Močové infekce spojené se zavedeným permanentním katétrem (CAUTI – catheter-associated urinary tract infections) patří mezi nejčastější infekce spojené s poskytovanou zdravotní péčí, které zvyšují morbiditu, mortalitu, prodlužují délku hospitalizace a mají nezanedbatelný vliv na cenu léčby. Nejvýznamnějším preventivním opatřením je zkrácení doby zavedení permanentního katétru (PMK) na nezbytnou dobu a zamezení zbytečného zavedení PMK. Léčba asymptomatické bakteriurie není doporučována. V případech závažné CAUTI je třeba zahájit razantní antibiotickou léčbu pokrývající multirezistentní uropatogeny. Tato doporučení jsou určena pro všechny medicínské odbornosti s cílem zlepšit péči o pacienty s permanentním katétrem v oblasti prevence, diagnostiky a léčby CAUTI v primární péči a následné dlouhodobé péči.
Catheter-associated urinary tract infections (CAUTI) are among the most common healthcare-associated infections, which increase morbidity, mortality, prolong the length of hospitalization and have a significant impact on the cost of treatment. The most efficient preventive method is removing catheters as soon as possible and avoid unnecessary catheterizations. Treatment of asymptomatic bacteriuria is not recommended. In cases of serious CAUTI, vigorous antibiotic therapy covering multidrug-resistant uropathogens should be initiated. These recommendations are intended for all medical specialties to improve the care of patients with indwelling catheters in the prevention, diagnosis, and treatment of CAUTI in primary care and subsequent long-term care.
Východiska: Infekce močových cest (Catheter-associated urinary tract infection - CAUTI) tvoří více než třetinu infekcí spojených se zdravotní péčí. Vedení dokumentace o katetrizaci močového měchýře je jedním z kroků prevence zmiňovaných infekcí, jednoznačně je doporučena elektronická podoba dokumentace v klinickém informačním systému.Cílem: Šetřením bylo mimo jiné zmapovat, jak je v nemocnicích poskytujících akutní lůžkovou péči v ČR vedena dokumentace o močové katetrizaci, jaké údaje jsou evidovány a zda jsou prováděny audity dokumentace. Dále byl sledován vztah mezi údaji v dokumentaci o katetrizaci a péči o pacienta s močovým katetrem a mezi jednotlivými typy nemocnic.Metody: Kvantitativní výzkum.Výsledky a diskuse: Sledováno bylo deset oblastí záznamů: evidence data katetrizace, jméno zdravotníka provádějícího katetrizaci, záznamy velikosti a typu katetru, péče o katetr, výměny sběrného systému, indikace a komplikace ke katetrizaci, odběrů moče na vyšetření a výsledků, dále provádění auditů dokumentace. Respondenti (98,2 %) potvrdili vedení záznamů data katetrizace. Statisticky významné se ukázaly rozdíly v případě typu nemocnice a odpovědí na otázky, zda je do dokumentace vždy zaznamenáván údaj péče o močový katetr (p-hodnota = 0,010, G = 13,253) a zda je vždy zaznamenávána výměna sběrného močového systému (p-hodnota = 0,002, G = 17,196).Závěr: Doporučením odborných společnosti v případě prevence CAUTI je, kromě dalších opatření, vedení dokumentace o katetrizaci, důležitá je i kontrola záznamů.

The relation between the data in the catheterisation documentation and the care of the patient with the urinary catheter and between the individual hospital types was also observed.Methods: Quantitative research.Results and discussion: Ten record areas were observed: recording of the catheterisation date, name of the catheterisation performing health care professional, records of the catheter size and type catheter care, collection system replacement, indication and complications related to catheterisation, urine sampling for testing and results, and documentation audits 98.2% respondents confirmed keeping the records of the catheterisation date in the documentation. There were statistically significant differences in the type of hospital and answers to the questions whether urinary catheter care is always recorded in the documentation (p-value = 0.010, G = 13.253) and whether the replacement of the urinary system is always recorded (p-value = 0.002, G = 17.196). Conclusion: The recommendation of professional companies in the case of CAUTI prevention is, among other measures, the maintenance of catheterisation documentation and, more importantly, control of the records.

