Kontext: Kvasinka rodu Candida je vzácnou, ale významnou příčinou infekční endokarditidy. Cílem studie bylo zjistit klinické údaje pacientů s kandidovou endokarditidou léčených na kardiologickém a kardiochirurgickém oddělení jedné nemocnice v letech 2012 až 2023. Metody: Dokumentace pacientů s diagnózou infekční endokarditidy byla prohlédnuta retrospektivně. Sledovaly se demografické údaje, anamnéza, klinické údaje a informace o výsledném stavu a léčbě. Výsledky: Čtyři (29 %) ze 14 pacientů byly ženy, v 10 (71 %) případech se jednalo o muže. Průměrný věk byl 49,7 (± 8,82) roku. Průměrná délka hospitalizace byla 39 dní (33–130 dní). Postiženo bylo 5 (38 %) aortálních chlopní, 2 (15 %) aortální + mitrální chlopně, 3 (23 %) mitrální chlopně, ve 2 (16 %) případech byla příčinou vegetace na elektrodě a v 1 (7 %) případě vegetace na plicní chlopni. Ze základních onemocnění trpělo 5 (35 %) pacientů diabetes mellitus, 5 (35 %) bylo na hemodialýze a 4 (30 %) měli kardiostimulátor. Jeden (7 %) pacient prodělal transplantaci srdce, další transplantaci jater. Sedmi (50 %) pacientům byla implantována umělá chlopeň a 7 (50 %) biologická chlopeň. Mikrobiologické vyšetření prokázalo v 8 (57 %) přípa- dech přítomnost C. parapsilosis, v 5 (35 %) C. albicans a v 1 (7 %) případě C. tropicalis. V 7 (50 %) případech byla provedena výměna chlopně. Ve vzorku odebraném z chlopně jednoho pacienta prokázala kultivace přítomnost C. albicans. Fluconazol byl podáván 6 (43 %) pacientům a echinokandiny v 8 (57 %) případech. V průběhu jednoletého sledování zemřelo 6 (43 %) pacientů. Vyšetření magnetickou rezonancí prokázalo embolii v centrálním nervovém systému u 6 (43 %) pacientů. Závěry: Kandidová endokarditida je spojena s vysokou mortalitou, přitom nejvhodnější způsob léčby lze vzhledem k jejímu vzácnému výskytu velmi obtížně formulovat.
Background: Candida is a rare but important cause of infective endocarditis. The study aimed to describe clinical data on patients with Candida endocarditis treated in cardiology and cardiovascular surgery hospital between 2012-2023. Methods: Patients diagnosed with infective endocarditis were reviewed retrospectively. Demographic information, medical histories, clinical data, and information on outcome and treatment were presented. Results: Four (29%) of 14 patients were female, and 10 (71%) were male. The mean age was 49.7 (±8.82) years. The mean hospital stay was 39 days (33-130 days). The valves involved were 5 (38%) aortic valves, 2 (15%) aortic + mitral valves, 3 (23%) mitral valves, 2 (16%) vegetation on the lead, and 1 (7%) in the pulmonary valve. In the underlying diseases, 5 (35%) had diabetes mellitus, 5 (35%) had hemodialysis, and 4 (30%) had pacemakers. One (7%) of the patients had a heart transplant, and 1 had a liver transplant. Seven (50%) had a prosthetic valve, and 7 (50%) had a natural valve. The detected microbiological factors are: 8 (57%) had Candida parapsilosis (C. parapsilosis), 5 (35%) Candida albicans (C. albicans ) and 1 (7%) Candida tropicalis (C. tropicalis). There was a valve change in 7 (50%) of them. C. albicans grew in the valve culture of one patient. Fluconazole was used in 6 patients (43%), and echinocandins were used in 8 (57%) patients. Six patients (43%) died during 1-year follow-up. Embolism in the central nervous system was detected in 6 patients (43%) on magnetic resonance imaging (MRI). Conclusions: Candida endocarditis has a high mortality, and it is not easy to define the most appropriate treatment because of its rarity.
