Cíl práce: Cílem práce bylo představit diagnostický a léčený postup při proximální tibiofibulární luxaci. Materiál a metody: Autoři popisují diagnostiku a léčbu anterolaterální proximální tibiofibulární luxaci u 27letého pacienta po pádu na levé koleno. Diagnostika se zakládala hlavně na klinickém vyšetření, které bylo doplněno rentgenem (rtg) a vyšetřením výpočetní tomografie (CT). Zavřenou repozici autoři provedli v analgezii pacienta, a to při flexi v koleni a dorzální flexi nohy tlakem na hlavu fibuly. Po repozici bylo provedeno kontrolní CT vyšetření a naložena ortéza. Pacient byl a je bez neurologické léze. Výsledky: Pacient je půl roku od úrazu bez potíží. Pohyby v koleni jsou plné, bez známek nestability. Závěr: Popsaný postup při tomto raritním poranění je v souladu s literárními poznatky.
Purpose of study: The aim of this work was to introduce diagnostic and terapeutic process in therapy of proximal tibiofibular dislocation. Material and methods: The authors describe diagnostic process and therapy of proximal anterolateral tibiofibular dislocation in 27 years old patients after fall to left knee. The diagnosis was founded on clinical examination appenden with plain films and computed tomography. After analgetization of patient dislocation was treated by closed reduction. The knee was flexed and the foot dorsiflexed and externally rotated. Pressure was applied over the fibular head, until a “pop” was heard. Afret reduction authors made control computed tomography and applied orthesis. Patient was without neurologic laesion. Results: Patient is now without any problems. Patient is able to burden fully his limb and the range of motion (ROM) in knee joint is without any restrictions. Conclusion: Described diagnostic and terapeutic procedure are accord with literature.

PURPOSE OF THE STUDY: The aim of the study was to describe the indication criteria and operative technique and to present the first conclusions of a prospective study dealing with an assessment of functional and radiographic findings in patients with concurrent fractures of the distal ulna and distal radius; the former was treated by osteosynthesis using an LCP Distal Ulna Plate (De Puy Synthes, USA) and the latter by plate osteosynthesis. MATERIAL AND METHODS: Between August 2013 and September 2014, 18 patients (3 men and 15 women; average age, 58 years; range, 25-74 years) with a fracture of the distal ulna and a concurrent distal radius fracture were treated by plate osteosynthesis. The indications for surgery included displaced ulnar styloid base fractures in 10 patients, displaced comminuted fractures of the ulnar head in seven patients and a subcapital fracture of the ulna in one patient. The average follow-up was 9 months, with 13 (72%) patients being followed up for 6 months at least. The outcome of surgery was evaluated on the basis of X-ray views and multiplanar reconstruction of CT scans. Functional results were based on measuring the range of motion at wrist and forearm and hand grip strength and on the scores obtained from the Mayo Wrist Score System and Quick DASH Questionnaire. RESULTS: The average values for wrist and forearm range of motion were as follows: 68° in flexion (60-80°), 71° in extension (40- 90°), 87° in pronation (70-90°), 81° in supination (50-90°). This corresponded to a 92% (74-100%) range of motion at the contralateral wrist and forearm. The average hand grip strength measured with a dynamometer was 80.6% (53-100%) of the gripping force exerted by the unaffected extremity. Post-operative X-ray and CT findings showed good reduction of both distal radius and distal ulna fractures as well as good fragment retention by means of LCP implants. During follow-up complete bone union and full stability of the radioulnar joint were achieved in all patients. The functional outcome of treatment was shown by the average Mayo wrist score and Quick DASH score of 84 (55-100) and 7.4 (0-47.7) points, respectively. DISCUSSION: The functional outcome of treatment in our patients corresponded to the severity of their distal forearm fractures. The patients with more serious distal radius fractures, frequently associated with ulnar head fractures, had poorer functional results than the patients with less serious fractures. According to the AO classification, in the distal radius fractures, prevailing fracture types were 23-C3 (78%) and 23-C2 (16%). In relation to the distal ulna fracture type, the patients with ulnar styloid base fractures had better functional results than the patients with ulnar head fractures, because a fracture of the styloid process does not affect the congruency of the distal radioulnar joint (DRUJ). Fractures of the ulnar styloid process base, following distal radius fracture osteosynthesis, were invariably associated with DRUJ instability. This fracture type is always accompanied by injury to the distal radioulnar ligament of the triangular fibrocartilage complex (TFCC) and, in case of a displaced fracture, surgical management is indicated. In agreement with the results of other authors it was found in our study that, when the anatomical position of the distal radioulnar joint had been achieved, both the functional and the radiographic findings were post-operatively very good. The final average values for the range of wrist and forearm motion, hand grip strength and hand and arm function were assessed as very good. CONCLUSIONS The assessment of our results and their comparison with the literature data showed that osteosynthesis of distal ulna fractures with concurrent distal radius fractures managed by open reduction and internal fixation are indicated in displaced fractures of the ulnar styloid process base, displaced comminuted fractures of the ulnar head where fragments interfere with DRUJ congruency, and unstable displaced subcapital fractures of the ulna. The early results of this study provide evidence for the efficiency of plate osteosynthesis in the treatment of distal ulna fractures.

