The effects of clotrimazole (CLO) and dexamethasone (DEX), both detected in the aquatic environment, were assessed on inhibition of cytochrome P450 (CYP450) in hepatic microsomes of rainbow trout. Activity of three CYP450 isoforms: ethoxyresorufin O-deethylase (EROD; CYP1A), 7-benzyloxy-4-trifluoromethylcoumarin O-debenzylase (BFCOD; CYP3A) and p-nitrophenol hydroxylase (PNPH; CYP2E1-like protein) was investigated in the presence of four concentrations of CLO and DEX. Clotrimazole in a concentration range of 1-100μM decreased the activity of EROD and BFCOD. The inhibition was reversible, as pre-incubation of the microsomes with CLO, before addition of the substrate, had no effect. EROD activity was non-competitively inhibited with a Ki of 0.5μM, and BFCOD activity revealed competitive inhibition with a Ki of 0.04μM. The relatively low Ki for CLO inhibition of EROD and BFCOD activity may indicate that the ability of CYP1A and CYP3A to metabolize xenobiotics is reduced in the presence of CLO. PNPH activity was not affected by CLO. DEX showed no inhibitory potency on any investigated reaction. CLO, but not DEX, inhibited EROD and BFCOD activity by different mechanisms.

Stabilita antikoagulace je důležitá pro účinnost a bezpečnost léčby warfarinem i novými antitrombotiky. Lékové interakce hrají důležitou roli při vzniku předávkování nebo nedostatečné účinnosti antitrombotické léčby. Cesta od požití warfarinu k jeho léčebnému efektu je velmi složitá a jeho biodegradace je zprostředkována různými mikrosomálními cytochromy P450, u warfarinu se tedy vyskytují četné lékové interakce. Nejdůležitější z nich jsou interakce s inhibitory a induktory cytochromu P450 2C9, ale uplatňuje se i řada dalších farmakologických mechanizmů. Nové přímé inhibitory trombinu (dabigatran) a inhibitory f. Xa (rivaroxaban a apixaban) mají předvídatelnou farmakokinetiku a jednoduchý mechanizmus účinku. Farmakokinetika těchto antitrombotik vede k nízkému riziku lékových interakcí. Inhibitory a induktory P-glykoproteinu ovlivňují hladinu dabigatranu, léky inhibující současně P-glykoprotein a CYP3A4 zvyšují expozici rivaroxabanu a apixabanu a induktory P-glykoproteinu a/nebo CYP3A4 snižují hladinu rivaroxabanu a apixabanu.
The stability of anticoagulation is essential for the efficacy and safety of warfarin therapy and treatment with new antithrombotic drugs. Drug interactions play a dominant role in the emergence of overanticoagulation or an insufficient effect of antithrombotic therapy. The pathway from warfarin ingestion to its therapeutic effect is very complicated and its biodegradation is mediated by different cytochrome enzymes, with warfarin thus being prone to numerous drug interactions. The most important of them are interactions with inhibitors and inducers of cytochrome 2C9, but numerous other pharmacologic mechanisms are also involved. The new direct thrombin inhibitors (dabigatran) and factor Xa inhibitors (rivaroxaban and apixaban) have predictable pharmacokinetics and a simple mechanism of action. The pharmacokinetics of these antithrombotic drugs leads to a low risk of drug interactions; they are limited to P-glycoprotein inhibitors and inducers, which influence the level of dabigatran, drugs inhibiting both P-glycoprotein and CyP 3A4, which increase rivaroxaban and apixaban exposure, and inducers of P-glycoprotein and/or CyP 3A4, which induce a reduction in rivaroxaban and apixaban levels.

Azoles antifungals are widespread used drugs for various medicinal indications. These drugs are well known for their numerous drug-drug interactions, which are believed to occur via inhibition of CYP3A4 enzymatic activity and consequently altering pharmacokinetic of co-administered drugs. In the current communication a complex view on the molecular interactions between azoles antimycotics and CYP3A4 is presented. Beside inhibition of CYP3A4 catalytic activity, azoles influence transcriptional activity of pregnane X receptor (PXR) and consequently expression of drug-metabolizing enzymes, including CYP3A4. Interactions between azoles and PXR occur by multiple mechanisms, including modulation of ligand-dependent activation of PXR (agonism, antagonism) or affecting recruitment of PXR co-activators SRC-1 (steroid receptor co-activator 1) and HNF4α (hepatocyte nuclear factor 4α). Miconazole and ketoconazole are antagonists of glucocorticoid receptor (GR), therefore these drugs inhibit GR-mediated expression of PXR and drug metabolizing cytochromes P450. In addition, PXR and GR are key regulators of intermediary metabolism (e.g. carbohydrate, lipids or bile acids homeostasis) and many other cellular functions (e.g. immune response), hence, the interactions between azoles and PXR/GR are of broader physiological importance. In conclusion, while inhibition of CYP3A4 enzymatic activity by azoles is considered as primary cause of azoles drug-drug interactions, the effects of azoles on PXR and GR should be taken in account. Apart from CYP3A4, azoles influence the expression and activity of others drug-metabolizing cytochromes P450.