BACKGROUND: Congenital Heart Disease (CHD) predisposes to Infective Endocarditis (IE), but data about characterization and prognosis of IE in CHD patients is scarce. METHODS: The ESC-EORP-EURO-ENDO study is a prospective international study in IE patients (n = 3111). In this pre-specified analysis, adult CHD patients (n = 365, 11.7%) are described and compared with patients without CHD (n = 2746) in terms of baseline characteristics and mortality. RESULTS: CHD patients (73% men, age 44.8 ± 16.6 years) were younger and had fewer comorbidities. Of the CHD patients, 14% had a dental procedure before hospitalization versus 7% in non-CHD patients (p < 0.001) and more often had positive blood cultures for Streptococcus viridans (16.4% vs 8.8%, p < 0.001). As in non-CHD patients, IE most often affected the left-sided valves. For CHD patients, in-hospital mortality was 9.0% vs 18.1% in non-CHD patients (p < 0.001), and also, during the entire follow-up of 700 days, survival was more favorable (log-rank p < 0.0001), even after adjustment for age, gender and major comorbidities (Hazard Ratio (HR) 0.68; 95%CI 0.50-0.92). Within the CHD population, multivariable Cox regression revealed the following effects (HR and [95% CI]) on mortality: fistula (HR 6.97 [3.36-14.47]), cerebral embolus (HR 4.64 [2.08-10.35]), renal insufficiency (HR 3.44 [1.48-8.02]), Staphylococcus aureus as causative agent (HR 2.06 [1.11-3.81]) and failure to undertake surgery when indicated (HR 5.93 [3.15-11.18]). CONCLUSIONS: CHD patients with IE have a better outcome in terms of all-cause mortality. The observed high incidence of dental procedures prior to IE warrants further studies about the current use, need and efficacy of antibiotic prophylaxis in CHD patients.

Cíle: Zpracování souboru konsekutivních pacientů podstupujících extrakci implantabilního elektronického kardiologického přístroje z jakékoliv indikace. Posouzení základních demografických dat, úspěšnosti extrakce, počtu komplikací a mortality v čase, určení prediktorů mortality. Metodologie: Do souboru byli zařazeni prospektivně všichni pacienti, u kterých byla provedena extrakce přístroje či elektrod v letech 2017-2020. Základními sledovanými parametry byly demografické údaje, stáří přístroje, typ přístroje, metoda extrakce a jednoměsíční a jednoroční mortalita, zjišťovaná telefonickým průzkumem. Výsledky: Celkem bylo zařazeno 172 pacientů, 59,3 % indikací k extrakci bylo z důvodu infekce systému. Úspěšnost extrakce (plná klinická i radiologická) dosáhla 92 %, jednoměsíční mortalita 6,4 %, jednoroční 14,5 %. Průměrné stáří zavedených systémů bylo 7,4 roku. Závěr: Perkutánní extrakce elektrod je etablovanou metodou, dosahující v našich podmínkách bezpečnosti i účinnosti srovnatelné s velkými evropskými centry. Nutnou podmínkou bezpečného provádění extrakcí je urgentní dostupnost kardiochirurga a dostatečný počet výkonů, zajišťující adekvátní trénink personálu.
Aims: Objective of this study is to register data and trends in population of patients indicated for lead extraction or complete system removal. Basic demographic data, extraction success rate as well as complications, mortality, and its predictors were collected. Methodology: Prospectively collected consequent patients indicated to lead extraction or system removal from our centre were included. Data regarding demography, device age, device type, extraction method and one month and one year mortality were collected. Outcomes: 172 patients were enrolled between years 2017-2020. 59.3% patients were indicated because of cardiac system infection. Success rate (full clinical and radiological) reached 92%, one month mortality 6.4%, one year mortality 14.5%. Explanted systems were 7.4 years old in average. Conclusion: Percutaneous lead extraction is an established method, which is in our conditions safe and effective as in large European centres. Urgent cardiac surgery availability as well as adequate number of performed procedures are necessary safety precautions.