Úvod: Příčiny nestabilit můžeme rozdělit na stavy poúrazové, při kterých dochází k poškození vazů, a nestability v důsledku hypermobility kloubní na podkladě vrozené poruchy syntézy kolagenu typu I. V klinickém obrazu skafolunátní (SL) nestability bývá ve většině případů přítomná bolest zápěstí, zpočátku pozátěžová, v chronicitě pak klidová. Hlavním dynamickým stabilizátorem os scaphoideum radiální strany zápěstí je musculus flexor carpi radialis (FCR), který vzhledem k průběhu své šlachy používá tuto kůstku jako kladku k dosažení svého distálního úponu na II. metakarpu. Cílem pilotního projektu bylo zhodnotit míru benefitu posílení FCR v konzervativní terapii dynamických SL nestabilit. Pacienti a metody: V letech 2012–2014 bylo na našem pracovišti diagnostikováno a zařazeno do terapeutického programu 56 testovaných pacientů (49 žen, 7 mužů), věkového rozpětí 18–49 roků. Kritériem pro zařazení byla diagnóza predynamické nebo dynamické formy skafolunátní nestability s pozitivním Watsonovým testem. Třináct pacientů mělo diagnostikovánu nestabilitu SL po primární fraktuře distálního radia, u 11 probandů byla přítomna klidová bolest, u 45 pak pozátěžová, omezující aktivity denního života, a v 9 případech bylo současně verifikováno poškození triangulárního fibrokartilaginózního komplexu (TFCC). Hodnotili jsme míru bolesti prostřednictvím visual analog scale (VAS) u každého z probandů při první terapii a následně s odstupem 1,5 a 3 měsíců od začátku fyzioterapie. Byly vybrány 4 specifické cviky pro aktivitu musculus flexor carpi radialis. Výsledky: Při prvním vyšetření škály bolesti bylo její rozmezí u testovaných jedinců 5–8. Po prvním kontrolním měření změn s odstupem 1,5 měsíce od začátku terapie se intenzita bolestí zápěstí snížila do rozhraní 3–6 a po druhém kontrolním měření bylo subjektivní hodnocení bolesti od 2 do 7. S odstupem 3 měsíců byl z daných měření největší nárůst hodnot v intenzitě 3, tedy významné zklidnění obtíží. Pokud budeme brát hodnoty bolesti 5–8 VAS škály, pak u 19 probandů (33 %) z 56 nedošlo k výrazným změnám ve smyslu zmírnění obtíží, nicméně 5 probandů mělo současně poškozen komplex TFCC a 8 bylo po primární fraktuře distálního radia, zbylých 6 v tomto rozhraní mělo diagnostikovanou pouze dynamickou formu skafolunátní nestability. Osm pacientů vyžadovalo další chirurgickou intervenci pro neustupující bolesti zápěstí a značné limity denních činností – activity of daily living (ADL). Diskuze: V našem souboru 56 pacientů s diagnostikovanou dynamickou skafolunátní nestabilitou došlo k inhibici bolesti u 66 % testovaných v případě rozmezí škály VAS hodnot 0–4, nicméně u všech probandů došlo po 3 měsících k alespoň nějakému snížení intenzity bolesti po cílené rehabilitační intervenci. U 8 jedinců byly obtíže natolik limitující, že byl chirurgický zásah nezbytný. Potvrdilo se nám, že facilitace FCR má jednoznačný benefit pro pacienty s predynamickou a dynamickou formou a je schopna v určité míře zamezit předčasnému chirurgickému řešení. Nicméně můžeme usuzovat, že v případech, kdy je k samotné SL nestabilitě přidružena i jiná patologie (traumatizace TFCC, fraktura distálního radia), bude efekt samotné rehabilitace méně uspokojivý a následná chirurgická intervence více pravděpodobná. Závěr: Pohledem traumat měkkotkáňových struktur zápěstí řadíme SL disociace k frekventovaným obtížím v segmentu ruky. Jejich včasná a korektní diagnostika a následná cílená léčba může zajistit úspěšný výsledek na poli terapeutickém. Naší snahou je nejen rozšířit povědomí o specifičnosti dané problematiky a nutnosti komplexnosti léčby, ale také najít vzájemnou mezioborovou spolupráci, dál posunout vlastní zkušenosti a neomezit se jen na již zavedené a nekomplikované.
