Terapeutické monitorování léčiv je specifická metoda oboru klinické farmakologie, která napomáhá monitorovat farmakoterapii celé řady onemocnění pomocí stanovení koncentrace léčiva v séru následované erudovanou interpretací. Jedná se o léčiva, jejichž klinický účinek je mnohem bližší dosažené koncentraci v organismu než podávané dávce. Cílem článku je prezentovat informace týkající se využití metody terapeutického monitorování léčiv v rutinní klinické praxi.
Therapeutic drug monitoring is a specific method of clinical pharmacology for monitoring of pharmacotherapy using measurement of drug serum concentrations followed by erudite interpretation. The concept of the method rests on the assumption that clinical effect correlates better with drug concentration than with the dose. We would like to present informations relating to utilization of therapeutic drug monitoring method in routine clinical practice.

BACKGROUND: The aim of the study was to evaluate the bioequivalence of two itraconazole 100 mg capsule formulations. RESEARCH DESIGN AND METHODS: The single-center, open-label, randomized, three-period, three-sequence, reference-replicated, cross-over study included 38 healthy subjects under fed conditions. In each study period (separated by a 14-day washout), a single oral dose of the test (T) or reference (R) product was administered. Blood samples were collected at pre-dose and up to 72.0 h after administration. The calculated pharmacokinetic parameters, based on the plasma concentrations of itraconazole and hydroxy itraconazole, were AUC0-72h, AUC0-∝, Cmax, Tmax, T1/2 and Kel. RESULTS: The 90% CI for the test/reference geometric means ratio for the parent compound, itraconazole, was in the range from 85.29% to 116.07% for AUC0-72h. Since the coefficient of variation (CV) for the reference product was 44.95% for Cmax, the 90% CI for this parameter for itraconazole was 93.49-133.78%, which was within the proposed limits of the EMA for bioequivalence of 72.15-138.59%. During the study, 4 subjects encountered a total of 14 mild adverse events. CONCLUSIONS: The use of the reference-scaling approach with 3-period design (TRR, RTR, and RRT) was an efficient way to demonstrate that two commercially available oral itraconazole formulations met the predetermined bioequivalence criteria.

BACKGROUND: Prevalence of oral mucosal fungal infections increases with the frequent administration of antibiotics, corticosteroids and immunosuppressive drugs. Therapeutically used antifungals are usually associated with a variety of drug interactions. Furthermore, there has been a noticeable increase in microorganisms resistant to these preparations. Mucoadhesive buccal films represent a modern therapeutic system for the treatment of oral mucosal fungal infection paired with a high degree of patient compliance. Ciclopirox olamine applied directly onto the oral mucosa offers an attractive alternative to treatment with systemic antifungals thanks to its low incidence of resistance and side effects. OBJECTIVE: The aim of this work was to evaluate the pharmacokinetic parameters of ciclopirox olamine after the buccal application of mucoadhesive film prepared by the solvent casting method. METHOD: A chromatographic method using an internal standard was developed and validated for evaluation of ciclopirox olamine plasma concentrations. Method accuracy was 88.5-104.6% and 89.5-99.7% for interday and intraday assays, respectively. RESULTS: The pharmacokinetic properties of ciclopirox olamine were studied in New Zealand White rabbits. The mucoadhesive films containing ciclopirox olamine in a total dose of 34.4 (33.0; 35.9) mg kg-1 were applied to all the rabbits. Plasma ciclopirox olamine concentrations were determined during the 12 h following application. The time taken to reach maximum plasma concentration was 1.7 (1.1; 2.2) h after the drug administration with cmax 5.73 (4.18; 7.28) μg mL-1. Overall elimination half-life was 3.8 (1.9; 10.8) h. CONCLUSION: The experiment suggests that oral mucoadhesive film may be a valuable alternative ciclopirox olamine administration.