AIM: Understanding cardiac electronic device infective endocarditis epidemiology is essential for the management of this serious complication. Only monocentric and limited data have been published regarding patients in the Czech republic so far. The aim of this study was to describe the current profile, microbiology and clinical characteristics of this population. PATIENTS AND METHODS: National data from the prospective ESC-EORP EURO-ENDO registry were collected. 57 consecutive patients with a diagnosis of cardiac device-related infective endocarditis (CDRIE) from 11 Czech centres were included. RESULTS: Staphylococcus spp. was responsible for 43.9% of isolates, whereas Culture negative endocarditis was documented in 26.3% episodes. The most frequent complications under therapy were acute renal failure (17.5%), septic shock and heart failure (both 10.5%). Extraction of device was performed in 75.4% of all patients, and the 1-year mortality was 22.5%. CONCLUSIONS: The high proportion of culture-negative endocarditis is alarming and warrants further investigation. Cardiac device related infective endocarditis is a serious complication with a high 1-year mortality in a highly polymorbid spectrum of patients.

AIMS: Infective endocarditis (IE) is a life-threatening disease associated with high mortality and morbidity worldwide. We sought to determine how socioeconomic factors might influence its epidemiology, clinical presentation, investigation and management, and outcome, in a large international multicentre registry. METHODS AND RESULTS: The EurObservational Programme (EORP) of the European Society of Cardiology EURO-ENDO (European Infective Endocarditis) registry comprises a prospective cohort of 3113 adult patients admitted for IE in 156 hospitals in 40 countries between January 2016 and March 2018. Patients were separated in three groups, according to World Bank economic stratification [group 1: high income (75.6%); group 2: upper-middle income (15.4%); group 3: lower-middle income (9.1%)]. Group 3 patients were younger [median age (interquartile range, IQR): group 1, 66 (53-75) years; group 2, 57 (41-68) years; group 3, 33 (26-43) years; P < 0.001] with a higher frequency of smokers, intravenous drug use, and human immunodeficiency virus infection (all P < 0.001) and presented later [median (IQR) days since symptom onset: group 1, 12 (3-35); group 2, 19 (6-54); group 3, 31 (12-62); P < 0.001] with a higher likelihood of developing congestive heart failure (13.6%, 11.1%, and 22.6%, respectively; P < 0.001) and persistent fever (9.8%, 14.2%, and 27.9%, respectively; P < 0.001). Among 2157 (69.3%) patients with theoretical indication for cardiac surgery, surgery was performed less frequently in group 3 patients (75.5%, 76.8%, and 51.3%, respectively; P < 0.001), who also demonstrated the highest mortality (15.0%, 23.0%, and 23.7%, respectively; P < 0.001). CONCLUSION: Socioeconomic factors influence the clinical profile of patients presenting with IE across the world. Despite younger age, patients from the poorest countries presented with more frequent complications and higher mortality associated with delayed diagnosis and lower use of surgery.