Introduction: The causes of instability may be classified as post-traumatic with ligament damage and those resulting from joint hypermobility due to congenital impairment of type 1 collagen synthesis. In most cases of scapholunate (SL) instability, the clinical picture includes wrist pain, initially post-exertional and later, in the chronic stage, resting. The main dynamic stabilizer of the scaphoid bone on the radial side of the wrist is the flexor carpi radialis (FCR) muscle. Along the course of its tendon, the muscle uses this bone as a pulley to reach its distal insertion at the second metacarpal bone. This pilot project aimed at assessing the benefit of FCR strengthening in conservative treatment for dynamic SL instabilities. Patients and methods: In 2012–2014, a total of 56 tested patients (49 males and 7 females) aged 18–49 years were diagnosed and enrolled in the therapeutic program at our center. The inclusion criterion was a diagnosis of pre-dynamic or dynamic SL instability made with Watson’s test. Thirteen patients were diagnosed with SL instability following a primary distal radius fracture. There was resting pain in 11 subjects and post-exertional pain limiting activities of daily living (ADL) in 45 persons. At the same time, a triangular fibrocartilage complex (TFCC) injury was confirmed in 9 cases. In each participant, pain intensity was measured using the Visual Analog Scale (VAS) during the first treatment session and at 1.5 and 3 months from the initiation of physiotherapy. Four specific types of exercise were selected for FCR muscle activity. Results: During the first measurement, the tested individuals had a pain intensity of 5–8. At 1.5 and 3 months from the initiation of therapy, the subjective pain intensity was reduced to 3–6 and 2–7, respectively. At 3 months, the largest increase was observed for a pain intensity of 3, suggesting a significant reduction of difficulties. When considering VAS pain intensity ranging from 5 to 8, nineteen out of the 56 subjects (33%) had no significant alleviation of problems. However, five individuals also had a TFCC injury and 8 had sustained a distal radius fracture; the remaining 6 patients with that range of pain intensity were only diagnosed with dynamic SF instability. Eight individuals required further surgical intervention for persistent wrist pain and severely limited ADLs. Discussion: In the group of 56 patients diagnosed with dynamic SL instability, pain was inhibited in 66% of the tested subjects with VAS pain intensity ranging from 0 to 4. In all patients, however, there was at least some reduction in pain intensity after three months of targeted rehabilitation intervention. In 8 patients, the difficulties were so limiting that they required surgical intervention. FCR facilitation has been confirmed to have a clear benefit to patients with pre-dynamic and dynamic SL instability and may, to a certain extent, prevent the need for early surgery. It may be assumed, however, that when SL instability is associated with another pathology (e.g. TFCC injury or distal radius fracture), the effect of rehabilitation alone would be less satisfactory and subsequent surgical intervention will be more likely needed. Conclusion: Among injuries to soft tissue structures of the wrist, SL dissociation is a common problem. Early and correct diagnosis as well as subsequent targeted therapy may guarantee successful results. It is our effort not only to extend the awareness of these issues and the need for comprehensive treatment but also to seek multidisciplinary cooperation and to extend our own experience without having to rely on the already established and uncomplicated approaches.