PURPOSE OF THE STUDY A very serious complication following joint replacement surgery is periprosthetic joint infection that can be caused by a urinary tract infection. Insertion of an indwelling urinary catheter constitutes a risk factor that may result in urinary tract infections. The aim of this prospective randomised study was to compare the occurrence of significant bacteriuria and symptomatic urinary tract infections during antibiotic prophylaxis at the time of removal of an indwelling urinary catheter by cotrimoxazole in two doses and with no administration of antibiotics. We also monitored the incidence of potential periprosthetic infection following the endoprosthesis implantation. The findings of preoperative urine tests were compared with the declared negative preoperative examination. MATERIAL AND METHODS The study included patients indicated for a total hip or knee replacement with a negative urine culture as a part of the preoperative testing. Where leukocyteria was detected, urine culture by mid-stream clean catch urine was obtained. The second part included patients, in whom an indwelling urinary catheter had to be inserted postoperatively for urine retention and/or monitoring of fluid balance and who were divided into two groups on a rota basis. No antibiotics were administered to the first group, whereas Cotrimoxazol 960 mg tablets p.o. was administered to the second group, 14 and 2 hours before the removal of the catheter. The urine culture test was performed 4 hours after the removal of the indwelling urinary catheter, in both the groups. The test was repeated after 14 days and a questionnaire was filled in to report urinary tract complications. Considered as significant bacteriuria by urinalysis was the laboratory finding of > 10x4 CFU/ml in case of a single pathogen or > 10x5 in case of multiple pathogens. The results were statistically processed by Fischer's exact test with the level of significance = 0.05. RESULTS In the first part of the study leukocyturia was detected by a test strip in 112 of the total of 478 patients. In 10 women, significant bacteriuria was found. Altogether 50 women and 50 men were randomly assigned to the second part of the study. The indwelling urinary catheter was in place for 4 days on average. In men, no statistically significant difference was detected in significant bacteriuria findings, in women a statistically significant difference of p = 0.00162 was found after the removal and after 14 days the borderline of statistical significance of p = 0.0507 was achieved, but no symptomatic urinary tract infection was present. In the period from 20 to 32 months following the total joint replacement, no periprosthetic infection caused by urinary tract infection was reported. DISCUSSION There is enough evidence to prove the correlation between the symptomatic urinary tract infection and periprosthetic infection. On the other hand, asymptomatic bacteriuria is a common finding in patients before the planned hip of knee joint replacement and its treatment is not recommended. No consensus has been achieved as yet regarding the method of antibiotic prophylaxis for an inserted urinary catheter. Antibiotics are administered throughout the period of catheterisation by an indwelling urinary catheter, during its removal, or are not administered at all. In our study antibiotics were administered during the removal of an indwelling urinary catheter and a statistically significant difference was found in women. It concerned, however, only a higher incidence of asymptomatic bacteriuria not treated by antibiotics, which in the next follow-up period did not lead to periprosthetic infection. CONCLUSIONS Despite the negative pre-operative urine culture, frequent incidence of leukocyturia and symptomatic urinary infections were detected in a fairly high number of cases. Therefore, we recommend asking the patients during the hospital admission process specifically about the urinary infection symptoms. The results of our study show that antibiotic prophylaxis during the removal of indwelling urinary catheters placed for a short-term is unnecessary. Úvod: V léčbě neurogenních dysfunkcí dolních močových cest existuje nezanedbatelná skupina pacientů, která je odkázána na dlouhodobou nebo trvalou derivaci moči permanentním katétrem (PMK). Trvale zavedený PMK přináší riziko močové infekce a při infekci "urea‑splitting" kmeny pak riziko inkrustací katétru, které způsobují jeho neprůchodnost. Mezi preventivní opatření patří časté výměny PMK, léčba močové infekce a acidifikace moči. Materiál a metoda: Do prospektivní randomizované studie bylo zařazeno 43 pacientů. První skupina měla zavedený permanentní katétr se standardní péčí. Ve druhé skupině byl dvakrát týdně prováděn preventivně proplach katétru 100ml roztoku URO TAINER® Suby G. V obou skupinách bylo pravidelně prováděno kultivační vyšetření moči a pravidelná výměna katétru po 21 dnech. Doba studie byla devět týdnů a sledovaným parametrem byla neprůchodnost katétru při pozitivním nálezu kmene Proteus mirabilis, Pseudomonas aeruginosa, Klebsiela pnemumoniae a Providencia species a symptomatická močová infekce. Výsledky: Ve skupině s proplachy katétru jsme zaznamenali signifikantně méně epizod neprůchodnosti katétru (p‑value = 0,009). Také počet symptomatických infekcí byl ve skupině s proplachy signifikantně nižší. Studii nedokončili pro nežádoucí účinky dva pacienti ve skupině s proplachy katétru. Závěr: Preventivní proplachy PMK u skupiny pacientů s neurogenní dysfunkcí dolních močových cest vedly k signifikantnímu snížení epizod neprůchodnosti katétru při jeho inkrustaci a také k signifikantnímu snížení počtu symptomatických močových infekcí.
