BACKGROUND AND AIMS: Matrix Gla protein (MGP) is a natural inhibitor of vascular calcification critically dependent on circulating vitamin K status. Growth differentiation factor 15 (GDF-15) is a regulatory cytokine mainly of the inflammatory and angiogenesis pathways, but potentially also involved in bone mineralization. We sought to determine whether these two circulating biomarkers jointly influenced morbidity and mortality risk in patients with chronic coronary heart disease (CHD). METHODS AND RESULTS: 894 patients ≥6 months after myocardial infarction and/or coronary revascularization at baseline were followed in a prospective study. All-cause and cardiovascular mortality, non-fatal cardiovascular events (myocardial infarction, stroke, any revascularization), and hospitalization for heart failure (HF) were followed as outcomes. Desphospho-uncarboxylated MGP (dp-ucMGP) was used as a biomarker of vitamin K status. Both, increased concentrations of dp-ucMGP (≥884 pmol/L) and GDF-15 (≥1339 pg/mL) were identified as independent predictors of 5-year all-cause or cardiovascular mortality. However, their coincidence further increased mortality risk. The highest risk was observed in patients with high dp-ucMGP plus high GDF-15, not only when compared with those with "normal" concentrations of both biomarkers [HR 5.51 (95% CI 2.91-10.44), p < 0.0001 and 6.79 (95% CI 3.06-15.08), p < 0.0001 for all-cause and cardiovascular mortality, respectively], but even when compared with patients with only one factor increased. This pattern was less convincing with non-fatal cardiovascular events or hospitalization for HF. CONCLUSIONS: The individual coincidence of low vitamin K status (high dp-ucMGP) and high GDF-15 expression predicts poor survival of stable CHD patients.

Popisujeme pozoruhodný případ pacienta se stenózou ve větvení kmene levé věnčité tepny, postihující jak odstup ramus circumflexus, tak první marginální větve, která byla úspěšně léčena provizorním zavedením jednoho stentu. Intrakoronární zobrazení a invazivní funkční hodnocení bylo použito k posouzení anatomické a funkční složitosti tohoto případu, pro vlastní techniku zavedení stentu a ke zhodnocení konečného výsledku.
We describe a remarkable case of a patient presenting with left main bifurcation stenosis involving the origins of both the left circumflex and the first acute marginal arteries which was successfully treated using single-stent provisional technique. Intracoronary imaging and invasive functional evaluation were used to assess the anatomical and functional complexity of this case, to guide the stenting technique, and to evaluate the final result.

PCI chronických totálních uzávěrů (CTO) koronárních tepen je jednou z nejdynamičtěji se rozvíjejících oblastí intervenční kardiologie. Samotné téma s sebou nese některé kontroverze. Jedná se o velmi komplexní výkony, které na jedné straně mohou řadě pacientů výrazně zlepšit kvalitu života, na druhé jsou ale zatíženy vyšším rizikem periprocedurálních komplikací než běžné non-CTO PCI. Skutečnost, že i v našich podmínkách vznikly na některých pracovištích dedikované CTO programy s rostoucími počty těchto výkonů, ukazuje, že jde o téma aktuální. V tomto přehledném článku předkládám zkrácenou verzi "state-of-the-art" dokumentu vytvořeného respektovanými světovými experty na problematiku CTO. Základní stavební kameny úspěšného a bezpečného programu CTO PCI jsou tu rozděleny do sedmi obecných principů od indikace, přes plánování výkonů a jejich technických aspektů až po minimalizaci rizika vzniku a případné řešení komplikací.
PCI for chronic total occlusion (CTO) of coronary arteries is one of the most dynamically developing fields in interventional cardiology. The issue itself involves some controversy. These are very complex procedures which, on the one hand, can significantly improve the quality of life of many patients, but on the other hand, are burdened by a higher risk of periprocedural complications compared to common non-CTO PCI. The fact that, even in our conditions, dedicated CTO programmes with increasing numbers of these procedures were set up at some centres implies that it is a current issue. The review article presents an abridged version of a state-of-the-art document developed by world-renowned experts in the subject of CTO. The basic principles of a successful and safe CTO PCI programme are divided into seven general principles, ranging from indications to planning of procedures and their technical aspects to minimizing the risk of developing and/or possibly managing complications.

