BACKGROUND: Strokes after left atrial appendage closure (LAAC) prophylaxis are generally less severe than those after warfarin prophylaxis-thought to be secondary to more hemorrhagic strokes with warfarin. Hemorrhagic strokes are similarly infrequent with direct oral anticoagulant (DOAC) prophylaxis, so the primary subtype after either LAAC or DOAC prophylaxis is ischemic stroke (IS). OBJECTIVES: The purpose of this study was to compare the severity of IS using the modified Rankin Scale in atrial fibrillation patients receiving prophylaxis with DOACs vs LAAC. METHODS: A retrospective analysis was performed of consecutive patients undergoing LAAC at 8 centers who developed an IS (ISLAAC) compared with contemporaneous consecutive patients who developed IS during treatment with DOACs (ISDOAC). The primary outcome was disabling/fatal stroke (modified Rankin Scale 3-5) at discharge and 3 months later. RESULTS: Compared with ISDOAC patients (n = 322), ISLAAC patients (n = 125) were older (age 77.2 ± 13.4 years vs 73.1 ± 11.9 years; P = 0.002), with higher HAS-BLED scores (3.0 vs 2.0; P = 0.004) and more frequent prior bleeding events (54.4% vs 23.6%; P < 0.001), but similar CHA2DS2-VASc scores (5.0 vs 5.0; P = 0.28). Strokes were less frequently disabling/fatal with ISLAAC than ISDOAC at both hospital discharge (38.3% vs 70.3%; P < 0.001) and 3 months later (33.3% vs 56.2%; P < 0.001). Differences in stroke severity persisted after propensity score matching. By multivariate regression analysis, ISLAAC was independently associated with fewer disabling/fatal strokes at discharge (OR: 0.22; 95% CI: 0.13-0.39; P < 0.001) and 3 months (OR: 0.25; 95% CI: 0.12-0.50; P < 0.001), and fewer deaths at 3 months (OR: 0.28; 95% CI: 0.12-0.64; P < 0.001). CONCLUSIONS: Ischemic strokes in patients with atrial fibrillation are less often disabling or fatal with LAAC than DOAC prophylaxis.

RATIONALE: Oxygen is essential for cellular energy metabolism. Neurons are particularly vulnerable to hypoxia. Increasing oxygen supply shortly after stroke onset could preserve the ischemic penumbra until revascularization occurs. AIMS: PROOF investigates the use of normobaric oxygen (NBO) therapy within 6 h of symptom onset/notice for brain-protective bridging until endovascular revascularization of acute intracranial anterior-circulation occlusion. METHODS AND DESIGN: Randomized (1:1), standard treatment-controlled, open-label, blinded endpoint, multicenter adaptive phase IIb trial. STUDY OUTCOMES: Primary outcome is ischemic core growth (mL) from baseline to 24 h (intention-to-treat analysis). Secondary efficacy outcomes include change in NIHSS from baseline to 24 h, mRS at 90 days, cognitive and emotional function, and quality of life. Safety outcomes include mortality, intracranial hemorrhage, and respiratory failure. Exploratory analyses of imaging and blood biomarkers will be conducted. SAMPLE SIZE: Using an adaptive design with interim analysis at 80 patients per arm, up to 456 participants (228 per arm) would be needed for 80% power (one-sided alpha 0.05) to detect a mean reduction of ischemic core growth by 6.68 mL, assuming 21.4 mL standard deviation. DISCUSSION: By enrolling endovascular thrombectomy candidates in an early time window, the trial replicates insights from preclinical studies in which NBO showed beneficial effects, namely early initiation of near 100% inspired oxygen during short temporary ischemia. Primary outcome assessment at 24 h on follow-up imaging reduces variability due to withdrawal of care and early clinical confounders such as delayed extubation and aspiration pneumonia. Geriatric syndromes involving instability are associated with a higher risk of further complications in patients with trauma requiring subsequent surgery. Acute stroke symptoms require prompt recognition and initiation of reperfusion treatment. This case study describes the patient's stroke symptoms in a timeline and explores the benefit of objective assessment using screening tests and their importance in the differential diagnosis in nursing. Dysphagia is one of the stroke symptoms that can lead to aspiration pneumonia and increase the risk of mortality. This article aims to inform general nurses about the importance of early recognition of dysphagia and other stroke symptoms using adequate screening tests to ensure quality care. Geriatric syndromes encompass instability, resulting in a higher risk of complications, especially in trauma patients. This case study describes a patient with acute ischemic stroke with vague and unrecognized symptoms of dysphagia and explores the rationale for objective screening tests in nursing care.
A 58-year-old patient presented with a severe neurological deficit due to a stroke caused by an occlusion of the left internal carotid artery siphon. Standard treatment failed and neurosurgical consult was delayed. Because of a favorable perfusion imaging finding, microsurgical revascularization via an extra-intracranial bypass (left superficial temporal artery - left middle cerebral artery) was performed 36 hours after the onset of the symptoms. The outcome of the patient was favorable. The authors want to emphasize the need to actively seek patients with a severe neurological deficit and still viable brain tissue. The time window and treatment alternatives are discussed.