Aim: Despite of advanced treatment of neurogenic lower urinary tract dysfunction there is a not inconsiderable group of patients reliant on long-term or permanent urinary diversion using indweling urinary catheter (IC). IC increase the risk of urinary tract infection and infections with urea-splitting strains can cause catheter encrustation and catheter blockage. Preventive measures include frequent changes of IC, treatment of urinary infections and acidification of urine. Methods: In this prospective study, we randomized 43 patients. The first group had inserted a IC with standard care. In the second group the bladder was washed out twice weekly using URO TAINER® Suby G solution for 10 minutes. The catheter was changed in both groups after 21 days. The period of study was 9 weeks and the endpoint was the number of catheter encrustations caused by Proteus mirabilis, Pseudomonas aeruginosa, Klebsiela pnemumoniae and Providencia species and the incidence of symptomatic urinary tract infection. Results: In the group with catheter washout, we recorded significantly fewer episodes of catheter encrustation (p-value = 0.009). Also the number of symptomatic urinary tract infections in the group with wash-out was significantly lower. Two patients in the wash-out group did not complete the study due to adverse events. Conclusion: Preventive catheter wash-out in the group of patients with neurogenic lower urinary tract dysfunction led to significant reductions in episodes of catheter encrustation and significant reductions in the rate of symptomatic urinary tract infection.

Bladder probing is an invasive procedure that is often used by paediatric and other surgeons. We present a case who was ureter catheterized accidentally while placing a probe into the right ureter in an 18 months old female. If different catheter localization is determined during the examination, the process must be terminated immediately and the relevant specialist should be informed right away.
Autoři tohoto přehledu prezentují různé formy mikčních abnormalit a uvádějí přehled konvenčních způsobů léčby i nejnovějších terapeutických modalit za účelem poskytnout informace urologům, kteří chtějí poskytovat péči pacientům s neurogenní dysfunkcí dolních močových cest na základě nejnovějších poznatků. Dostupné možnosti řešení dysfunkce dolních močových cest neurogenního původu nejsou dostatečně konzistentně ani efektivně využívány. Významné pokroky na poli urodynamiky dosažené v uplynulých 20 letech umožnily lépe porozumět složitým mechanizmům zodpovědným za vznik neurogenní dysfunkce dolních močových cest a vyvíjet vhodné terapeutické strategie. Dysfunkce dolních močových cest neurogenního původu u dospělých představuje velmi častý urologický problém postihující muže i ženy trpící poruchami centrálního, periferního a autonomního nervového systému nebo kombinací těchto poruch. Autoři ve svém aktuálním přehledu hodnotí roli základního a urodynamického vyšetření u pacientů s různými dysfunkcemi dolních cest močových neurogenního původu a zabývají se možnostmi medikamentózní, nemedikamentózní, min. invazivní a rekonstrukční operační léčby. Pozornost je věnována výskytu dysfunkce dolních močových cest neurogenního původu u pacientů s běžnými neurologickými poruchami, ke kterým patří cerebrovaskulární příhody, Parkinsonova nemoc, roztroušená skleróza, nádory mozku, poranění míchy, myelodysplázie, nádory míchy, herniace meziobratlové ploténky, poranění periferních nervů, syndrom cauda equina a syndrom medulárního konu. Autoři prezentují rovněž nejnovější poznatky v léčbě dysfunkce dolních močových cest neuro-genního původu u dospělých pacientů, které se dnes praktikují v referenčních akademických centrech po celém světě. V současné době je kladen důraz na praktickou a klinickou klasifikaci neurogenní dysfunkce dolních močových cest do kategorií s nízkým a vysokým rizikem a její využití při léčbě pacientů s touto poruchou. Pravidelné a doživotní urologické sledování může pomoci ochránit horní močové cesty, zlepšit výsledky léčby, zabránit vzniku komplikací a potenciálně zlepšovat kvalitu života a očekávanou délku života pacienta. In this review, the authors sought to present different patterns