BACKGROUND: Data on safety and efficacy of voriconazole for invasive aspergillosis (IA) and invasive candidiasis/esophageal candidiasis (IC/EC) in pediatric patients are limited. METHODS: Patients aged 2-<18 years with IA and IC/EC were enrolled in 2 prospective open-label, non-comparative studies of voriconazole. Patients followed dosing regimens based on age, weight and indication, with adjustments permitted. Treatment duration was 6-12 weeks for IA patients, ≥14 days after last positive Candida culture for IC patients and ≥7 days after signs/symptoms resolution for EC patients. Primary analysis for both the studies was safety and tolerability of voriconazole. Secondary end points included global response success at week 6 and end of treatment (EOT), all-causality mortality and time to death. Voriconazole exposure-response relationship was explored. RESULTS: Of 53 voriconazole-treated pediatric patients (31 IA; 22 IC/EC), 14 had proven/probable IA, 7 had confirmed IC and 10 had confirmed EC. Treatment-related hepatic and visual adverse events, respectively, were reported in 22.6% and 16.1% of IA patients, and 22.7% and 27.3% of IC/EC patients. All-causality mortality in IA patients was 14.3% at week 6; no deaths were attributed to voriconazole. No deaths were reported for IC/EC patients. Global response success rate was 64.3% (week 6 and EOT) in IA patients and 76.5% (EOT) in IC/EC patients. There was no association between voriconazole exposure and efficacy; however, a slight positive association between voriconazole exposure and hepatic adverse events was established. CONCLUSIONS: Safety and efficacy outcomes in pediatric patients with IA and IC/EC were consistent with previous findings in adult patients.

V současné době je pro klinickou praxi dostupná nová léková forma posaconazolu, širokospektrého triazolového antimykotika – enterosolventní tablety. Na rozdíl od p. o. suspenze je možné dávkování jednou denně s dosažením vyšších a stabilnějších plazmatických koncentrací. Kvalita resorpce není ovlivněna dietou ani pH. Hlavní indikace posaconazolu je v profylaxi u pacientů s hematologickým onemocněním, ve 2. linii invazivní aspergilózy a zygomykózy žaludku, tolerance je velmi dobrá. Krátký přehled uvádí jeho hlavní vlastnost a léčebné použití.
Clinical practice has recently been enriched with a new dosage form of posaconazole, a broad-spectrum triazole antifungal – enterosolvent tablets. Unlike p.o. suspension can be dosed once daily with achieving higher and more stable plasma concentrations. The quality of the resorption is not affected by diet or pH. The main indications od posaconazole is prophylaxis in patients with haematological disease, the second line of invasive aspergillosis and zygomycosis of stomach, tolerance is very good. This brief summary provides its main properties and therapeutic use.