Úvod: Poranění extenzorů v zóně I je velmi časté poranění, které při nesprávné léčbě může vést k postižení funkce ruky. Pro toto poranění je typické flekční postavení distálního článku v DIP bez možnosti aktivní extenze. Materiál a metody: Do souboru vyšetřovaných pacientů bylo zařazeno 48 pacientů s izolovaným poraněním dorzální aponeurózy jednoho prstu, u kterých uběhly minimálně 4 měsíce, maximálně 15 měsíců od primárního poranění. Indikací k chirurgické léčbě zastaralých poranění bylo selhání konzervativní terapie. Všichni pacienti byli operováni v lokální anestezii v ambulantním režimu. Z hrotu prstu byl zaveden jeden Kirchnerův drát o průměru 0,8–1,2 mm, který byl následně zanořen pod kůži. Fixace byla odstraněna 8.–9. týden. Bezprostředně po vyjmutí K-drátu byla nasazena fixace – dlaha typu OVALE s postupným vysazováním dlahy během dne po dobu 2–3 týdnů. Celková doba aplikace dlahy přes noc po odstranění K-drátu se pohybovala mezi 2–3 měsíci. Výsledky: Léčba byla pacienty přijímána velmi dobře vzhledem k minimálnímu omezení v běžném životě. U žádného pacienta nedošlo k infekčním komplikacím. Šest pacientů si krátce stěžovalo na zvýšenou citlivost v oblasti jizvy pod nehtem. U 2 pacientů, mužů, došlo k selhání léčby, DIP kloub ztuhl v semiflekčním postavení, bez aktivní extenze, jednalo se o pacienty, u kterých již před operací bylo obtížné prst rozcvičit do extenze. Celkem 27 pacientů mělo úplnou úpravu hybnosti ve smyslu flexe i extenze v celém rozsahu; 14 pacientů mělo omezenou plnou extenzi do 10–30 stupňů.U 12 pacientů byla omezenaflexe v DIP o 20–30 stupňů ve srovnání se zdravou rukou. I přes tato mírná pohybováomezení byly výsledky léčby pacientypozitivně přijaty. Závěr: Chirurgická léčba zavřených poranění extenzorů v zóně I je indikována u zastaralých poranění, která nebyla léčena nebo u kterýchselhala léčba konzervativní. Podmínkou léčby je rozcvičení DIP kloubu minimálně do extenze. Okamžitá konzervativní léčba u akutních úrazů mívá lepší výsledky než léčbachirurgická u zastaralých poranění.
Introduction: Extensor tendon injury in zone I is a very common injury, which could lead to impaired hand function without proper treatment. This injury is characterized by flexion in DIPJ without possibility of active extension. Material and methods: The study group included 48 patients with isolated injury to dorsal aponeurosis of one finger, between 4 months to 15 months from the primary injury. The indication for surgical therapy of chronic injuries is a consequence of failure of conservative management. All patients underwent surgery under local anaesthesia on out-patient basis. There was a 0.8 - 1.2mm Kirschner wire introduced from the apex of the finger, and it was subsequently hidden under the skin. Fixation was removed after 8-9 weeks. Immediately after removal of the K-wire, OVALE type splint was applied. This was used for night splinting after K-wire removal for a total of 2-3 months, while splinting during the day lasted only for 2-3 weeks during which it was gradually removed. Results: Treatment was very well accepted by the patients due to the minimal limitation in their daily life. None of the patients developed infectious complications. 6 patients complained shortly of greater tenderness in the area of the scar under the nail. 2 male patients experienced failure of therapy, DIPJs became stiff in semiflexion; there was no active extension. These were patients in whom it was difficult to mobilize the finger to extension before the surgery. 27 patients achieved full range of motion to flexion and extension. 14 patients had extension limited to 10-30 degrees. 12 patients had reduced flexion in DIPJ by 20-30 degrees in comparison with the other hand. In spite of these minor reduction of ROM, the results of therapy were accepted well by the patients. Conclusion: Surgical therapy of closed extensor tendon injuries in zone I is indicated in chronic injuries, without previous treatments or in case of a failure of conservative management. The condition for treatment is good passive range of motion in DIPJ, at least in extension. Immediate conservative management of acute injuries has usually better results than surgical therapy in chronic injuries.