of voiding abnormalities and conventional and emerging treatment options in order to provide timely information for urologists who seek to optimize their care for patients with neurogenic lower urinary tract dysfunction. Management options of proven benefit in neurogenic lower urinary tract dysfunction are not uniformly utilized effectively. As significant and rapid advances in therapy have been made in the field of urodynamics in the past two decades, it is becoming increasingly important for clinicians to gain a deeper understanding of complex disease mechanisms associated with neurogenic lower urinary tract dysfunction and to develop appropriate treatment strategies accordingly. Adult neurogenic lower urinary tract dysfunction is a very common urological condition affecting men and women worldwide occurring in patients with central, peripheral, autonomic or mixed neurological conditions. In this up to date treatise, basic and urodynamic evaluation of patients with various neurogenic lower urinary tract conditions is discussed along with non-pharmacological, pharmacological, minimally invasive, and reconstructive surgical treatment options. Lower urinary tract dysfunction in common neurogenic problems including cerebrovascular accidents, Parkinson‘s disease, multiple sclerosis, brain tumors, spinal cord injury, myelodysplasia, spinal cord tumors, herniated intervertebral disk, peripheral nerve injury, cauda equina syndrome, conus medullaris syndrome, among others are discussed. Contemporary advances in the management of adult neurogenic lower urinary tract dysfunction as practiced in academic centers worldwide in 2015 have been presented in this review. Current emphasis on the practical and clinical classification of neurogenic lower urinary tract dysfunction into a low and high risk category and utilization of such categorization in the management of patients with neurogenic lower urinary tract dysfunction has been discussed. Regular and lifelong urological follow up can preserve the upper urinary tract, improve the outcomes, prevent complications, and potentially result in an optimized quality of life and life expectancy.

Infekce močových cest vznikající v důsledku katetrizace (CAUTI) představují jednu z hlavních příčin infekčních onemocnění, k nimž dochází během hospitalizace pacienta. Vedle významného dopadu na morbiditu i mortalitu pacienta má tento typ infekce rovněž nezanedbatelné ekonomické důsledky. Vhodná péče o pacienty se zavedeným katetrem má tedy zcela zásadní význam. V tomto přehledovém článku předkládáme nejnovější informace (založené na důkazech) o léčbě CAUTI. Speciální pozornost je věnována různým možnostem prevence a léčby, jako jsou např. programy pro kontrolu infekce a upomínací systémy, povrchové úpravy různých materiálů, profylaxe antibiotiky a vhodná léčba symptomatických infekcí souvisejících s katetrizací. Optimální technologie pro prevenci CAUTI nebyla dosud vyvinuta a ačkoli nejrůznější přístupy vykazují slibné výsledky, zatím nejúčinnější formou zůstává prevence zbytečného zavádění katetru a jeho co možná nejrychlejší odstranění. Při manipulaci s katetry je nezbytné dodržovat doporučení stanovená na základě důkazů a postupovat v souladu s patřičnými guidelines.
Catheter‑associated urinary tract infections (CAUTI) are a major source of hospital acquired infections with significant consequences to morbidity and mortality and having substantial financial implications. Therefore appropriate catheter care is of utmost importance. In this review we present an evidence‑based update on the management of CAUTI, with special emphasis on the different measures of prevention and treatment, such as infection control programs and reminder systems, surface material modifications of the foreign bodies, antibiotic prophylaxis, and the appropriate treatment of catheter associated symptomatic infections. An optimal technology to prevent CAUTI is still not available and although there are promising results with different approaches the most efficient means are still to avoid unnecessary catheterizations and to remove catheters as soon as possible. Evidence based catheter management and adherence to guidelines is mandatory.