Funkčně vedená revaskularizace myokardu je v současnosti široce akceptovaným konceptem léčby nemocných s ischemickou chorobou srdce. Přestože klinicky jsou nejvíce probádány stabilní formy ICHS, pevné místo získává tento koncept i v oblasti akutního koronárního syndromu. Vedle hodnot vyžadujících zavedení speciálních koronárních vodičů za koronární lézi: frakční průtokové rezervy myokardu (FFRmyo) založeném na poměru středních tlaků v aortě a za stenózou v průběhu maximální, farmakologicky navozené hyperemie, se v současnosti využívají i indexy klidové (iFR - instantaneous Free Ratio) aj., posuzující především diastolickou fázi tlakové křivky. Hodnoty FFRmyo ≤ 0,80 či iFR ≤ 0,89 značí funkčně významné koronární postižení, které je ve většině případů indikací k revaskularizaci. Funkční významnost koronárních stenóz je však možné posoudit i pomocí neinvazivních testů, od běžně používaných perfuzních zobrazovacích metod, jako jsou perfuzní scintigrafie, kardiovaskulární magnetická rezonance či pozitronová emisní tomografie, až po 3D modely koronárních tepen získaných z počítačové tomografie (FFRCT) nebo invazivní koronární angiografie (QFR). Využití funkčně vedené revaskularizace je v praxi spojeno s významným snížením počtu koronárních intervencí a příznivou prognózou v případě nálezu funkčně nevýznamných lézí. Stávajícím cílem tedy není dosažení anatomicky, ale funkčně kompletní revaskularizace.
The concept of functional revascularization has been broadly accepted. Although the best clinical evidence has been provided in patients with chronic coronary syndromes, recent data strongly advocate its use also in patients with acute coronary syndromes. Invasive pressure-derived indexes: hyperemic FFRmyo (myocardial fractional flow reserve) and resting iFR (instantaneous free ratio) or others require an interventional procedure by wiring the diseased vessel. Values of FFRmyo ≤ 0.80 and of iFR ≤ 0.89 are suggestive of functionally significant coronary disease mostly indicated for revascularization. Moreover there are several non-invasive functional tests that can be used in clinical practice: perfusion scintigraphy, cardiovascular magnetic resonance and positron emission tomography, and the recently introducer FFRCT or quantitative flow ratio (QFR). In routine practice, the concept of functional revascularization avoids unnecessary coronary interventions and, in the case of functionally non-significant disease/stenoses, the patients may be treated conservatively with a very good prognosis. Currently, the "functionally complete revascularization", instead of ananatomical one, should become our new target of treatment.

Standardní radiální přístup z pravého či levého zápěstí se stal dominantní přístupovou cestou ke koronárním katetrizacím a intervencím s doporučením IA dle posledních guidelines Evropské kardiologické společnosti pro revaskularizaci myokardu z roku 2018. Od té doby bylo publikováno několik článků o možnosti provádět intervenční výkony na koronárních tepnách z tzv. distálního radiálního přístupu. Tento článek popisuje naše první zkušenosti s tímto novým přístupem.
Standard radial approach from the right or left wrist has become a dominant arterial approach for coronary catheterizations and interventions with IA recommendation in 2018 Guidelines for myocardial revascularization of European Society of Cardiology. Since that time several articles has been published about distal radial approach. This article describes our first experience with this new approach.

BACKGROUND: Clopidogrel associated with aspirin is the recommended treatment for patients undergoing elective percutaneous coronary intervention (PCI). Although severe PCI-related events are rare, evidence suggests that PCI-related myocardial infarction and myocardial injury are frequent complications that can impact the clinical prognosis of the patients. Antiplatelet therapy with a potent P2Y12 receptor inhibitor such as ticagrelor may reduce periprocedural ischemic complications while maintaining a similar safety profile as compared with conventional dual antiplatelet therapy by aspirin and clopidogrel in this setting. METHODS: Assessment of Loading with the P2Y12 inhibitor ticagrelor or clopidogrel to Halt ischemic Events in patients Undergoing elective coronary Stenting (ALPHEUS) (NCT02617290) is an international, multicenter, randomized, parallel-group, open-label study in patients with stable coronary artery disease who are planned for an elective PCI. In total, 1,900 patients will be randomized before a planned PCI to a loading dose of ticagrelor 180 mg or a loading dose of clopidogrel (300 or 600 mg) in addition to aspirin. Patients will then receive a dual antiplatelet therapy with aspirin and ticagrelor 90 mg twice daily or clopidogrel 75 mg once daily for 30 days. The primary ischemic end point is PCI-related myocardial infarction (myocardial infarction type 4a or 4b) or major myocardial injury within 48 hours (or at hospital discharge if earlier) after elective PCI/stent. Safety will be evaluated by major bleeding events (Bleeding Academic Research Consortium type 3 or 5) at 48 hours (or discharge if it occurs earlier). CONCLUSION: ALPHEUS is the first properly sized trial comparing ticagrelor to clopidogrel in the setting of elective PCI and is especially designed to show a reduction in periprocedural events, a surrogate end point for mortality.

