Among all oral diseases, teeth infection is a commonly encountered problem in dentistry with teeth blackening commonly seen in kids. One of the major causes of teeth blackening are the chromogenic bacteria which form biofilm in the teeth. So far there is no significant and safe treatment available for teeth blackening. The current study focuses on teeth blackening by chromogenic bacteria and various factors promoting the biofilm formation and its treatment by an alternative herbal approach. Distinct colonies isolated from dental swabs of children with black stained teeth were grown individually on oyster shell to identify the particular isolate responsible for tooth discoloration. The interaction of traditionally known substances like NaCl, glucose, charcoal powder, mustard oil, KMnO4 and H2O2 on the growth of bacterium and biofilm forming ability were studied. Further, the leaf and stem extracts of Achyranthus aspera and Alternanthera sessilis (available locally) on the growth and biofilm formation was also evaluated. Finally, the efficacy of the various agents was determined by the reduced biofilm formation as well as change in the membrane potential of the isolates. From our results we propose a non-toxic approach comprising herbal compounds and household reagents against teeth blackening to improve oral hygiene.

Širokospektrálne beta-laktámové antibiotiká našli rozsiahle medicínske uplatnenie, ich časté používanie v klinickej praxi vedie ale k šíreniu rezistencie proti nim. Vážnym problémom v tejto oblasti je stúpajúci výskyt infekcií spósobených producentami beta-laktamáz skupiny 1(klasifikácie podla Bushovej), ku ktorým patria aj zástupcovia rodu Enterobacter. Rezistencia proti beta-laktámom móže byt u týchto významných nozokomiálnych patogénov podmienená 1) znížením permeability vonkajšej bunkovej membrány pre antibiotikum v dósledku zmien v lipopolysacharidoch alebo v proteínoch vonkajšej membrány (porínoch); 2) producciou beta-laktamáz, ktoré inaktivujú beta-laktámy a móžu spósobovat rezistenciu aj nehydrolytickým mechanizmom označovaným „vychytávanie" antibiotika (trapping). HIinicky velmi významná je produkcia plazmidovo-kódovaných širokospektrálnych beta-laktamáz, ale najetá chromozomálne kódovanej inducibilnej cefalosporinázy AmpC, ktorá móže byt v dósledku spontánnych chromozomálnych mutácií syntetizovaná konštitutívne vo velkých množstvách. Najlepšiu účinnost v liečbe infekcií spósobených druhmi rodu Enterobacter si zachovávajú cefalosporíny štvrtej generácie a karbapenémy, ale kombinácia vysokej hladiny produkcie beta-laktamáz a zníženej permeability vonkajšej bunkovej membrány, ktorá nie je u Enterobacter spp. zriedkavá, spósobuje rezistenciu aj proti týmto liečivám.
Extended-spectrum beta-lactam antibiotics have found great medical importance, but their wide use in clinical practice leads to increasing resistance to them. The more frequent occurrence of infections caused by Bush group 1 beta-lactamase producing organisms, including Species of the genus Enterobacter, is a serious problem in this field. Resistance to beta-lactams in this important nosocomial pathogens can be due to 1) reduction in outer membrane permeability to antibiotics caused by alterations in outer membrane lipopolysacharides or proteins (porins); 2) production of beta-lactamases, which inactivate beta-lactams and can also lead to resistance by non-hydrolytic mechanistu called trapping. Production of plasmid-mediated extended-spectrum beta-lactamases, but especially chromosomally-mediated inducible cephalosporinase AmpC, which can be synthesized constitutively in large amounts as consequence of spontaneous chromosomal mutations, are of great clinical importance. Fourth-generation cephalosporins and carbapenems are the most effective in the treatment of infections caused by Species belonging to the genus Enterobacter, but combination of high level beta-lactamase production and decreased outer membrane permeability, which is not rare in Enterobacter spp., leads to resistance even to these drugs.
Riziko ulcerací na noze s lokální i generalizovanou infekcí se u diabetických pacientů po transplantaci ledviny, resp. pankreatu a ledviny, může zvyšovat. Hluboká infekce u syndromu diabetické nohy ohrožuje končetinu, musí se proto léčit velmi intenzivně. Cílem práce bylo zjistit rozdíly ve výskytu mikrobiální flóry ve stěrech z ulcerací a ve způsobu antibiotické léčby syndromu diabetické nohy u pacientů po transplantaci ledviny, resp. pankreatu a ledviny, a u pacientů bez transplantace orgánů. V retrospektivní studii byl hodnocen soubor 20 pacientů s diabetickou nohou po transplantaci orgánů a 97 pacientů s diabetickou nohou bez transplantace léčených v podiatrické ambulanci IKEM v letech 1995-1996. Výsledky naší práce prokázaly, že u pacientů se syndromem diabetické nohy po transplantaci orgánů byl ve srovnání s pacienty bez transplantace signifikantně vyšší výskyt Enterobacter species ve stěrech z ulcerací, což vedlo v praxi k nutnosti používat signifikantně častěji širokospektrá antibiotika se zaměřením na gramnegativní bakterie.
The risk of foot ulcers with a local and generalized infection may increase in diabetic patients after transplantation of the kidney or transplantation of the pancreas and kidney. Deep infection in a diabetic foot threatens the extremity and must be treated aggressively. The objective of the presented work was to assess differences in microbiological findings in swabs from diabetic ulcers and the use of antibiotics in the management of diabetic foot infection in patients after organ transplantation and in patients without transplantation. In a retrospective study, a group of 20 patients with the diabetic foot after organ transplantation and a group of 97 patients with the diabetic foot without transplantation, who were treated in our podiatric outpatient unit in 1995-1996, was evaluated. Results of our study showed a significantly higher occurrence of the Enterobacter species in patients with the diabetic foot syndrome after organ transplantation in comparison with patients with the diabetic foot syndrome without transplantation. In practice, this led to significantly more frequent use of broad-spectrum antibiotics focused on Gram-negative bacteria.