Názor na chirurgickou léčbu izolované scapho-trapezio-trapezoidní artrózy není doposud jednotný a ani etiologie vzniku izolované artrózy tohoto kloubu není dostatečně objasněna. Nejčastěji užívaná léčebná metoda u symptomatické scapho-trapezio-trapezoidní artrózy je artrodéza, která způsobuje omezení pohybu zápěstí a je zatížena komplikacemi především v podobě vzniku bolestivého pakloubu a rozvojem radioscaphoidního impingementu. Další metodou operačního řešení je resekce distálního pólu scaphoidea, která často způsobí zvýraznění midkarpální malpozice mezi proximální a distální karpální řadou a v některých případech dochází k progresivním změnám charakteru midkarpální destrukce kloubu. Popsaný případ upozorňuje na stav, kdy izolovaná scapho-trapezio-trapezoidní artróza není ve své podstatě primární, ale je sekundárním důsledkem chronické, asymptomatické, dorzální midkarpální nestability. Četnost těchto stavů a vhodný způsob léčby není dostatečně literárně dokumentován.
The opinions on the surgical treatment of isolated scapho-trapezoid trapeziometacarpal arthritis remain divided, and even the aetiology of the origin of osteoarthritis of the joint has not been sufficiently explained. The most commonly used method of treatment for symptomatic scapho-trapezoid-trapeziometacarpal arthritis is arthrodesis, which uses restriction of movement in the wrist and is burdened with complications, especially in the form of painful pseudo-arthritis and development of radioscaphoid impingement. Another method of surgical treatment is the resection of the distal pole of the scaphoid, which often causes the disparity in midcarpal malposition between the proximal and distal carpal row and in some cases leads to progressive changes in the character of midcarpal joint destruction. This case highlights the condition where isolated scapho-trapezoid-trapeziometacarpal arthrosis is not inherently primary, but secondary consequence of chronic, asymptomatic, dorsal midcarpal instability. The frequency of these conditions and suitable method of treatment is not sufficiently documented in literature.

PURPOSE OF THE STUDY: The aim of this study is to compare functional and X-ray results before and after four-corner arthrodesis using an angular stable dorsal circular plate in patients with post-traumatic degenerative changes of the wrist. METHODS: Nine consecutive patients with scapholunate advanced collapse (SLAC) or scaphoid nonunion advanced collapse (SNAC) of the wrist, stage III, underwent scaphoid excision and four-corner fusion using angular stable plate fixation. The wrists were immobilised using a split for two weeks and then a removable wrist brace with assisted physiotherapy for four weeks. Minimal followup was 18 months. Clinical assessment before and after surgery included the wrist range of motion (ROM), grip strength, presence of pain, disabilities of the arm, shoulder and hand (DASH) scores, and radiographic findings of the carpal height and ulnar translation ratios. Differences were statistically tested. In addition, patients' satisfaction with functional outcomes was evaluated. RESULTS: In the post-operative period the patients experienced less pain during daily activities and had a better carpal height ratio than before surgery; these differences were statistically significant. In the other criteria, although showing better post-operative results, improvement was not statistically significant. Eight patients were satisfied with the final outcome. One patient required limited denervation of the wrist for pain relief. There was no non-union. DISCUSSION The functional outcomes (ROM, grip strength) in our patients at follow-up were fully comparable with the previously published studies. Although, by most postoperative treatment protocols, physical therapy is started at four to eight weeks of rigid fixation of the wrist, no non-union was found in our group of patients who had a shorter period of post-operative fixation. CONCLUSIONS: Four-corner arthrodesis of the wrist using angular stable dorsal circular plate fixation provides pain relief with acceptable preservation of the range of motion and no consolidation problems. Long-term follow-up and a larger group of patients would be necessary to confirm these hopeful results.