Cíl: Analyzovat a syntetizovat výsledky kvalitativních studií týkajících se životních zkušeností mladých pacientů po prodělané ischemické cévní mozkové příhodě (iCMP). Design: Syntéza kvalitativních studií. Byl dodržen doporučený postup dle ENTREQ pro zvýšení transparentnosti při předložení syntézy kvalitativních výzkumů. Metody: Pro vyhledávání kvalitativních studií bylo využito elektronických vědeckých databází CINAHL Plus (EBSCO), MEDLINE (OvidSP), PsycInfo (EBSCO), ScienceDirect (Elsevier), SocINDEX (EBSCO), Web of Science Core Collection a ProQuest, ve kterých bylo dohledáno 5 785 zdrojů. Pro metodologické posouzení kvality studií bylo využito CASP (Critical Appraisal Skills Programme Qualitative Research Checklist). Celkem bylo do literárního review zařazeno 7 kvalitativních studií zaměřených na životní zkušenosti mladých dospělých po iCMP. Výsledky: Cévní mozková příhoda v mladém věku způsobuje narušení ve třech oblastech: oslabení sebe sama a vlastní identity, sociální narušení a oslabení blízkých osobních vztahů. Na druhou stranu byla nalezena tři analytická témata související s copingovými strategiemi nebo osobními změnami: znovunalezení nového já, znovunalezení sociální role a pomoc ve vztahu. Závěr: Dohledaná zjištění našeho review mohou přinést hlubší porozumění zkušenostem mladých dospělých po cévní mozkové příhodě a současně jejich věkově přiměřeným potřebám v dlouhodobém rehabilitačním procesu.
Aim: To report a synthesis of evidence presented in qualitative studies related to the experience of living with ischemic stroke in young patients. Design: A research synthesis of qualitative studies. The ENTREQ statement was followed to enhance transparency in reporting the synthesis of qualitative research. Methods: The electronic databases CINAHL Plus (EBSCO), MEDLINE (OvidSP), PsycInfo (EBSCO), ScienceDirect (Elsevier), SocINDEX (EBSCO), Web of Science Core Collection and ProQuest Science Database were used to gather data for a review of qualitative studies, yielding 5,785 sources. The methodological quality of studies was reviewed using the Critical Appraisal Skills Programme Qualitative Research Checklist. In total, seven sources were included in this literature review, focused on young adults' experience of ischemic stroke. Results: Stroke at a young age can cause a disruption of three domains: disruption of self and self-identity, social disruption, and disruption of close personal relationships. On the other hand, three analytical themes related to coping strategies or personal challenges have been found: re-establishing a new self, restablishing a social role, and relationship help. Conclusion: The findings of our review can provide a deeper insight into understanding the experience of a stroke at a younger age and their age-appropriate needs in the long-term rehabilitation process.
During the last 5-7 years, tremendous progress was achieved in the reperfusion treatment of acute ischaemic stroke during its first few hours from symptom onset. This review summarizes the latest evidence from randomized clinical trials and prospective registries with a focus on endovascular treatment using stent retrievers, aspiration catheters, thrombolytics, and (in selected patients) carotid stenting. Novel approaches in prehospital (mobile interventional stroke teams) and early hospital (direct transfer to angiography) management are described, and future perspectives ('all-in-one' laboratories with angiography and computed tomography integrated) are discussed. There is reasonable chance for patients with moderate-to-severe acute ischaemic stroke to survive without permanent sequelae when the large-vessel occlusion is removed by means of modern pharmaco-mechanic approach. Catheter thrombectomy is now the golden standard of acute stroke treatment. The role of cardiologists in stroke is expanding from diagnostic help (to reveal the cause of stroke) to acute therapy in those regions where such up-to-date Class I. A treatment is not yet available.

INTRODUCTION: Despite improvements in device technology, only one-third of stroke patients undergoing endovascular thrombectomy (EVT) achieve first-pass effect (FPE). We investigated the effect of arterial tortuosity and thrombus characteristics on the relationship between first-line EVT strategy and angiographic outcomes. PATIENTS AND METHODS: Patients with thin-slice baseline CT-angiography from the ESCAPE-NA1 trial (Efficacy and safety of nerinetide for the treatment of acute ischemic stroke) were included. Tortuosity was estimated using the tortuosity index extracted from catheter pathway, and radiological thrombus characteristics were length, non-contrast density, perviousness and hyperdense artery sign. We assessed the association of first-line EVT strategy (stent-retriever [SR] versus contact aspiration [CA] versus combined SR+CA) with FPE (eTICI score 2c/3 after one pass), final eTICI 2b/3, number of passes and procedure duration using multivariable regression. Interaction of tortuosity and thrombus characteristics with first-line technique were assessed using interaction terms. RESULTS: Among 520 included patients, SR as a first-line modality was used in 165 (31.7%) patients, CA in 132 (25.4%), and combined SR+CA in 223 (42.9%). FPE was observed in 166 patients (31.9%). First-line strategy was not associated with FPE. Tortuosity had a significant effect on FPE only in the CA group (aOR = 0.90 [95% CI 0.83-0.98]) compared with stent-retrievers and combined first-line approach (p interaction = 0.03). There was an interaction between thrombus length and first-line strategy for number of passes (p interaction = 0.04). Longer thrombi were associated with higher number of passes only in the CA group (acOR 1.03 [95% CI 1.00-1.06]). CONCLUSION: Our study suggests that vessel tortuosity and longer thrombi may negatively affect the performance of first-line contact aspiration catheters in acute stroke patients undergoing EVT.

