Úvod: Všeobecní praktičtí lékaři mají mimořádné postavení v systému preventivní péče. Mají k pacientům nejblíže, překládají preventivní strategie do individuální péče a realizují preventivní programy. Prevence zahrnuje zejména systematické preventivní prohlídky v intervalu 2 let, screeningové programy a příležitostné intervence rizikových faktorů. Výbor Společnosti všeobecného lékařství ČLS JEP se rozhodl uskutečnit reprezentativní sociologický průzkum a prověřit výše uvedenou problematiku z pohledu českých pacientů. Cíle a metody: Cílem výzkumu bylo zjistit, jaké jsou postoje české populace k vlastnímu zdraví, především k preventivním prohlídkám, ke screeningu kolorektálního karcinomu a zároveň i prověřit postoje občanů k dílčím aspektům fungující praxe, jako je možnost objednávání a využití různých způsobů komunikace. Výzkum byl koncipován jako nezávislý sociologický. Terénní šetření bylo provedeno technikou standardizovaného řízeného rozhovoru žadatele s respondentem. Statistické zpracování dat bylo provedeno programem SASD 1. 4. 10 Výsledky: Více než polovina registrovaných pacientů navštíví během roku svého všeobecného praktického lékaře. Průzkum dokumentuje zájem o preventivní prohlídky a setkávají se s aktivním přístupem všeobecných praktických lékařů k preventivním prohlídkám. Z pohledu pacientů roste organizační úroveň praxí. Všeobecný praktický lékař je klíčovým a aktuálně nenahraditelným zdrojem informací ve screeningu tlustého střeva a konečníku.
Introduction: General practitioners have a special place in the system of preventive care. They are closest to the patients, directly apply preventive strategies into individual care, and implement preventive programs. Prevention principally includes biannual systematic preventive examinations, screening programs, and the occasional intervention of risk factors. The Committee of the Society of General Practice in the Czech Republic (ČLS JEP) decided to make a representative sociological survey in order to examine the above issues from the perspective of Czech patients. Objectives and methods: The aim of the research was to find out the attitudes of the Czech population towards their own health, especially towards preventive examinations, colorectal cancer screening, as well as to examine the stance of citizens on individual aspects of practice operations, such as the possibility of making appointments and using various methods of communication. The research was designed in accordance with independent sociological methodology. The field survey was carried out in the technique of standardized controlled interviews of the interviewer with the respondent. The computer program SASDM 1. 4. 10 did the statistical data processing. Results: More than half of all registered patients visit their general practitioner at least once a year. The survey documented patient interest in preventive examinations, as well as an active approach of general practitioners towards preventive care. From the perspective of the patients, the standard of practice operations is on the rise. General practitioners are a crucial, and, currently, irreplaceable source of information for the patients in the screening of the colon and rectum..

The authors investigated the relationship between household environmental tobacco smoke (ETS) exposure and prevalence of respiratory symptoms and diseases as well as absenteeism related to respiratory illness in schoolchildren. The study sample consisted of 1,074 children aged 7–11 years from three primary schools in Niš (Serbia). ETS exposure was associated with wheezing (OR=1.48; 1.09–2.01), bronchitis (OR=1.66; 1.23–2.23), headache (OR=1.45; 1.08–1.95), and fatigue (OR=1.38; 1.02–1.85) in exposed children. The other risk factors with possible influences weren't assessed. There was no statistically significant difference in the number of physicians' visits as well as in absenteeism from school due to illness in children exposed to ETS in comparison to non exposed children. The tobacco smoke effect on children is an essential and urgent problem with life lasting negative health effects which are preventable.

BACKGROUND: Sublingual allergen immunotherapy (SLIT) is a commonly used alternative route of administration to standard subcutaneous immunotherapy (SCIT) in Europe. Despite its wide use, the cost-effectiveness of SLIT vs SCIT has not been well established. OBJECTIVE: To evaluate the cost and effectiveness of SLIT compared with SCIT in patients with allergic rhinoconjunctivitis during a 3-year specific allergen immunotherapy (SIT) from a third-party payer's, a patient's, and society's perspectives. METHODS: We performed an open-label randomized clinical trial of patients receiving SLIT (n = 19), patients receiving SCIT (n = 23), and a control group (n = 22). The outcome measures were Rhinoconjunctivitis Quality of Life Questionnaire score, visual analog scale score, symptomatic medication reduction, and direct and indirect costs. RESULTS: SLIT offered clinical benefits to patients comparable to those provided by SCIT. From the perspective of a third-party payer, the total average direct medical cost per patient of 3-year SIT was estimated at Euro 416 vs Euro 482 in the SLIT and SCIT groups, respectively. A patient who received SLIT paid less than a patient who received SCIT for all out-of-pocket costs (Euro176 for SLIT vs Euro 255 for SCIT) but more for sole allergen extracts (Euro 72 for SLIT vs Euro 55 for SCIT). When both direct and indirect costs were considered, the 3-year SIT expenditures per patient reached Euro 684 vs Euro 1,004 in the SLIT and SCIT groups, respectively. CONCLUSIONS: SLIT represents a less expensive alternative relative to subcutaneous administration from all perspectives. However, from a patient's perspective, SCIT offers a less expensive alternative for patients who do not experience loss of income and travel costs associated with treatment.
