu. Popsaný případ upozorňuje na stav, kdy izolovaná scapho-trapezio-trapezoidní artróza není ve své podstatě primární, ale je sekundárním důsledkem chronické, asymptomatické, dorzální midkarpální nestability. Četnost těchto stavů a vhodný způsob léčby není dostatečně literárně dokumentován.
The opinions on the surgical treatment of isolated scapho-trapezoid trapeziometacarpal arthritis remain divided, and even the aetiology of the origin of osteoarthritis of the joint has not been sufficiently explained. The most commonly used method of treatment for symptomatic scapho-trapezoid-trapeziometacarpal arthritis is arthrodesis, which uses restriction of movement in the wrist and is burdened with complications, especially in the form of painful pseudo-arthritis and development of radioscaphoid impingement. Another method of surgical treatment is the resection of the distal pole of the scaphoid, which often causes the disparity in midcarpal malposition between the proximal and distal carpal row and in some cases leads to progressive changes in the character of midcarpal joint destruction. This case highlights the condition where isolated scapho-trapezoid-trapeziometacarpal arthrosis is not inherently primary, but secondary consequence of chronic, asymptomatic, dorsal midcarpal instability. The frequency of these conditions and suitable method of treatment is not sufficiently documented in literature.

Artrotické postižení zápěstí je poměrně časným zdrojem obtíží pacientů. Stále často je tento stav řešen totální artrodézou zápěstí, což sice pacienta zbaví bolesti, ale za cenu ztráty pohybů. V současné době je ve světě prosazovaná filozofie, kdy i minimální nebolestivý pohyb je pro pacienta důležitý. Autoři ve svém článku představují základní principy vzniku artrózy zápěstí na podkladě anatomické stavby, poukazují na základní klinické příznaky a diagnostické metody. Načrtávají principy operačních metod, které zachovávají pohyb, jako je selektivní denervace karpu, proximální karpektomie, čtyřrohá artrodéza karpu se resekcí od scafoideum či radioskafolunátní artrodézu.
Arthritis of the wrist is affected relatively early source of difficulties patients. Currently, in the world of philosophy promoted when the minimum painless movement is important for the patient. Úvod: Artrotické poškození zápěstí je častý důsledek neadekvátně léčených úrazů zápěstí. K odstranění bolestí, které způsobuje, byla publikována celá řada metod, jež však často omezují či zcela odstraňují pohyb. Selektivní denervace karpu je metoda, kterou lze použít ke snížení bolestí zápěstí. Soubor pacientů a metoda: Autoři představují soubor pacientů (31 zápěstí), u nichž byla provedena selektivní denervace karpu excizí nervus interosseus anterior a posterior. Soubor byl předoperačně zhodnocen pomocí Distability of the Arm, Shoulder and Hand (DASH) skóre a stejný soubor pacientů byl zhodnocen dva roky po operaci. Zároveň byla posuzována spokojenost pacientů na stupnici 1–5. Výsledky: Pacienti hodnotili svůj stav podle DASH skóre v průměru 43,52 body před operací a 24 body po ní. Zlepšení tedy bylo v průměru 19,52 bodů. Při hodnocení na škále 1–5 uvádělo devět (29 %) pacientů hodnocení 1 (velmi spokojen), 2 (spokojen) 12 (39 %), 3 (ani spokojen, ani nespokojen) šest (19 %), 4 nespokojen čtyři (13 %) a 5 (velmi nespokojen) neuvedl žádný pacient. Diskuze: Selektivní denervace karpu je metoda, která může u velké části pacientů snížit bolesti, aniž mění rozsah pohybů. Její výhoda je technická jednoduchost a možnost provedení jiných metod, pokud selektivní denervace karpu nevede ke zlepšení stavu.
Introduction: Arthritis of the wrist is a frequent consequence of inadequately treated wrist injuries. A range of methods has been published for pain reduction but these frequently limit, or completely eliminate, movement. Selective denervation of the wrist is method that can be used to reduce wrist pain. Patients and methods: The authors present a set of 31 wrists selectively denervatied by resection of nervus interosseus anterior and posterior. The file was preoperatively evaluated by Distability of the Arm, Shoulder and Hand (DASH) score and the same group of patients was evaluated two years after the surgery. Patient satisfaction was evaluated on a scale of 1–5. Results: Preoperative mean on the DASH score was 43.52 points and postoperative 24 points. Therefore, patients improved by a mean of 19.52 points. On the 1–5 scale, nine (29%) patients reported 1 (very satisfied), 12 (39%) reported 2 (satisfied), six (19%) reported 3 (neither satisfied nor dissatisfied), four (13%) marked 4 (unhappy) and none used 5 (very dissatisfied). Discussion: Selective denervation of the wrist is a method that can reduce pain with no effect on motion in a large proportion of patients. Its advantages include its technical simplicity and an opportunity to still use other treatments if selective denervation of the wrist does not improve the condition.