The EUROASPIRE surveys (EUROpean Action on Secondary Prevention through Intervention to Reduce Events) demonstrated that most European coronary patients fail to achieve lifestyle, risk factor and therapeutic targets. Here we report on the 2-year incidence of hard cardiovascular (CV) endpoints in the EUROASPIRE IV cohort. EUROASPIRE IV (2012-2013) was a large cross-sectional study undertaken at 78 centres from selected geographical areas in 24 European countries. Patients were interviewed and examined at least 6 months following hospitalization for a coronary event or procedure. Fatal and non-fatal CV events occurring at least 1 year after this baseline screening were registered. The primary outcome in our analyses was the incidence of CV death or non-fatal myocardial infarction, stroke or heart failure. Cox regression models, stratified for country, were fitted to relate baseline characteristics to outcome. Our analyses included 7471 predominantly male patients. Overall, 222 deaths were registered of whom 58% were cardiovascular. The incidence of the primary outcome was 42 per 1000 person-years. Comorbidities were strongly and significantly associated with the primary outcome (multivariately adjusted hazard ratio HR, 95% confidence interval): severe chronic kidney disease (HR 2.36, 1.44-3.85), uncontrolled diabetes (HR 1.89, 1.50-2.38), resting heart rate ≥ 75 bpm (HR 1.74, 1.30-2.32), history of stroke (HR 1.70, 1.27-2.29), peripheral artery disease (HR 1.48, 1.09-2.01), history of heart failure (HR 1.47, 1.08-2.01) and history of acute myocardial infarction (HR 1.27, 1.05-1.53). Low education and feelings of depression were significantly associated with increased risk. Lifestyle factors such as persistent smoking, insufficient physical activity and central obesity were not significantly related to adverse outcome. Blood pressure and LDL-C levels appeared to be unrelated to cardiovascular events irrespective of treatment. In patients with stabilized CHD, comorbid conditions that may reflect the ubiquitous nature of atherosclerosis, dominate lifestyle-related and other modifiable risk factors in terms of prognosis, at least over a 2-year follow-up period.

BACKGROUND: We hypothesized that fractional flow reserve (FFR)-guided percutaneous coronary intervention (PCI) would be superior to medical therapy as initial treatment in patients with stable coronary artery disease. METHODS: Among 1220 patients with angiographically significant stenoses, those in whom at least one stenosis was hemodynamically significant (FFR, ≤0.80) were randomly assigned to FFR-guided PCI plus medical therapy or to medical therapy alone. Patients in whom all stenoses had an FFR of more than 0.80 received medical therapy and were entered into a registry. The primary end point was a composite of death, myocardial infarction, or urgent revascularization. RESULTS: A total of 888 patients underwent randomization (447 patients in the PCI group and 441 in the medical-therapy group). At 5 years, the rate of the primary end point was lower in the PCI group than in the medical-therapy group (13.9% vs. 27.0%; hazard ratio, 0.46; 95% confidence interval [CI], 0.34 to 0.63; P<0.001). The difference was driven by urgent revascularizations, which occurred in 6.3% of the patients in the PCI group as compared with 21.1% of those in the medical-therapy group (hazard ratio, 0.27; 95% CI, 0.18 to 0.41). There were no significant differences between the PCI group and the medical-therapy group in the rates of death (5.1% and 5.2%, respectively; hazard ratio, 0.98; 95% CI, 0.55 to 1.75) or myocardial infarction (8.1% and 12.0%; hazard ratio, 0.66; 95% CI, 0.43 to 1.00). There was no significant difference in the rate of the primary end point between the PCI group and the registry cohort (13.9% and 15.7%, respectively; hazard ratio, 0.88; 95% CI, 0.55 to 1.39). Relief from angina was more pronounced after PCI than after medical therapy. CONCLUSIONS: In patients with stable coronary artery disease, an initial FFR-guided PCI strategy was associated with a significantly lower rate of the primary composite end point of death, myocardial infarction, or urgent revascularization at 5 years than medical therapy alone. Patients without hemodynamically significant stenoses had a favorable long-term outcome with medical therapy alone.