Léčba fungálních onemocnění prošla dlouhodobým vývojem a vzhledem k narůstající četnosti systémových mykóz s vysokou mortalitou je hledání optimálních léčiv i farmakoterapeutických postupů stále aktuální. Ve srovnání s minulostí máme v současné době k dispozici léky výrazně účinnější s menším rizikem nežádoucích účinků. Významný vývoj ve smyslu omezení nežádoucích účinků a zvýšení selektivity k patogenu zaznamenala také skupina azolových antimykotik. Přesto farmakoterapii systémových mykóz nadále komplikuje řada lékových interakcí, které se mohou vyskytnout až u 95 % hospitalizovaných pacientů léčených kombinací azolových antimykotik s dalšími látkami. Nejdůležitějším a nejlépe prostudovaným mechanizmem lékových interakcí azolových antimykotik je inhibice biotransformačních enzymů cytochromu P450 (CYP). Ovšem v minulých letech byly navíc studovány a popsány i další mechanizmy lékových interakcí těchto látek na úrovni farmakokinetiky. Jedná se především o inhibici membránových transportních proteinů, zejména P-glykoproteinu (P-gp), a ovlivnění exprese genů pro biotransformační enzymy prostřednictvím pregnanového X receptoru (PXR), především CYP3A4. V tomto přehledovém článku se zaměříme na poslední dva zmíněné mechanizmy lékových interakcí azolových antimykotik, tedy ovlivnění transportních funkcí P-gp a modulace exprese enzymů I. i II. fáze biotransformace.
Altough the therapy of fungal infections has been developing for many years, searching of optimal drugs and pharmacotherapeutic approach is still desirable due to the increasing prevalence of systemic mycoses with high risk of mortality. In comparison with the past, currently we dispose of more efficacious drugs with lower adverse effects. Significant progress has been done in terms of reduction of adverse effects of azole antifungals and heightening of their specificity for pathogen. Nevertheless drug-drug interactions, which may occur in 95 % in patients treated with combination of azole antimycotics and other drugs, still complicate the pharmacotherapy of systemic mycoses. The most important and best known mechanism of drug-drug interactions caused by azole antimycotics is the inhibition of metabolizing enzymes of cytochrome P450 (CYP) family. Moreover during several years additional mechanisms of pharmacokinetic drug-drug interactions have been studied and described for the drugs such as inhibition of membrane transport protein, namely P-glycoprotein (P-gp), and regulation of expression of metabolizing enzymes, especially CYP3A4 via pregnane X receptor (PXR). In present review we focus on the last two mentioned mechanisms of drug-drug interactions caused by azole antimycotics, affecting the transport functions of P-gp and modulation of expression of enzymes involved in I. and II. phases of biotransformation.

Olomoucine II is a cyclin-dependent kinase inhibitor and a potential antineoplastic agent because it can arrest animal cell cycles. This study examines its interactions with human liver microsomal cytochrome P450 (P450) enzymes. Spectroscopic and high-performance liquid chromatography (HPLC) methods were used to estimate the degree of olomoucine II-mediated inhibition of enzymatic activities of eight drug-metabolizing P450s in vitro. In addition, mass spectrometry coupled with HPLC was used to identify an olomoucine II metabolite (2,5-dihydroxyroscovitine) formed in the reaction mixtures, and CYP3A4 was found to be responsible for the hydroxylation of the N(6)-benzyl ring at position 5, leading to this compound. Olomoucine II significantly inhibited the enzymatic activities of CYP1A2, CYP2C9, and (to a lesser degree) CYP3A4. The results indicate that use of olomoucine II as a drug could affect the activities of CYP3A4, CYP1A2, and CYP2C9 in vivo. Hence, the clinical relevance of these interactions should be carefully evaluated.