BACKGROUND: Infective endocarditis (IE) in people who inject drugs (PWID) is an emergent public health problem. OBJECTIVES: The purpose of this study was to investigate IE in PWID and compare it with IE in non-PWID patients. METHODS: Two prospective cohort studies (ICE-PCS and ICE-Plus databases, encompassing 8,112 IE episodes from 2000 to 2006 and 2008 to 2012, with 64 and 34 sites and 28 and 18 countries, respectively). Outcomes were compared between PWID and non-PWID patients with IE. Logistic regression analyses were performed to investigate risk factors for 6-month mortality and relapses amongst PWID. RESULTS: A total of 7,616 patients (591 PWID and 7,025 non-PWID) were included. PWID patients were significantly younger (median 37.0 years [interquartile range: 29.5 to 44.2 years] vs. 63.3 years [interquartile range: 49.3 to 74.0 years]; p < 0.001), male (72.5% vs. 67.4%; p = 0.007), and presented lower rates of comorbidities except for human immunodeficiency virus, liver disease, and higher rates of prior IE. Amongst IE cases in PWID, 313 (53%) episodes involved left-side valves and 204 (34.5%) were purely left-sided IE. PWID presented a larger proportion of native IE (90.2% vs. 64.4%; p < 0.001), whereas prosthetic-IE and cardiovascular implantable electronic device-IE were more frequent in non-PWID (9.3% vs. 27.0% and 0.5% vs. 8.6%; both p < 0.001). Staphylococcus aureus caused 65.9% and 26.8% of cases in PWID and non-PWID, respectively (p < 0.001). PWID presented higher rates of systemic emboli (51.1% vs. 22.5%; p < 0.001) and persistent bacteremia (14.7% vs. 9.3%; p < 0.001). Cardiac surgery was less frequently performed (39.5% vs. 47.8%; p < 0.001), and in-hospital and 6-month mortality were lower in PWID (10.8% vs. 18.2% and 14.4% vs. 22.2%; both p < 0.001), whereas relapses were more frequent in PWID (9.5% vs. 2.8%; p < 0.001). Prior IE, left-sided IE, polymicrobial etiology, intracardiac complications, and stroke were risk factors for 6-month mortality, whereas cardiac surgery was associated with lower mortality in the PWID population. CONCLUSIONS: A notable proportion of cases in PWID involve left-sided valves, prosthetic valves, or are caused by microorganisms other than S. aureus.

Cíl a metodika: Charakter infekční endokarditidy (IE) se dlouhodobě postupně mění. Tato práce si klade za cíl zmapovat výskyt infekční endokarditidy v jižních Čechách v letech 2013 až 2019 a na základě nasbíraných dat tak posoudit aktuální obraz tohoto onemocnění. V netříděném retrospektivním souboru pacientů s diagnostikovanou IE v letech 2013 až 2019 byla sledována incidence v jednotlivých letech, charakter infekce, etiologické agens, terapeutický postup a mortalita. Zvlášť byly hodnoceny skupiny případů způsobených bakterií Staphylococcus aureus (STAU) a streptokoky. Výsledky: Celkem bylo ve sledovaném období zjištěno 171 případů IE s incidencí 3,4 případu na 100 000 obyvatel za jeden rok. V celém souboru bylo zachyceno 59 % případů IE nativní chlopně (NVE), 30 % případů IE protetické chlopně (PVE) a 11 % případů IE spojené se srdečním přístrojem (CDRIE). Nejčastějším etiologickým agens byl Staphylococcus aureus, který způsobil 38 % všech případů. Celkem 58 % pacientů s IE chlopenního aparátu podstoupilo kardiochirurgický výkon s hospitalizační mortalitou 28 % a celkovou mortalitou 35 %. Mortalita pacientů, u kterých bylo postupováno konzervativně, byla zprvu nižší (hospitalizační mortalita 21 %), celková mortalita v tomto souboru však dosáhla 58,5 %. Mortalita STAU IE byla vyšší ve všech sledovaných aspektech. Závěr: Výsledky analýzy potvrzují publikované trendy o stárnutí populace pacientů s IE, narůstajícím zastoupení PVE a vysokém podílu STAU na etiologii. Ve shodě s uveřejněnými daty zůstává mortalita tohoto onemocnění vysoká. © 2020, ČKS.