Cíl: Cílem této studie bylo určit souvislost mezi hormony produkovanými hypotalamo-hypofyzárně-tyroidní (tyroidní) osou (thyroid axis produced hormones; TAPH) a kognitivním stavem po prodělané akutni ischemické CMP (acute ischemic stroke; AIS). Materiál a metody: U jedinců s AIS z tří různých výzkumných a klinických center byla stanovena hladina TAPH v séru, vč. tyreostimulačního hormonu, volného tyroxinu a volného trijodtyroninu (FT3), a to po přijetí pacienta a před jeho propuštěním. Kognitivní funkce byly hodnoceny během akutní ubakutní fáze AIS pomocí Mini-Mental State Examination (MMSE). Výsledky: Byla získána data týkající se 194 jedinců s AIS v akutní fázi a 89 jedinců s AIS v subakutní fázi. Během akutní fáze AIS byla nezávislou determinantou kognitivního stavu hladina FT3 (R2 = 0,016; p = 0,017). Během subakutní fáze AIS nebyly zjištěny žádné nezávislé asociace mezi změřenými hladinami hormonů v séru a MMSE. Závěr: Hladina FT3 v séru změřená při přijetí pacienta s AIS v akutní fázi může predikovat jeho kognitivní stav hodnocený pomocí MMSE.
Aim: The aim of this study was to determine association between hypothalamic-hypophysis-thyroid axis (thyroid axis) produced hormones (TAPH) and cognitive performance after experienced acute ischemic stroke (AIS). Materials and methods: Individuals with AIS from three different research and clinical centers were evaluated for serum TAPH levels, including thyroid stimulating hormone, free thyroxin, and free triiodothyronine (FT3) during admission and before discharge. Cognitive outcomes were evaluated using the Mini-Mental State Examination (MMSE) test during an acute and a subacute AIS periods. Results: Data were available for 194 and 89 individuals during an acute and a subacute AIS periods, respectively. FT3 (R2 = 0.016; P = 0.017) was an independent determinant of cognitive performance during an acute AIS period. No independent associations were established between measured hormone serum levels and MMSE estimate during subacute AIS period. Conclusion: FT3 serum levels on admission could predict cognitive performance assessed by the MMSE during an acute AIS period.

Stroke is the most commonly identified cause of epilepsy in adults older than 35 years and accounts for more than 50% of all new cases of epilepsy in the elderly population. Stroke patients have higher risk of developing epilepsy (up to 10%) and those patients who develop it have significantly higher disability and are less likely to be independent in activities of daily living. Despite increasing knowledge about processes that underlie epileptogenesis no effective antiepiletogenic therapy is available for clinical use. Lack of reliable biomarker limits further development and verification of new therapeutic strategies. We assume that severity of blood-brain barrier (BBB) dysfunction and neuroinflammation induced by vascular stroke correlate with occurrence of epileptiform phenomena and epileptogenesis. The aim of this project is to verify MR imaging of BBB dysfunction, cytokines levels and high density EEG as biomarkers of postichemic epileptogenesis.
Cévní mozková příhoda (CMP) je nejčastější příčinou epilepsie u dospělých starších 35let a je zodpovědná za více než 50% nových případů epilepsie ve starší populaci. Pacienti po prodělané CMP mají větší riziko rozvoje epilepsie (až 10%) a pacienti, u kterých se epilepsie rozvine, mají signifikantně větší postižení a při běžných denních aktivitách jsou závislý na pomoci druhých osob. I přes významný rozvoj znalostí o dějích, které provázejí epileptogenezu, není k dispozici žádná účinná antiepileptogenní terapie vhodná pro klinické použití. Limitujícím faktorem pro další rozvoj a verifikaci léčebných postupů je nedostupnost vhodného biomarkeru. Předpokládáme, že rozsah poškození hematoencefalické bariéry (BBB) a zánětu vzniklých po ischemické CMP korelují s výskytem epileptiformních fenoménů a epileptogenezou. Cílem tohoto projektu je ověřit, že MR zobrazení poruchy BBB, hladiny vybraných cytokinů a high density EEG jsou vhodné biomarkery epileptogeneze.