Stručný přehled seznamuje s novinkami v oblasti léčby a diagnostiky invazivních mykotických onemocnění. Do klinické praxe byl uveden isavuconazol – nové triazolové antimykotikum, které bylo registrováno v roce 2015 v USA i v Evropě pro léčbu aspergilózy a mukormykózy, recentní studie 2. fáze ukazuje účinnost i v léčbě kandidózy jícnu. V České republice byla do klinické praxe uvedena nová léková forma posaconazolu – enterosolventní tableta s významně lepšími farmakokinetickými vlastnostmi. Léčba invazivní aspergilózy kombinací voriconazolu s anidulafunginem statisticky významně nepředčila monote-rapii voriconazolem, i když u vybrané populace pacientů byl zjevný příznivý trend v ovlivnění mortality. Voriconazol v monoterapii zůstává nadále léčbou první volby u pacientů s invazivní aspergilózou. Text dále uvádí příspěvek k úspěšné časné deeskalaci léčby kandidózy echinokandiny u nemocných na jednotkách intenzivní péče a dále je krátce zmíněn význam nepřímé diagnostiky, tj. stanovení biomarkerů mykotické infekce, pro včasnou diagnózu, monitorování a predikci léčebné odpovědi a mortality, ale také pro racionální zahájení i ukončení léčby. V závěru pak článek informuje o aktivitách Evropské asociace pro výzkum a léčbu rakoviny (European Organization for Research and Treatment of Cancer, EORTC) a leukemické skupiny ECIL (European Confe-rence on Infections in Acute Leukaemia) v oblasti úpravy diagnostických kritérií invazivních mykóz a o novelizovaných doporučeních pro diagnostiku a léčbu invazivních mykotických onemocnění.
This is a brief overview of novel approaches to the treatment and diagnosis of invasive fungal diseases. Isavuconazole – a new triazole antifungal agent registered in 2015 for the therapy of aspergillosis and mucormycosis in the US and Europe – has been introduced to clinical practice; recent phase 2 trial also shows its effectiveness in the therapy of esophageal candidiasis. A novel form of posaconazole was introduced in the Czech Republic – an enterosolvent tablet with much better pharmacokinetic qualities. The treatment of invasive aspergillosis using combination of voriconazole and anidulafungin was not statistically better that voriconazole monotherapy; however, it was associated with a trend towards decreased mortality in a selected patient population. Voriconazole monotherapy remains the treatment of choice in patients with invasive aspergillosis. The text also mentions successful early de escalation of candidiasis treatment with echinocandins in intensive care unit patients. It deals briefly, too, with the significance of indirect diagnostic instruments, i.e. with biomarkers of mycotic infections, for the early diagnosis, monitoring, and prediction of therapeutic response and mortality but also for rational therapy initiation and termination. In the end, this article informs about the editing activities of the European Organization for Research and Treatment of Cancer (EORTC) and European Conference on Infections in Acute Leukaemia (ECIL) concerning the diagnostic criteria of invasive fungal infections and about the revised recommendations for the evaluation and treatment of invasive fungal diseases.

Cílem tohoto článku je seznámit čtenáře s možnostmi a úskalími použití terapeutického monitorování léčiv (therapeutic drug monitoring, TDM) u některých skupin léčiv na základě zkušenosti podpořené uvedenými kazuistikami. Důraz je kladen na multidisciplinární spolupráci a velmi komplexní interpretaci výsledků měření koncentrace látek v daném biologickém vzorku. TDM je nástrojem racionální terapie, která by měla vést nejen ke zlepšení stavu, ale i k zabránění vzniku nežádoucích následků terapeutické intervence, nebo alespoň k jejich zmírnění.
The purpose of this article is to introduce to readers the possibilities, and challenges while practicing therapeutic drug monitoring (TDM) in some drug groups. The core of the article is based on personal experience supported by a series of cose presentations. Emphasis is placed on interdisciplinary work and very complex interpretation of the results of measuring the concentration of substances in the given biological specimen. TDM is a tool for rational pharmacotherapy, which should lead to an improvement in the patient's health while avoiding or at lest minimizing the adverse effects of therapeutic interventions.

Monensin A is a commercially important natural product isolated from Streptomyces cinnamonensins that is primarily employed to treat coccidiosis. Monensin A selectively complexes and transports sodium cations across lipid membranes and displays a variety of biological properties. In this study, we evaluated the Jacobsen catalyst as a cytochrome P450 biomimetic model to investigate the oxidation of monensin A. Mass spectrometry analysis of the products from these model systems revealed the formation of two products: 3-O-demethyl monensin A and 12-hydroxy monensin A, which are the same ones found in in vivo models. Monensin A and products obtained in biomimetic model were tested in a mitochondrial toxicity model assessment and an antimicrobial bioassay against Staphylococcus aureus, S. aureus methicillin-resistant, Staphylococcus epidermidis, Pseudomonas aeruginosa, and Escherichia coli. Our results demonstrated the toxicological effects of monensin A in isolated rat liver mitochondria but not its products, showing that the metabolism of monensin A is a detoxification metabolism. In addition, the antimicrobial bioassay showed that monensin A and its products possessed activity against Gram-positive microorganisms but not for Gram-negative microorganisms. The results revealed the potential of application of this biomimetic chemical model in the synthesis of drug metabolites, providing metabolites for biological tests and other purposes.
