PURPOSE: Prompted by the extensive biases in the immunoglobulin (IG) gene repertoire of splenic marginal-zone lymphoma (SMZL), supporting antigen selection in SMZL ontogeny, we sought to investigate whether antigen involvement is also relevant post-transformation. EXPERIMENTAL DESIGN: We conducted a large-scale subcloning study of the IG rearrangements of 40 SMZL cases aimed at assessing intraclonal diversification (ID) due to ongoing somatic hypermutation (SHM). RESULTS: ID was identified in 17 of 21 (81%) rearrangements using the immunoglobulin heavy variable (IGHV)1-2*04 gene versus 8 of 19 (40%) rearrangements utilizing other IGHV genes (P= 0.001). ID was also evident in most analyzed IG light chain gene rearrangements, albeit was more limited compared with IG heavy chains. Identical sequence changes were shared by subclones from different patients utilizing the IGHV1-2*04 gene, confirming restricted ongoing SHM profiles. Non-IGHV1-2*04 cases displayed both a lower number of ongoing SHMs and a lack of shared mutations (per group of cases utilizing the same IGHV gene). CONCLUSIONS: These findings support ongoing antigen involvement in a sizable portion of SMZL and further argue that IGHV1-2*04 SMZL may represent a distinct molecular subtype of the disease.

AIMS: Littoral cell angioma (LCA) is a rare primary splenic tumour that is frequently associated with internal malignancies. Immunohistochemistry can demonstrate a distinct hybrid endothelial-histiocytic phenotype of littoral cells, and is a helpful adjunct for making the correct diagnosis. The aims of this study were to present a series of 25 LCAs, with an emphasis on the frequent association of the neoplasm with visceral malignancies, and to provide a detailed immunohistochemical analysis by employing new markers. METHODS AND RESULTS: All 25 cases with available tissue blocks were immunohistochemically stained for endothelial and histiocytic markers. Clinical and follow-up data were retrieved from the respective institutions. The tumours were obtained from 16 males and nine females, whose age ranged from 32 to 86 years (mean 56.2 years). Clinical information was available for 24 of 25 patients, and follow-up for 11 of 25 patients (range 2-19 years; mean 11.6 years). Immunohistochemically, all cases were positive for LYVE-1, factor VIII, FLI-1, vascular endothelial growth factor receptor (VEGFR)-2, VEGFR-3, claudin-5, ERG, LMO2, CD31, CD163, lysozyme, and CD4, but negative for D2-40, CD8, and factor XIIIa. Fifteen of 25 cases were associated with various malignancies, including epithelial, mesenchymal and haematological tumours. CONCLUSIONS: The cohort of 25 patients is the largest series of LCAs published to date. By using antibodies against recently introduced endothelial markers, we have expanded the immunoprofile of LCA. We have further highlighted the clinical significance of LCA, as more than half of the patients in this study also harboured a coexisting visceral malignancy. Therefore, we conclude that the finding of splenic LCA mandates a thorough clinical evaluation for a concomitant malignancy.

The objective of this registry study was to analyse the outcome of patients who underwent allogeneic or autologous haematopoietic stem cell transplantation (HSCT) for hepatosplenic T-cell lymphoma (HSTL), a rare and extremely aggressive peripheral T-cell lymphoma subtype. Patients were eligible if they had histologically verified HSTL and underwent HSCT between 2003 and 2011. Seventy-six patients were identified in the European Society for Bone and Marrow Transplantation database. Additional baseline and follow-up information could be obtained from the referring centres for 36 patients. Eleven of these were excluded following histopathology review, leaving 25 patients in the final study cohort (alloHSCT 18, autoHSCT 7). With a median follow-up of 36 months, 2 patients relapsed after alloHSCT, resulting in a 3-year progression-free survival of 48%. After autoHSCT, 5 patients relapsed and subsequently died. This study indicates that graft-versus-lymphoma activity conferred by alloHSCT can result in long-term survival for a substantial proportion of patients with HSTL.

T-cell gamma/delta hepatosplenic lymphoma is a primary extranodal lymphoma, distinct from other T/LGL lymphomas. The clinical course is aggressive, and despite use of the multiagent chemotherapy and young age of the patients, the median survival is less than 1 year. Curability of this disease is still uncertain. We reported a case of the successful intensive treatment with splenectomy, chemotherapy a first line autologous stem cell transplant. High intensity cytarabine- platinum containing regimen (EDHAP) may overcome primary resistance to conventional alkylating agents.

v materiálu 8 slezin a 13 kontrol se podařilo paušálně prokázat, že při ITP je výrazná imunitní aktivace. Destrakce trombocytů je však zřejmě nejvýznamnější rolí sleziny u této nemoci. Probíhá klasicky systémem ADCC. Nepodařilo se však prokázat roli jiných buněk než monocytů v Billrothových trámcích. Destrukce trombocytů u ITP je zřejmě velice rychlá a nemusí zanechat ve slezině nemocných žádné zbytky. Sleziny nejsou také výrazně zvětšeny.
Immunologic activation of the splenic white pulp seemed to be an important feature of the structural changes in the 8 splenectomized patients with ITP. However, destruction of the platelets with CD 16+ monocytes was the most prominent change in the Billroth's cords of the splenic red pulp. Classical NK cells did not play an important role in platelets destruction.
Analyzovali sme jeden nekroptický a dva bioptické prípady primárného hepatosplenického lymfómu, diagnostikované v početných biopsiách kostnej drene, sleziny a pečene. Ide o zvláštny „nový" typ zriedkavého T-bunkového lymfomu charakterizovaného zvyčajne přeskupením yδ reťazcov T-bunkového receptora. Tento lymfóm je tvorený histocytologicky pomeme monotónnou prolifee velltých lymfoidných buniek, s diagnosticky typickým intrasínusovým Šírením v kostnej dreni, slezine a pečeni. Typickou črtou je chýbanie lymfadenopatie. Imunohistochemické vyšetrenia dokázali konštantnú pozitivitu nádorových buniek pri dókaze CD3 antigénu a negativitu dókazu antigénov B- a myelomonocytámeho radu, s nepravidelnou koexpresiou antigénov CD45R0 a CD43. Na rozdiel od iných a častějších primárných lymfómov sleziny B-póvodu je jeho klinické chovanie agresívnejšie.
We analyzed one autopsy case and two biopsy cases of primary (hepato-)splenic lymphoma, diagnosed in numerous trephine bone marrow, spleen and liver biopsies. It is a distinctive" new "type of a rare T-cell lymphoma characterized usually by rearrangement of yδ chains of T-cell receptor. Morphologically, the lymj nphoma is composed of a cytologically monotonous proliferation of small to medium sized lymphocytes, with diagnostically characteristic intrasinusoidal spread in the bone marrow, spleen and liver. The involvement of the lymph nodes is always absent. Immunohistochemically, the tumor cells expressed constant CD3 positivity and negativity for Band myelomonocytic antigens, together with an inconstant coexpression of CD43 and CD45RO. In contrast to other and more common primary B-cell spilenic lymphomas, its biological behaviour is more agressive.