Úvod: Artrotické poškození zápěstí je častý důsledek neadekvátně léčených úrazů zápěstí. K odstranění bolestí, které způsobuje, byla publikována celá řada metod, jež však často omezují či zcela odstraňují pohyb. Selektivní denervace karpu je metoda, kterou lze použít ke snížení bolestí zápěstí. Soubor pacientů a metoda: Autoři představují soubor pacientů (31 zápěstí), u nichž byla provedena selektivní denervace karpu excizí nervus interosseus anterior a posterior. Soubor byl předoperačně zhodnocen pomocí Distability of the Arm, Shoulder and Hand (DASH) skóre a stejný soubor pacientů byl zhodnocen dva roky po operaci. Zároveň byla posuzována spokojenost pacientů na stupnici 1–5. Výsledky: Pacienti hodnotili svůj stav podle DASH skóre v průměru 43,52 body před operací a 24 body po ní. Zlepšení tedy bylo v průměru 19,52 bodů. Při hodnocení na škále 1–5 uvádělo devět (29 %) pacientů hodnocení 1 (velmi spokojen), 2 (spokojen) 12 (39 %), 3 (ani spokojen, ani nespokojen) šest (19 %), 4 nespokojen čtyři (13 %) a 5 (velmi nespokojen) neuvedl žádný pacient. Diskuze: Selektivní denervace karpu je metoda, která může u velké části pacientů snížit bolesti, aniž mění rozsah pohybů. Její výhoda je technická jednoduchost a možnost provedení jiných metod, pokud selektivní denervace karpu nevede ke zlepšení stavu.
Introduction: Arthritis of the wrist is a frequent consequence of inadequately treated wrist injuries. A range of methods has been published for pain reduction but these frequently limit, or completely eliminate, movement. Selective denervation of the wrist is method that can be used to reduce wrist pain. Patients and methods: The authors present a set of 31 wrists selectively denervatied by resection of nervus interosseus anterior and posterior. The file was preoperatively evaluated by Distability of the Arm, Shoulder and Hand (DASH) score and the same group of patients was evaluated two years after the surgery. Patient satisfaction was evaluated on a scale of 1–5. Results: Preoperative mean on the DASH score was 43.52 points and postoperative 24 points. Therefore, patients improved by a mean of 19.52 points. On the 1–5 scale, nine (29%) patients reported 1 (very satisfied), 12 (39%) reported 2 (satisfied), six (19%) reported 3 (neither satisfied nor dissatisfied), four (13%) marked 4 (unhappy) and none used 5 (very dissatisfied). Discussion: Selective denervation of the wrist is a method that can reduce pain with no effect on motion in a large proportion of patients. Its advantages include its technical simplicity and an opportunity to still use other treatments if selective denervation of the wrist does not improve the condition. Omezení extenze palce nebo méně častěji tříčlánkového prstu u dětí vzniká, na rozdíl od podobné afekce v dospělém věku, na podkladě lokálního zesílení šlachy hlubokého flexoru. Stav je často mylně přičítán poranění prstu nebo jeho následkům. U části dětí mohou obtíže vymizet spontánně během několika měsíců až let, proto jsou názory na způsob léčby kontroverzní – od prostého sledování, přes fyzioterapii a dlahování, miniinvazivní perkutánní uvolnění šlachy až ke klasickému operačnímu výkonu. Zde prezentujeme 3letého pacienta, u kterého bylo po ročním sledování provedeno pro přetrvávající obtíže chirurgické ošetření. Kazuistika je doplněna přehledem literárních poznatků.
The restriction of thumb extension or less frequently, of digits extension in children is caused by the profundus flexor tendon local thickening, contrary to a similar affection in adults. This situation is often incorrectly considered to be a finger trauma or its consequence. We can see the spontaneous resolution of some paediatric trigger fingers and it brings forward controversial approaches to treatment possibilities – from “wait and see” to physiotherapy, splinting, minimal invasive percutaneous tendon release to surgical open release. We present a three-year-old patient treated by open surgical release after a one-year period of no improvement. This case report is supplemented with an overview of literature.