Purpose and methods: The characteristics of infective endocarditis (IE) have long been changing. The aim of the present study was to map the incidence of IE in south Bohemia in the 2013-2019 period and to review the current status of the disease. Using an unselected retrospective group of patients diagnosed with IE in the 2013-2019 period, we analyzed the incidence in individual years, characteristics of infection, pathogens therapeutic strategy and mortality. Subgroups of patients with IE caused by Staphylococcus aureus (STAU) bacteria and streptococci were assessed separately. Results: Overall, 171 cases of IE with an incidence of 3.4 cases per 100,000 inhabitants/year were identified. The whole group of patients included 59% of cases of native valve endocarditis (NVE), 30% of prosthetic valve endocarditis (PVE) and 11% of cardiac device-related infective endocarditis (CDRIE). The most frequent pathogen was Staphylococcus aureus accounting for 38% of all cases. A total of 58% of patients with IE involving the valve apparatus had cardiac surgery with in-hospital and all-cause mortality rates of 28% and 35%, respectively. While the mortality rate of patients receiving conservative therapy was initially lower (in-hospital mortality, 21%), all-cause mortality in this patient subgroup was as high as 58.5%. Mortality from STAU IE, both in-hospital and all-cause, was higher in all characteristics. Conclusion: Results of our analysis are consistent with published data on trends in aging of the population of IE patients, increasing proportion of PVE and a high proportion of STAU in terms of etiology. In line with published data, mortality from the disease remains high.

Kardiostimulátory, implantabilní kardiovertery-defibrilátory a přístroje pro srdeční resynchronizační terapii jsou potenciálně život zachraňující léčbou u řady srdečních onemocnění, ale nejsou bez rizika. Nejvíce znepokojující je riziko infekce srdečních implantabilních elektronických zařízení (CIED), která je spojena s významnou morbiditou, zvýšenou nutností hospitalizace, zkrácením přežití a zvýšenými náklady na zdravotní péči. Doporučené preventivní postupy, jako je podávání intravenózních antibiotik před implantací, jsou dobře známy. Zůstávají nejasnosti ohledně úlohy různých preventivních, diagnostických a léčebných opatření, jako jsou kožní antiseptika, aplikace antibiotik do kapsy přístroje, antibakteriální obálky, prodloužené podávání antibiotik po implantaci a další. Doporučení týkající se použití nových alternativ implantabilních zařízení, u nichž se očekává, že budou méně náchylné k infekcím, a nových perorálních antikoagulancií jsou rovněž omezená, stejně jako definice minimálních požadavků na kvalitu péče implantačních center, erudice operatérů a počtu výkonů. Navíc chybí dokument o mezinárodním konsenzu ohledně léčby infekcí CIED. Rozpoznání těchto témat, šíření výsledků důležitých randomizovaných studií zaměřených na prevenci infekce CIED a pozorované rozdíly v řešení infekcí souvisejících s implantabilními přístroji, které byly zjištěny v celosvětovém průzkumu European Heart Rhythm Association, poskytlo silnou motivaci k vytvoření mezinárodního konsenzuálního dokumentu s použitím nejmodernějších poznatků vědy z roku 2019 týkajících se posuzování rizik, prevence, diagnostiky a léčby infekcí CIED.
Pacemakers, implantable cardiac defibrillators, and cardiac resynchronization therapy devices are potentially life- -saving treatments for a number of cardiac conditions, but are not without risk. Most concerning is the risk of a cardiac implantable electronic device (CIED) infection, which is associated with significant morbidity, increased hospitalizations, reduced survival, and increased healthcare costs. Recommended preventive strategies such as administration of intravenous antibiotics before implantation are well recognized. Uncertainties have remained about the role of various preventive, diagnostic, and treatment measures such as skin antiseptics, pocket antibiotic solutions, anti-bacterial envelopes, prolonged antibiotics post-implantation, and others. Guidance on whether to use novel device alternatives expected to be less prone to infections and novel oral anticoagulants is also limited, as are defi -nitions on minimum quality requirements for centres and operators and volumes. Moreover, an international con-sensus document on management of CIED infections is lacking. The recognition of these issues, the dissemination of results from important randomized trials focusing on prevention of CIED infections, and observed divergences in managing device-related infections as found in an European Heart Rhythm Association worldwide survey, provided a strong incentive for a 2019 International State-of-the-art Consensus document on risk assessment, prevention, diagnosis, and treatment of CIED infections.
